Personal risk assessment processes, also known as Take 5 or SLAM (Stop, Look, Assess, Manage) have been utilised in high hazard industries for over 30 years. However, there is limited research to understand if a personal risk assessment process improves safety and health outcomes. The author’s learnings during discussions with training participants in G3/G-MIRM (Risk Management Systems) and G2 (Risk Assessment for Supervisors) courses have provided them with significant insight into understanding if a personal risk assessment process is valued across a number of mining industry organisations. These discussions arise when the four-layer model is presented (Figure 1) where ‘Layer 4’ represents the last resort risk checking activity. In this context, training participants agree that this process has value. However, the consistent opinion is that the personal risk assessment process is not effective, that it is a ‘tick and flick exercise’ and is not achieving the desired outcomes.

"Société le Nickel exploits the Dôme lateritic nickel orebody at its Tiébaghi operations in New Caledonia. The site geology has an obvious bearing on recovery and metallurgical performance, with controls that are extremely complex at all scales, including lithology, oxydo-reduction conditions, mineralogy and multivariate geochemistry. In this context, establishing an adapted recoverable resource estimation method that is efficient and transparent proves an interesting challenge. The method must address the key notions of: support effect – the exploration data set can only warrant the estimation of large panels of 20 × 20 × 3 m3 that are much larger than the selective mining units (SMUs) of 5 × 5 × 3 m3 information effect – the selection at the production stage uses estimates based on information that is much denser than that available for mine planning.The problem is rendered more complex in this type of deposit by the fact that the additive variables used for the estimation (metal accumulation and ore tonnage) are not the ones used to establish the selection at the mining stage (ie the nickel grade of SMUs, which is the ratio of accumulation on tonnage). Eventually, the selection criteria not only involve Ni grades, but also other elements such as Al2O3, Fe2O3, MgO and SiO2.The solution presented in this paper is to construct an SMU multivariate simulation in the saprolitic horizon and construct an efficient post-processing aimed at producing multivariate recoverable resource estimates. The efficiency constraints imposed by the number of blocks to be simulated (1.5 million) lead to resorting to direct block simulations. The underlying multivariate discrete Gaussian model, which had its validity tested beforehand, is put to good use to mimic the selection process that happens at the mining stage by offering the ability to simulate a composite value located at random within each block.The paper also presents the application of a scenario reduction algorithm to pick a representative subset of a few simulations to help appraise the risk attached to the downstream (reserve optimisation and mine sequencing) phases of the project. The original version of the paper (see Deraisme, Bertoli and Epinoux, 2014) used an early version of the scenario reduction plug-in (named S2RM) built in Isatis® software, where measuring the distance between the initial set of scenarios and the reduced set was based on Ni only. The implementation presented here is for the second generation of S2RM, where measuring the distance between the initial and the reduced set of realisations is based on all study variables. The Pajingo gold operation in Queensland is approximately 50 kilometres south of Charters Towers and 134km south-west of Townsville. The resource was discovered in 1983 and first open pit gold production began in 1986. The mine traditionally employed a bottom-up sequence – a combination of modified and traditional Avoca methods with run-of-mine waste backfill. This practice caused many high-grade crown pillars to be left in place, yet accessible. In recent years, these crown pillars have been extracted by injection grouting of rockfill. This process facilitated blind up-hole open stoping. To date, thirteen separate injection campaigns have been completed, of which ten were deemed successful. In general, the two key