The underwriting of professional negligence has proved to be an evolving science. As with most insurance products coverage offered in the past was very broad but as legal precedent demonstrated to Insurers areas of unforeseen exposure policy documents per se have become more restrictive until today the insurance market through attrition have produced a basic policy structure with various purchasable extensions which together provide insuring professionals with coverage that has proved to be adequate for their needs. The basic policy provides indemnity and protection for fortuitous negligence. In a practical sense, however, professional indemnity policies are really a defence document in so far as policy holders are relying upon the expertise of Insurers to protect them in the event that a claim is made upon them. Whether damages are awarded is in many respects a secondary consideration, the most important factor is to ensure that policy holders are protected in terms of the legal defence mechanism. By way of illustration we are currently in the process of defending a large professional firm which purchased from us a policy with a limit of indemnity of $10M. The case has been running in a Supreme Court now for 14 months and the defence costs which we have incurred on their behalf are rising close to $8M as I put pen to paper. Defence costs of that magnitude would in effect terminally cripple all but the very largest professional operations in Australia. In more recent times coverage has been provided in Australia for breaches of the Trade Practices Act and similar legislation enacted in various States. No-one really appreciated the comprehensive impact which the Trade Practices Act would have on commerce in Australia. Most professionals thought that indeed they would be exempt from prosecution under this Act. The passage of time has clearly indicated that this is not the case and we have found that it is much more difficult to defend a claim involving breaches of the Trade Practices Act than it would be to defend a claim for breach of tortuous liability.

