While less than 1% of reported accidents are associated with slope stability problems, slope failures were responsible for about 15% of all U.S. surface mine fatalities between 1995 and 2001. Small rockfalls, which may involve hand-sized rocks weighing only a few kilograms, can cause fatal injuries to workers away from the protection of large machinery. Massive highwall failures containing a million cubic meters of material or more can be fatal even for heavy equipment operators. As part of an ongoing study at the Spokane Research Laboratory of the National Institute for Occupational Safety and Health, several remote-sensing technologies are being evaluated as tools to monitor slopes for hazards and to assess slope stability. Field tests of a hyper spectral imager were conducted to assess its value for improving geologic maps of potentially unstable alteration zones on mine slopes. An interferometric radar device capable of detecting very small displacements on slopes awaits final assembly and field testing. Computerized monitoring methods using images from digital and video cameras are being assessed for application to mine slope surveillance. These and other techniques will eventually provide new tools to augment current methods for monitoring ground control hazards in mines.

This report describes three cases of carbon monoxide (CO) poisoning in a manhole, including one fatality, from CO migrating through soil after nearby use of explosives. A municipal sewer project involved installation of new pipes and manholes. Work of the general construction contractor was interrupted when a subcontractor detonated 265 pounds of nitroglycerin-based explosive 40-60 feet south of the manhole to break up underlying rock. A construction worker who descended into the manhole 45 minutes after the explosion collapsed within minutes, and two co-workers descended into the manhole to rescue him. One rescuer retrieved the unconscious worker, and the other rescuer died in the manhole. All workers had elevated carboxyhemoglobin levels. In the subsequent NIOSH investigation, air monitoring was conducted with real-time instruments, and air samples were collected in Tedlar bags. Laboratory analyses of the bag samples collected near the bottom showed 1905 parts per million (ppm) CO, 19.5% oxygen, and 3% carbon dioxide. Direct reading instruments showed progressively higher concentrations as the sensor was lowered into the manhole. Subsequent chamber tests on sample explosive yielded 27 liters CO per kilogram detonated. Based on this value, the surface blast at the construction site may have produced about 3,250 liters (114.8 cubic feet) CO. The CO in this incident most likely was released from the nearby explosion and migrated through soil and fractured rock into the manhole. The blasting and construction industries should be made aware of this previously unrecognized CO exposure hazard associated with surface blasting. The extent of CO exposure from explosives used in construction is not known, and additional information on the extent of CO exposure must be collected. In addition, confined space entry procedures (including monitoring confined space atmospheres before entry) should be observed; CO monitoring of confined spaces in the presence of blasting can prevent future incidents such as this one.

A thermal radiation probe was constructed and tested in pulverized-coal-fired furnaces. A rotatable sensing head on the side of a water-cooled probe permitted measurement of radiant flux within a 7.20 cone from any desired direction to a point within a furnace. A response curve for a corner-fired furnace showed radiant flux of 130,000 Btu per sq ft per hr, when viewing flame, and 60,000 Btu per sq ft per hr, when sensing wall reflection. Detailed drawings and fabrication procedure are given.

This is the third in a series of four articles that discuss underground fire-fighting preparedness. As with the first two articles, it is based on interviews with 214 miners conducted at 7 underground coal mines (referred to as Mines ?A? through ?G?) by researchers of the National Institute for Occupational Safety and Health?s (NIOSH) Pittsburgh Research Center [Vaught et al. 1996]. In the first article, the authors presented an overview of the study conducted by NIOSH on mine fire response preparedness and provided a general perspective on underground mine fires. The second article discussed miners? preparedness to evacuate a fire and their experience with incipient fires. In this article, we describe miners? experience in fighting underground mine fires and present their perceptions of training and readiness for fire-fighting.

The Bureau of Mines developed and evaluated a detector-sorting device to separate the copper-bearing portion from the barren portion of native copper ores. Small-scale sorting tests were run on minus 1-inch plus 1/2-inch size materials from the following Michigan samples: Winona, Kingston, and Centennial ores, and "poor" rock from the Champion mine dump. Results showed that 60 to 85 pct of the copper was recovered in 9 to 19 wt-pct of the sorted fraction. About 90 pct of the copper recovered was obtained in the first pass through the sorter. For the Kingston and Centennial ores, combinations of sorter concentrates and fines generated during crushing, resulted in recoveries of 93 and 94 pct, respectively, of the total copper in the samples. The combined products constituted about 66 wt-pct of the total sample and allowed the balance to be discarded with only a 6 to 7 pct copper loss.