technical difficulties in this mining technique are: maintaining a consistent high-quality ground consolidation of the rock fill material; and controlling the drill and blast practices during stoping. Here, effective drill-and-blast control was found to be essential to achieve the dual aims of 100% extraction, and maximum sill pillar stability to minimise dilution. During stoping, the technical performance of the consolidated rock-fill was recorded; both to verify the design, and to identify operational improvements. The results showed that Pajingo has realised both reduced production down-time through use of early strength gain grout, and more rapid and thorough grout consolidation through changes to the emplacement method. The optimisation of consolidated rock fill has led to several quantifiable benefits to the mine, including: profitable extraction of previously sterilised crown pillars resulting in both increased ore reserves and production profile; improvements to the stability of grouted stope crowns, thereby reducing safety and production related; risks associated with open up-hole stopes; reduction in cost and time constraints, with respect to the grouting of loose fill; and reduction in grout curing time – from 14 days to 8 days – thereby minimising production down-time. This paper is a case study of the evolution and optimisation of the up-hole open stoping mining method at Pajingo. The challenges and successes are discussed in terms of their key mining outcomes. PT Freeport Indonesia (PTFI), operating in the Ertsberg Mining District in the Province of Papua, is developing the Grasberg Block Cave copper-gold mine. The site aims to output 160 000 t/d and will be located underneath the current Grasberg Open Pit. Initial studies of the potential to develop a block cave mine in the Grasberg orebody have revealed that the mine will require significant volumes of air. The Grasberg Block Cave (GBC) project requires four primary fans (3750 kW each) to support production at full capacity.Fan #1 commissioning was accomplished successfully on 9 February 2012. However, Fan #2 chamber, which is separated by a 20 m pillar, had not been completed. The main challenge was Fan #2 chamber still needed to be benched. The risk of damage to the critical infrastructure in Fan #1 chamber, including ground support, cracks in pedestals, and misalignment in the impeller shaft were considered. The challenges were blasting the bench without damaging the surrounding infrastructure, completing the work safely and meeting the schedule. This was achieved with the engineering control methodology. EnergyAustralia’s Sydney Central Business District (CBD) cable tunnel network will form a loop 6.2 km in length. The 3.2 km long, 3.5 m, diameter city east cable tunnel (CECT) currently in design, will complete the loop in 2014. This paper discusses the ventilation design of the CECT, considering operation for four functions:1. normal (unmanned),2. access (manned),3. purge, and4. emergency fire modes.The dominant design criterion which determines the required ventilation capacity for CECT is the specification of an upper air temperature limits of 35°C (dry bulb) and 24°C (wet bulb) for manned access, during all ambient weather conditions and cable heating loads. Simplistic static heat transfer analyses do not yield a sufficiently accurate calculation of tunnel air temperature and humidity to provide a safe and yet economical design. The primary refinement in the analysis discussed in this paper is associated with modelling of the system’s dynamic behaviour.Our means of analysis is through a thermo-fluid-dynamic simulation of the interactions between the diurnally and seasonally varying loads (cable heat generation, ambient air conditions and groundwater inflow), tunnel airflow and the resulting tunnel air temperature and humidity. These simulations are validated using experimental data from the existing city south and city west cable tunnels.Fire life safety scenarios are also analysed and discussed in association with the ventilation design. Ventilation strategies are developed for dealing with such emergency fire situations.The understanding and insight provided by the work offers the ability to optimise ventilation plant provisions in future cable tunnels, particularly long and high energy density tunnels.