"The Gosowong Gold Project is owned and operated by PT Nusa Halmahera Minerals (PT NHM), an Indonesian company 75 per cent owned by Newcrest. It is located on Halmahera Island, in the North Maluku Province of the Republic of Indonesia, approximately 2450 km north-east of the national capital, Jakarta (Newcrest, 2017).Kencana is one of two underground gold and silver mines at the Gosowong Operation where ground conditions vary from competent to very poor. Development headings occurring in poor rock mass conditions require high capacity ground support including long tendon cable bolts. Manual installation of cable bolts in these development headings has been a time consuming process due to borehole collapse, blockages requiring subsequent re-drilling and a minimum eight hour grout curing time interval. The installation of cable bolts including the setting, plating and tensioning can impact the critical path for development heading advance resulting in development cycle delays.A review was conducted by PT NHM on current industry practices for alternatives to manual cable bolting. PT NHM subsequently approached Minova to assist with the development and trialling of self drilling anchors (SDAs) installed using jumbos with a rapid setting pumpable resin, CARBOTHIXTM. A series of system development trials were conducted and a reliable bolting system was developed utilising the existing jumbo drilling fleet with minimal required modifications.The system developed was a ‘one pass’ bolting process to install and grout SDAs using a two component resin with a remote, mechanised system. Improvements achieved from the system include: eliminating hole closure issues and more immediate support in poor rock mass effectively reducing personnel exposure to poor ground conditions by removing the requirement to access the drive backs reducing grout set time with full installation times for 6 m SDAs averaging 25 min.The resin grouting system has now been successfully implemented to effectively manage areas of poor ground conditions and to accelerate advance rates in high priority headings.CITATION:Pateman, D and Sprague, T, 2017. Reducing metal prices have forced mining operations to become more efficient and innovate in order to do more with the existing base. Much of this effort has focused on improving the efficiency of load and haul operations. The key to this is to understand and quantify where delays and missed opportunities exist. Metrics such as utilisation of availability, efficiency utilisation and asset availability are helpful indicators of performance; however, uptime-based time usage models and performance analysis tools using productive operating time models help to identify additional improvement opportunities.Phu Kham is a copper-gold mine operated in the Lao People’s Democratic Republic (Laos) since 2008 by PanAust Limited. The mine is a single large open pit of 450 m depth, running a fleet of 51 100 t class rigid dump trucks and 300 t and 200 t shovels controlled with the aid of a fleet management system in order to achieve ex-pit material movements of 56 Mt/a.Commencing in late 2014, a range of initiatives were implemented in order to improve the efficiency of the mine haulage fleet. These include hot seating of all operator changeouts via changeout ramps, split shifting of crews to minimise queuing delays, maximising blast volumes and minimising blast event days to reduce blast exclusion delay events including opportune refuelling of equipment during blasting events, inspections of trucks during refuelling events, implementation of a dedicated haul road services team to help reduce wet weather related delays and infrastructure development for haul path efficiency improvements.These and other initiatives have allowed the mine haulage fleet utilisation of availability (UofA) to be sustainably increased from 85 per cent to 97 per cent (see Figure 1), efficiency utilisation to increase from 85 per cent to 90 per cent, and productive operating time (POT) to increase from 73 per cent to 84 per cent. The corresponding increases in efficiency of the haulage fleet has allowed an equivalent reduction of eight haul trucks from the fleet to meet continuing life-of-mine production requirements.CITATION:Monro, K R and Young, S P, 2016. Transporting coal by means of conveyor belts is very common in Australian underground coal mines. Major conveyor belt-related mining incidents are now relatively infrequent as a result of recent improvements in engineering standards and use of fire-resistance materials. However, conveyor belts have the potential for incidents capable of causing either personal or structural damage. In addition, any unexpected break down of rolling components or failure of belts creates a significant interruption to the production that is a major concern for operators whose responsibility is to achieve the optimum mining production.Currently, underground coal conveyor belts are examined for the presence of any overheating, smouldering or unsafe conditions likely to cause a fire. Periodic inspection of operating conveyor belts is carried out with the aim of detecting any local heat sources in an early stage. This includes temperature measurements of the driving head, idlers and rollers using hand-held temperature sensors such as infrared cameras or laser temperature sensors. The disadvantages of current conveyor belt temperature monitoring systems include high labour and equipment costs, safety risk to personnel who are in close proximity to moving conveyors, the ability to take line of site measurements only and potential for missed overheated idlers. Some of the conveyor idlers in underground coal operations cannot be accessed for traditional temperature measurements. This introduces a significant risk to the operation of critical infrastructure.A distributed temperature sensing (DTS) system can be used to develop an advanced conveyor belt temperature monitoring system that provides real-time, accurate and continuous temperature measurements along a conveyor belt particularly at the rolling components. The DTS system comprises of fibre optic detection cable, DTS controller and interfacing module. The advantages of the system include the accuracy, low-cost, easy installation, robustness, safety, immunity to radio frequency and electromagnetic interference, real-time data collection, and secure data transmission. This paper presents a scoping study on the development and utilisation of a fibre optic-based DTS system for underground coal conveyor belts. The mechanism of heat generation, the operation temperature range and the installation issues of fibre optic cable along the conveyor belt particularly in close proximity to each end of the spindle on every idler will be discussed.CITATION:Aminossadati, S M and Yang, B, 2013. Tunnel boring machines (TBMs) excavation is nowadays more and more considered a valuable alternative to conventional tunnelling. As a matter of fact, thanks to its advantages such as greater rate of advance and safer working conditions, the application of TBMs has increased significantly in recent years. Despite the possible excellent performance of TBMs in favourable ground conditions, the advance rates could be significantly slowed down or even obstructed in limiting geological situations. This obviously traduces in potentially longer tunnel construction times and higher costs and, therefore, in a preference for conventional method. One of the major geological hazards encountered during tunnel construction, is related to the presence of fault zones or heavily jointed and fractured rocks. The most common geotechnical problems resulting from tunnelling in these difficult conditions include the instability of the excavation face, instabilities of side-walls producing large overbreaks on the tunnel profile, excessive deformations in case of squeezing grounds, frequently changing stresses in terms of magnitude and direction, high water inflows, etc. The decisions aids for tunnelling (DAT) is a model and a computer code that can be used to evaluate the influence of the uncertainties related to both the geological/geotechnical conditions and the construction process on final tunnel construction time and costs. In this framework, the DAT is used for assessing the effects that challenging ground conditions may have on tunnelling operations in terms of costs and delays. This is done by simulating the tunnel construction considering several geological profiles. The aim of the study is comparing different excavation methods (ie drill and blast versus TBM) in changing ground conditions along the tunnel alignment ranging from good rock, to very poor and fractured formations. An optical computing system was used to determine spacings and frequencies in geological fabrics and engineering joint fabrics.Three-dimensional arrays of joints were plotted on an equal-area net. Directions in which true spacings are seen were determined from these plots. Photographs taken along these directions were used for optical computing.

