Laboratory column leach tests are used routinely to evaluate gold and silver ores for heap leaching. The tests are fairly straightforward. The ore to be tested is placed into an appropriately sized leach column. Then an alkaline cyanide solution is applied to the ore and the effluent solutions are collected. The effluent solutions are then put through a column of activated carbon to recover the gold and silver in solution. Zinc precipitation may also be used for silver ores or if the ore contains a significant amount of soluble silver in addition to gold. The length of the leach time in the tests is mainly a function of how quickly the precious metals are recovered from the ore. At the completion of the tests, the tailings are assayed and the recoveries determined from the calculated head grades. The amount of ore to be used for a test is partly a function of its crushed size. The coarser the size, the larger the sample size for the column. A typical small-scale laboratory column test on ore crushed to -12 mm (-0.5 in.) would usually be conducted in a 100- to 150-mm-diam (4- to 6-in.-diam) column 2 m (6.6 ft) in height, containing about 20 to 45 kg (44 to 99 lb) of ore. A comparable test on the same ore crushed to 50 mm (2 in.) would be conducted in a 200- to 300-mm-diam (8- to 12-in.-diam) col¬umn 2 m (6.6 ft) in height, containing about 80 to 200 kg (176 to 441 lb) of ore. The Azores islands are located on a triple junction where the American, African (AFR), and Euroasian (EUR) plates meet. The Mid-Atlantic Ridge (MAR), oriented N-S, represents two branches of the T; an E-W branch defines the EUR/AFR boundary called the Terceira Rift. The nature and structure of this plate boundary, as well as the dynamics of the interaction of tectonics and volcanism remain unconstrained. The Azores plateau is seated on a mantle domain of anomalously low seismic S-wave velocities, and gravity and geoid anomalies, where the apparent thick crust is thought to relate to larger degrees of melting initiated at greater depths [1]. The cause of these anomalies is controversial but many authors consider the presence of an anomalously hot/wet enriched mantle probably supplied by a plume [2]. The geochemistry of the subaerial and most submarine lavas from the plateau reinforces the plume model beneath the Azores [3-4]. The origin, size and present location of the plume is under debate but an area near Terceira is the favoured plume centre [5]. Introduction The relationship between precious metal miners and gold refiners has been uneasy over the years. Reasons for this uneasiness and lack of trust include technical factors, economics of scale, and cultural differences. Technical factors take two forms. First, concentrates, precipitates, and sponge produced at the mine site are notoriously difficult to sample. A nonrepresentative sample can be the cause of misunderstanding and distrust. Dote, however, is relatively homogeneous and can be correctly sampled and analyzed with greater reliability. Second, the technology and chemistry of fine gold and silver production is complex and quite different from dote production. Most mining companies are content not to be involved with additional processing steps. From the standpoint of process economies, it is easy to see why miners and refiners are separate groups. Mine location is dictated by the natural occurrence of ore bodies. Heavy capital investment requirements for infrastructure in remote locations usually limits companies to produce the first marketable product from a mine. Beyond that point, the economies of scale and large capital investments for refineries generally dictate that the outputs of several mines be combined to feed a single refinery. The refin¬ing process is so complex that refineries are generally built in larger cities with a skilled labor pool. Therefore, only the largest mines can afford to refine their own output. Lastly, there are cultural differences between the miner and the refiner. The miner, particularly the small gold miner, is by definition a risk taker in a risky business. Refineries, on the other hand, have historically been operated under legal or implied monopolies approved by the government. The government is interested in owning gold, either for currency reasons or because many national mints are major users of gold coins. It is not surprising, then, to find that miners may regard refiners as part of an unresponsive government bureaucracy or, worse, as "city slickers." It is easy for the two groups to distrust each other with these different backgrounds. The interface between miner and refiner does exist and affects nearly all precious metal mining companies and most refineries. This paper discusses the technical and economic aspects of the interface. It points out how both parties can improve the relationship. Technical interface Precious metals selectively dissolve to produce a pregnant solution. They are then recovered from solution by zinc precipitation (Merrill-Crowe process) or by absorption onto activated carbon. This is followed by stripping and electrowinning. At this point, the mining company must decide whether to ship precipitates (or sponge) to refiners or to produce a dote. Some factors the company must consider are the ability to sample precipitates, concentrates or sponge, security of storage, and production rates. An important factor is the incremental cost (capital and operating) of converting precipitates or sponge into dote at the mine. Generally, mines prefer to melt precipitates or sponge to dote to avoid prob¬lems associated with sampling them. There are three types of fur¬naces used by mining operators to melt precipitates to dote. The first and most widely used is a sil¬icon carbide crucible heated ex¬ternally with a gaseous liquid fuel. The crucible is easy to use and slag can be skimmed before pouring dote. This simple system is capable of handling 1 t/a (32,000 oz per year) gold. It could cost as little as $20,000. More complex systems could cost up to $200,000. The second type of furnace is the gas- or propane-fired rever¬beratory furnace. This is usually used only by large-scale opera¬tors in the silver business. Precip¬itates in reverberatory furnaces come in direct contact with the flame. The large volumes of flue gas can carry significant amounts of dust and fume that must be re¬moved from the gas before dis¬charging it to the atmosphere. In the days when the Rand re¬finery in South Africa used reverberatory furnaces, the silver leaving the furnace in the form of vapor and fume was estimated at 2.2% of the total silver charged to the furnace. The gold in the gases was about 0.03% of the gold charged. Most of this silver and gold was recovered in the exhaust gas scrubber system. These scrub¬ber systems can be more costly than the furnace itself.

