Cross passageways between adjacent tunnels in passenger rail or transit systems may be used during train fire emergencies to evacuate passengers to points of safety in the non-incident tunnel. Rescue trains can be brought into the non-incident tunnel to remove passengers to expedite evacuation times as compared to walking along bench walls to the nearest egress stairs. In cases of multiple train emergencies, the required ventilation capacities can result in high pressure differences between the incident and non-incident tunnels. These pressure differences can make it difficult or impossible for a normal person to open doors of a cross passageway between adjacent tunnels. If opened, the velocity of the airflow through an open cross passageway can reach levels high enough to impede passenger evacuation. This paper describes the analysis of this problem for a sample rail tunnel. A ventilation concept that mitigates the problems of high cross passageway door opening forces and high velocities through open cross passageways is presented.

Getting the best match between truck and haul road requires examination of many factors and their effect on the bottom line. Most mines use a bottom line measure based in some way on cost per ton of material moved. This measure focuses on the two components of truck haulage cost per ton: productivity (or performance) and operating cost. Productivity can be measured or predicted for specific tasks using either field measurement or modelling. Individual tasks can be combined to provide a larger scale, longer term net figure for total mine output. True cost is more difficult to capture as they are time and task oriented and can be influenced by a summation of factors over a truck lifetime. For example, "allocated" costs, such as owning (depreciation) and some repair costs, are spread over long periods of time. Other cost, such as fuel and tires can be tied more directly to certain tasks such as fuel and tire cost. This paper focuses on the more direct, task-oriented factors that affect truck productivity and cost.

In the world mining scene, precious metals (particularly gold) have recently monopolized the attention in exploration as well as new production activities. As the major world producer of gold, South Africa plays and will continue to play a very significant role in this commodity. This role will, of course, have its consequential effects on the world's gold supply and, hence, also on the international price of gold. It is essential, therefore, and in the interests of all gold producers that the likely future pattern of gold production in South Africa be estimated such that some measure of production uncertainties can be predicted. For this purpose, some elementary geostatistical models will be used, and the main factors affecting the situation will be analyzed in broad outline only under the following headings: South Africa's basic gold ore potential in the three categories of: (1) ore remaining in existing mines; (2) extensions to existing mines via unexplored areas and/or reef horizons of lower grade; and (3) new mines, potentially deep or ultradeep. Cutoff grades - operating, marginal, and flotation, for the foregoing three ore categories, respectively. The availability of production, financial, and other resources required. Cutoff grades will, of course, depend on cost levels (operating and capital), future inflation and cost escalation trends in South Africa (SA), and/or gold price trends (in US$ and via the exchange rate in SA Rand terms). The differentials in inflation and escalation trends with those in overseas countries, mainly the USA, will, in general, set the scene for future exchange rate levels of the Rand. To predict the effect of various cutoff grades on future gold production from South Africa, it is essential to use geostatistical models. The applicability of such models is tested against past records of price, cost, and grade trends and then applied to the estimated ore potentials. These models were referred to briefly in a paper at a recent seminar on gold (Krige, 1986) but are discussed in detail herein. This lecture was intended originally to cover only technical aspects, but, in view of South Africa's position internationally and the possible effect of sanctions, internal unrest, and strikes on gold production, a brief reference to the third factor above (availability of production, financial, and other resources) seems appropriate.

Many cases of derailments, train accidents and traffic interruptions due to the slope failure and rockfall have been reported in the past on Konkan Railway trackline because of unstable rock slopes. The authorities installed stability enhancement measures such as wire netting, rock bolting and shotcreting which were found insufficient. The failed geotechnical measures prompted authorities to excavate the unstable slopes. Rock blasting is generally not preferred for the excavation of slopes along the transportation route. Stabilisation and widening of slopes by blasting need closure of the route for a considerable time. Discontinuing traffic on Konkan Railway route was undesirable as it connects two important port cities Mumbai and Mangalore. The paper presents an innovative directional controlled blasting (DCB) technique for excavating three unstable slopes (i.e. cuttings) in hilly terrain without disturbing the traffic. New empirical formulae for small geometry blasting to estimate burden and throw have been developed. A unique sequence of firing and excavation with powder factor varying between 0.04 and 0.39 kg/m3 irrespective of the actual free face was used for rock blasting. The process controlled the throw of blasted material over the track and resulted in a smooth and stable wall. The slope angle has been changed from 80 to 82° to 45–47° and probability of wedge, planar and toppling failures was eliminated. About 100 such accident-prone slopes (52 km cumulative length, 7% of total route) situated at discrete locations along Konkan Railway route were stabilised using the technique. No incident of rockfall and slope failures along these slopes has been observed after the stabilisation.