The Wattle Dam gold mine, located near Kambalda in the Goldfi elds region of Western Australia has recently progressed to an underground operation and is now believed to be Australia's highest grade operating gold mine. At end of March 2010, the deposit had reserves of 132 000 t @ 20.8 g/t for 88 000 oz and by the end of June 2010 had produced 110 000 oz from open-pit and underground mining. The Wattle Dam main lode is characterised by very coarse gold mineralisation. Gold occurs as cm to decimetre fracture fi ll veinlets, shear surface coatings and erratic fl akey clusters. The gold has no quartz or silica association and lacks adjacent low-grade mineralisation or alteration assemblage. Geologically it appears atypical of Archaean lode gold deposits and may represent an extreme end member of the type. Definition of the lode is largely dependent on effective geological identification of the host lode zone and an understanding of the erratic distribution and grade behaviour of a high-nugget, coarse gold deposit. Dependence on assay data to defi ne the lode zone would result in the deposit being mined incorrectly and much of the deposit being left unexploited. This paper discusses the geology, coarse gold characteristics and defi nition methods which are used to successfully defi ne and mine the deposit.

The major copper orebody at Mount Isa is being extracted at a rate of four million tonnes per year. The method used requires an equivalent rate of back filling with (mainly) cemented fill. There were three major incentives for developing this mining system. The first was the prospect of high ultimate recovery of the orebody. The target is 100% recovery. Also the proposed stoping layout appeared to offer significant advantages in development and production costs. Finally, extensive subsidence above the northern end of the orebody could have disturbed permanent surface installations and jeopardised a possible future open cut. The full implications of this ambitious strategy were not apparent in the early years. Now, after twelve years of production the benefits are being realised and they can be weighed against the costs. Probably the most interesting result which comes from this analysis is the significance of cutoff grades and dilution factors in balancing mining and filling rates.

Nowadays, technology has improved to extract dimensional stone blocks at quarry sites and also to process these at processing plants for making slabs and tiles. In the process of quarrying and processing, performance evaluation of machinery is very important to achieve optimum output at minimum cost with improved technology. Cost analysis of improved technology includes parameters such as manpower, energy consumption, tool consumption, depreciation, maintenance and also others. The chainsaw machine with long boom and rail in marble quarries and multi-diamond wire saw machine for slab making from granite blocks at processing plants have recently been introduced in India. Thus, a study was carried out under a research project on these machines to assess their suitability. In this paper, performance evaluation and cost analysis of chainsaw and multi-diamond wire saw have been carried out with existing machines on the basis of research findings.

In tectonically disturbed zones within coal seams, decreases in the amount of kaolinite in the cellular structure of inertiniterich coal are accompanied by increasing kaolinite contents in fractures within the coal and/or rocks of immediately overlying horizons. This is due to fracturing of the inertinite microstructure by tectonic disturbance and remobilization and redistribution of free, fine-grained material (kaolinite and/or inertodetrinite.) Ascending waters are believed to transport the kaolinite in suspension, and redeposit it in the fractures and pores in the overlying horizons. This removal of material from the inertinite-rich coal leads to an increase in porosity (higher free gas storage), a presumed reduction in coal strength and an apparent increased proneness to instantaneous outbursts of coal and gas.

