Within the metal/nonmetal mining sector, fall-related incidents account for a large proportion of fatal and nonfatal injuries that occur in mining in the United States. However, the events and contributing factors leading up to these incidents have not been fully investigated. To help provide a clearer picture of these factors, an analysis of imminent danger orders issued by the U.S. Mine Safety and Health Administration (MSHA) from 2010 through 2017 at both surface and underground metal/nonmetal mine sites revealed that most orders are associated with fall risks. Of these cases, 84 percent involved the workers not using fall protection, fall protection not being provided or the improper use of fall protection. Fall risks for workers most frequently occurred when standing on mobile equipment, performing maintenance and repairs on plant equipment or working near highwalls. In most cases, a single, basic, corrective action — such as using fall protection — would have allowed workers to perform the task safely. Overall, these findings suggest that a systematic approach is needed to identify, eliminate and prevent imminent danger situations. Furthermore, to protect mineworkers from falls from height, frequently performed tasks requiring fall protection should be redesigned to eliminate the reliance on personal fall protection.

This paper describes the installation and operation of a Texas Nuclear on-line silica analyzer (NOLA) coupled with a Leeds and Northrup Microtrac particle-size monitor (Microtrac) at U.S. Steel's Minntac taconite concentrator. An automatic sampling system provides the NOLA with samples of final concentrate. After the sample is analyzed for silica by the NOLA, it is pumped to the Microtrac for size and surface area measurement. A sample is analyzed every six minutes, providing the control room with the silica concentration, the percent passing 270, 400, and 500 mesh, and an equivalent Blaine surface area number. When fed a slurry of final concentrate containing between 20 and 70% solids and 5 to 6% silica, the NOLA analysis is within ±0.2% of the wet chemistry silica. The agreement between the Microtrac output and standard screen analyses is ±1.2% at 270 mesh; ±1.5% at 400 mesh, and ±2% at 500 mesh. The poorer disagreement at the finer sizes is mainly due to inaccuracies in screening at 400 and 500 mesh. The agreement between Microtrac surface area a d laboratory Blaine surface area is about 9 ±80 cm 2/g. These instruments provide rapid and reliable feedback information to the concentrator and feed forward information to the agglomerator.

Ongoing Outokumpu development in Solvent Extraction and Electrowinning Technology, involving all aspects of process chemistry, equipment and control, extends over a period of close to 30 years. The development in SX started with molybdenum and rhenium, continued with zinc, manganese, cobalt and nickel and is now focusing on copper. The OutoCompact VSF? SX concept, which has been recently introduced, is a honed version of earlier VSF? technology. The Outokumpu VSF? ("Vertical Smooth Flow") SX package includes the DOP ("Dispersion Overflow Pump") units, the Spirok Mixers, the OutoCompact Settlers including their DDG ("Dispersion Depletor Gate") fences and tank farm equipment such as the LOS ("Loaded Organic Scrubbing") tank as well as the CEP ("Copper Electrolyte Purifying") tank. Although the VSF? SX package can be implemented according to several optional lay-outs, the so-called Track version has lately been introduced to the advantage of new plants. The new OutoCompact SX technology decreases the footprint of future VSF? plants. The lay-out, including plant size flexibility, have further been improved by Outokumpu?s new OutoReverse settler technology. Two of several SX plants currently under construction rely on reverse settlers in order to maximize their performance. The Outokumpu EW technology includes process know-how as well as proprietary equipment. The copper EW package consists of permanent stainless steel cathodes based on wax-less technology and lead anodes utilising polymer concrete cells with the double contact bus bar system and ventilation hoods fitted for efficient mist control. The lifting of cell hoods, as well as cathode washing, is done by the cathode pulling crane. Proper electrolyte flows and matching current density further ensure high productivity of LME grade copper cathodes.