Laboratory and small-scale continuous pilot-plant investigations made at the Tucson Metallurgy Research Laboratory of the Bureau of Mines have demonstrated that the copper segregation process is applicable to the treatment of oxidized and mixed oxide-sulfide copper ores regardless of the gangue constituents. The process comprises heating the ore to a temperature of 750° C. with small quantities- of sodim chloride and coke to produce metallic copper, which then is recovered by flotation. Tests were made on' 23 copper ores of different types containing 0.8 to 5 percent Cu, .The copper recoveries ranged from 73 to 96 percent in concentrates assaying 10 to 62 percent Cu, depending on the grade and type of ore, The direct operating cost for a 1,000-ton-per-day furnace and flotation plant is estimated at about- $3 per ton of ore.

The Leeville underground mine has been in operation since 2006 and currently produces 3,175 t (3,500 st) of ore per day with an average gold grade of 12.6 g/t (0.4 opst). The mine has a large horizontal extent of 762 by 152 m (2,500 by 500 ft) and a vertical extent of 6 to 61 m (20 to 200 ft). Based on the characteristics of the ore bodies, the deposits are divided into several mining zones. The geotechnical properties of mining zones are also different from one zone to another. A successful geotechnical collection system is very important for mining design and operation in the Leeville underground mine. Three methods are used to collect the geotechnical data in the Leeville underground mine: • Core logging. Most geotechnical data are collected by this method, which is based on exploratory drills and estimates parameters for rock quality designation (RQD), fracture frequency (FF), compressive strength and rock mass rate (RMR). • Site investigation. This method collects more detailed geotechnical data and geostructural data from work done in the underground drifts and stopes. Parameters of RQD, FF, compressive strength, RMR, rock tunneling quality index (Q) and geostructural data are estimated. • Data collected from drill holes made for the critical infrastructure facilities. This method provides the best detailed geotechnical information for areas in which the critical infrastructure located. A complete geotechnical database has been constructed using this geotechnical data collection system. The database can be used for the design of ground support, stopes and stope blasts, among other critical applications.

The longterm aim of this research project is to find fast ways of simulating the recoverable reserves of a deposit for input into financial Monte Carlo simulations without having to conditionally simulate the whole orebody many times. The first step in this re- search is to visualize and quantify the ore tonnage T and the metal quantity Q above an arbitrary cutoff z. This paper presents detailed results for one specific distribution, the lognormal. These lead to a suggestion of a fast way of simulating QTz curves.

In this paper I use the term "industrial minerals" in its broad general sense, and I distinguish two main groups. The first consists of the bulk materials, characterized by low unit price, nearness of deposits to markets, relatively simple methods of extraction and processing, and a tendency to be closely related to the construction industries. The second group includes those materials valued for exceptional purity, or for some special property such as hardness, immunity to acids, or dielectric strength. These materials are produced in smaller amounts, at high unit values, from deposits notably restricted in. distribution; and they require more or less sophisticated methods of production and beneficiation. The applications of this second group are not closely tied to any particular part of the economy. From the way in which the commodity committees of the Industrial Minerals Division are set up, I take it that the chief interest of most members is with the first group of materials mentioned above. Hence this paper will deal mainly, though not exclusively, with such bulk commodities as sand and gravel, clay, and stone. The first word in the title of the paper also requires brief explanation. It is here given a threefold interpretation. First is the role of the industrial minerals themselves in industry; this would probably be taken as the conventional meaning of the title* The second is the role of the industrial-mineral producers as citizens of the community. The third is what may be called the scientific role of these producers.

Fatigue-related risk is a persistent safety concern for the mining industry. However, fatigue and sleepiness are often treated interchangeably, which can lead to confusion and potentially less effective reduction of safety risk. To provide clarity, we present an overview of similarities and differences between work-related fatigue risk and sleepiness including definitions, theories, measurements, and mitigation strategies. As a supplement, a summative visual model which highlights these similarities and differences is presented. Expanding industry knowledge in this area will assist safety professionals in crafting more targeted risk management practices appropriate for work-related fatigue risk, sleepiness, or both.