Table 10 and figure 4 show the number of fires and fire injuries occurring at the surface of underground metal/nonmetal and stone mines by state during 1990-2001. Table 10 also shows the injury risk rates, employees' working hours, and lost workdays. A total of 12 fires occurred in 11 states; 5 of the fires caused 5 injuries. The yearly average was one fire and 0.42 injury. Five fires with two injuries occurred at metal mines, five fires with two injuries occurred at nonmetal mines, and two fires with one injury occurred at stone mines. None of the fires involved contractors. The Ewhr value was 58 x 106 hr (Irr = 0.017), and the LWD value was 75. Nevada had the most fires (two fires and no injuries). Ohio, New Mexico, Missouri, South Dakota, and Idaho each had one fire with one injury. Of these states, Missouri had the highest injury risk rate value (In = 0.25). Table 11, partly illustrated in figure 5, shows the number of fires, fire injuries, risk rates, employees' working hours, and lost workdays by time period. The number of fires decreased during most of the six time periods. The number of fire injuries and employees' working hours decreased during all of the periods. The Irr values follow patterns similar to those shown by the injury values. Tables 12-17 show the number of fires by ignition source, method of detection and suppression, equipment involved, location, and burning material by time period. Figure 6 shows the major variables related to fires for 1990-2001. Table 18 shows the number of fire injuries per number of fires causing injuries and total fires by year, ignition source, equipment involved, and location.

The Bureau of Mines dried and carbonized two wood wastes, one softwood (redwood) and one hardwood (oak), using continuous, vertical entrainment techniques previously developed for use with coal. It was demonstrated that these techniques are readily applicable to wood waste and that appreciable changes in the yields of carbonization products can be obtained by varying operating conditions. A limited amount of experimental work was done on briquetting the char-coals. Firm briquets were formed with oak charcoal, although greater quantities of starch binder than used commercially for kiln-produced charcoal were required. Redwood charcoal did not briquet successfully under the test conditions employed.

The Bureau of Mines conducted studies related to SO2 air pollution to determine attritioning rates, SO2 sorption rates, and sulfur loads that could be obtained on commercial alkalized alumina sorbent pellets. Fluidized-bed attritioning rates ranged from 0.025 pct/hr for alkalized alumina with a crushing strength of 4 kg to 0.075 pct/hr for 0.5 kg crushing strength mate¬rial. Airlift attritioning rates ranged from 0.50 pct/hr for 3 kg crushing strength alkalized alumina to about 3 pct/hr for 0.5 kg material. Attritioning rates for the commercial alkalized alumina were comparable to those obtained for sorbent pellets prepared in the Bureau of Mines laboratory. Sorption rates were related to S02 concentration in flue gas, temperature, and presence of nitrogen oxides in the gas stream. Sorption rates at 300° C were 0.35 X 10-6 gS/gp/sec (grams sulfur per gram pellet per second) in 0.045 pct so, gas, 5.1 x 10-5 gS/gp/sec in 0.34 pct SO2 gas, and 65 X 10-5 gS/gp/sec in 10 pct SO2 gas. For the 0.34 pct S02 gas, sulfur loads of 0.10 gS/gp (the projected load for regeneration) were obtained in 47 min at 130° C and in 116 min at 300° C. The presence of either NO or NO2 in the 0.34 pet S02 gas increased the SO2 sorption rates 2 to 3 times at 300° C, but had no effect at 130° C.

