Many organisations struggle to yield meaningful outcomes from incident investigations. This paper will demonstrate that we can discover systemic issues by rethinking the ‘Why’ and ‘How’ of investigations, fostering genuine risk reduction. Key points: 1. Shift from ‘prevent recurrence’ to ‘learn and reduce risk’: conventional investigations focus narrowly on recurrence prevention, resulting in narrowly focused investigations with limited ability to identify and reduce organisational risk. Redirecting our objective to learning and improvement opens avenues for broader risk mitigation. 2. Understanding daily work for risk insights: recognise that work is not always completed in full compliance with procedures. Workers will adapt how they complete tasks influenced by the conditions and management systems that exist every day. Understanding the conditions and systems affecting everyday work is vital for uncovering systemic risks. 3. Unearth systemic issues through in-depth exploration: many investigations address surface concerns such as lack of training, incorrect procedures, equipment not being maintained, and missing overarching organisational risks. Systemic, organisation-wide risks are not identified. To discover these systemic issues and create shared learnings, investigators must question management systems, including resourcing, planning and risk management. 4. Design actions for safe failure: common actions like discipline and training fall short and do not reduce risk. Resilience emerges from well-designed controls. Incorporate planning, better resources, interlocks, and guarding to bridge the gap between task execution and risk. 5. Investigation Reports as a Business Case: Investigation reports should not simply state historical information regarding an incident. They are to facilitate decision-making and be presented as a business case to drive change. Organisations can move beyond localised risk and ineffective administrative actions by changing investigation objectives and methods. They can facilitate learning, understand daily work intricacies, identify systemic issues, and enhance the capacity for safety, driving lasting risk reduction.

During the ten year span from 1978 to 1987, the number of fires which occurred in under-ground coal mines doubled from the previous ten years. Anyone involved with underground coal mines is well aware of the associated tragedy that can result from these fires, both to personnel and to property. Every day mine management and mining personnel face this threat of a possible mine disaster or fire. Their ability to deal with such a situation safely and effectively depends on the level of emergency preparation developed and maintained. As a direct result of the ever-increasing interest in the Mine Emergency Preparedness Program and the Mine Fire Training Programs currently being conducted, the Emergency Preparedness Center was created. As a branch of the Mining Extension Service at West Virginia University, the Center provides preparedness training at several different levels on a varying range of topics. Classes are conducted at a new especially-designed training laboratory at the National Mine Safety & Health Academy in West Virginia. By maintaining the proper equipment and levels of training allowing for immediate response, the personnel who are part of the Center believe that the impact of a mine fire or other such emergency situation can be minimized. This paper will present recommendations for maintaining an efficient mine emergency preparedness program and will provide information regarding the Mine Fire Training Program as conducted by the Emergency Preparedness Center and describe the advantages of the newly constructed training facility.

The demands for increased productivity on the 1967 coal industry have generated new operating trends and fresh approaches to old methods, which have enabled the industry to keep pace with the expanding economy. Changes, while not radical in concept or design, have been plentiful and are marked by the refinement of the already tried, tested and proven. Perhaps not since World War II have there been demands on equipment manufacturers to design and deliver such diversified types of machines as those required by the industry in the past year. Equipment developments-following a plan which calls for higher productivity, lower maintenance costs and greater machine availability- have tended more toward increased horsepower and heavier, more rugged equipment. Electrical and mechanical components, designed in many instances with maximum reliability rather than initial costs as the main criterion, have contributed to the improvements in over-all performance. High-strength, specialty steels are finding use in a wide range of equipment applications. The horsepower capacity of electric motors being installed is more frequently being rated on a continuous basis rather than the former one hr or less and, as a result, fewer failures now occur because of overloading. These developments reflect the pyramiding cost of equipment down-time and represent definitive progress in reducing the magnitude of the problem.

