Regulators have an important role to play in fostering and enabling reliable, effective, environmental management. Given this, it is perhaps surprising that there has been relatively little debate on exactly how regulators can enhance their own approaches, in order to promote improved outcomes overall. The authors argue that applying research on High Reliability Organisations, as well as the University of Pennsylvania’s Penn Program Framework for Regulatory Excellence, to regulators has the potential to facilitate positive environmental results across the mine life cycle.

Safety is a key value in most leading mining companies. Despite years of effort, and significant expense in ‘programs’, operating sites seem to have been left with no real improvement in either the elimination of fatalities or sustained reduction in injuries.Safety systems and processes have grown and evolved, often to the point where they have become overly complicated and marginally useful.MMG Limited, following the consolidation of its Australian operations under a single management structure, undertook a program to effect real and sustained change in its approach to managing safety. The program aimed at improving safety by better engaging its workforce and by making safety processes simple, useful and effective. It defined the role of the supervisor in creating safe work, the role of the people doing work and the role of safe work methods.A safety diagnostic tool was developed to assess site against safety-critical mindsets, behaviours, cultures and systems. The outcome of the diagnostic is considered by the organisation to be the true lead indicator of safety performance.While still a work-in-progress, the program at the Australian Operations of MMG Limited shows that challenging common paradigms in safety management can create real change in safety performance.CITATION:Webb, M E, 2016. Cyclic voltammetry (CV) has been evaluated as a technique for monitoring the quality of an industrial zinc sulphate electrolyte. The basis of the method involves measuring a change in the polarisation of the zinc nucleation reaction at an aluminium surface and relating this to the impurity content and/or the overall quality of the electrolyte. Specifically, the technique aims to be able to indicate and quantify detrimental synergism between impurity elements, something which traditional analytical techniques are incapable of providing. The technique has found some success with regard to monitoring glue additions in a range of metal electrolytes. It has been found that investigations by a number of workers over the past 15 years have produced variable and often misleading results due to the inadvertent use of commercial purity aluminium working electrodes. It has been shown that impurity elements in the working electrode can give rise to a range of responses that are not obtained when pure aluminium is used and that the response is variable due to the lack of surface homogeneity of commercial purity aluminium electrodes. Furthermore, it was demonstrated that previous detection limits reported in the literature were based on the presence of additional peaks in the cathodic scan and not polarisation effects. The results of this study have shown that when a pure aluminium working electrode was used, the method was incapable of detecting any change in polarisation for an electrolyte containing three common impurities (Co, Ni and Ge), at concentrations that would significantly reduce optimum electrolytic performance. A range of methods was tried to sensitise the technique, including raising the temperature and acidity, and adding antimony in order to induce some synergism. In all cases the polarisation response was unchanged. Consequently, apart from the commercialised application for monitoring additions of antimony and levelling agents such as glue, CV has very limited application as an electrolyte screening technique.

The main assets of a mining company are its orebodies; the second are the people who find and use them.The graphs in Figure 1 were derived from a paper by Rob Macdonald of Rothschild Australia presented to The AusIMM Centenary Conference in Adelaide earlier this year. They show that the value of the industry on the Australian Stock Exchange halved over the past decade, despite the re-investment of capital equivalent to its full market capitalisation. Why is this so? The average dividend yields for the ASX Mining Index was never above five per cent during that period and barely averaged 2.5 per cent. Why then is $Ai million capital re-investment per employee since 1960 not enough to generate a rate of return to investors better than lousy bank interest? Do the investing public no longer regard our industry's assets to be world class?

Waihi Gold Mining Co Ltd [WGM], operated by Normandy Mining, has been internationally recognised for its achievements in environmental management at the Martha Mine at Waihi. In a proactive approach to dealing with community concerns regarding blast vibrations on the North Wall cutback, trials were performed using the Cardox System [Cardox], to determine feasibility of achieving economic fragmentation of andesite overburden whilst bettering management levels of 5 mm/s peak particle velocity. Cardox is a non-explosive pressure breaking system which uses high pressure CO2, Results against conventional Ammonium Nitrate/Fuel Oil [ANFO] using a variety of delays showed that conventional blasting techniques could produce marginal fragmentation within the management levels with occasional excursions, whereas Cardox produced the required fragmentation while not registering on the vibration monitoring equipment. Fragmentation to enable normal excavation by a Hitachi EXII00 hydraulic excavator was achieved with a yield of 16m3 per round. The trails indicated that fragmentation with Cardox at the yields required of over 18m3 per round was possible with further product development. The trial demonstrated that non-explosive pressure breaking is feasible and its application warranted where environmental concerns affect the cost-benefit economics of production blasting in sensitive areas.