An extensive suite of elements are routinely assayed on all resource drill core samples to assist in deposit scale resource definition, local mine scale delineation of ore zones, and ore characterisation for the planning and scheduling of mining and processing of ores at Olympic Dam. This paper discusses a deposit scale geochemical relation that can be used by mine geologists to find more high-grade ore at the local mine area scale at Olympic Dam.A 3D grade shell representing 0.8 per cent Cu was created for a mine area using drill hole assay data. This defined the geometry of the ore zone. The Cu-grade shell was then overlain onto the existing resource block model to evaluate the relationships between the Cu-grade shell, copper and other elements. The strongest correlation was between copper and manganese. The block model showed a dramatic decrease in the concentration of manganese just below the below the Cu-grade shell.Several drill holes that intersected the Cu-grade shell were selected to evaluate the copper manganese relation at the drill hole scale. As expected, the individual drill holes displayed a strong inverse relation between copper and manganese. A geochemical footwall was then modelled representing 0.1 wt per cent Mn. Using both the Cu– and Mn-grade shells, several drill holes were designed/drilled and all hit high-grade copper.For the vast majority of the deposit the inverse relation between copper and manganese is valid; however, in areas with intense chlorite alteration (related to amounts of mafic to ultramafic rocks) this relation is not as strong, due to increased manganese in the rock mass. Even though using the copper manganese relation as a drill targeting method was not always as reliable in a few small areas of the deposit, it demonstrates the value of using existing assay data integrated with the resource block model to discover local mine scale geochemical relationships which can be used to more effectively design drill holes to discover high-grade ore at Olympic Dam.CITATION:Griffith, J and Ehrig, K, 2017. Mineralisation was identified at Gladstone Hill as far back as the late 1870s-early 1880s, shortly after the initial discovery of mineralisation at what is now the Martha Mine. Indeed, early recovery problems associated with the Martha mineralisation led to the preferred development of ore bodies within what is now the Union Hill Joint Venture tenement; ore bodies such as Silverton, Amaranth, and Union to name a few. Gladstone Hill forms the basis of the Union Hill Joint Venture, a 50:50 joint venture managed by Au-Ag Resources Ltd (a fully owned subsidiary of Otter Gold Mines Ltd), and Welcome Gold Mines Ltd (a fully owned subsidiary of the Normandy Group). The joint venture participants also operate the adjacent Martha Mine. Mineralisation of Gladstone Hill, as with mineralisation at the adjacent Martha Mine, has been interpreted as a low sulphidation epithermal Ag-Au system hosted within hydrothermally altered andesites of the lower Coromandel Group. Drilling in recent years has successfully identified extensive mineralisation associated with a number of vein sets throughout the tenement including the Gladstone Vein set, the Moonlight Lode and more recently along a north-easterly trend towards the historic Favona workings. A wide range of vein styles is apparent, ranging in character from thin stock works to broader lode-style banded chalcedonic to cherty quartz and have been interpreted as representing multiple phases of emplacement. At shallower levels mineralisation is often hosted by vein clasts within vent breccias. Visible gold is rare, with mineralisation generally associated with very fine-grained pyrite, arsenopyrite and minor electrum. Percentage levels of combined sphalerite-galena-(+/- chalcopyrite) have been intersected at depth however these have been gold poor to date. The potential for the Gladstone Hill area remains to be clearly defined as drilling to date has yet to close off mineralisation both along strike and down dip. If further drilling is successful, the joint venture partners believe Gladstone Hill could contribute significantly to the economic future of current operations.

Mining is an activity which has, in the past, been regarded as often leaving land in a condition unfit for further use. As the need for development that is both economic and ecologically sustainable has grown, so the issue of re-use of land after mining has become significant. During operation it is usual that mine environmental conditions which minimise unnecessary or avoidable environmental impact are agreed with the regulatory authorities . These conditions are usually set down in such a way that they can be interpreted reasonably well by both miners and regulators. However, the conditions to be met by a mining company before it can be absolved of responsibility of a site are often less clear. The close-out criteria by which the success of rehabilitation programs will be judged have received little attention in the past. These conditions are now growing in importance as the costs of rehabilitation of a mine site to an agreed post-mining land use assume greater significance in the economics of mining. This paper briefly describes the status of close-out criteria presently being applied across Australia. A comparison is made with similar guidelines from other countries.