Integrating a research and development department into an engineering company's corporate structure can be rewarding to the engineering division, the R&D group, and, most of all, the company's clients. Actually, the R&D department is a logical extension of the conventional processing group within an engineering organization-but the R&D group provides services as needed to the engineering department while it is carrying out regular testing, pilot plant, and preliminary feasibility studies. Backup Services Provided With an R&D group, the engineering division does not need a process engineering group as such. The R&D group provides project backup services for feasibility studies, preliminary and detailed engineering, construction, and startup. The R&D group, in turn, benefits from its close association with project engineers, draftsmen, estimators, and computer capabilities in the engineering division. The client also benefits from the arrangement because R&D and engineering skills are available to both departments on an as-needed basis without standby eost. To handle the integrated responsibility of R&D projects, as well as conceptual design and preliminary feasibility studies, the R&D staff must be carefully selected. These research engineers should: • have a good background in plant practice as a mill superintendent or chief metallurgist; • be capable of organizing and supervising a testing program or a pilot plant campaign; •be innovative and willing to accept new ideas and concepts; • be able to translate lab test results into a workable flowsheet through sound conceptual design and feasibility studies; and •be able to communicate with clients. In short, the R&D engineer must be a jack of all process-engineering trades to successfully take a project from concept to operating reality. Introduction The domestic copper industry has suffered for some time from declining ore grades; rising costs associated with conventional mining, milling, smelting and refining; tightening environmental restrictions; and shrinking markets due to materials substitution. Ore grade depletion is a major problem. It is illustrated by the decline in average copper yield shown in Fig. 1. From 1979, copper grade has dropped 60%, lowering yields from 1.2% to 0.49% (Sousa, 1981). The US copper industry is taking drastic measures to enhance its competitive posture and to guarantee its future worldwide position. In a sense, this involves a general restructuring of the processing philosophy of US copper producers. Judging from recent trends, this restructuring relates to the application of solution mining techniques to the treatment of copper ores and low grade waste material. Recovering copper by leaching techniques has been practiced for centuries. As early as the mid-16th century, some Hungarian copper mines were recycling copper bearing leach solutions through waste heaps (Nash, 1912). In the US, recovering copper from dilute mine waters has been carried out for more than 100 years. There are, however, certain technical innovations that have elevated the importance of solution mining as a process for copper recovery. These include: • Advances in solvent extract-electrowinning (SX-EW); • Developments in acid and acid ferric cure processes for oxide and mixed oxide sulfide ores; • Improvements in heap and dump construction; and • Advances in in situ leaching technology. Generally, solution mining enjoys certain intrinsic advantages over conventional mining and milling. Compared to conventional milling, the combined capital and operating costs of leaching facilities are normally lower, start-up times are faster, and the leaching operation usually has less impact on the environment. Furthermore, solution mining represents an expedient way of extracting metals from small shallow deposits, and is particularly suited to the treatment of low grade sources. And, in the case of waste dump leaching, there is a tremendous resource in-place that has the mining cost already off the books. This paper updates current US copper solution mining trends. Solution mining systems Schlitt (1980) summarized the characteristics of the principle leach systems in the US. He pointed out that the wide variations in copper deposits make solution mining a more site-specific activity than conventional mining and milling. Heap leaching is usually selected for moderate to high grade ores having a copper mineralization that is amenable to acid leaching (oxides, silicates and certain sulfides). Higher grade ores mined specifically for heap leaching are usually treated to optimize copper extraction, therefore, crushing and some pretreatment (acid curing) may be justified. On the other hand, dump leaching practice usually involves leaching low grade waste rock placed on large dumps as dictated by topography and haulage costs. Mine waste is therefore treated in an uncrushed run-of-mine condition. Finally, in situ leaching is appropriate for either very deep ore bodies or for recovering copper from low grade rock left behind in abandoned pit walls, stopes, and subsidence zones from earlier mining activities. True in-situ solution mining should be defined as the leaching of ore in its original geologic setting. Heap leaching Heap leaching has a long history as a hydrometallurgical process for copper recovery. The methodology of heap leaching established at Rio Tinto in Spain more than 300 years ago is basically the same as that used today. Magnetic surveying and map production is well established in land based mineral exploration. Magnetic data are routinely used to map geology in covered terrains, to estimate the depth to basement in overburden covered areas, and to identify altered zones, mineralization, bedding attitudes, and fault networks both regionally in map view, and through interpretation to depth in 3D. Best practices for magnetic survey methods and magnetic interpretation exist in all areas of land based geophysical survey methodology - mineral exploration, hydrocarbon exploration, geological mapping, archeological mapping, and in military applications. At sea, and particularly under the sea, sophisticated magnetic surveys are being done, but it can be argued that these are primarily in niche applications, for example in naval mine-hunting and UXO applications. Magnetic data is not normally collected on AUVs and ROVs conducting mapping and hydrographic survey operations. OFG has developed physical and mathematical calibration routines so that magnetometers can be mounted directly onto AUV and ROV bodies where significant heading errors and magnetic gradients exist. This simplifies the routine deployment of magnetometers on AUVs and ROVs for mineral exploration and other magnetic survey applications without the need for towed bodies, ungainly mounting frames, or degaussing of the survey equipment. Our experience shows that magnetometers can be operated routinely on AUVs and ROVs and that with careful processing useful maps can be produced routinely at low cost.

