This presentation addresses the sources and weighting of project risk in coarse gold deposits with relevance to experiences at the Ballarat mine and Castlemaine goldfield. With sample representation and reliability often a source of great error in the resource estimation of coarse gold deposits, it follows that this matter should be of greatest risk to a mining project - but is it? What are the costs and efforts required to best quantify the sampling error due to nuggetty effect? After all, the main problem in Victorian gold mining ventures in the current decade is not the amount of gold available, but defi ning which portion of it can be converted to Ore Reserves or more so profitable gold bars. The author postulates that greater risk to the project lies in the geological interpretation and matching resource spatial parameters to mining methods, than the degree of uncertainty related to grade due to nuggetty gold. Recent experience in the Victorian goldfi elds is that the investor market has already severely discounted Mineral Resources reported in accordance with The JORC Code - so if they don't believe in the capacity to upgrade the resource why should we? To this end, Castlemaine Goldfi elds Ltd at its Ballarat gold project does not intend to estimate its gold mineralisation for public release, instead choosing to focus on the geological interpretation and mine planning elements. So with coarse gold deposits, is your Mineral Resource reported with The JORC Code a glass half full or half empty? Our mining forefathers in Victoria had no predictive tools which could define resources to any degree and yet were successful. By over emphasis on the definition of mineral resources and particularly the sampling error, are we ignoring the basics of cost control, solid geological interpretations and maximum mining flexibility. The new law on Environmental Protection and Management (Republic of Indonesia Law 32 of 2009) was passed by the Indonesian parliament in September 2009. This law aims to protect Indonesia's rapidly degrading environment by providing tougher provisions for both polluters and law enforcers. In doing so, the law introduces a number of new constraints and potential business risks that will affect all companies in Indonesia, particularly mining companies. Some of the newly introduced concepts follow.   Mining companies will now require an environmental license, issued by the Minister of Environment or local counterparts. This license is a prerequisite for all other business and operational permits. The Minister can revoke the environmental license at any time in case of an environmental incident or dispute. Mining companies may be exposed to additional environmental taxes and bonds besides already existing reclamation and mine closure bonds.   There is an increased risk of environmental lawsuits, due to more stringent controls on one side and greater protection for NGOs on the other side who make claims, false or true, against the company. Company executives and managers can be sued and they will be held accountable if environmental degradation has indeed occurred due to the company's activities.   Companies may face increased liabilities due to a change in the definition of contamination. The new definition is more stringent (and on occasion may prove too stringent to be practical); it also provides little room for flexibility in its application.   As implementing regulations are prepared over the coming two to three years, mining companies will need to adjust to the new law. A proactive approach is needed to minimise risk. Recommended actions include: upgrading existing environmental impact assessments to a higher standard; conducting baseline studies and thoroughly documenting environmental conditions on a regular basis; closely tracking regulatory changes, especially the forthcoming implementing regulations on the new environmental law; and regularly engaging key government and community stakeholders on environmental issues.

This report will differ slightly from the traditional scientific and technical discussions given at this type of conference. It will focus on the personal experiences and choices that I undertook when deciding on my graduate position and location after completing university studies. Throughout the duration of my course I had experience in both fly-in, fly-out operations in the Pilbara and residential operations in the West Australian Goldfields. These experiences, though limited, allowed me to explore the two different methods and their pros and cons, and consequently to make my choice for graduate employment. Throughout the duration of my degree I believed it was important to gain a basic understanding of the two methods, focusing on lifestyle and personal interactions. The decision that a residential site suited my personality and lifestyle then allowed me to refine my job applications to a more specific list of mine sites and companies, resulting in a residential position with BHP Billiton at their Iron Ore operations in the Pilbara, Western Australia.

Organisations’ incident classification processes are plagued by the overclassification of events, often leading to excessive workloads and suboptimal outcomes. This presentation addresses the pressing need to rethink how incidents are classified to maximise organisational learning and minimise systemic risk. A major issue in the current incident classification landscape is the tendency to classify events at high levels, overwhelming organisations with investigations they cannot efficiently handle. This results in inadequate risk reduction and learning opportunities. The link between injury classification and investigation methods further compounds these problems, creating unnecessary administrative clutter. Challenging the misconception that every event, no matter how minor, requires a comprehensive investigation, this presentation advocates for a shift in perspective. The key recommendation is to decouple injury classification and risk matrices from the decision-making process regarding what and how to investigate. Instead, the emphasis should be placed on identifying opportunities for organisational learning and risk reduction. Organisations can streamline their incident management processes by adopting a more pragmatic approach that aligns incident classification with the organisation’s investigation capacity. This transformation focuses on achieving meaningful outcomes, enhancing safety and improving performance. This presentation provides an innovative process for organisations to revamp their incident classification practices, enabling them to harness the full potential of incident data for learning and risk mitigation. It highlights the importance of aligning incident classification with the opportunity for organisational learning, ultimately leading to more effective incident management and enhanced safety outcomes.