Recent increase in steel demand has led to the increased usage of high grade iron ores. During the mining, crushing and washing of ore, huge quantities of fines and slimes (<150 µm) are generated, majority of which is dumped into tailing ponds owing to their high alumina contents (~10-15%). Due to rapid depletion of high grade iron ore, it is the need of the hour to utilize low grade iron ore fines and slimes. These fines and slimes contain high iron values assaying 45-60% Fe. Dumping these valuable resources in ponds locks enormous land space and causes environmental hazards. The constituent minerals present in Indian iron ores comprise of hematite, goethite, gibbsite, kaolinite and quartz. Our research is focused on beneficiation of iron ore slimes through selective flocculation. We use density functional theory simulations to screen the reagents and perform experiments with screened reagents to determine the experimental conditions for effective performance of the reagents. Our studies revealed that reagents like starch, guar gum and carboxymethyl cellulose exhibit excellent selectivity in high alkaline pH range. However, the higher cost of alkali makes the process economically less attractive. In the present study, a commercial reagent from NALCO Water (Hi-Tex 83570) has been explored as selective flocculant for beneficiation of iron ore slimes in wide pH range (4-11). However, the reagent was also found to be the most selective in alkaline pH range only. Full factorial design of experiments was done considering three parameters namely, pulp density, flocculant dosage and settling time. Regression models were developed for yield, iron grade, alumina grade and iron recovery in the concentrate, using experimental results. Further parameter optimization was done to achieve high Fe recoveries and low alumina recoveries. The optimal parameters thus arrived at were validated with the experiments. We were able to achieve a concentrate grade of 60.0% Fe and 5.1% Al2O3 with 68.8% Fe recovery from a feed containing of 53.1% Fe and 8.9% Al2O3. The performance of flocculant Hi-Tex 83570 was also compared with reagents starch and guar gum.

The progressive development of underground mines frequently requires increased ventilation flow rates through mine shafts that were originally designed for lower rates. Where these shafts are equipped with conveyances running on rope guides, the higher air flow rates adversely affect the lateral motion of conveyances and could cause conveyances to collide with the shaft walls or with one another. Hence the safety impact of higher flow rates requires proper assessment. This can be undertaken by use of computer simulation models to predict the dynamic motion of the conveyances during hoisting. It can also be undertaken entirely by field testing, or using a combination of simulation and field testing. This paper discusses these techniques and makes cautionary remarks about setting an absolute generic maximum flow velocity for all shafts. Cupreous pyrite deposits in Cyprus have many geologic similarities to Canadian massive sulphide copper-zinc ores as well as certain significant differences. A comparison of the Cyprus and Canadian deposits indicates a complex genetic history for the latter and suggests that their origin should be re-examined. The Cyprus ores occur in subaqueous extrusive rocks near and at the top of a Cretaceous volcanic pile or igneous complex. The rocks have not been deformed, nor have they been deeply buried or deeply eroded. Apparently the ores formed near or at surface in the top of the volcanic pile by a volcanic-exhalative process and from an ore-fluid of volcanic derivation. The Canadian deposits are of comparable composition and have similar geologic set-tings, but occur in the most ancient Precambrian rocks. Their enclosing Keewatin greenstones have, in most places, been deformed, regionally metamorphosed and invaded by granitic intrusions. Comparison suggests that the Canadian ores also formed initially near surface in ancient Keewatin volcanic complexes by a syngenetic, volcanic-exhalative process. A greenfield open pit gold mine in Latin America suspended operations less than three years into an eight-year mine plan after a waste rock dump and heap leach facility sustained substantial damage from a deep-seated landslide. No injuries to employees were recorded because of the slow-moving failure. Due to early detection by the operations team, the owner had sufficient notice of the potential failure and successfully rinsed the heap leach facility prior to the failure event. Engineers estimated approximately 35 million cubic meters of mass movement below the waste rock and heap leach facilities. The original eight-year mine plan produced approximately 60,000 ounces of gold per year. After the failure, the owner terminated gold production, re-configured the mine site to a safe, standby condition and launched a detailed investigation into the root-cause of the failure. Approximately two years after the shutdown, legal interests filed a statement of claim and named three engineering design firms and individuals involved in the project. Several years of litigation, including discovery and deposition proceedings, followed as the legal process considered evidence spanning a 20-year timeline. Changes in project ownership and consulting firms during the development and operations phase complicated the process. In 2017, the plaintiff and last defendant settled. The author was involved as the corporate representative for one of the engineering design firms.