A way back in 1851-2 an army of diggers had commenced the conquest, of the now famous goldfields of Australasia. At that time, and in the Colony of Victoria, more that 10,000 were camped on and around the slopes of the silurian ridges over which Ballarat East h'as been built. A wonderfully rich golden harvest was soon gathered from the alluvial found high on these slopes, and from runs (or gutters) which got their rise on them, and were traced down gullies into a broad valley. In due course it was ascertained that the valley carried a main gutter which represented the unity of the higher ones, and further explorations proved this gutter to have a westerly and then a southerly trend toward and under a basaltic plateau, on the top ofwhich Ballarat West and Sebastopol now stand. In the records of alluvial gold mining all the world over the Ballarat field can be classed amongst the richest. Gold in pieces the size of beans, in, stringy filigree bits, in dust and in solid lumps (the largest of which weighed more than 2,200 ozs.), was obtained in the comparatively shallow ground on the slopes; and speaking generally, great and uniform richness was met with along a belt of country on which, in a north and south line, the present quartz mines of Ballarat East are at work. It was noticed by the diggers that when the gutters crossed the upturned edges of the ,strata, the gold deposits in them were patchy, and that when they trended north and south on this beltthey carried rich deposits for longer distances. In time it was also noticf!d that a class of stringy, irregular shaped quartz occurrences, in a bottom of soft slates, in the, immediate vicinity of tpe g)ltter, meant greater richness, and (in the aIluvial) on the higher ground slabs of quartz would now and then be met with, in which strings and spongy masses of gold were found. As the shallow alluvial became worked out the diggers turned their attention to the quartz occurrences, and, now and again the striking of a flat make of a very golden nature would cause quite a stir in the local mining world.

The Otago Schist is crossed by numerous deformation zones that formed in the latter stages of metamorphism and uplift of the metamorphic belt. These deformation zones are long (tens of km) linear features at the regional scale, and are generally narrow (km scale or less). Many of these structural zones separate schist domains with different rock types and structural/metamorphic histories. The Hyde-Macraes Shear Zone is one such deformation zone, and this zone hosts the active Macraes gold mine. The Rise & Shine Shear Zone, 80 km NW of Macraes, is also enriched in gold and has been mined historically. Mineralisation in both these shear zones occurred during the transition from ductile to brittle deformation as the schist belt was uplifted from greenschist facies metamorphic conditions. Recrystallisation of quartz and micas in the early stages was overprinted by cataclasis, with sulfide and gold precipitation throughout. Many of the other late metamorphic deformation zones formed under similar greenschist to sub-greenschist facies conditions, with similar overprinting of recrystallisation textures and folds by more brittle textures such as breccias. Under these conditions, originally regionally pervasive deformation at the highest metamorphic grades was evolving towards more focussed high strain zones at lower temperatures and pressures. Shear zones initially focussed fluid flow along microshears and mineral grain boundaries, resulting in 1 to 100 m scale alteration and replacement in the host schist, with only local development of quartz veins. Fluid flow was not controlled by open fractures, and the rate of fluid movement was probably slow and pervasive. After uplift into the brittle regime, the schist belt underwent tectonic extension. Extension was accompanied by development of swarms of steeply dipping veins, with localised mineralised normal faults. Hydrothermal fluid flow throughout this uplift history was a normal consequence of late stage devolatilisation of the metamorphic belt. Hence, all late metamorphic deformation zones should be considered prospective for gold accumulation.

The construction of two drainage tunnels and the associated subvertical drain system at the Po Shan area in Hong Kong was completed in December 2009. This project is the first of its kind to utilise drainage tunnels and a network of subvertical drains to regulate the groundwater table to enhance the long-term stability of the hillside of Po Shan area.The groundwater levels, monitored by preinstalled piezometers, once exhibited dramatic drops during drilling of the subvertical drains; however, have recovered from natural replenishment after effectively plugging of the drains. The level of groundwater drawdown is in line with the design function of the subvertical drains. In order to understand the groundwater flow pattern in the Po Shan area during and after construction and to set out operation schemes for the subvertical drain system so as to control the groundwater table in the Po Shan area, a post-construction review has been carried out to examine the behaviour of the drainage system and its impact on the surrounding subsurface groundwater flow.This paper presents a review of the groundwater monitoring data during and after the construction of the drainage tunnel system covering a period of over 3.5 years. A simplified conceptual hydrological model is developed, which has been verified by the monitoring data, to capture the key elements with regard to groundwater variations in different stages of the construction. The performance of the tunnel and subvertical drain system as revealed from these field measurements is discussed.

