The Pittsburgh Safety and Health Technology Center (PSHTC) of the Mine Safety and Health Administration (MSHA) provides engineering and scientific support to the inspectorate personnel of MSHA. One of the Center's myriad roles is to assist in the investigation of mining accidents, especially those involving fatalities. Center personnel utilize forensics to help uncover the causes of accidents and to make technical recommendations on the prevention of future similar mishaps. To exemplify a typical forensic investigation, this paper documents the methodology used in determining the reasons for a boat sinking, which resulted in a double fatality on a mine company pond.

Several factors need to come together in creating an integrate mining equipment dispatching system. At the hub of these factors ace the central dispatching formulas. This paper details the development of these central dispatching formulas and explores the combining of these formulas with other elements to create an integrated dispatching decision. WHY DISPATCHING? Truck dispatching implies and in reality should equate to mine equipment efficiency. Good truck dispatching will yield good mine equipment efficiency while poor truck dispatching will yield the alternative. Truck dispatching can either be automated through a computerized, process or it can be manual. Every mine with haul trucks has a truck dispatching system whether it is recognized or not. Even a system wherein the hauler operators determine where they are going for their next load is a truck dispatching system.

The geologic modeling of multiple domains is required for the prediction of mineral resources and reserves. In general, explicit methods have wide acceptance among modelers due to the ability to manually add deemed important trends and geological features into the models. As an alternative, implicit geologic modeling techniques are increasingly adopted to reduce professional time requirements. These methods aim for objective, repeatable and flexible models by replacing the manual time-consuming interpretation of sections with the interpolation of signed distance values. These techniques, however, have been developed for the binary case of two domains at a time. The current paper presents an algorithm that extends the binary case to multiple domains. The method is able to transfer large-scale geological features from data to geologic models. A case study illustrates the benefits of the algorithm and practical implementation considerations.

In the last few years major industrial companies have begun to realize the importance of Reliability as an enterprise strategy that, properly implemented, will improve financial performance. Typical results of an effective Enterprise Reliability Strategy include a 20% -50% reduction in maintenance cost accompanied by a 5% - 10%increase in real production capacity, with no capital investment in production equipment. One factor fueling the race to acquire and implement enterprise reliability is the pressure for companies to be increasingly cost-effective and competitive in global markets. In a recent study, Asset Productivity: The Next Wave, the Boston Consulting Group (BCG)stated that, “In order to compete for funds, companies must offer investor returns that are competitive with other opportunities. Alternative investments such as dot-com and others with non-traditional business models have raised the bar.” (APDF version of the study can be downloaded at www.bcg.com). BCG views asset productivity as the most powerful mechanism for increasing shareholder return. The author believes that the most effective way to increase Fixed Asset Productivity is to maximize asset utilization by increasing the Reliability of the Assets and the associated processes. Developing and implementing an ERS requires significant effort and knowledge. Most companies have difficulty finding a sufficient supply of these valuable commodities to implement an ERS Program internally. Many companies also struggle with figuring out how to get started and what direction to proceed.

Next year, in 1964, Butte will celebrate its Centennial. It was in May, 1864, that the first recorded mining activity took place. During that summer, several rich placers were found on Silver Bow Creek and its tributaries, and a gold rush followed. As placer mining proceeded in the creek beds, the quartz lodes on the hillsides were located and the veins were prospected. Mining in Butte entered its second phase when the rich silver mines, the Alice, Moulton, and Lexington, were opened. The silver boom lasted until the decline in the price of silver in the early 1890's forced the closure of many of the mines. An indication of the beginning of the copper mining era in Butte was in 1866 when it was noticed that green copper carbonate discolored some of the, lode material. It was not until ten years later, however, that the Anaconda and St. Lawrence claims were located by Michael and Ed Hickey. The claims were sold in 1880 to Marcus Daly and in 1881, Daly obtained financing and organized the Anaconda Silver Mining Company to exploit the vein The silver venture was a failure because of the predominance of copper which diluted the bullion to only 400 fine.