Oaky North is AustraliaÆs highest producing underground coal mine. In 2011, the mine produced 8.7 Mt of high-quality coking coal. The high quantities are a result of the constant strive for optimising the operating time of major production units, which involves investing numerous resources, including diesel equipment, in supporting and maintaining their operation.In July 2011 it was identified that the diesel particulate matter (DPM) levels in the underground workings were above acceptable levels, due to the high level of diesel activity. Over the next 12 months, several controls were implemented to reduce exposure to personnel: the main ventilation system was upgraded; the production forecast was adjusted to better manage the diesel fleet, removing surges in diesel usage; and an electronic diesel tag board was introduced to centrally manage the distribution of diesel usage around the mine.From the start of the campaign in emission reduction, an intense DPM monitoring regime was established. Within a few months of the introduction of the centralised tag board, there was a major reduction in emission levels. After the installation and commissioning of the new main fans, the emission levels dropped further, such that over-exposures are now a rarity.Oaky North continues to explore new systems to better utilise their diesel fleet while still maintaining control over DPM emissions.CITATION:Melloy, A, Holt, D and Jurak, M, 2013. This paper uses the results of research trials by Landcare Research and Forest Research Institute and assessments of reclamation plantings at mines and other drastically disturbed sites to describe methods of enhancing restoration of native forests. The characteristics of mine and forest environments are compared to highlight the difficulties facing restoration and techniques which result in successful restoration are described. Selection of the most appropriate technique is illustrated by describing restoration of lowland beech-podocarp forest at a West Coast coal mine. The key to successful reclamation is to understand the factors controlling and limiting plant establishment and growth, then to design rooting media, surfaces and management regimes to create a favourable environment. This paper concludes with a summary of the planning and technical factors which are critical for successful restoration of native forest.

Surface wave imaging is a relatively new seismic technique that generates subsurface depth images without having to drill boreholes into the ground, and is rapidly becoming a standard tool in non-destructive geotechnical characterization. This paper presents a case study of the application of these surface wave techniques to image shallow abandoned mine workings at the Montague Gold Mines district near Dartmouth, Nova Scotia. This district has numerous steeply dipping narrow vein workings from historical mining activity, and has actively subsiding and collapsing crown pillars. Due to the rugged conditions at the site and the relative narrowness of the openings (often less than 1 m), a surface seismic system was developed that had improved portability and lateral resolution over other available seismic imaging systems. Seismic surveys were conducted over two sites where independent evidence indicated the presence of shallow mine workings. At one site, a comparative gravity survey was also conducted. The seismic data was processed to generate velocity vs depth images. Overall, the velocity images show vertical zones of zero velocity in the same lateral positions where historical mapping, current subsidence and gravity surveys indicate the presence of steeply dipping workings. Further, the velocity sections are consistent with the thickness of the till cover at each site and the possible weakening of crown pillars at one of the sites.