Because of growing pressure from government and consumers to produce more coal, the mining industry must improve existing techniques and devise more efficient mining methods. Mining of the thicker, shallower and geologically superior coal seams in the western United States is being subjected to increasing pressure from environmentalists; and so in order to meet the demand, the coal industry is exhausting the more accessible surface seams and beginning to mine seam of over 1,000 feet in depth and less than. 30 inches in thickness. The longwall mining system is the natural choice for extracting deep and thin coal seams, as it operates almost independently of the overburden rock stress. Due to the added fact that longwall mining is an inherently more productive method than room and pillar mining, many coal producers are looking to it as a means of increasing productivity without a corresponding cost increase. The consistent rise in application of longwall mining in the United States in the last decade illustrates the excellent potential of this technique for the future. American mining conditions are geologically different from those in Europe where longwall mining has been highly developed and extremely successful. Although numerous successes have been recorded in the U.S. with this mining system, a substantial number of failures have also occurred. These failures are due primarily to roof control problems usually over the face, in the tail entry, or both. Another problem area for American operators contemplating the use of this method has been correct selection of equipment, which should be custom fitted to local mining conditions for optimum productivity. It is essential that correct estimates for chock resistances, support densities, supported and unsupported roof spans, as well as rates of advance and length of working faces be provided to optimize mining conditions. Unless extensive pilot projects are planned engineers must choose their equipment correctly at the outset. A typical investment in longwall equipment will amount to several millions of dollars.

This paper is directed at interested parties outside the Beach Sand Industry : the scope is quite broad. The geomorphology described is a successful working hypothesis which guided the author during prospecting and pegging campaigns : the author is not an expert in this field. The test data used in the text, for illustrative purposes, is original and may constitute a contribution to the state of the art. The operation- al and design philosophy embodied in the text is personal and describes the author's methods : all practising exponents would differ in some degree amongst themselves on these subjects. In this paper, no criticism of any existing operation or brand of equipment is intended and names are used to maintain a real contact with the subject matter. The author believes that the operational and design techniques evolved in the Beach Sand Industry under continual duress, have a wider application in the mining industry. This glimpse of the Industry is intended to enhance the respect for the present and past members. Space does not permit individual acknowledgements.