Almost 90 % of phosphate in Indonesia is consumed for natural and manufactured fertilizers, while the rest is used in chemical industries. The consumption of phosphate always increases each year, while Indonesian phosphate production is almost constant at around 75,000 tons annually, except the production in 1988 was 165,000 tons. Due to the limited amount of domestic production, the imported phosphate is increasing each year. Phosphate trading in Indonesia is unique. Some of the domestic production is exported to Taiwan, while phosphate from USA, Morocco, Jordan and Nauru is imported. Indonesian phosphate includes guano phosphate, caving phosphate and small amount of marine phosphate sediment. Problems with Indonesian phosphate include: scattered deposits in difficult locations and a relatively small amount of reserves. Exploitation of phosphate deposits for domestic purpose has a good prospect, mainly because of the rapidly increasing phosphate demand as natural fertilizer or as a raw material for manufacturing fertilizer to support an agricultural sector.

According to Arbiter (Reference 1), the ball mill was invented by Brückner in Germany in 1876 and was first used in the United States on ores about 1905. The basic concept was a simple rotating cylindrical drum in which the tumbling action of the charge of material and grinding media resulted in size reduction of the material. Despite the very low mechanical efficiency of this method a number of factors have caused the tumbling mill to remain, by far, the most widely used piece of equipment for mineral grinding. The principal factors influencing this situation have been simplicity of design, ease of manufacture, ready adaptability to familiar material handling layouts and the absence of practical alternatives acceptable to the industry. In the early mills, the material was ground in batches, the mills being charged and discharged through doors in the shell. The shell was supported by means of solid trunnion shafts resting in plain bearings and the drive was accomplished through one of these shafts. In some later batch mills, the mill shell was supported on rollers which were also used to drive the mill. At an early stage, continuous operation was made possible by feeding and discharging the mill through hollow trunnions, the method still in most widespread use. The mechanical and structural design of the early mills was not an onerous engineering task. Norse sense and good practical knowledge were key qualifications. The mill shells had to be heavy enough to withstand the physical abuse they would receive in the field and the bearings had to have low enough unit pressures and sufficient wear retol to ensure several months of operation before the need for replacement occurred. When these requirements were satisfied, stresses developed in the equipment during operation were insignificant and the majority of structural failures were probably due to the use of inferior materials in manufacture. Incident investigation is of utmost importance in most high-risk industries. This study was undertaken to study the content of past investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of 5 elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of 3 Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed. INTRODUCTION The importance of learning from incidents is well documented in the literature. For instance, some scholars regard learning from incidents as important for improving safety [1,2]. Thus, many organisations spend considerable resources in identifying lessons from incidents and subsequently on the process of improvement that follows the identification of lessons. It is therefore important for such organisations to determine the effectiveness of their learning from incidents processes, in order to identify specific areas where improvements are needed, so that interventions can be as specific as possible. The learning from incidents process models the plan-act-check-do approach of management systems, often consisting of four major stages, namely; data collection and incident analysis, planning interventions, implementing interventions and follow-ups and evaluation [3,2]. The investigating and analysis of incidents have been regarded as one of the vital stages of the learning process since it is the stage where lessons are identified [3,2,4]. It has also been noted elsewhere in the literature that results of the previous stage of the learning process are important for the next stage [3]. For example, ineffective incident analyses could result in the planning of ineffective remedial measures. One of the primary aims of incident investigations is to identify lessons from previous incidents which when implemented can prevent future recurrences or minimise the effects of future incidents. There is thus the need for organisations to assess the quality of their investigations periodically since it is one of the means they adopt to improve safety and the quality of remedial measures to some extent depends on the effectiveness of the investigations. Although previous studies [5,2] have assessed the effectiveness of the learning process, and some aspects of the learning process, such as incidents reporting, there is less focus on the effectiveness of incident investigations. The research described in this paper was therefore conducted to contribute to this gap, focusing more closely on incident investigation reports.