Many mine accidents will affect the mine ventilation system when they occur. At present, there is a lack of monitoring means to capture the occurrence of accidents in time by monitoring the ventilation system, so as to take emergency measures to reduce the loss of personnel and property. Pressure wave is an elastic wave existing in air. Most accidents in mine will cause obvious changes of pressure wave. It is an innovative idea to monitor the occurrence of accidents in mine by measuring pressure wave. The development and functions of the pressure monitoring device and related software are introduced in detail. The pressure wave monitoring test is carried out in San Xavier underground mining laboratory, and the D-wave and S-wave waveform data are collected. The waveform characteristics are analysed, and the main frequency distribution of the pressure wave is found to be within 1 Hz by using FFT. At the same time, the influence of the propagation distance of the pressure wave on the waveform is analysed. The propagation velocity of D-wave and S-wave are roughly calculated, and they are 279.6 m/s and 397.3 m/s respectively. S-wave is slightly faster than D-wave. Although there are errors and uncertainties in the test, the test verified the feasibility of the existence and collection of pressure waves in the ventilation roadway, and provides a method for the future study of the propagation of pressure waves in the mine ventilation system.

Determination of the mineralogical location (deportment) of minor and trace elements (including credit and penalty elements) in base metal concentrates can indicate whether further processing is likely to (a) reduce the levels of the penalty elements, and/or (b) direct selected elements into other plant products, thereby increasing current concentrate grade or increasing payment for credit elements. To determine the deportment of elements requires knowledge of the proportions of the minerals present and their individual element contents. In addition, knowledge of the liberation and locking is important, as this can affect any possible further processing. Mineral proportions can be determined by a variety of methods including chemical analysis, point-counting, area-counting, QEM*SEM, image analysis and mineral separation. In the case of a Broken Hill zinc concentrate, several of these methods were used and gave consistent results.

Cliffs Natural Resources Pty Ltd (CNR) operates iron ore mines in the Koolyanobbing region of Western Australia, about 50 km north of the town of Southern Cross. Ore is trucked from three geographically isolated sources to the crusher at Koolyanobbing, where it is blended before and during crushing. Lump and fines products are produced and railed to Esperance for ship loading and export to Asian customers. CNR is examining alternative processing paths, from mining to ship loading, with the aim of improving efficiency and reducing costs. Modifications to the system must be consistent with potential future expansions and maintain the low inter-shipment grade variability on which CNR prides itself and has built a strong relationship with its customers. In searching for the optimum process design, many options from mine face to ship loading must be evaluated and compared. Pilot plant studies are infeasible, while complex mineralogical interactions, competing goals and numerous possible system configurations limit the applicability of theoretical analysis. It was therefore concluded that simulation modeling would provide the confidence to take the next step into production trials. This paper describes techniques applied at CNR to simulate grade variability resulting from potential process design changes. The simulation models are easily run Excel-based modules, with each module representing a different part of the process. The modules use extensive Visual Basic macros driven by Excel's user-friendly interfaces. Presentation of the results is enhanced by Excel's excellent graphical capabilities. The simulation software stores and graphically presents time-stamped data from a run, enabling detailed analysis of different process configurations. Final success of a simulation run is measured by inter-shipment variability (standard deviation and process capability) and in-process ore tonnages. Meaningful results from the simulations require the initial input data contain the same correlations present in the real production environment, between the mineral components, production linkages and across time. The data also have to allow simulation of potential changes to mining method and introduction of new pits into the blend. Mining data from the real operations under study are therefore used, with average grades and variability adjusted to match potential future development proposals. It is also necessary to filter out medium and long-term variations from the production data as this variability is best controlled through the conventional medium to long-term mine planning process, not by the process design being studied. The filtering was carried out using a Fourier transform technique, which is described.

Mining and mineral processing makes a vital contribution to AustraliaÆs national export income, and our society today is critically dependent on its products. Despite the importance of this industry, its operations often come under scrutiny because of its high potential impact on local environments û both social and ecological. The challenge for the minerals industry today is to make the most responsible and best possible use of the national resource base, maximising overall resource utilisation.Research efforts and industry implementation of improvements have been moving in this direction for a considerable time already, and the industry has a long history of process innovation and improvement in Australia. Examples range from the early development of gold cyanidation and mineral flotation to more recent work undertaken under the auspices of AMIRA or through the various CRCs and research organisations such as the AJ Parker CRC for Hydrometallurgy, the Cooperative Research Centre for Mining Technology and Equipment and the Julius Kruttschnitt Mineral Research Centre.However, the nature of research and innovation in the mining and mineral processing sector is changing, as the industry itself copes with the pressures introduced by globalisation, industry consolidation and commodity price cycles. Large firms that have historically maintained a tradition of corporate technology and a culture of innovation are moving to outsource their technological and research competencies and are adopting æfast-followerÆ mentalities. At the same time, the challenges of sustainable development that need to be addressed are many-faceted, multidisciplinary, and require a holistic approach for significant achievements to be realised. Many sustainability issues, particularly those pertaining to environmental and safety issues, are shared across large sections of the minerals industry, and are seen as æindustryÆ problems beyond the scope of a single company.In this environment, collaborative research efforts are becoming the dominant mechanisms for building on past innovations and ensuring the continued success of the mining and mineral processing industries within a framework of sustainable development.