The conventional method of mining underground, using narrow rooms and wide pillars, has proven to be poorly adapted to the powerful earth forces that act on underground openings. The high costs of roof bolting, timbering, steel arching, and other means of artificial roof support for both room-and--pillar operations and longwall gate roadways attest to the limitations of traditional mine design. Too often, productivity halts when corners buckle under "cutter roof' stresses, floors heave, and roof strata fall. In the 1960s, an entirely different approach to strata control was developed. The Stress Control Method (SCM) stabilizes failing roofs and heaving floors by removing the cause of failure - highly concentrated stresses around the openings. This is achieved by regrouping three to 10 rooms into a single entry where the rooms are separated by very narrow yielding but stable pillars. SCM employs a specified excavation sequence to redirect earth pressures away from the mine openings. The yield pillars act as scaffolding until a permanently protective secondary stress envelope forms around the entry (lower part of Fig. 1). The result is a dramatic improvement in roof stability and a significant reduction in artificial support costs. Some roof bolting must still be done, mainly to meet Mine Safety and Health Administration (MSHA) requirements. Potash, salt, coal mining The stress control method was first applied to the deep potash mines of Saskatchewan. The new SCM design actually increased room widths to enlarge the stress envelope, redistributing the stress concentration away from the entries (Serata and Schultz, 1972). This resulted in a turnaround from pervasive roof fall/floor heave conditions that threatened operations in the eight new mines opened in the Saskatchewan potash field during the 1960s. In addition to eliminating all roof bolting, SCM successfully stabilized ground conditions and substantially increased productivity over the original design (Serata, 1976, 1982). Similarly, the deep Boulby potash mine of Yorkshire, England, was about to close after various conventional means of combating roof failure had failed. An SCM test area was excavated that resulted in the ground quickly stabilizing without need o artificial support (Serata, 1982; Chilton and Laird, 1982). Another example is the Diamond Crystal salt mine in Louisiana. Here, multiple level mining was being extended from the third to the fourth level downward in the Jefferson Island salt dome. Extensive roof failure was occurring at 396 m (1300 ft) ii the third level. SCM design principles in creased the room size from 24 to 49 m (80 t( 160 ft) on the fourth level at a depth of 457 m (1500 ft). This unprecedented expansion of room width was first questioned and later ap¬proved by MS HA, which expressed satisfaction in seeing the improvement documented by field validation data in 1973. The most recent SUM application to salt mining has been in the Michigan Basin a: Goderich, Ontario. The anticipated abandonment of Domtar Chemicals Group/Sifto Salt Division's deep operation in bedded sale was prevented by a multi-year SCM implementation. The success of SCM in dry mining[lecc] to its application to solution mining of salt This adaptation of the method is now being implemented in bedded salt at Fort Saskatchewan, Alberta by Dow Chemical Co., and in St. Clair, MI by International Salt Co. Application of SCM to solution mining in a salt dome is proceeding at Napoleonville. LA by Dow Chemical. The same technology has also been applied to cavern design for the Louisiana Offshore Oil Port (LOOP), and the compressed air energy storage (CAES)

The Dolph Mine fire site is located in Lackawanna County, PA, approximately three miles northeast of the city of Scranton. The fire was burning in both coal refuse (culm) and underground anthracite mine workings. It was reportedly begun by rubbish burning on top of a culm bank. A site investigation was completed in 2006 to define the extent of the fire and to recommend fire control measures. A cut off trench was selected as the principal control method. Plans and specifications were prepared in late 2006. The project was bid and a contractor selected in early 2007. The trench construction was completed in 2007. This paper describes the investigation and selection of the fire control method and discusses the trench construction.

The East Side Access (ESA) Project will extend Long Island Rail Road (LIRR) into midtown Manhattan via 11 km of tunnels, a new terminal station and associated support facilities. In addition to the hard-rock tunneling currently underway in Manhattan, soft-ground tunnels in Queens will traverse beneath Amtrak?s Sunnyside Yard and Harold Interlocking, the busiest rail interlocking in North America. This paper details ESA?s coordinated approach to interfacing with both Amtrak and LIRR with regard to the planned tunnels, shafts and related construction beneath or adjacent to the operating railroads. This interface includes integration of railroad staff, extensive presentations, reviews and agreements with the railroads regarding settlement mitigation, real-time automated geo-instrumentation monitoring, ground improvement, and geotechnical/environmental controls.

To meet increasing demand of coal, underground mining is largely being replaced by the exploitation at deeper horizons and with greater complexities. The exploitation-proposition may be below not-to-be-disturbed surface properties or below water-bodies, posing a constant threat of inundation. Also, the mounting pressures to limit the adverse impacts on the environment bring the extraction-undertaking to face more challenges. An understanding of subsidence-mechanism and a rational quantification of subsidence parameters is a requisite herein. This paper presents such pragmatic quantification, validated at mine-sites in India. In comparison, better productivity, conservation and safety is ensured in this approach.

1967 will be remembered as the year the world realized it was running out of silver. In May of this year a new era in tin the history of the white metal was begun as the power of the U.S. Treasury to regulate world .silver markets was staggered by one of the most intense speculative flurries in recent times. In the past few months the price of silver has risen from $1, 29 to over $1.80 per troy ounce and with it have spiraled the prices of a host of silver-bearing consumer items. Legislation has, been enacted to conserve our coinage and meager silver stocks and today large quantities of non-existent silver are trading hands at a fast pace on the commodity exchanges throughout the world. Now that the price of silver has been set loose, forces are in motion to dull the world's appetite for silver. At the same time there is strong incentive to provide greater quantities of silver to satisfy the world's hunger' for the metal.