The solvent extraction section of an old Uranium Plant in the Vaal River region (North West Province in South Africa) produced a substance called CRUD (Chalk River Unidentified Deposit) that formed within the aqueous/organic interface of the settler of the solvent extraction circuit. The CRUD was deposited together with gold tailings to the tailings dam many years ago. Previous research done on the CRUD confirmed that exposed CRUD leaches Mn and Ni at concentrations that are well above their acceptable risk limits as well as Zn that leaches at concentrations that are slightly lower than its acceptable risk limit. Soil washing and bioremediation technologies were employed to treat the contaminated soil.   Various laboratory test work and the pilot scale studies were conducted in parallel with each other and the results of the laboratory studies were used to determine critical parameters. Four lixiviants, ie sulfuric acid, acetic acid, oxalic acid and ethylene diamine acetic acid were tested at three concentration levels ie 0.001 M, 0.0001 M and 0.00001 M, respectively. Column tests showed that the leaching of the heavy metals increases as the soil porosity increases. Silica sand was added to increase porosity of the contaminated soil. Highest Mn removal was achieved mainly by sulfuric and oxalic acid at 0.001M at all soil levels. EDTA was significantly efficient in the removal of Ni and Zn. The recovery of heavy metals, particularly Mn, decreased as the depth of sampling increased.   Heterogeneous bacteria and filamentous organism plate counts were conducted to evaluate the growth of the bacteria and filamentous organisms in the soil. Results showed that more organism growth took place in the bioremediated soil with a fertilizer added to it. The leaching of metals from the soil that was bioremediated in the pilot study was generally higher than the extraction of the metals from the soil that was leached with no bioremediation carried out. The results from this study suggest that the leaching of heavy metals from the contaminated soil is to a large extent dependant on the pH of the leaching solution. By increasing the soil's porosity and bioremediating the soil, higher extractions of metals were achieved.

CaF2 has been a common constituent in a wide variety of welding fluxes, including those used for submerged arc welding (SAW). The widespread use of CaF2 stems from its role in lowering the liquidus temperature as well as the viscosity of the molten slag that covers the weld pool. However, the harmful effect of fluoride evaporation from molten flux, on the environment as well as the health of the users is becoming increasingly evident over the years. This has led to increasing attention towards reducing the CaF2 concentration in the fluxes. The fluoride-free formulations face the challenging requirement to maintain the thermo-physical properties (viscosity, liquidus temperature etc) like those observed in fluoride-containing fluxes. In the present work, three fluxes with compositions in the CaO-MnO-SiO2-Al2O3 system have been identified as promising alternatives to the CaF2-rich fluxes. However, the thermo-physical properties of any such flux need to be comparable to CaF2-based commercial SAW fluxes before it can be adopted for regular use. The single hot thermocouple technique (SHTT) has been used by several researchers in the past as a tool for observing the melting and solidification characteristics of fluxes. This device has been developed in the present work and is used to characterise the melting behaviour of the fluxes. The measured solidus and liquidus temperatures from SHTT were compared with the results from differential thermal analysis (DTA) and simulations using the FactSage™ software for a comprehensive understanding. In spite of having a slight deviation in the measured values from the multiple techniques, the results obtained from these multiple methods show that the selected compositions could be potential CaF2 free alternative welding fluxes.

ABSTRACT Before any large plant is moved onto an oil sands dominated surface, particularly those surfaces encountered in-pit, it is imperative that the ground be evaluated for bearing capacity. A 2230 tonne mobile in-pit hopper/crusher facility designed for operation in the Canadian oil sands was modeled. This evaluation focused on track and outrigger pad versus ground interactions as the key performance indicators of appropriate ground performance below the unit in both operating and repositioning propel modes. A study of cyclic oil sand behaviour was also inferred from laboratory cyclic plate load data as an extension of the simulation model indications.

Modeling and simulation are two major tools available to a mineral process engineer for a consistent and systematic process analysis. The application of these tools to the analysis of specific mineral processing unit operations is well documented in the literature; nevertheless, they are used only to a limited extent in the industry. This is because the fundamental principles of modeling and simulation in the context of mineral processing context are not available. This paper represents the first in a series of papers planned to meet this need. General principles of process modeling and a unified mineral process model development methodology are presented. A simple example of model development using the flotation unit operation is given. A discussion of the practical significance, challenges and opportunities associated with mineral process modeling is included.