THE reduction of iron from the ferric to the ferrous state prior to its titration by permanganate or dichromate may give erroneous results in the presence of other reducible metals such as titanium. Stannous chloride and metallic zinc both reduce tetravalent titanium to a lower form, and so increase the consumption of the oxidizing standard: solution. Sulphuretted hydrogen and sulphurous acid do not reduce titanium, but are slow in action and troublesome to manipulate.Sulphuretted hydrogen is considered to be the best reducing agent for iron in the presence of titanium.* In the method to be described the advantages of the H 2S reduction are retained without the disadvantages of using the gas. The reduction is carried out by the action of a metallic sulphide having a solubility product of such a value that the sulphide iom, are utilized as fast as they are formed, and but little free sulphuretted hydrogen is given off.Cadmium sulphide was the first to be tried, but its solubility product (3.6 x 10 - 2~)t was apparently too low. The reduction was satisfactory, but the excess was very difficult to remove. Zinc sulphide (S.P. = 1.2 x 10 - 23) was found to be quite satisfactoryin both respects. The zinc sulphide is preferably used in the form of an emulsion prepared from pure zinc sulphate as follows: About 100 grm. of ZnS047H20 is dissolved in 2 litres of water. H2S is passed until no further precipitate is obtained, the acid formed being barely neutralized from time to time by the addition of ammonia. The precipitation of the sulphide in a slightly acid solution gives a finer emulsion than that prepared in an alkaline solution, and any iron present remains dissolved. Operational Grade Engineering (GE) is the early (pre-processing) discard of low-grade mineralised and/or barren rock to improve the ore grade and/or reduce deleterious elements within the run of mine ore to be processed. GE activities generally have an impact on primary and ancillary mining activities, equipment utilization and overall equipment productivity. By discarding material which would have reported as ore (in a non-GE mine plan) means that GE must increases the total ore and total rock mining rate requirements to sustain mill feed targets. One challenge of GE and the planning of GE activities is that there are several indirect parameters which impact operational productivity. Modelling these potential impacts and determining if they can be overcome with alternate mine sequences, equipment application or possibly a different equipment fleet altogether is a key question requiring investigation. Overcoming these potential productivity obstacles associated with a more complex operation by introducing new operational parameters and destinations for ore and possibly sub-grade blocks, requires a GE mine plan that is coupled with detailed equipment productivity metrics and a sound operational plan incorporating all GE infrastructure requirements. Several GE mine plans and equipment productivity models indicate that mining rates generally need to increase by approximately 5% to 15% to sustain mill targets and often negatively impact equipment unit productivity metrices through increased cycle times, additional queuing and destination changes. These changes to productivity metrics are however offset by step change increases in asset value. This paper details a methodology to develop GE mine plans that incorporate equipment modelling, flexible mine sequencing and equipment-based production scheduling. In addition, the paper will outline impacts of GE planning on equipment unit productivities through worked examples and models.

McArthur River Mine is located approximately 50 km south-east of the NT township of Borroloola and approximately 120 km from the Gulf of Carpentaria. The mine hosts a series of layered shallow dipping tabular orebodies, with current reserves of approximately 22 Mt, grading 13.6 per cent zinc, 6.3 per cent lead and 65 g/t silver. Owing to economic considerations, the lower most orebody (No 2 orebody) has been selected for initial mine production. Following the completion of the No 2 orebody, mining will transfer to the lower grade and metallurgically more complex No 4 orebody. The mine employs a total of 235 people throughout the underground and surface areas. The majority of the workforce work a fly-in/fly-out, seven-day- on/seven- day- off roster with 12-hour shifts. Presently, a further 18 contractors are involved in trial mining areas. An apparent dip room and pillar technique has been selected as the primary mining method. Officially opened in September 1995, the current production rate of 1.2 Mt per annum will eventually escalate to 1.5 Mt per annum. Though most production is achieved through the use of conventional loaders, trucks and jumbos, the recent development of methods utilising scrapers, hand held machines and modified jumbos has produced encouraging results in steeper dipping areas of the mine. Initial production has been hindered by several factors including extensive and largely unpredictable faulting, worse than expected ground conditions and difficulties with some mining equipment. Compounded by the relatively thin 3 m ore seam, initial mining resulted in high dilution and poor production. This paper describes the development of unique planning and production systems that were evolved in order to overcome these problems and achieve the targeted production rates and head grade.