Spent potliner is generated in the electrolytic process of reducing alumina to make aluminum metal using molten fluoride electrolytes. Spent potliner (SPL), obtained at the end of the pots life contains a small amount of cyanide and fluoride which present environmental problems. SPL became a listed hazardous waste under RCRA (KO88) in July, 1990. One of the possible uses of spent potliner is as a substitute for Calcium Fluoride (CaF2) as a fluidizer in iron- and steelmaking and in the glass industry. A low viscosity and a low melting point are prime requirements for accomplishing the above. In the present study, viscosity measurements were performed. Effect of various additives, such as sand, limestone, dicalcium silicate dust, and MgO baghouse dust on the viscosity of SPL was studied. The results showed that with the addition of sand and limestone, very high fluidity of SPL melt can be obtained at significantly low temperatures. The results suggest that spent potliner mixed with additives could potentially be used as a substitute fluidizer.

The collection zone residence time distribution (RTD) of mechanical self-aerated flotation cells was characterized for liquid and solid per size class (-45 mm, +45-150 mm and +150 mm). It was found that a single large industrial cell with a volume of 130 to 250 m3, now used in plant operations, can be well represented by an LTST (large and small tank-in-series) model plus dead time. From a scale-up point of view, results showed a similar mixing behavior for industrial cells ranging from 130 to 250 m3. Also, single mechanical cells showed a similar mixing pattern (RTD) when compared to large pneumatic flotation columns. It was confirmed that for practical purposes, the mixing condition in a flotation bank of three or more cells in series can be well described by the classical N tanks-in-series model.

Much has been said and written about the world's, and particularly the united states of America's, energy crisis. The relative changes between cost components of production have altered because the days of cheap energy have now gone. Decisions that made capital investment reduction attractive to treat large tonnage lower grade ore bodies appear to be no longer valid because average energy costs for comminution that were less than 1 cent per kilowatt hour in 1972, are now more than 3 cents and will shortly become 6 cents according to some projections. Increased capital investment to improve processing energy efficiencies are certainly going to be pursued. Energy today is the largest cost component in the average mineral production facility. It takes more than 12000 kilowatt hours to produce one ton of finished steel and more than 60000 kilowatt hours for each ton of copper and aluminum.

Today's highly publicized issues of resource development and environmental protection are not new. But recently these issues have been played one against the other, and some would have us believe they are mutually exclusive. Why? Because modern man has realized that the shortest route to achieving economic progress is to subdue nature. He extracts mineral resources from the earth to provide the raw materials upon which other processes and services are based. But often in his search for economic progress he finds that other highly- held values (e.g., an unspoiled environment) have been violated. The natural response is a counter action - to attempt to protect nature. But this response is basically unsatisfactory because it is reactive.

Application of depositional analysis concepts is becoming an ever-increasingly more important and more useable tool in exploration and development of coals in the Appalachian basin. Over the past decade, a rather large number of studies, papers, and publications have addressed various aspects of depositional environments of coals and, to some extent, applications to development. As would be expected as regards a subject so complex as Pennsylvanian sedimentation in the Appalachian basin, certain academic studies have produced conclusions at variance in some aspects to those of other studies. Overall, however, there has been a building- block progression and general refinement with time of understanding of the depositional settings and other factors controlling Appalachian coal distribution, quality, and other minability parameters. Most attempts to explain coal seam characteristics have revolved around a basic deltaic model. Some have attributed certain seam attributes to upper delta plain deposition, other attributes to transitional-lower delta plain settings, and yet others to lower delta plain environments, and so on. While these approaches have provided valuable insight into some general coal characteristics, they are somewhat incomplete and are, in a sense, too broad for specific mine-scale applications. Further, it is apparent that in order for widespread peat accumulation to occur, there must be a drastic decrease in the energy of the depositional system and amount of sediment being brought in, compared to the active deltaic and coastal processes responsible for deposition of the large volume of clastic sediments that dominate the Pennsylvanian system of the Appalachians. Consideration of this fact implies that peats (coals) may not necessarily reflect depositional settings that are directly coincident with those associated with adjacent rocks. Cecil and Englund (1985) address this point, and quite correctly state that "the primary controls on major stages of peat formation are allocyclic processes involving paleoclimate, eustatic changes in sea level, and tectonics." While this statement is inarguable, it does not negate the importance and usefulness of depositional analysis in predicting reserve characteristics that may impact mining and marketability.