"INTRODUCTION The orienting phases of the current investigation of cushioned blasting in the Explosives Division of the Bureau of Mines were indicated in a previous publication (5) 3/. Pertinent literature reviewed therein indicated a highly controversial field. This emphasized the need for experimental studies of many factors that affect the health and safety of miners and the efficiency of mining operations. Results of some orienting tests showed marked effects on practical hazards. Further, they emphasized the necessity and importance of the contemplated investigation. The scope and general plan of these practical inquiries into the safety of cushioned blasting were indicated.This investigation of cushioned blasting relates to the various means employed to reduce the intensity of application of energy from explosive actions in boreholes. It has been suggested (from p. 19 of reference 5) that the divisions of this work would correspond to the salient hazards affected by cushioned blasting. These include the ignition hazard, hazards from poisonous gases, and other practical hazards in the blasting technique. The ignition hazard was resolved into two components of approximately equal weight. These include studies of effects upon (1) the incendivity of blown-out shots and upon (2) the occurrence of such shots. Experimental studies of effects on the incendivity of blown-out shots were first undertaken. Questions of the best procedure for making these evaluations have been adduced, and the interpretation and utilization of the results of gallery testing seemed to require detailed attention; some preliminary general considerations of such testing were therefore necessary."

The brecciated dolostone-hosted (Mississippi Valley-type) and associated shale-hosted stratiform Pb-Zn deposits at McArthur River occur adjacent to a major syndepositional fault, the Emu fault. Base-metal zoning, mineral parageneses, and Pb and stable isotope ratios indicate that both types of deposits formed contemporaneously from metalliferous brines that flowed into the host sediments from the Emu fault. Derivation of brines from underlying evaporitic sediments is suggested. Geochemical evidence indicates that the mechanism of ore formation was sulfate-reduction coupled with oxidation of organic matter. The textural relationships of stratiform sulfide mineralization to diagenetic dolomite concretions, chert blebs, volcanic glass shards, and inter-ore breccia beds, indicate that the reactions proceeded diagenetically within about 100 m of the sediment/water interface.

The roof rock in underground limestone mines in Northern Appalachia can be subject to high horizontal stresses in spite of the shallow depth of the workings. The high stresses can cause roof stability problems. The National Institute for Occupational Safety and Health staff have observed a distinctive asymmetrical mode of failure at the pillar-roof contact in two underground stone mines, which is different from the typical failure mode observed in response to excessive levels of horizontal stress. A dip of greater than 5° was identified as a possible cause of this mode of failure. Numerical model experiments showed that an increase in the dip of the workings can cause an increase in the stress at the up-dip corner of the roof beam. A case study is presented in which failure at the pillar-roof contact was observed where the dip of the workings was 7° in a high horizontal stress field. Numerical modeling showed that the relatively small dip of the workings could have induced stresses changes in the immediate roof that would explain the failures. The model results also showed that this mode of failure is less likely to occur if the limestone pillars contain weak bedding planes that provide stress relief. In addition, the results showed that for the conditions at the case study site, the stress in the roof beam was not sensitive to the thickness of the roof beam or to the excavation span. The high horizontal stresses at this site are an important contributing factor to the observed failures.

The possibility of using surfactants as wetting agents for the suppression of sulphide ore dust has been investigated. Numerous surfactants were examined to determine their characteristics and to investigate their ability to improve the wetting of the sulphide ore dust. However, the surfactants selected as suitable dust suppressors proved to have a negative effect on the flotation of the ore contaminated with such reagents. A method was developed to eliminate the detrimental influence of dust-suppressing surfactants. It consisted of surfactants adsorption on activated carbon and separation of loaded carbon prior to flotation. The technique is simple and requires minimal changes in the plant flowsheet.

The Geotechnical Engineering research group at James Cook University has been working closely with all the leading mines in Australia in the area of minefills. They have carried out substantial research and consulting work on the drainage and stress developments within hydraulic fills. The objective of this paper is to summarize their work during the past five years on hydraulic fills. The paper will serve as a one-stop reference, pointing to relevant literature including the authors? own publications, on various issues covering drainage and stress developments within hydraulic fills. More than 15 different hydraulic fills, representing five major mines in Australia, were studied. Their grain size distributions and placement characteristics including permeability, porosity and dry density were studied and comparisons were made with the limited in situ data available in the literature. Stress development within hydraulic fills has been studied to minimize the risk associated with barricades failure. FLAC and FLAC3D were used to model stress developments within the hydraulic fills and were compared against the approximate closed form solutions available in the literature. A laboratory model was developed to study arching effects within the hydraulic fills.