Deposits of the metalloids arsenic and antimony occur in quartz veins of mesothermal hydrothermalsystems in the South Island as arsenopyrite (AsFeS) and stibnite (Sb2S3). Arsenopyrite and stibnite occur in natural exposures in gold-mining areas, and additional arsenopyrite and stibnite have been exposed to the atmosphere by mining activity. At present the drinking water limits for dissolved As and Sb are 10 ppb and 3 ppb respectively. Sampling and analysis techniques are critical to determination of metalloid concentrations at these low levels. Endeavour Inlet in the Marlborough Sounds is an historic mining area (late 19th Century) with elevated As and Sb in waters emanating from mineralised rocks. Waters running out of abandoned adits have concentrations of As and/or Sb up to ppm levels. Natural waters have high Sb and low As, and the As levels increase markedly where anthropogenic activity has exposed rock. Changes in Sb/As ratio could potentially be used to quantify the anthropogenic component of metalloid mobility on mine sites on the 100-year time scale.

Conventional industry tests used to determine leach recoveries are often fully monitored and may be time-consuming and expensive. Such tests may be limiting in geometallurgical applications where large numbers of samples are required to be processed for characterisation of an ore deposit. BHP Billiton currently use a fully instrumented leach test for characterisation of Olympic Dam uranium-rich samples. Researchers at the Commonwealth Scientific Research and Industrial Organisation (CSIRO) were approached to develop an alternative to these full-scale leach tests that could allow for the ranking of samples in terms of their leachability. The novel geometallurgical test was required to use samples of small mass and be affordable and rapid.Three leach test protocols were pursued in developing the small-scale tests. Kinetic leach tests were performed using mild conditions at higher pH, low temperature and low oxidation reduction potential, moderate conditions at intermediate pH, temperature and oxidation reduction potential and aggressive conditions at lower pH, higher temperature and higher reduction potential.This paper presents results from 44 leach tests on 11 samples with varying uranium mineralogy. The developed small-scale tests correlate well with data from the more costly conventional industry laboratory tests, indicating the possibility of using this alternative test as a geometallurgical leach proxy. Simple regression analysis confirmed the importance of pH, reduction potential and temperature on uranium extraction. Experimental data were fitted to published leach models to predict uranium leach extraction for a range of operating conditions.The ultimate aim of the test work is to combine a limited number of large-scale calibration tests, a large number of small-scale geometallurgical tests and leach models for geometallurgical sample characterisation and evaluation of a wide range of scenarios or processing options.CITATION:Turner, N L, Kuhar, L L, Francis, N, Liebezeit, V, Ehrig, K and Robinson, D J, 2013. Roadheaders were introduced to mining and tunnelling in the early-1960s. Since this period their main field of application has been for roadway drivages in coal mines and for mineral production, for instance in coal, rocksalt, potash and gypsum mines. Their main field of application in tunnelling is in softer and partly friable rock formations, a suitable area for non-blasting excavation. However, in longer tunnel sections excavators with high ripping forces have replaced roadheaders in many situations. The main arguments against roadheaders have been and still are:a sharp increase in excavation cost with increasing rock strength and increasing abrasivity; and greater risks in conditions which have not been, or could not be, investigated to a sufficient extent. These arguments have, to a great extent, restricted practical roadheader application in production mining and in a lot of æstandardÆ tunnel projects. To address these limitations, factors which could make roadheaders attractive for such conditions and could overcome existing technological difficulties have been investigated. Extension of the economic field of application in hard rock was found to be the most important and the most effective step forward.