What is the preferred electrolytic cell design and why? Hall: Considerable research remains to be done on electrolytic cell design. Studies are presently being conducted to determine optimum voltage and amperage. Zadra cells, which were used in some of the first carbon plants, are efficient when properly installed. Gold and silver are deposited on large-capacity, cylindrical steel wool cathodes. At times high value sludge falls from the cathode to the bottom of the cell. Rectangular cells, which were developed at some smaller operations in Nevada, require less floor space and have some desirable features in that cathodes can be moved and sludge can be removed from cell bottoms without actually shutting down the circuit. In South Africa two types of cells using graphite and diaphragm configurations are being tested. Duncan: The rectangular cell has one problem it's easy for shortcircuiting to occur. If you don't have a good seal on the side of the tank and on the bottom, the solution is going to want to flow around the bottom and sides of the cathodes and you're going to get poor performance in the cell. That is the advantage the Zadra cell has over the rectangular cell. With good sealing, which is a must, you should be able to get a ratio of the value of the solution going in to the value of the solution coming out of 10 to 1. Ken, can you tell me what you're attaining on the Zadra? Hall: At the Homestake gold operation in Lead, the cathode in the No. 1 electrowinning cell is loaded to about 600 ounces when it is moved to the refinery. At this time the No. 2 cathode is moved to the No. 1 cell. The No. 3 cathode is moved to the No. 2 cell and a cathode with new steel wool is placed in the No. 3 cell. In rectangular cells this sometimes is not possible because the loading on the cathode in the last cell may be as high as that in the No. 1 cell. As Don has pointed out this can probably be prevented by good seal design between cathodes. Potter: At the risk of sounding as though I'm straddling the fence, I really believe that either the round cell, as developed by Jack Zadra, or the rectangular cell will do an acceptable job. The Zadra is a fine unit. Leaks are not unknown but there are not very many. However, the rectangular cell is very saving of floor space. Plastics have been greatly improved during the past year or two and there is a very efficient design for the rectangular cell. It would have six cathodes in it and contain about 25 ft of steel wool. The body of this rectangular cell would be reinforced fiberglass plastic but with an integral lining of a high temperature plastic. I think either cell is so efficient and reliable that I would almost be tempted to flip a coin depending on floor space requirements. Either cell can be used to pull a loaded cathode and to advance the lesser loaded cathodes. Don, what is the configuration of your cell? Duncan: The Pinson rectangular cell has dimensions 2 ft wide, 30 in deep, and 10 ft long-long enough to take a dozen cathodes and anodes. We are currently using six of each but are not up to full capacity as yet. It's a steel tank coated with rubber. We have copper bus bars, which brings to mind that the last cathode in the tank, in one instance, was loaded heavier than the first one. That was probably a function of poor contact between cathodes and the bus bar. You can have erosion of the bus bar and if you don't get current flowing into the anode and out of the cathode, the cells will not function properly. Kappes: Just a few comments on cell design. In an alcohol stripping circuit we are running in Beatty„ NV, we are using PVC pipes for all circuitry piping and a commercial polypropylene electrolytic cell. They seem to be working quite nicely. The cell itself has eight cathodes measuring 18 in2. At six gallons per minute flow into the cell, at an average gold content of about 500 ppm, we seem to get good loading on the first three to four cathodes and not much on the end cathodes. At 12 gallons a minute flow we seem to be seeing gold uniformly along all eight cathodes. We are running at quite a high amperage level, about 400 amps into that cell. Gene McClelland, US Bureau of Mines, Reno Research Center (from the floor): I was associated with a company where the packing density of their steel wool cathodes was much too high and created some short-circuiting problems in their electrowinning circuit. I would like to ask the panel if they can recommend a packing density for cathodes in pounds of steel wool per cubic foot. Hall: The weight of steel wool put on one of our cathodes is 10 lb and the dimensions of the cathode are such that density would be about 0.75 lb/ft3. Duncan: I made a rough calculation and I'm estimating about 2 lb/ft3. Potter: In checking over a number of electrowinning operations, I found apparently very satisfactory operation within the limits that have been mentioned. I have come to a figure of about 0.5-1.0 lb of medium grade steel wool per cubic foot packed as uniformly as possible, of course. One consideration in packing is the fact that there should be good contact between the steel wool and the conductor. Operators tend to compact the steel wool more than would be desirable just to try to maintain good contact.