In a survey conducted by symplexi with underground miners in Australia; 78 per cent of participants believed that ‘safety’ is all about people. If ‘safety’ is about people, then is the mining industry focusing on the wrong thing? It would be fair to say that the mining industry is heavily systems focused. Companies develop a safety management system and use this to show regulators that they comply with legislation – however, that is only half of the picture.

Bow-tie analysis is a commonly used tool to identify and display risks that have the potential for an undesired hazardous event. The output of the analysis are bow-tie diagrams, which are an effective and versatile communication format that can be applied to any type of risk. Contrary to their simplicity, a proper bow-tie diagram that maintains its usefulness and clear communication is difficult to build. Over two decades assisting Major Hazard Facilities with bow-tie analysis for Safety Cases, several common mistakes have been identified. This paper aims to highlight those issues with examples and provide improvement strategies. A sample of the most common mistakes are: - overly complicated risk analysis - poorly scoped and defined terms - poor descriptions and justifications - identifying too many dependent controls. Overly complicated risk analysis leads to wasted time in obtaining data for little gain. The bow-tie diagram is only a model. Effort should be redirected into identifying and managing controls that will make a tangible difference to site risk. Broad scopes lead to enormous and confusing bow-tie diagrams for both the user and the workshop team. Defining the scope of the study, the consequence severity of interest and critical control characteristics creates focused workshops and targeted analysis. With more definitive scope, there may be more bow-tie diagrams, however, they are succinct and specific. Well described causes are imperative for clear communication to users, regulators and future bowtie review workshop groups. It is best to use common wording structures and explain the event stepby- step. Also, clearly document any likelihood calculations and use data references where applicable. Identifying all possible controls creates a false sense of security that there are many layers of protection. In reality, most controls are dependent on a single system or person and could all be compromised from a single failure. A definition of a ‘bow-tie’ control is required whereby it must be independent from other controls. Improving the bow-tie analysis process will assist operators in developing meaningful and ‘operational’ bow-tie diagrams to support risk management systems and practices.

The Kalgoorlie Consolidated Gold Mines (KCGM) Fimiston Open Pit is located on the eastern boundary of the Kalgoorlie-Boulder City and is mining an area traditionally known as the Golden Mile. The pit area has been continuously worked since 1893, predominantly by underground mining methods and more recently through open cut mining. There are over 2000 km of old mine headings and numerous stopes under the open pit area. KCGM has interpreted and digitised the underground mine plans and stope sections into a common database. A 2D-model software package, called Pits, was developed in-house and allows old workings to be displayed at any bench level; this is integrated with 3D Vulcan modelling software, which allows triangulated surfaces to be displayed. Almost every mining activity is affected by underground workings. The effective management of the interaction between current mining and the underground workings is very important for safety and production. A specific ævoidÆ management department has been formed and its broad objective is to minimise the impact of underground workings upon production while maximising safety. Special criteria and procedures have been developed and implemented to keep personnel and machinery from being exposed to unsafe ground. It is of primary importance to accurately define the location and outline of underground workings in advance. Probe drilling has been used as the key tool for that purpose. Various geophysical methods have also been trialled on site to detect underground workings; these are discussed in some detail. This paper discusses ævoidÆ management issues such as data integration, void detection and flagging. The impact of underground workings on mine planing, production and slope stability is also discussed.