[For a year or more, statements have appeared in the technical press, and Especially in various trade journals, indicating that valuable metals such as gold, silver, platinum, and other rare metals, have been found and also have been successfully extracted from oil shales. These reports stated tat decarbonization and special methods of chlorination treatment, leaching and subsequent Precipitation, also cyanidation followed by table concentration of the residues after cyanidation, all indicated that told ad silver and other metals could be, and were, successfully recovered. Such articles have caused much misunderstanding, and as the correctness of some re of the statements made in them were questioned by some of the technical journals and by various chemists and metallurgists, an investigation of the subject. was begun by the metallurgical division of the Bureau of Mines, in order, if possible, to determine to what extent, if any, metals of value do occur in oil shales, and if so, the feasibility of attempting their recovery by tie processes said to have been used for teat Purpose. Inasmuch as the Bureau's station at Salt Lake City is investigating the recovery of oil from oil shales, the investigation of too recovery of metals from oil shales was assigned to my direction as superintendent 'of that station. An agreement had been entered into with the owner of m extensive oil shale deposit in Wyoming whereby I would personally collect the samples' of shales for use in the investigation.]

"Devices and flowsheets for recovering fine placer gold are reviewed. Inefficiencies of gravity processing techniques are documented and discussed. Prospects of combining gravity and froth flotation techniques are presented. Possibilities of using other separation techniques, such as magnetic and electrostatic devices are also discussed.IntroductionSeveral new methods for beneficiating different types of placer ores have been developed in the last two decades. Improved efficiencies in the recovery of fine particles have been achieved by improving gravity flowsheets and machines as well as by developing multiple treatment techniques involving flotation, magnetic and electrostatic separations. Placer gold processing, however, has been an exception to some extent. Gravity concentration using riffled sluices accompanied by mercury amalgamation is still the main method in use today. Because of this, the recoveries of fine gold and the other associated values are often similar to those achieved several decades ago. For example, very few sluice systems recover significant placer gold finer than 100µm in size.Only a few proposals for using alternatives to simple gravity methods for recovering fine placer gold can be found in the published literature. The fixed, low gold price discouraged research in this field for many years. Development of more efficient processes has not kept pace with the recent rapid escalation in the price of gold. Froth flotation for saving fine placer gold might now prove economic, for example. This paper reviews literature published on the recovery of fine placer gold over the period 1907 to 1980. Suggestions for improving the performance in this field, based on the authors' current testwork, are also presented."

Water has played a key role in the Pilbara’s 60-year history of iron ore operations. In contrast to many of the current-day mining environments, iron ore mining was initially hosted by geological formations that were typically above the water table. Such mining operations required no dewatering or dewatering at rates that were less than mine and mining township requirements. The challenge for these operations was not pit dewatering but water supply for mining operations and municipal supply for nascent mining towns.The current industry sees new deposit styles being increasingly targeted, including channel iron and Marra Mamba Formation deposits. These deposits are often lower in the landscape and can require extensive dewatering. Furthermore, increased water demand for ore beneficiation has resulted in major programs of water-supply borefield development. Added to this is the emergence of magnetite projects and their significant levels of water use in ore beneficiation.The large volumes of water that are being extracted to support Pilbara iron ore mining is attracting attention. The Western Australian Department of Water has recently commissioned a study to assess the potential use of surplus mine water as a medium- to long-term supply for irrigated agriculture.This paper outlines the changing landscape of water management in Pilbara iron ore operations and explores strategies for maximising mining productivity. For base metal sulphides, iron rejection starts in mineral processing. This review focuses on the changes in plant practice specifically to improve iron sulphide rejection by controlling contaminant ion effects and discusses selected innovations in mineral processing that can be expected to play a role in iron control. One change to control contaminant ions is size reduction (the use of stage grinding and inert stirred milling) and another is system chemistry (order of reagent addition and exploitation of the flexibility of sulphoxy species). Plant examples illustrate the successes, and indicate that there are several options available. The general innovations consider the new tools for measuring gas dispersion properties and quantifying froth characteristics from images. Examples of their application to improve concentrate quality illustrate the potential. Other innovations adapt geostatistics to model variations in ore flotation response to provide feed forward control and rigorously apply the statistical experimental methodology in plant test work. An example from one concentrator shows that significant improvements to reduce the iron content of concentrates can be achieved by applying these basic principles.