Sunshine Mining Co.'s Kellogg operation is the primary silver producer in the US. Maintaining a comfortable working environment in this deep underground mine historically has been a complex and costly problem. To improve the ventilation and cooling systems at a time of declining and un-certain silver markets, it was proposed to optimize the performance of existing facilities at minimum cost. This paper discusses a two-year, $400,000 optimization program that resulted in increased total mine airflow, cooler ambient temperatures, reduced compressed air usage, and lower energy consumption by primary fans, air compressors, and refrigeration plants. This saved the company about $650,000 per year.

"Due to recent coal bursts in Australian coalmines, research of coal bursts has attracted intensive interest from mining researchers and engineers worldwide. Previous research has shown that the sudden drop of fractal dimension may be a precursor of catastrophic failure of rock. This paper examines the fractal characteristics of temporal-spatial distribution of acoustic emission signals released during the compression loading process of coal. This research suggests the possibility of coal burst forecasting based on acoustic emission data. The concept and calculation equation of fractal dimension is introduced in this paper. Additionally, uniaxial compression experiments of Australian coal samples are conducted, and the acoustic emission waveform and 3D location are recorded simultaneously by eight sensors Express-8 AE monitoring equipment in the loading process. The experimental results indicate that the continued slowdown of fractal dimension in time sequence can be used as a precursory signal of coal sample brittle failure. The fractal dimension of the spatial distribution of acoustic emission events decreases with the evolution of coal fracturing.INTRODUCTIONCoal bursts refer to the catastrophic failure of overstressed coal subject to the dynamic and static loads. In recent years, coal bursts have occurred in major coal mining countries and this phenomenon has attracted the research interests of international mining and geotechnical scholars. A total of five coal burst accidents happened in Australian coalmines since 2014. On October 20, 2018, a coal burst occurred at Longyun Coalmine in China’s Shandong province, which led to 21 fatalities. Unfortunately, even after decades of research, it seems neither more accurate forecasting methods nor comprehensive controlling measures put forward by researchers have solved the problems posed by coal bursts.As an instantons energy releasing process, coal bursts are always associated with energy dissipation in the forms of acoustic emission, micro seismicity, and electromagnetic radiation (Yamada, Masuda, and Mizutani, 1989). It is believed by many researchers that the frequency–magnitude relationship of micro seismicity induced by rock failure satisfies Gutenberg-Richer relationship as well (Feng et al., 2017). Hence, based on the principal and theory of earthquake source locating, micro-seismicity monitoring equipment has been adopted in many coalmines to locate seismic event origins associated with coal and rock failure in an attempt to better forecast coal and rock bursts. Although many parameters such as b value (parameter of Gutenberg and Richter’s law) (Lu et al., 2015) have been adopted as indicators of risk, the forecasting of coal bursts is still of low accuracy. The same is true with earthquake prediction, as it remains as a worldwide problem even after decades of research and practice. Acoustic emission (AE) methods are now widely adopted to detect rock failure processes under an applied stress for application predicting the abrupt failure of rock in the laboratory (Li et al., 2017). Previous studies have found that significant overlap exists between research regarding AE and micro seismicity with respect to generation process (both are concerned with the generation and propagation of elastic wave) and event origin location, as geophysical signals of both are same (Lockner, 1993). That is, the frequency–magnitude relationship and early-warning indicators found by AE research in the laboratory have a potential applicability to in-field micro seismic monitoring of coal bursts. The drum begins to turn and the bits catch the helper's clothing. He is thrown violently to the ground and suffers multiple contusions. ? A farmer's wife helplessly watches her husband drawn into the claws of farm machinery while she frantically and unsucessfully searches for the control to stop the machine. ? A mechanic on the night shift replaces plugs in the lubrication systems of all three engines of an airliner following routine maintenance. In doing so', he fails to notice that "0" ring seals have not been mounted on the plugs. A few hours later, as the fully loaded airliner is cruising high over the ocean, oil pressure on one engine drops precipitously. The pilots quickly shut down the engine and turn for home. Almost immediately, the two remaining engines lose their oil and fail.