Impact-induced rock fragmentation is a mechanism that is commonly used for rock breakage in drilling and crushing. Additionally, rock fragmentation by blasting is frequently used in rock excavation operations. Experience shows that the presence of discontinuities can significantly influence the impact-induced damage and fragmentation of rock. Quantification of the response of jointed rock masses to impact loads is complicated by the fact that the available laboratory tests are mainly designed for aggregates or intact rocks. It can be argued that neither of these tests adequately represent a jointed rock mass. This paper presents the results of a series of numerical simulations used to investigate the influence of pre-existing discontinuities on the impact-induced fragmentation of rock masses. The methodology includes determination of the static and dynamic mechanical properties of a rock unit by conducting a series of laboratory tests on intact rock samples collected from a quarry in Canada. A 2D distinct element code, Particle Flow Code, was used to generate a bonded particle model in order to simulate both static mechanical properties (uniaxial compressive strength, elastic modulus, Poisson’s ratio and indirect tensile strength) and dynamic mechanical property (drop weight tensile strength) of the intact rock. The calibrated numerical model was then used to construct large-scale synthetic rock mass samples by incorporating discontinuity networks of different intensity into the bonded particle model. Finally, the impact-induced fragmentation inflicted by a rigid projectile particle on the jointed rock mass samples, simulated by a synthetic rock mass model, was determined. Soil improvement technologies have been widely employed over the recent decades to overcome tunnelling problems in difficult grounds, their range of application growing proportionally to the technological development. The constant advancement in the different mechanical (flexible and more powerful equipment) and electronic (enhanced reliability and direct quality control) aspects of these technologies, fostered the confidence of engineers and tunnellers and, ultimately, allowed the entire tunnel industry to approach difficult excavations in a safer and cost effective manner.This paper outlines the main features of commercially available soil improvement techniques, with particular emphasis on permeation grouting and jet grouting. These two technologies, developed concurrently in Europe and Japan, and there customarily adopted in the design of underground structures, have known extensive and extreme applications worldwide in recent years. Case histories and current practice of soil improvement techniques, as they apply to tunnelling, are also rendered.

This paper briefly describes the construction in 1979/80 of a 70m deep trial open pit coal mine near Bowmans S.A. and describes more fully the hydrogeological aspects of the exercise. The primary purpose of the Trial Pit was to provide a large sample (700 tonnes) of coal for combustion tests to be carried out in West Germany and the United States of America. Consideration was given to obtaining the sample of coal by drilling and by shaft sinking as alternatives to open pit methods. It was expected that economic mining of the deposit on an operational scale could only be achieved by open cut methods and it was recognised that major uncertainties existed in planning such a mine in the deep unconsolidated multi-aquifer deposit. It was therefore elected to obtain the bulk sample by constructing a trial open pit, which would be closely monitored to provide as much data as possible for future planning of a full scale mine. The dewatering of the Trial Pit was achieved using 26 deep pumping wells, assisted in the later stages by several hundred well points. This was probably the largest dewatering project currently in operation in Australia.

In 2009, George Fisher mine began to seriously consider the transition from pyrotechnic detonators to electronic detonators. Initially, the electronic detonators were used for complex crown shots or areas where ground conditions had the potential for excessive overbreak. After critically evaluating the potential safety benefits, production increases and ability to redesign stopes to better manage a thinly bedded hanging wall, the mine decided to embrace the technology.