The Bureau of Mines drilled and sampled the more important magnesite deposits in the Currant Creek Mining District, White Pine County, Nev. The properties of the Ala-Mar Magnesium Co., Inc., and the Nevada Magnesite Co. were examined in August 1942 by D. W. Butner, engineer of the Bureau of Mines, to determine the advisability of investigating the deposits under the strategic minerals program. Development involving diamond drilling, trenching, and sampling was started September 1, 1942, and was completed in the latter part of November 1942. The deposits are on unsurveyed public land in T. 12N., B. 59 E., M.D.B.&M., in an area of high relief ranging from 7,300 to 8,100 feet in altitude. Both properties are situated northwest of the Tonopah-Ely Highway, U.S. Highway 6, from which a dirt road extends about 4.6 miles northwesterly, about 35 miles southwest of Ely, to the Ala-Mar property, and another dirt road extends about 10 miles westerly, about 26 miles southwest of Ely, to the Nevada Magnesite property. The magnesite occurs as irregular nodules, veins, and lenses in a bedded calcareous tuff formation, which overlies a group of volcanic rocks, and which is capped by remnants of rhyolite flows.

"The platinum group metals (PGMs) and associated minerals are particularly refractory to most traditional direct leach processes. However, the remoteness of many new ore deposits, socio-political risks, the scarcity of skilled labor and constraints on the availability of water and electrical power makes it increasingly more difficult to implement the conventional mill-float-smelt-refine process routes to extract and refine PGMs in remote areas. This paper discusses a process to extract PGM’s from low grade ore and concentrate using a sequential stage heap leach process entailing heap bioleaching and high temperature cyanide leaching, and evaluates the feasibility of implementing a carbon-in-pulp process to recover the PGMs from the pregnant leach solution. The heap bioleaching extracts the base metals in an acidic sulfate medium using a mixed culture of thermophiles. After heap bioleaching the heap is reclaimed, rinsed, agglomerated and restacked for high temperature cyanide leaching where the cyanide liquor is directly heated via solar energy in panels. Palladium, gold and platinum are recovered during the cyanide leaching stage. The pregnant leach solution was evaluated for potential carbon-in-pulp adsorption and subsequent elution similar to gold processing. The kinetics of adsorption and elution compared well with gold adsorption and elution."

The role of dedicated operational support in the ramp-up and stabilisation of the FeNi-II furnace for ferro-nickel production at PT Antam?s smelter located in Pomalaa, Indonesia is described. The furnace was uprated to 32MW capability in a rebuild performed in 2005. This primarily involved installing a larger transformer, coupled with adoption of HATCH water-cooled copper plate, waffle and flanker coolers with integrated metal and slag tapblocks, HATCH air-cooled copper fin lower sidewall cooling and HATCH vertical sidewall binding system ? ?holddowns?. Following a smooth commissioning period, the progress in attaining the preliminary ramp-up target load of 24MW is described. This initial target was set by Antam, subject to some prevailing smelter equipment constraints, particularly kiln capacity. Stabilisation of the furnace operation is discussed, together with some of the specific commissioning and operational challenges surmounted by the joint Antam-HATCH team. These included: ?Mastering furnace integrity monitoring and operation with copper coolers, especially metal tapblock planned maintenance and repair strategies; ?Transition from traditional ?immersed electrode? to ?brush-arc? mode of operation approaching as closely as possible to the productive ?shielded-arc? mode of operation within existing equipment constraints; ?Reductant selection and control issues; ?Utilising the sophisticated furnace monitoring to provide detailed indication and warning of potentially destructive furnace upset conditions such as Si reversion and CO boils.