A combination of new radiometric age dates, local field relationships and the regional tectonic history indicate that the low sulphidation, fault breccia-hosted, epithermal gold-silver mineralisation at Umuna is related to continental rifting processes rather than subduction induced deformation and magmatism. K-Ar and Ar-Ar age dating of secondary sericite associated with the gold-silver mineralisation indicates that the alteration crystallised between 4.0 and 3.2 Ma, whereas relative age relationships and SHRIMP II U-Pb dating of the abundant Boiou porphyry intrusions on Misima confirm that the calc-alkaline plutonism is significantly older (8.1 f 0.4 Ma) and related to a different tectonothermal event. The Woodlark Basin sea floor spreading centre occurs to the north of Misima and has continued to propagate westwards, into the continental basement of the Papuan Peninsular, since the end of the Miocene. The westernmost tip of sea floor spreading centre is presently to the east of the D'Entrecasteaux Islands but would have been to the east of Misima roughly 4 Ma. Many of the young tectonothermal features in the D'Entrecasteaux Islands (hydrothermal activity and associated epithermal mineralisation, metamorphic core complex exhumation, detachment faulting, peralkaline volcanism, half graben type basin formation) can be attributed to major extensional deformation ahead of the propagating rift system. Similar tectonothermal features on Misima are temporally associated with the epithermal gold-silver mineralisation at Umuna, suggesting that the D'Entrecasteaux Islands are a modern day tectonic analogue for Misima at the time of precious metal mobility and deposition. Modern petroleum refining offers a complexity of environments, each requiring different metals for the satisfactory performance of-the various sections of the refinery, and this paper first outlines the varied enva.x¦on-ments thus met.It follows that the metal types in use will also vary to suit each particular environment, and the paper' consequently covers the use of ferrous-metals, ranging from plan carbon steels to the high alloy types and the non-ferrous metals, particularly copper and its alloys.The failures of some,of these metals and alloys are then illustrated by experiences it Kwinana. These failures are duo to (a) corrosion, (b) erosion, (c) faulty welding, and (d) mechanical defects,' of which the first is by far the most prevalent. The protective measures taken to prevent subsequent failures are then given under one or other of the following headings :(1) alterations in process conditions, (2) the use of alternative ,alloys,(3) suitable heat treatment, (4) redesign of the failed section, (5) cathodic protection, and (6) use of corrosion inhibitors.Reference is then made to the use of specialised metals and alloys.Metals, generally in the form of oxides or salts, are also used as catalysts in refining processes. The investigations of the Leningrad ruining lilstitute on this problem are carrying out in the following main directions: 1) The analysis and study of contaminants migration at particular sites of mining development; 2) The working out of theoretical and me- thodical basic principles of aquifers migration tests; 3) The work- ing out of methods of the forcasting of groundwater pollution pro- cesses; 4) The theoretical and uethodical substantiation of hydro- geological observations of groundwater pollution processes; 5) The wort ing out of the measures at the groundwater control and their rational use at the particular sites of mining development. A common problem for many mines is insufficient total primary airflow or insufficient airflow in the correct places as the mine changes over its life. This has a number of impacts on the business operation and may result in the need for a major and unexpected capital upgrade of the system, cancellation of a planned production increase, reduction in production targets, increased operating costs, failure to provide contracted metals to customers or acceptance of substandard conditions in the workplace. In some cases, it can even result in premature closure of the mine. In recent times, higher metal prices have given an expected economic boost to many mines, potentially prolonging their life but simultaneously requiring the mine to develop to deeper depths or requiring assets such as shafts and fans to have their service life extended. Insufficient primary airflow is a particular problem in deeper mines as the ventilation circuits become longer, more convoluted, have higher shock losses and the actual airflow requirements increase due to a combination of leakage, dispersion of the workings and the problems of managing the additional heat loads due to higher virgin rock temperatures and autocompression. This paper discusses the reasons why mines experience unexpected shortfalls in primary ventilation capacity with its associated impacts on metal targets, profits and frequently on the health, safety and morale of the workforce, as well as its productivity.