Electrochemical Behaviour of Massive Chaclopyrite Bioleached Electrodes in Presence of Acidithiobacillus ferrooxidans and Acidithiobacillus caldus In this work, the monitoring of bacterial and chemical dissolution of massive chaclopyrite was evaluated using cyclic voltammetry (CV); the mineral surface was examined using scanning electron microscopy (SEM) and the elements were examined by Energy Dispersive X-ray Analysis (EDAX). The electrolyte employed for the electrochemical tests was the medium used to bioleaching the electrodes (3.0 g/L (NH4)2+SO4, 0.5 g/L K2HPO4, 0.5 g/L MgSO4+7H2O and 0.1 g/L Ca(NO3)2).The results showed differences in voltagramms carried out by Acidithiobacillus ferrooxidans and Acidithiobacillus caldus: In bioleached electrodes tested in the presence of Acidithiobacillus ferrooxidans, the anodic and cathodic current signals were larger than with Acidithiobacillus caldus. The analysis of CV results allowed attributing the different peaks observed in both the direct and reverse potential scanning to the oxidation of chaclopyrite to a secondary copper mineral (bornite, chalcocite and covellite) and its reduction via different non stoichiometric copper sulphides intermediaries. Surface evidences were observed by SEM, and chemical elements evidences were also investigated by EDAX.

Preliminary work being undertaken on the optimisation of decision strategies for an open-pit metalliferous deposit is presented. The Optimal Control method has been selected rather than the easier to implement Dynamic Programming method of optimisation since a fundamental criteria of Dynamic Programming is violated. Essentially a system of state equations and decision variables is developed for the operation under consideration.

"Estimation of stress state as well as elastic modulus is important for the prediction of a rock slope deformation in an open pit mine since it can strongly influence the mining-induced deformation of the rock slope. This study applies a method to evaluate regional stress state and Young’s modulus by analysing the mining-induced deformation of ground. The effect of changes in regional strain state due to tectonic plate motion can be accounted for using the proposed method. Mining-induced deformation can manifest itself as changes in ground surface geometry resulting from mining activity. There are three underlying factors that can contribute to the deformation: displacement induced by the effect of gravity displacement induced by the effect of horizontal regional strain incremental change in the horizontal regional strain. The relative displacement arising from the mining excavation at the Torigatayama limestone mine in Japan has been measured by Global Positioning System (GPS). Both the regional stress state and Young’s modulus were estimated by back analysis of the change in distance between GPS stations using a 3D finite element method. It was shown that the reliability of the estimation improved with an increase in displacement due to mining progression. It was also shown that values for Poisson’s ratios of less than 0.10 are reasonable estimates with values for Young’s modulus between about 5?GPa and 10 GPa. The magnitude and direction of the maximum compressive stress in the district surrounding the Torigatayama mine were estimated to be between -6.4 MPa and -4.4 MPa and around N25E respectively.CITATION:Kodama, J, Sasaki, T, Fukuda, D, Kaneko, K, Fujii, Y and Kudo, R, 2014. Mining is an international business and vital contributor to national and global economies. It depends on the trust and confidence of investors and stakeholders for its financial and operational well-being (Committee for Mineral Reserves International Reporting Standards (CRIRSCO), 2016). The Social Licence to Operate (SLTO) is an increasingly multifaceted and multistakeholder risk that can impact investment decisions. Community challenges to broader political and economic decisions have given rise to protest delaying or stopping projects or closing mines and has led to loss of life (Ernst and Young, 2016). Some governments are now giving greater powers to stakeholders to make the final decision on approving mining and metals activities (EY, 2016). With billions of dollars in project investment at stake, ongoing engagement, collaboration and effective communication with stakeholders is crucial and mutually beneficial solutions are increasingly expected (EY, 2016). It is therefore essential that the industry communicates the risks (and opportunities) associated with investment effectively and transparently in order to earn the necessary level of trust with investors and stakeholders to underpin its activities (CRIRSCO, 2016). The selection and application of an effect- ive firing system first require a clear under- standing of the initiating abilities of the com- ponents in such systems. The conditions under which charges can be prematurely initiated by a detonating cord downline must be appreciated. This is a principal consideration in the design of a successful downline/'Anodet'/primer assembly. The advantages and limitations of an electric and several non-electric initiating systems are explained. The factors which in- fluenced selection of the two most suitable systems for use at Isa Mine (viz. downline/ 'Anodet'/primer assemblies and the reinforced long-lead 'Anodet'-type delay system) are dis- cussed. Blasts are detailed in which each of these has been applied. The initiating design for a hypothetical 2 x 106 tonne blast using electric short delay detonators and multi-channel exploders is presented. In this design, charges in remote blastholes are activated after the detonation of earlier firing charges by protecting the firing lines with sandbags.