Structural and topographic relief in the Sangre de Cristo Mountains of northern New Mexico provides a remarkable cross section through the recently recognized 23-26 m.y.-old Questa caldera and cogenetic volcanic and plutonic rocks. Although largely eroded, remnants of an ash-flow sheet of silicic-alkalic rhyolite and associated more mafic lavas of the Latir volcanic field are preserved as far as 45 km beyond the source caldera. Within the caldera, the tuff ponded to a thickness of 2-3 km and enclosed chaotic megabreccia fragments which slumped from the caldera walls. At the time of ash-flow eruption, the volcanic field was severely deformed along northwest-trending faults related to the Miocene Rio Grande rift, with most intense deformation occurring within the caldera. Strata were steeply tilted and locally overturned along presumably listric faults. Ash- flow tuffs locally underwent secondary flowage over concurrently developing fault scarps and accumulated within structural basins as rheoignimbritic lava flows. Cogenetic batholithic granitic rocks, exposed over an area of 20 x 35 km, range from mesozonal quartz monzonite to epizonal porphyritic granite and aplite, with the shallower and more silicic phases most abundant within the caldera. Compositionally and texturally distinct granitic phases define a highly evolved resurgent intrusion within the caldera, an incomplete ring dike along its southern margin, and a large mass of less fractionated quartz monzonite south of the caldera. A negative Bouguer gravity anomaly is closely confined to the area of ex- posed granitic rocks; it also reflects boundaries of the Questa caldera. The gravity anomaly is interpreted as defining the extent of the underlying batholith, emplaced into lower parts of the volcanic sequence and underlying Precambrian rocks. Paleomagnetic determinations indicate that the granitic rocks were above Curie temperatures at the time of caldera formation and regional listric faulting, yet most are little different in radiometric age from the intensely deformed volcanic rocks. These relations indicate that the batholithic complex represents the source magma body for the volcanic rocks, into which the Questa caldera collapsed, and was largely liquid at the time of regional tectonic disruption. Compositions of the volcanic and plutonic phases changed from early calc-alkaline metaluminous rocks, to weakly peralkaline silicic rhyolite and equivalent afredsonite-acmite granite at the time of the caldera formation, then back to post- caldera calc-riebeckite granitic rocks. Maximum concentrations of alkalis and minor elements such as Rb, Th, U, Nb, Zr, and Y were reached at the time the caldera formed about 25-26 m.y. ago, but the major molybdenum mineralization of the Questa district took place when the late calc-alkaline granitic ring intrusions were emplaced about 23 m.y. ago, along the south margin of the caldera.

"Coal is mainly composed of energy-producing elements carbon and hydrogen. In addition to these elements, coal usually contains many other valuable elements, including iron, aluminum, silicon, rare earth elements, lithium, gallium, germanium, vanadium, uranium, titanium, scandium and phosphorus. These coalassociated elements are significantly enriched in coal combustion byproduct fly ash because the majority of the main components, carbon and hydrogen elements, being removed. Table 1 provides the chemical composition range of the main element oxides in 12 Chinese fly ash samples collected from coal power plants located in Northeast China, Northwest China, Southwest China and Southeast China. It can be seen from Table 1 that the major mineral components of different ashes are similar, but the bulk chemistry of different fly ashes may vary due to the changes of ratios of the mineral components in different coals. The major mineral components in various Chinese coal combustion fly ash are silicates, iron oxides and unburned carbon.Chinese coal fly ash usually contains about 5 to 15 percent of unburned carbon, which are generally chars with irregular shapes and wide particle size distribution. In addition to unburned carbon, XRD analysis of four Chinese fly ash samples from Liaoning, Shanxi, Guizhou and Shandong provinces indicates that the fly ash samples are composed of minerals such as quartz (SiO2), mullite (3Al2O3.2SiO2 or 2Al2O3.SiO2), hematite (Fe2O3), maghemite (gamma form of hematite, ?-Fe2O3) and magnetite (Fe3O4). The mullite and quartz particles are usually present as spherical particles because they are formed by melting clays, feldspars, quartz, calcite and other common minerals in coal during coal combustion. The silicate particles have usually entrapped gas to yield hollow spherical particles because their lower melting point facilitates gas entrapment.The iron oxides are usually spherical also. The magnetite (Fe3O4) is derived from pyrite (FeS2), hematite (Fe2O3), siderite (FeCO3) and limonite (2Fe2O3.3H2O) in the coal. Maghemite (?-Fe2O3) is formed by dehydration of the mineral hydrohematite (?-FeO(OH)) and/or oxidation of magnetite (Fe3O4) during coal combustion. Maghemite (?-Fe2O3) and magnetite (Fe3O4) are strongly magnetic, so they are recovered from Chinese coal fly ash and widely used for coal processing in China (Fan et al., 2005, 2000)."