The manganese in solution was derived from the weathering of Precambrian manganifer.ous s.edimexsts, None of the deposits are of hydrothermal origin, nor are they related to igneous rocks in any war, other than as ultimate source rocks. There is ho evidence-'t.o.:sugge,st that meta- morphism has been responsible for.the emplacement of any deposit. The tenor of the ore varies'aec'ording'to the quantity of gangue in the orebody and- also to the nature of the part- icular ore minerals. The main ore.mine.rals are the oxides pyrolusite and cryptomelane and the 'o'xide-silicate braunite. The main gangue minerals are hematite, limonite and quartz. The deposits of high grade oreò(-above.45 per cent Mn) range in size from 50 tons to many thousands of tons but most of them are in the 500-5,000-tons r .nge .

Fragmentation is the basic concern in rock blasting and serves as the main measure of blasting effectiveness. A good estimation of fragment size distribution before blasting is very useful and helpful for selecting the proper equipment for loading, transporting, and crushing rocks. A fragmentation model is presented that is based on a hole by hole analysis and correlates the rock properties, burden, spacing, and explosive column with the blasted volume and fragmentation obtained. The drill bench is divided into three separate areas corresponding to different breakage mechanisms. They are: 1) a main breakage or casting area, 2) a secondary breaking area, and 3) an overbreak area. The boundary of the main breakage area is found from the magnitudes of strains in the stress waves and the tensile strength of the rock. The secondary breaking area is determined by stress wave reflections at discontinuities and interactions between colliding stress waves from adjacent boreholes. The overbreak area is that beyond the main breakage area "where the reflected tensile waves no longer break the rock. This analysis is applied on a hole by hole basis to locate the breaking points. Then connecting the breaking points on each side of a borehole forms the triangle of the main breakage area. This is repeated for the next hole and the remaining area is the secondary breakage area. After the angle and area for each hole are found the Kuz-Ram model is used to predict the fragmentation size distribution for each hole. Then summing up the size distribution for each hole gives the total fragmentation distribution. This analysis has been successfully applied to field tests performed in a limestone bench where the entire spoil pile was screened to determine the fragment size distribution. Knowing the explosive and rock properties the breaking area was found for each hole, the strain and stress values, the breaking point, and angle of breakage (104 degrees). The fragmentation size distribution was then calculated and compared to the field data with excellent agreement.

During a mineral exploration programme in an environmentally sensitive area near Mount Painter in South Australia, a helicopter was used to position, construct and service drilling rigs in order to avoid the need to create new access tracks. The drilling programme was designed to optimise the advantages of natural landforms in order to minimise the environmental impact of site construction and drilling activities. This minimisation of land disturbance reduced the required amount of rehabilitation measures to the extent where all impacts of the helicopter-supported programme had disappeared within twelve months of completion of work.

In the process of primary copper smelting in flash smelting furnace (FSF), the ratio of iron to silica (Fe/SiO2 mass ratio) primarily determines the slag viscosity and magnetite formation. Viscous slag does not only cause difficulties in daily operation, such as in tapping, it also increases the copper losses to slag. Thus, keeping the Fe/SiO2 mass ratio at the desired level is an important aspect of FSF and slag cleaning furnace (SCF) operation. In this study, the improvement of the Fe/SiO2 mass ratio was conducted in two steps: First, the optimal Fe/SiO2 mass ratio was derived based on thermodynamic data. Based on this analysis, appropriate control limits were determined. Second, this range of favourable Fe/SiO2 mass ratios was implemented into operation by an advice-based control system, which suggests the addition of silica based on the analysis of SCF slag. As this approach was successful, the same advice-based control system principle was adapted for the coke addition to the SCF. After testing the new advice-based control for several months, the production data confirm the predictions by thermodynamics: since the prediction was implemented as an advisor, the copper and magnetite content in SCF slag is significantly reduced and viscous slag is reported less often by operations.