When coal in an oil/water slurry is ground to minus-200-mesh in a ball mill, some of the mill energy, normally wasted as heat, can be usefully recovered to help emulsify the oil. If the discharge flows onto a suitable sized trommel screen, water and ash material are rejected through the screen while coal and oil ball together to give a clean coal product. A small (6-lb/hr) continuous plant has demonstrated the feasibility and the limitations of this process for New Mexico coals.

Pre-cambrian scheelite deposits in Northeastern Brazil are widespread in a high grade metamorphic terrain. Previously these deposits had been interpreted as contact-metasomatic. In this paper, a volcanigenic model is invoked based on the volcanic setting (proximal to distal) and the absence of associated felsic intrusives and spatially associated metasomatic skarns. The tungsten was introduced into sediments by volcanic- exhalative processes and subsequently remobilized and further concentrated by heat and metamorphis fluids associated with high-grade metamorphism and migmitization. An alternative syngenetic - diagenetic model is discussed which requires the transport of tungsten by heteropoly acids and deposition of the tungsten as a function of Eh and Ph conditions and different clastic depositional environments. While such stratabound tungsten deposits are considered rare in North America, the hypotheses developed in this paper may open new areas for exploration.

The behavior of non-sulphide gangue in chaloopyrite flotation circuits has been investigated. Plant and laboratory tests were carried out on five different ores, and linear relationships were found to exist between the rates of recovery of water and free nonsulphide gangue in the concentrates although there was evidence from pilot plant tests that these were linear approximations to parabolic curves over the limited regions which were observed. There was evidence that the free non-sulphide gangue enters the concentrate by hydraulic classification and a classification function has been proposed to define this behavior. This function is suitable for inclusion in a mathematical model of the process.

Traditionally Australia has been a leader in underground mining methods with world standards being established at Mount Isa, Broken Hill, Kalgoorlie, and Mount Lyell. With decline in higher-grade reserves and discovery of a multitude of near-surface oxide gold de-posits attention has shifted to bulk mining of low-grade gold ores by open pit methods, heap leaching and activated carbon recovery of gold values. The paper examines historical change in mining philosophy and the economic effects of isolation.

"The Norwegian University of Science and Technology (NTNU) and Nordic Ocean Resources AS, with support from Statoil ASA have carried out a project reviewing the knowledge about seabed mineral resources in Norway to identify and prioritize needs for further research and development.A special focus area in the project has been to increase the knowledge of possible massive sulfide mineralizations along the Mid-Atlantic Ridge. This Ridge is a subsea mountain range which separates the Eurasian and the North American continental plates. In this area, seafloor massive sulphides (SMS) are formed from the volcanic and hydrothermal activities along the Ridge. A large part of the North-Atlantic Mid-Atlantic Ridge between Jan Mayen and Spitsbergen is located on the Norwegian shelf.Analysis and interpretations of multibeam data have been carried out by NTNU in cooperation with the Russian Institute for Geology and Mineral Resources of the Oceans in order to disclose relevant areas for the formation of SMS deposits. This analysis has identified several promising areas."