While in some cases it is vitally important to measure the virgin stress in rock prior to tunnelling there are a number of cases where such stress measurement is really not required. The concept of having to have a number to put in a numerical model representing a continuum is often quite inappropriate. This paper discusses the cases when virgin stress measurement is required and the most appropriate means to measure it. The techniques discussed cover all of the overcoring systems, hydrofracture and borehole breakout measurements. It also covers the use of rock surface stress measurement, which is easy to perform and should be used to check for changing stress conditions. Unfortunately this technique has been ignored in recent times because engineers are addicted to numerical models requiring far field stresses rather than measuring the real values that exist where they matter – at the tunnel wall. A case is presented in which the measurement of surface stress in a tunnel boring machine tunnel enabled the far field stress to be determined in what must be the world’s largest overcore.CITATION:Gray, I, 2014. It has been almost a decade since the release of the International Council of Mining and Metals (ICMM) Health and Safety Critical Control Management: Good Practice Guide (2015a). During this time the ICMM member companies, along with many others across industry, have been busy designing, deploying and enhancing their own critical control management (CCM) programs. So, what have we learnt? This paper discusses the scope, application and learnings from the implementation of CCM within the mining industry. What’s going well? What are the common challenges? What are the opportunities to strengthen the approach? What are the threats to successful implementation? The paper draws on the author’s experience as one of the key contributors to the development of the CCM Good Practice Guide and an adviser to many mining companies, including six of the ICMM members. The paper focuses on the practical (operational) application of CCM and the cycle of identifying, defining, implementing, verifying and improving critical controls at the frontline. The paper also includes a discussion on the safety performance of the ICMM member companies over the past decade and provides a recommendation for how CCM can be incorporated into a more proactive model to measure safety performance. The paper concludes with practical recommendations that mining companies, regardless of their CCM maturity, can consider as they look ahead. They are supplemented by three additional recommendations for consideration at the industry level.

Titanium (ilmenite) concentrate was extracted from a New Zealandilmeno-magnetite deposit. Tailings from the existing processing line at the Waikato North Head ironsand concentration plant were used as a raw material feed. The titanium bearing mineral extracted was defined as intergrowths of ilmenite and hematite in the size range below 150 um. The Owen-Honda equation was used to determine the conditions forilmenite/hematite magnetic separation. Three distinct regions of magnetic field strength for the mineral separation have been defined. Theoretical estimates from this calculation have been confirmed experimentally. Ilmenite concentrate was obtained using medium field magnetic andelectrostatic separation. The final material contains 40.6 - 42 per cent TiO2 and 39.1 - 39.8 per cent Fe (total). The actual yield from extraction was about seven per cent of the Zircon, leucoxene and monazite were found as byproducts of thetitanium concentration process. Preliminary experiments have shown the viability of industrial zircon benefication.