[The Bureau of Mines has measured the low-temperature heat capacities and high-temperature enthalpies of crystalline K2Si2O5 as part of a program for advancing mineral resources technology with energy economy. High-temperature enthalpies of glassy and liquid K2Si2O5 and crystalline K2SiO3 were also mea¬sured. An adiabatic calorimeter operating over the range 5 to 308 K was used for the heat capacity measurements for K2Si2O5 from which the standard entropy at 298.15 K, S°298 = 45.55 ±0.05 cal/deg-mole, was calculated. Tabulated values of Cp°, S°, -(G° -HO°)/T, and H°-HO° from 5 to 300 K are given for K2Si2O5. A copper-block drop calorimeter was used in the enthalpy measurements, operating from 298.15 to 1,198 K for K2SiO3 and from 298.15 to 1,453.8 K for K2Si2O5. No transitions were found for K2SiO3. The KI2Si2O5 showed two reversible first order transitions, one at 510 K with a heat of transition of 0.29 kcal/mole and one at 867 K with a heat of transition of 0.38 kcal/mole. The heat of fusion for K2Si2O5 20 5 was calculated to be 8.42 kcal/mole. Relative enthalpies are given in equation form, and high-temperature values are tabulated for Cp°, S°, - (G°-H°298 )/T, and H°-H°298 . A method for evaluating the relative incendivity of detonating cord in natural gas-air mixtures was developed. The method consisted of firing short lengths of cord in bundles to determine the number of strands per bundle which would ignite natural gas-air mixtures 50 percent of the time in the 45-cubic-foot gallery of the Bureau of Mines. The relative efficiency of a flame quenching agent, potassium acid tartrate (KHT), when incorporated in both the pentaerythritol tetranitrate (PETN) core and the polyethylene sheath, was explored. The effects of sheath thickness, sheath composition, core weight, and core composition were evaluated. The bundle method established a quantitative scale for evaluating the relative incendivity of cords. The relative incendivity determined by the bundle method was then compared with the incendivity of 100-foot lengths of detonating cord when initiated in 8 percent natural gas in the 640-cubic-foot gallery of the Bureau of Mines.

The Bureau of Mines, through contract with Ultrasystems, Inc., obtained data on the oxidative degradative behavior of a number of materials used in coal mines. Tests were performed using a stagnation burner arrangement. Materials examined included those used in typical coal mining operations such as (1) polyvinyl chloride-containing materials. (2) neoprene compositions, (3) rigid foams (urethanes/isocyanates), (4) miscellaneous materials (including jute, fiber glass, and polyethylene), (5) coal, (6) fluids (phosphate esters, organic esters, oil-water emulsions), and (7) composites. The composite comprised a mixture of a phosphate ester fluid with (1) coal, (2) a representative polyvinyl chloride sample, or (3) a neoprene-type composition. A number of decomposition products are identified that could represent a potential health hazard to miners including HC1, CO, SO2 and H2S, if they were generated to exceed toxic levels.

The Bureau of Mines performed a pilot study examining the effects of posture on back strength and Maximum Acceptable Weight of Lift (MAWL) on six healthy male subjects (M = 32 years + 4 SD). Six hack strength measurements (3 static and 3 dynamic) were made while the subjects were kneeling and standing. In addition, these subjects (who were unaccustomed to lifting in these postures) volunteered to participate in a study of psychophysically determined NAWL in both postures. Results of the back strength tests showed a significantly lower peak torque per body weight output in kneeling versus standing hack strength measurements for five out of six test comparisons (p < .05). Subjective estimates of lifting capacity in the kneeling posture were significantly lower than those for the stooped posture (p < .05). The surface tension from 240° to 844° C and the density from 236° to 583° C of 99.999 percent purity liquid tin has been measured using the maximum-bubble-pressure method. Results for surface tension (y) show y (dynes/em) = 566.84 -4.76 X 10-2 T (° C) with a standard error of 1.9 dynes/cm. Results for density (p) show p (g/cm3) = 7.16 -6.3 X 10-4 T (° C) with a standard error of 0.012 g/cm3. Comparisons with other data found in the literature are discussed. The experimental results are considered to be as good as any data presently available.

Predict the spontaneous combustion potential of a coal, using an expert system computer program. Background Approximately 15% of underground coal mint fires in the United States are attributed to spontaneous combustion. These spontaneous combustion fires frequently occur in worked-out or abandoned areas of a coal mine, This makes the control and extinguishment of the fire difficult and often requires large sections of the mine or even the entire mine to be scaled for long periods of time. A prior knowledge of the self-heating potential of the coal can be useful in preventing spontaneous combustion fires through the use of aggressive monitoring and control measures at existing coal mining operations, and in the planning oh new mines. Many factors can affect the spontaneous combustion process in a mine, including the self-heating potential of the coal, ventilation, geologic factors, and mining conditions and practices. Various test methods have been used to evaluate the relative self-healing potential of coals, including crossing point temperature and adiabatic heating methods, and oxygen adsorption techniques. However, these tests arc usually time-consuming and difficult to conduct, limiting their use to laboratories.