The concept of initiating a large action-oriented contract, directed toward inherently safe mining systems, was conceived by the U. S. Bureau of Mines in January 1970. At that time we were in the process of establishing research contracts and grants, about 100 of which were let in FY 71 to deal individually with fairly specific technological areas. These projects in the aggregate covered a wide spectrum of health and safety problems. What appeared to be necessary was the creation of a project or program that would provide a focal point for the demonstration of many of the results of this research and also in the action sense culminate with working hardware and systems that could be tested and demonstrated over a fairly long teen. The concept of working demonstrable systems, rather than just a conceptual study, was, felt to be necessary for two significant reasons. In the development of any new system, the translation from concept to working hardware is usually a costly and time consuming process which acts as a deterrent to implementation. Secondly, many decisions in the mining community to change equipment or systems ,are based on first hand observation of the new system, in operational practice, an attitude which is certainly understandable in view of the high capital costs of equipment. At LKAB's plants in Lappland, Sweden, sintered pellets are manu-factured according to three different industrial processes, viz. shaft furnace at Malmberget, straight-grate at Kiruna and grate-kiln at Svappavaara. At present the total annual capacity amounts to 4.7 million tons. However, since it has been decided to erect a new straight-grate plant (2.5 million tons) at Malmberget, and because of the decision to extend and rebuild the existing plants, it is estimated that the annual capacity will be increased to somewhat over 8 million tons by 1973. The great majority of the pellets produced is exported. They are thus reloaded several times and generally speaking roughly handled, resulting in larger or smaller product losses on arrival at the point of use. It is there-fore of great economical interest to have a good control over the transport strength of the pellets by appropriate quality testing at the point of production. In addition to good transport strength, further demands are ob-viously made on pellets destined for blast furnace operation. The blast furnace burden must maintain a good permeability for the gas. The pellets must thus have a favourable size distribution and must not be so weakened by reduction that they disintegrate under the stresses and clog the blast furnace shaft. Furthermore, the burden must have good reducibility. An increase of the rate of reduction can obviously mean increased blast furnace capacity and reduced coke consumption. In order to optimise the properties of the pellets in these respects, the producer must also be thoroughly familiar with their behaviour during reduction, particularly as the customer's interest in the reduction properties of the pellets' has increased over the last few years. Up to now the only quality demands the customer has made have generally had to do with the cold strength and the screen analysis. Thus a quality control of in addition to good transport strength, further demands are obviously made on pellets destined for blast furnace operation. The Miami mine is located near the town of Miami, Gila County, Arizona. The Miami mine started mining in 1910 and finished in July, 1959. The orebody was divided and mined as follows: Tons High Grade 24,400,000 - Top slicing, sub-level caving, etc. Low Grade 105,100,000 - Block caving Mixed ore 9,800,000 - " Low Grade #2 13,100,000 - " Total Mining 152,400,000 GEOLOGY Ore minerals are largely in pre-Cambrian Pinal schist intruded by tertiary Schultz granite porphyry and covered to some extent by Quarternary Gila conglomerate. The structures are highly faulted and shattered. The Miami fault on the east cuts off the ore and the Pinto fault on the southwest caused re-oxidation of enriched sulphides producing mixed ore. MINERALOGY The chief mineral is chalcocite, with chalcopyrite, bornite, covellite, malachite, azurite, chrysocola, cuprite, native copper, and molybdenite as minor minerals. The ore minerals occur in seams, veinlets and disseminated particles. In block caving there is always some dilution, and drawing of ore stops when the grade drops past an economical point. This results in a small amount of copper left in the stopes below the capping. There is some copper in the capping, probably below 0.03% or less than 0.6 lbs. per ton. Over the low grade #2 orebody and portions of the high grade there was an oxide capping which was not mined. The crushed pilars are another source of copper. The leaching at Miami is an attempt to recover this last remaining copper of a worked-out mine. The enrichment at the Miami mine had gone more nearly to completion than most ore bodies in the southwest. There is practically no pyrite in the capping and very little in the ore.

"The presence of enargite (Cu3AsS4) is problematic in gold processing not only because it is environmentally hazardous due to its arsenic content but also because it is refractory, causing both cyanide and oxygen consumption to increase. Selective leaching and treatment of the arsenic would prove advantageous prior to applying conventional gold leaching methods. The reactivity of enargite samples from Butte in Montana and Quiruvilca in Peru were spectroelectrochemically studied under alkaline conditions – pH range of 8-13 – using Raman spectroscopy and cyclic voltammetry. Raman spectra of the surface were collected during and after potentially cycled voltammograms in order to determine and compare surface species against theoretical EH-pH diagrams constructed from mass-balanced thermodynamic calculations using STABCAL software. Under slightly oxidizing conditions, covellite (CuS) peaks were found in a short matter of time, above ~100mV vs. SHE for pH 9-13, suggesting arsenic leaching specifically occurred. At longer conditioning times, elemental sulfur was observed and tended to passivate the surface. The presence of elemental sulfur suggests that copper was eventually leached from the surface. By operating above pH 12, under slightly reducing conditions near -300mV vs. SHE, arsenic can be selectively leached as thioarsenate (AsS4 3-) or thiosulfate (AsO4 3-) without sulfur formation.IntroductionEnvironmental limitations on the pyrometallurgical processing of ores and concentrates bearing arsenic and other hazardous elements often outweigh those in hydrometallurgical processing, particularly when minerals like enargite (Cu3AsS4) are present. The hydrometallurgical processing of enargite-bearing materials can be broken down into two categories: the co-dissolution of arsenic and copper under acidic conditions, and the selective leaching of arsenic under alkaline conditions. The selective leaching of arsenic would preferably leave behind copperrich sulfide phases, typically chalcocite (Cu2S) and covellite (CuS), which then become more amenable to conventional copper (and gold) processing. Furthermore, selective leaching often employs sulfide (S2-), bisulfide (HS-) or polysulfides (Sx 2-) to first extract the arsenic as AsS4 3- and AsS3 3- (Delfini et al., 2003; Anderson and Twidwell, 2008; Tongamp et al., 2010a,b). However, hypochlorite leaching (Viñals et al., 2003; Curreli et al., 2005; Mihajlovic et al., 2007), ammoniacal leaching (Gajam and Raghavan, 1983), and finegrinding to mechanically activate the enargite surface (Balaz and Achimovicova, 2006) have been investigated to increase the normally slow reaction kinetics. Because gold ores are becoming more refractory due to increasing arsenic contents as a result of enargite, these methods are being examined more closely by the gold-mining industry."