The Fimiston reserve is being depleted at the rate of one million ounces per year; most of which is processed by the Fimiston plant, with the remainder being stockpiled. As for most other large open cut mines around the world, Fimiston has an excellent dispatch and tracking system. Reconciliation is used to determine how the various models used in mine planning (such as a reserve, budget and schedule models) are tracking against production. It is also used to gauge the performance of grade control estimation against production; however, analysing the results of this can be complicated and, at times, misleading. The reconciliation process involves adjusting the monthly production data to equal the mill reported tonnes and grade, plus the change in stockpiles. This adjusted number is then compared to the grade control reported tonnes and grade, the scheduled model (monthly planning model) tonnes and grade and against budget (Reserve) tonnes and grade. This paper includes explanations of all assumptions made in the reconciliation process, including the end of month closing stocks balance, the application of adjustment factors to the dispatch data to create a new balanced set of production data and the monthly sign off to comply with the Sarbanes-Oxley requirements of the North American owners. The reserve is reconciled by proved and probable classification. This tool is used to assess the robustness of the classification scheme utilised and identify any reconciliation issues as they arise. Within the Fimiston deposit there are still issues that are clouded by the assumptions made concerning the scale of observation, poor data support and the mining of the narrow lode areas of the deposit. The declared ore mined (DOM) number (which is defined in the paper) can be significantly affected by these assumptions, and these need to be clearly stated and understood when looking at the reconciliation data. The DOM tonnes and grade is a good indicator of real mine performance as it adjusts mined tonnes and grade to take into account the milled tonnes and grade each month, and prevents cumulative errors from building up in mine predicted dispatch numbers or end of period stockpiles. This number allows monthly monitoring of the mine to mill performance and allows valid comparisons to be made back to budgets and reserves over a period of time. KCGM uses three month rolling averages to monitor performance against reserve and budget models. This ensures that monthly distortions in the figures due to stockpiling issues or over mining in a particular period do not lead to false conclusions. Reconciliation performance over a longer period (12 - 18 months) is used to adjust reserve models should significant variance be observed. By closely monitoring the reconciliation performance at the Fimiston operation, fine tuning of all aspects of the grade estimation process from reserve through to grade control can be made. This allows the most accurate data to be used when planning from reserve data for the Life of Mine Plan through to grade control data for short-term mill feed.

"OceanaGold’s Didipio operation has been in operation for three years. Integral to the flow sheet design is a flash flotation circuit treating a portion of cyclone underflow, consisting of an Outotec SK-500 flash rougher and an Outotec TC10 flash cleaner and a SB-2500 falcon treating flash rougher concentrate to recover fine free gold.Flash copper recovery has been holding steady at 60 per cent, while flash gold recovery at 50 per cent. Gold recovery from the gravity circuit is 22 per cent, bringing the overall flash plus gravity gold recovery to 72 per cent.Operation of the flash rougher is mainly by pulp level control. Control of copper grade in the flash cleaner is mainly controlled by air inflow and flash cleaner pulp level control drives copper recovery. The Australian Constitution specifies certain powers of the Commonwealth Government to be used in its capacity as the national government. Certain of these broad powers result in the Com- monwealth having a facility to play an important part in the development of the mining industry. The State Governments, however, by virtue of their wide powers to regulate matters within the boundaries of a particular State, are more directly involved in the day to day administration and standards of activities of mining operations. In this respect the Common- wealth has something of a dual role because of its "State-like" responsibilities for the mainland Terri- tories. The components of the role that the Common- wealth Government discharges in relation to the mining industry are qualified to varying extents at particular times by the political persuasion of the Government. In absolute terms the components are as follows. 1. Economic management. The Commonwealth's fiscal and monetary policies have an important effect on industry as well as on State finances. In particular, the taxation regime employed by the Commonwealth Government is of direct importance to economic decision-makers in the mining industry. 2. International matters. International trade and commodity matters are, more and more, becoming an everyday part of the conduct of a nation's foreign affairs. 3. Exports. The Commonwealth Government, via the constitutional powers over overseas trade, have exercised varying degrees of control over mineral exports to achieve particular objectives. 4. Foreign capital and investment. Australia in general, and the mining industry in particular, is and will continue to be a net importer of capital. There appears to be bipartisan political agreement in Australia that foreign investment policy must be enunciated to ensure that investors. are in no doubt that Australia welcomes beneficial investment and technology from overseas. 5. Environment and Aborigines. Governments, both Commonwealth and State, have in recent years become more involved in these areas in response to their interpretations of community aspirations. 6. Energy. Access to assured supplies of energy is now viewed, by Governments worldwide, as a desirable asset to ensure the well-being of a nation's people and industries.