This paper demonstrates that finite-element modeling can be used to predict in situ coal pillar strength, especially under non ideal conditions where interface friction and roof and floor deformation are the primary controlling factors. Despite their differences in approach, empirical, analytical, and numerical pillar design methods have apparently converged on fundamentally similar concepts of coal pillar mechanics. The finite-element model results, however, are not intended to suggest a new pillar design criterion. Rather, they illustrate the site-specific and complex nature of coal pillar design and the value of using modeling procedures to account for such complex site-specific conditions. Because of the site-specific nature of coal pillar design, no single pillar design formula or model can apply in all instances. Understanding and accounting for the site-specific parameters are very important for successful coal pillar design. More work remains before the century-old problems related to pillar design are finally solved. Future research should focus on the cross-linkage of empirical, analytical, and numerical pillar design methods.

A computer code for laminar, steady-state, incompressible, two-dimensional flow developed by Gosman (2)3 was modified by the Bureau of Mines to calculate shock (minor) loss at the intersections of ventilation ducts. Turbulent flow was simulated using laminar flow equations and an appropriate wall shear stress. Results of the calculation showed good agreement with the experiment.

"The petroleum refiners of the United States made rapid strides in fuel economy in 1928, when the calculated number of B. t. u. required to refine a barrel of crude petroleum was 637,000 as compared with 698,000 in 1927, a decline of 9 per cent. The major portion of this indicated increase in fuel efficiency was undoubtedly due to the gain in use of the pipe-still and the continuous process of refining as contrasted with the old batch method of distillation.,Almost without exception the new refineries are equipped with one or more pipe-stills, and many of the older plants are changing over to this type of equipment. The refineries of the Appalachian area are among the oldest and most conservative of the country, yet a number of them dismantled their old shell and cheese-box stills and installed pipe-stills in 1928.The more extensive use of the pipe-still was not the only way in which the refiners lowered their fuel requirements in 1928. The use of heat-exchangers, in which the heat from hot vapors to be condensed is used to heat the incoming cold oil, also increased in 1928, and in addition the refiners have kept abreast of the times in saving fuel through better insulation and improved burners.For the year 1928 the refiners burned as fuel the following: 46,400,000 barrels of-fuel oil; 3.362,000 short tons of coal; 114,950,000,000 cubic feet of natural gas; 77,313, 000,000 cubic feet of refinery or still gases; and 470,000 short tons of coke, in addition to a little electricity. This fuel was calculated to have produced a grand total of 582,156 billions of B. t. u., as compared with 578,784 billions B. t. u. in 1927.The total of 46,400,000 barrels of fuel oil burned represents an increase over 1927 of about 5,000,000 barrels, which made it about equal to the 1926 figure. Practically all of the refinery districts used more oil as fuel in 1928. The most important increase - over 2,000,000 barrels - occurred in the East Coast district, where greatly expanded throughput of foreign crude was instrumental in increasing fuel-oil production.The use of coal for fuel at refineries again registered a material decline, the total for 1928 of 3,362,000 short tons being 33 per cent below that of the previous year. The outstanding decrease in the use of coal in 1928 occurred in the East Coast district, where coal was probably replaced by oil. The use of coal for fuel at refineries in the States of Ohio, Pennsylvania, and West Virginia declined materially in 1928; the substitute used in the majority of cases was oil burned in pipe-stills."

This report analyzes mobile equipment fires for all U.S. surface and underground coal and metal/nonmetal mining categories by state and 2 year time periods during 1990-1999. Risk rate values are derived, and ignition source, methods of fire detection and suppression, and other variables are examined. The data were derived from MSHA mine fire accident publications and verbal communications with mine personnel. The analyses will provide the National Institute for Occupational Safety and Health (NIOSH), the Mine Safety and Health Administration (MSHA), and the mining industry with a better understanding of the hazards associated with mobile equipment fires and will create a basis for future fire research programs.