For short tunnels, a reinforced cast-in-place lining is not economical and shotcreteis a logical substitution. In the past, reinforced shotcrete linings have been constructed with lattice girders and welded wire fabric. For the Russia Wharf Tunnel in Boston, MA, the Contractor, with the help of the Dr. G. Sauer Corporation offered the MBTA a VECP to substitute the reinforced cast-in-place concrete lining with a steel fiber reinforced shotcrete lining. The challenge was to adequately model the ductility of the steel fiber reinforced shotcrete and to test the shotcrete prior to and during construction to assure quality control. To our knowledge, this type of numerical simulation has not been used on any other tunnels in the US.

The Robinson Mining District near Ely, Nevada, is one of the major porphyry copper districts in the world. Within the district is an east-trending zone of altered porphyry, metamorphosed sedimentary rocks, and sulphide mineralization surrounded by Paleozoic limestone, dolomite, shale and sandstone, Tertiary volcanics, and Quaternary alluvium. Six major disseminated ore deposits have been exploited in this eight-mile zone. About 20% of the district production has come from disseminated ore in metamorphosed sedimentary rocks found adjacent to altered quartz-monzonite porphyry. The following brief statements summarize the inter-relationship of porphyry, alteration, metamorphism, and mineralization: 1. Disseminated copper ore does not occur in unaltered porphyry or in unmetamorphosed sedimentary rocks 2. Sedimentary rocks have been only weakly metamorphosed where intruded, by unaltered porphyry. 3. Intensely metamorphosed sedimentary rocks are found only adjacent to, altered porphyry. 4. Many hydrothermal minerals are common to both altered porphyry and adjacent metamorphosed sediments. 5. Disseminated ore is found in altered porphyry and adjacent meta¬morphosed sedimentary rocks. 6. In general, there is a direct correlation between grade of ore in porphyry and copper content in the adjacent rocks. 7. The situation of The Consolidated Mining and Smelting Company of Canada Limited, or Cominco, may be somewhat unique in that the Company is not only a manufacturer of ammonium nitrate, but as a mining company is a major user of its own product. This situation has resulted in highly successful application of ammonium nitrate-oil explosives. Several noteworthy techniques have been developed and others have been improved. Use of this material in the mining industry has led ammonium nitrate producers to adopt the term nitro-carbo-nitrate or INN for ammonium nitrate-oil blends. A small blending plant at Cominco's largest mining property, the Sullivan Mine, at Kimberley, B.C., is producing more than 80% of all the explosive used at the Sullivan, and is supplying two other Cominco mines. Application has required intensive research to determine the best forms of ammonium nitrate for use in small holes drilled for development and long-hole stoping. It has led not only to experiments to determine the best blending oil, but also to development of loading techniques to the point where NCN is being moved through loading tubes over 700 feet. It has required research into mining practices such as hole arrangement and blasting of development cuts. It has resulted in the development of a special product for secondary breaking. All this has been accompanied by the recognition of the importance of insuring safe practices at every stage.