This paper examines the occurrence of outbursts, coal bursts and rock bursts in underground mining. It then examines them from the viewpoint of the failure mechanism and velocity of particles. The presence of gas is extremely important as it can both act as the fluid pressure component of effective stress leading to failure, and as a substantial energy source in projecting material during a burst. For gas to drive a dynamic event it must be present as free gas held in voids within the rock or coal mass. This can pre-exist the burst event or be released from desorbing gas during it. The latter is typically part of a multi-stage outburst event which can reach very large proportions. Rock and coal bursts are driven by strain energy release and can vary in form. These tend to be single events associated with failure and strain energy release. It is also possible to conceptually have hybrid events where the failure of the mass and the energy release are due to both stress and expanding gas.

Since the days of mandatory high cost pilot plant testing in 6 ft + 2 ft SAG mills autogenous griding/semi-autogenous grinding (AG/SAG), mill specific energy is now calculated by various empirical formulae and pilot testing carried out in small laboratory mills. Barratt (1989) described a method using Bond Indices than adding a single factor of 1.25 to arrive at the AG/SAG specific energy. In 1991 the Canadians developed the SAGPower Index (SPI), test using 2 kg of material in a 300 mm diameter mill. This test has been further developed by Starkey, Hindstrom and Orser (2006). This test now includes a Bond Ball mill work index test.The Macpherson test is still being offered by some testing facilities. This dry test requires at least 180 kg of material.Morrell (2004) formulated an AG/SAG specific energy generator based on a drop weight index Value (DWi). This DWi value is related to the Ab parameter and the material specific gravity. Morrell reconfigured the BondÆs Third Theory formula to cause extra power requirements to be inputed. This technique incorporates the material specific gravity (SG). The AG/SAG specific energy values produced using this method are susceptible to higher values of crushing and rod mill work indices.This paper offered describes a more simple method to generate AG/SAG specific energies. This programme incorporates Bond work indices, the JKMRC parameter Ab measuring impact breakage and the parameter ta measuring the abrasion rate, then employs a series of efficiency values nominated for each transition size (AG/SAG T80). These efficiency values have been compiled over the years from actual operating plants and documented reports.CITATION:Burgess, D, 2012. Banded Iron Formations or more commonly known as BIF are sedimentary rock formations with alternating silica-rich layers and iron-rich layers that are mainly composed of iron oxides (hematite and magnetite), iron-rich carbonates, and/or iron-rich silicates. Considering the depletion of highgrade iron ore reserves in South Africa, there is a need to maximise resource utilisation through the exploitation of low-grade/uneconomic resources particularly BIF material, which makes up more than 65 per cent of current reserves. Previous test work conducted by alternative BIF sources globally indicates the need for fines processing in order to produce a concentrate. South African BIF however is amenable to coarse and fines processing producing an on-grade product or blended feedstock. C-Type material within South Africa’s Northern Cape Province grading at approximately 33 per cent Fe was supplied for research purposes. Top size optimisation via gravity Heavy Liquid Separation (HLS) tests was conducted. The laboratory results being perfect separation (Ep=0) were modelled to predict pilot performance, the results of which were tested on a pilot scale for the finest size fraction (-1 mm). It was observed that a top size of -2 mm provided optimum results in terms of upgrade and recovery, with crushing finer not necessarily producing superior results. At a target grade of 60 per cent Fe, recoveries of 65.6 per cent were achieved on the laboratory scale, whilst recoveries of approximately 58 per cent were achieved on the pilot scale. Targeting higher upgrades of 63 per cent would inevitably result in lower recoveries of 57.2 per cent on a laboratory scale and approximately 54 per cent on a pilot scale. This is in line with grades and recoveries achieved globally for BIF material albeit at finer grind sizes.

The mining industry in common with the rest of Australian industry is in the critical situation of facing a rapidly diminishing insurance market. Huge industrial complexes and massive mining risks with limits of liability of millions upon millions of dollars are becoming increasingly difficult to insure. Insurance underwriters are still reeling from the financial shocks of cyclones, floods and fires in Australia over the past couple of years. Risk Management which calls for manage- ment involvement in the control of losses offers a serious challenge to miners, industrialists and insurance underwriters alike. Without it the insurance market could disappear altogether.