THE distinguishing principle of the Tromp process is the use of a medium in the bath which is not stable, i.e., the solids will settle and the density of the medium increases with depth. A medium of uniform density can most easily be effected by agitation or by means which increase the viscosity of the medium. Either of these works against the sharpness of separation of coal from its impurities. The Tromp process grinds the medium solids to relative coarseness, allows them to settle downward in the bath without hindrance, and introduces horizontal currents of medium at intermediate levels between top and bottom of the bath. This prevents any accumulation of particles denser than the upper layer of the medium from which the clean coal is conveyed away or an accumulation of particles lighter than the bottom layer or medium whence the refuse is removed. In a relatively shallow bath making a two-product separation, the intermediate horizontal currents sweep these intermediate gravity materials away to be discharged with the refuse removed by the conveyor. Such a system provides a number of advantages. The gravity of separation can be changed simply by changing the density of the medium without changing particle size of medium solids or other conditions, as would be necessary with a system requiring uniform or stable density throughout the bath. Some installations in Europe separate at 1.40 sp gr on one shift, producing a superclean coal, and on the next shift reclean refuse from dry cleaning units and effect a separation at 1.80 sp gr. Medium solids may be much coarser in size consist, which reduces the viscosity of the medium and promotes its fluidity and in turn the sharpness of separation. Coarser medium solids with a more rapid settling rate are much more easily recoverable from the dilute medium rinsed from the products. Nowadays, one of the main problems in current flotation plants and new projects is the significant decrease in feed grades and changes in the mineral composition, which has an important effect on the mineral liberation and associations. Additionally, flotation plants usually operate with feed throughput higher than the original design, which has a significant impact on the feed particle size. All these conditions affect metallurgical performance, and must be evaluated and addressed, by improving control strategies for banks optimisation. For this purpose, the flotation of valuable minerals along the circuits requires better understanding, considering the compromise between performance and selectivity in flotation circuits. This paper presents a characterisation of the different minerals recovered along industrial flotation circuits. Plant sampling campaigns, performed in a copper industrial concentrator in Chile, provided the data for the study. Two rougher flotation banks consisting of cells of 130 m3 and 300 m3 were analysed. All the samples from the industrial surveys were analysed by screening and mineralogy, and the mineral recovery along the circuits was evaluated per particle size class, liberation class, type of associations, and others. Additionally, operating data on froth depth profiles complemented the evaluation of the mineral composition of concentrate streams, along the circuit. The results allowed for identifying and quantifying the main factors that affect the copper recovery and concentrate grade along the flotation banks. It was found that minerals with liberation lower than 50 per cent have a significant impact on the recovery. On the other hand, the decrease in the Cu grade in concentrates along the flotation circuits is mainly due to the relative increase of middling minerals (less liberated), non-valuable floatable minerals, such as free pyrite, and gangue entrainment. The characterisation of the gangue entrainment flow rate along the flotation banks showed a strong relationship with froth depth, which becomes more critical towards the last cells of the banks.

This paper describing the mining practice at the Utah Copper Co., Bingham Canyon, Utah, is one of a series being prepared by the Bureau of Mines on mining practices, methods, and costs in the various mining districts of the United States. The Utah Copper Co. operates the low-grade open-out copper mine at Bingham Canyon, Utah, which is 30 miles in a southwesterly direction from Salt Lake City. In addition to the mine the company operates two flotation concentrators, the Magma plant and the Arthur plant, which have a combined capacity at maximum metallurgical efficiency of 60,000 tons daily. The concentrators are at Magna, about 18 miles from the mine. The company operates its own standard-gage railway, known as the Bingham and Garfield Railway, between the mine and the mills (fig. 1). Concentrates are smelted at the Garfield smelter of the American Smelting and Refining Co., situated 4 miles from the mills.