In order to meet the increasing demand for coal for their Newcastle Steel Works, the Broken Hill Pty. Co. Ltd., on June 5, 1937, approved of a scheme to sink a tunnel to the Victoria Seam at their Burwood Colliery. This seam was virgin at this locality and lay nominally horizontal at a depth of 320ft. vertical from the surface.For the purpose of despatch of the coal produced it was decided that the tunnel be equipped with a rubber belt conveyor system.This was a comparatively new departure in Australia, and, indeed, was one of the first installations of its kind in the world, designed to raise coal from considerable depth by this means. Later the tunnel was extended to the Borehole Seam, which lay nominally horizontal at a vertical depth of 600ft. from the surface. The whole installation is the subject of this paper.In planning a project of this kind, consideration had to be given to the following:-1. "One of the key risks to the success of a mining operation is the accuracy of the estimates of both the economic mineral and any accompanying contaminants contained within the ore reserve. Misrepresenting the mineralogical characteristics of the ore reserve can result in considerable financial investment in an operation that is in fact uneconomic and/or carries a significant environmental risk. This in turn can result in an unplanned closure of the operation with associated financial, environmental and social legacy. One way to minimise this risk is to undertake high density resource sampling to gain a better understanding of the resource characteristics. However, such an approach is expensive, time-consuming and often impractical. The risk of not having an accurate model is typically assessed during a feasibility study utilising a Monte Carlo simulation method or some form of sensitivity analysis. Importantly, the error margins used in these types of analyses are often based on subjective or very general criteria such as ‘professional judgement’ or an ‘industry standard’. This presentation introduces a conditional simulation method for developing a range of mineralogical distribution models. Based broadly on the method outlined in Journel and Kyriakidis (2004) the proposed approach develops this work further. It combines a geostatistical analysis of the available data with automated mine design software to provide a rigorous method for assessing the confidence in the mineralogical characterisation of the ore reserve. The process is based on simulating possible mine plans based on the statistical characteristics of the available resource sample data. These plans are then assessed against high resolution resource models generated using a Sequential Gaussian Simulation method. The outcome is confidence intervals for important characteristics of the ore reserve estimate relative to the actual resource. Determining the mechanical properties of rocks is important for various civil engineering fields, including the mining of raw materials for aggregates used in the construction and paving industries. Traditionally, the mechanical properties of rocks are obtained through in situ and laboratory tests during geotechnical surveys. However, these time-consuming surveys involve many resources. In contrast, hyperspectral remote sensing methods make it possible to identify the mineralogical composition and crystallographic structure of the rock; properties that control the mechanical properties of the rocks. In this work, we characterise the mechanical properties of carbonate rocks by using a hyperspectral sensor in laboratory conditions. We collected about 150 cylindrical samples of carbonate rock, with a wide range of strength values from several rock outcrops in Israel. We used a point spectrometer in the range of 0.4–2.5 μm and a spectral image sensor in the range of 3.0–12.0 μm, scanning the samples to obtain the signature of their light reflections and spectral emissivity. We then measured the samples’ density, porosity, water absorption, and uniaxial compressive strength (UCS). We used sophisticated data mining to find statistical relationships between the hyperspectral signatures of the samples and their mechanical properties. We used this data to identify the most dominant wavelengths for predicting mechanical properties. We found that the density, porosity, and water absorption of carbonate rocks could be confidently predicted based on spectroscopy data, while the UCS of the rock could also be predicted, but less significantly. The results of the study pave the way for the development of measuring tools for the mechanical properties of rock, based on non-destructive tests of quarrying materials.