A variety of grades of very white muscovite mica are now being produced from sedimentary sands and clays in Central Georgia. The mica is naturally concentrated near the base of the lens-shaped kaolin deposits and also disseminated in associated quartz sands. The same weathering and bacterial alteration processes which have transformed the kaolins from dark organic muds to white pigment-grade industrial clays have also removed discoloring iron from the micas and developed unusually white muscovite. Further, chemical alteration has removed some potassium from the muscovite atomic structure, resulting in an easily delaminated mica which can be cleaved to unusually fine, high bulking, high aspect ratio products. The peculiar alteration history of these sedimentary micas enables production of some of the whitest, finest particle size, and thinnest aspect ratio muscovite mica products on the world market today. Raw mica feed is obtained from kaolin and sand processing by-products, and also from beneath the useful base of abandoned kaolin mines. The processed ultrafine and filler grade muscovite mica products are used for paint, plastics, cosmetics, mold release, thermal and electrical insulation, epoxy, and dry wall joint cement markets.

"A young private R & D organization, COREM celebrated its 10th anniversary in 2009. Managed by a consortium of nine mining and metallurgical companies, COREM has the mandate of increasing the competitiveness of its member companies through the development and transfer of mineral processing innovations. Typical technologies developed by COREM include alternative mineral processing techniques, novel operating strategies, innovative sensors and process simulation software. Many of them were developed in partnership with universities and selected research organizations.Because the COREM membership base is broad (from ferrous to base, to precious metal industries), its R & D program must deliver technologies with applications within a wide spectrum of mineral processes. Comminution, physical (gravity and magnetic) separation, flotation, cyanidation, pelletizing and thermal processes are the main topics of the R & D program. A number of new technologies have been developed and transferred to the plants of member companies. These examples of transferable technologies demonstrate the capability of COREM to impact the bottom-line of its member companies.INTRODUCTIONCOREM is a private mineral-processing R & D organization that operates from its own laboratories and pilot-plant with its own staff of professionals and technicians (about 95 persons). It is managed and financially supported by a consortium of nine mining and metallurgical companies and three provincial and federal ministries. "Why do some room and pillar retreat panels encounter abnormal conditions? What factors deserve the most consideration during the planning and execution phases of mining and what can be done to mitigate those abnormal conditions when they are encountered? To help answer these questions, and to determine some of the relevant factors influencing the conditions of room and pillar (R&P) retreat mining entries, four consecutive R&P retreat panels were evaluated. This evaluation was intended to reinforce the influence of topographic changes, depth of cover, multiple-seam interactions, geological conditions, and mining geometry.This paper details observations made in four consecutive R&P retreat panels and the data collected from an instrumentation site during retreat mining. The primary focus was on the differences observed among the four panels and within the panels themselves. The instrumentation study was initially planned to evaluate the interactions between primary and secondary support, but produced rather interesting results relating to the loading encountered under the current mining conditions. In addition to the observation and instrumentation, numerical modeling was performed to evaluate the stress conditions. Both the LaModel 3.0 and Rocscience Phase2 programs were used to evaluate these four panels. The results of both models indicated a drastic reduction in the vertical stresses experienced in these panels due to the full extraction mining in overlying seams when compared to the full overburden load. Both models showed a higher level of stress associated with the outside entries of the panels. These results agree quite well with the observations and instrumentation studies performed at the mine.These efforts provided two overarching conclusions concerning R&P retreat mine planning and execution. The first was that there are four areas that should not be overlooked during R&P retreat mining: topographic relief, multiple-seam stress relief, stress concentrations near the gob edge, and geologic changes in the immediate roof. The second is that in order to successfully retreat an R&P panel, a three-phased approach to the design and analysis of the panel should be conducted: the planning phase, evaluation phase, and monitoring phase."