INTRODUCTION The Wyodak Mine is located in the northern Powder River Basin of Wyoming and is thought to be the oldest continuously operating coal mine in the United States, having recorded yearly production since 1923. The mine has expanded over its 80-plus-year history from an underground room and pillar operation to a modem surface snip mine. Currently, the mine feeds four on-site power plants using conveyors, has a filly automated truck load-out, and ships coal by rail to nearby Wyoming power plants. Annual production of 4.8 MTFT is accomplished on a single-shift basis with 53 fill-time production, maintenance and administrative personnel. In 1990 a decision was made to modernize the coal feed system to the power plant complex with modular conveying equipment. The project involved constructing coal storage silos, overland conveyors, feeder breakers and modular conveyors to follow the mining benches. Total project cost in excess of $25 million was used to construe or commission: A 2,150-TPH 48-inch overland conveyor with horizontal curve A 2,150-TPH 48-inch plant feed conveyor 13 shuttle conveyors, each 150 feet in length 2 front-end loaders equipped with large coal buckets 2 crawler-mounted feeder breakers with compatible discharge conveyor 1 high-lift shuttle conveyor with adjustable discharge height 1 high-angle conveyor 2 4,500-ton coal silos with automated truck load- out capabilities. Conceptually, the plan was to develop 33 MTONS of reserve adjacent to the plant complex and use front-end loaders to tram ROM coal to in-pit feeder breakers. The primary crushing would reduce coal to 6-inch minus and feed through a series of shuttle conveyors to an overland supply conveyor. The overland conveyor would feed the secondary crusher, and the crushed product would be moved to any of four storage silos using conveyors. The in-pit crushing and conveying system is a unique fit for the power plants and provided many interesting challenges for the coal mine to overcome. System integration, maintenance windows, equipment availability and the like had a major role in engineering the system to a power plant complex having no emergency coal stock- pile and limited plant storage. In crediting the designers and operators, the in-pit conveying system at the Wyodak Mine has delivered coal to the power generators since 1994 with little or no downtime. ENGINEERING CONSIDERATIONS Yearly production for the mine is based on 3.5 MTPY which requires about 1,550 TPH on an 8 hour, one-shift- per-day, 5-days-per-week basis. Considering efficiencies for starting and stopping, crew transport, loader trams, and operational delays, a nominal conveyor feed rate of 2,150 TPH or 72% system utilization was used in engineering design application. This ton-per-hour basis is used to size and commission the remaining components of the in-pit system. Coal quality requirements also played a role in equipment selection. The mine reserve grades vertically and coal from Upper and Lower benches needed to be mixed in any given proportion. This required equipment to be filly variable and capable of producing all or part of the 2,150-ton-per-hour requirement. Bisecting the 80-foot- high coal face for quality required construction of a high lift module capable of reaching a single overland conveyor 40 to 50 feet above the pit floor. Designers built a high-lift, adjustable and portable module specific for this application Having two bench systems capable of each producing the required feed for the power plants required computerized control processes to control on-line production. RSLinx, RSLogix, and RSView were chosen as the communication and control software, and a Data Highway Plus serves as the communication backbone. The stability of shale gas wells drilled through current and future coal reserves can be compromised by ground deformations due to nearby longwall mining. Depending on the longwall-induced rockmass permeability, the high-pressure explosive gas from the damaged well may reach mine workings and overwhelm the mine ventilation systems. This study uses geomechanical models to estimate the rockmass permeability induced by mining. A two-panel longwall model of a deep, 341-m-cover mining site in southwestern Pennsylvania is constructed in 3DEC to explicitly model the rockmass by a discrete fracture network (DFN) technique. Stress-induced fracture apertures and permeabilities are calculated across the model and are validated against permeability measurements. A fracture flow code (FFC) is developed to use these results to predict potential inflow to the mine should a gas well breach occur. One hundred DFN realizations are simulated, and the results show that for a gas pressure of 2.4 MPa, the average of the predicted inflow rates to this deep-cover mine is 0.006 m3/ s, significantly lower than the average inflow of 0.22 m3/ s for a shallow-cover mine (145-m deep) studied in the previous work (Khademian, et al. 2021). The result can help assess the potential hazards of a shale gas well breach for mine safety and evaluate the ventilation requirements to mitigate the risk.