The occurrence and deportment of manganese in a number of ferromanganese and Mn-bearing iron ore samples from supergene-upgraded deposits, as well as from magnetite banded iron formation (BIF) and magnetite skarn deposits, was characterised in order to provide a basis for improved understanding of the processing characteristics of different ore types and investigate the potential for development of a robust geometallurgical classification for Mn-oxide ores. An understanding of the Mn mineralogy in mixed Fe-Mn deposits is necessary to inform any attempt to separate the Fe and Mn during beneficiation and to understand and predict the behaviour of lump and fine ore products during processing.A suite of analytical techniques was utilised, including hand sorting of lump ore particles, optical microscopy, X-ray fluorescence (XRF)/inductively-coupled plasma mass spectrometry (ICP-MS) chemistry, X-ray diffraction (XRD), scanning electron microscopy (SEM)/electron probe microanalysis (EPMA) and gas and solid phase pycnometry, in order to elucidate manganese mineralogy, mineral chemistry and mineral textures. Manganomelane group minerals (eg cryptomelane, hollandite), pyrolusite and lithiophorite were the most common manganese oxides in the samples examined. From the microprobe work, there was evidence of probable substitution of Mn, as well as Al and Si, into the structures of goethite and hematite. Manganomelane minerals were often intimately associated with goethite, in particular, and a clarification of this association and the associated Si and Al impurities is fundamental for improved understanding of beneficiation and sintering behaviour of Mn-oxide ores. Due to the poor or very fine crystallinity of some supergene manganese deposits and of some Mn oxide minerals, in particular the extent of overlapping peaks and their variable mineral chemistry, XRD was a problematic technique to equivocally determine Mn mineralogy.For lower-grade, earthy, poorly microcrystalline ores the mineralogical and textural association between mixed Mn, Fe and Al (¦ Si) phases was also critical. The deportment and potential separation of these elements in these deposits, especially Al, was complex, as they were characterised by forms of æhydrousÆ cryptomelane with higher levels of Fe-Al-Si substitution, as well as intimate intergrowths of ochreous-earthy goethite (probably aluminous in part) with cryptomelane, lithiophorite and/or kaolinite.Trace amounts of alkali elements in ferromanganese deposits were considered likely to be preferentially structurally associated with Mn oxides, while lateritic deposits with significant amounts of lithiophorite could contain elevated levels of deleterious impurities; Co and Ni in particular.A combination of solid and gas phase pycnometry proved to be a useful technique for broadly discriminating hematitic, goethitic and mixed Mn oxide-goethite lump particles based upon differences in their envelope and apparent densities. Based upon these preliminary results and ongoing Raman spectroscopy work on manganese deposits sensu stricto, Raman spectroscopy is a technique considered to have promise during targeted research to better link the optical and compositional properties of ferromanganese ore particles. To effectively plan for closure, we must have an end-point in mind. To undertake meaningful rehabilitation, a post-mine land use (or uses) must be known. However, it is becoming more apparent that the end-point or “goal-posts” are shifting - and may continue to do so throughout the life of a mine. As well as being safe, stable and not causing environmental harm, post-mine land uses should always be sustainable, achievable within the given land capability, and they should involve stakeholders in their design and development.The majority of mines in Australia, and around the World, were historically expected to return disturbed land to either native ecosystem or agriculture/forestry practices. However, examples of highly successful final land uses suggest that today’s focus on closure includes not only minimising future liability and impact but also – and importantly – a move towards the creation of economic opportunity (Murphy and Heyes, 2016).CITATION:Durbin, J, 2018. The management and operation of tailings storage facilities (TSFs) are often neglected until the process plant has to shut down as a result of this negligence or as a result of a catastrophic failure as was the case during the recent TSF failure in North America.TSFs are often regarded as non-assets and do not receive the attention they deserve in terms of technical expertise and financial provision. A TSF is not a simple structure but a complex and dynamic structure that changes throughout its operational life. The operational life of a TSF is the period in which there is the greatest opportunity for risks to occur. The failure of a TSF is normally not as a result of a single event but as a result of a consequence of events. These events are often not recognised in time due to a number of factors.Process plants and slurry pumping systems are operated at suboptimal conditions in order to maintain throughput. At the same time, the tailings properties are compromised in terms of slurry density and particle size distribution amongst other things. Tailings deposition cycles and drying cycles are not well understood and the tailings management strategy is not well tuned in terms of maximising consolidation and improving in situ densities. Pond control is also neglected. These are aspects that form the keystone to the effective utilisation of available deposition area as well as the frequency and cost of future embankment raises.Apart from operational issues, forward planning in terms of budget requirements, increases in tailings throughput, embankment raises, pipe replacements, etc are normally put on the back burner. Available deposition space at the time seems to render this planning obsolete.The bottom line is that responsible tailings management and proper forward planning can limit liability and reduce the bottom line in terms of cents per ton if correctly implemented by suitably qualified tailings managers and engineers.The aspects that need to be considered as part of responsible tailings management are highlighted in this paper.CITATION:Boshoff, J, 2015. On 30 June 2020, the Mine Land Rehabilitation Authority (the Authority) superseded the Latrobe Valley Mine Rehabilitation Commissioner as the independent overseer of Victoria’s declared mine rehabilitation. Currently, only the Latrobe Valley’s three brown coalmines are declared, with each mine at a different stage of rehabilitation and closure planning: Hazelwood ceased mining in 2017, Yallourn is scheduled to end mining in 2028 and Loy Yang in 2035. The three declared mines are very large, close together (Figure 1) and present significant environmental risks that must be mitigated during rehabilitation and post closure. In addition to fire risks, the brown coalmines are inherently unstable and require continuous monitoring and management of ground movements and groundwater pressures. Since 2007 several damaging events have occurred, highlighting the difficulties of achieving long-term stability, even with good landform design and rigorous ground control management plans

Fully grouted cable bolts have been used in underground mining industry for more than 40 years and are becoming more and more popular especially when severe geological conditions are encountered, such as high stresses and large cross-section excavations. The axial performance of cable bolts are mainly relied on the load transfer process between the cables themselves and surrounding rock masses via grout annulus. Therefore, the selected grouts play a significant role in determining the reinforcement quality for underground openings. In order to better understand the mechanical properties of Stratabinder HP grout, which is one type of cementitious grouts commonly used in Australian underground coalmines, plenty of unconfined compressive tests are conducted on it. Following diverse standards, two different kinds of samples are prepared and tested with the w/c ratio ranging from 0.35 to 0.45. During the test important mechanical properties including uniaxial compressive strength (UCS) values, Young’s modulus, Poisson’s ratios and so forth are recorded. Based on experimental results, the effect of w/c ratio on the mechanical properties of Stratabinder HP grout is studied. Beyond that, influences of sample size on the failure mode, peak strength as well as residual strength of this kind of grout are summarised and analysed.CITATION:Chen, J, Hagan, P C and Saydam, S, 2014.