INTRODUCTION South Coast Water District (SCWD) operates the Beach Interceptor Sewer Tunnel(BIST) in South Laguna, Orange County, California between Monarch Beach and Aliso Beach (Figure 1). The 10,200-foot long BIST houses a 24-inch gravity Techite sewer pipeline critical to the SCWD Sanitary Sewer Collection System. The original tunnel and sewer pipeline were constructed in 1954 and the sewer line was replaced in 1974.Periodic inspections by sewer pipeline maintenance crews documented deterioration of specific intervals of the tunnel. Rotting timber supports and rock falls in the tunnel as a result of the deterioration were endangering the sewer pipeline and a 731-foot section of tunnel located at Thousand Steps Beach was determined to be in immediate need of repairs. SCWD and consultants Hatch Mott MacDonald (HMM) and Tetra Tec prepared a Work Plan for emergency tunnel stabilization and pipe protection measures that was implemented in January 2007. Normal access to the beach and the section of tunnel requiring repairs was limited to public stairs referred to as Thousand Steps and, therefore, all materials and equipment needed for the work were transported to the beach by ocean-going landing craft. Additionally, the emergency repairs had to be completed without taking the pipeline out of service. Numerous environmental, logistical and operational requirements had to be overcome to complete the challenging emergency repairs. GEOLOGICAL SETTING The Laguna Beach area is located in the Peninsular Ranges of Southern California. The project area is located on the southwest flank of Niguel Hill along marine wave-cut cliffs and slopes. The tunnel was excavated in the San Onofre Formation, a marine deposit, characterized by abrupt lateral and vertical changes in lithologies that vary from boulder to cobble and gravel size breccias to medium coarse sandstones with gravel, sandy claystone and siltstone. The interval of the tunnel that required repairs is located within the San Onofre breccia, a moderately-to-well cemented, poorly bedded to massive unit with angular to subangular boulder and cobble sized clasts of schist. The sandy silt matrix of the breccia is composed of quartz, feldspar and schist grains (Figure 2). GENERAL TUNNEL DESCRIPTION The approximately 6-feet high by 6-feet wide 10,200-foot-long BIST was hand mined in 1954 at the base of the coastal cliffs. Primary access is through twenty-one access adits from public and private beaches. The tunnel is located in an easement that extends beneath private property with residential structures situated 50 to 80 feet above and horizontally in close proximity to the tunnel alignment. Fatigue in mining operations is a serious issue and a significant contributor to incidents and accidents. While mine operators are using or introducing new technology to monitor operator fatigue, there is little work that focuses on the subjective experience of fatigue and the use of fatigue monitoring technology. To investigate this issue, eight focus groups with a total of 44 operators of heavy haulage equipment working for two mining companies in the Mountain West of the USA participated in 1-h focus groups. The topic of the focus groups was the experience of operator fatigue and issues surrounding fatigue at the workplace. The results of the content analyses suggest that operators are well aware of the signs of fatigue, and that they understand the severity of these signs as well. In addition, they are aware of the effect of fatigue on driving performance and are using a wide range of methods to manage fatigue. Finally, based on participants’ assessment, current fatigue monitoring systems lack face validity and reliability and are perceived as not being effective. The results of this study emphasize the importance of a socio-technical systems perspective in the context of miner health and safety management. Only such a comprehensive perspective can result in improvements that are sustainable.
