PT Kaltim Prima Coal's 13.1 km overland conveyor was commissioned in 1991. It is of conventional 3 roller 350 troughed belt type. Cost and performance optimisation during initial design ended up with 5.4m/s belt speed, 1,000mm belt width to convey 1,350tph, and 7mtpa of coal. Despite high initial figure, the belt speed was lifted to 6.5 mps in 1996 to accommodate mine throughput increase. In 2002, new low loss ST2100 1,100mm wide belt replaced the old ST2250 due for change-out. An upgrade project was initiated at the end of 2003 to further increase the belt speed to 8.5 mps, hence to increase the OLC capacity to 30mtpa. Performance data, experience of site personnel, site observation and tests, benchmarking, and information from equipment manufacturers were major inputs to the design review. The project fast track was another challenge in addition to executing modifications on a production lifeline. The project was completed within fifteen months and the OLC was successfully commissioned at 8.5 mps belt speed in February 2005. The OLC has been to date conveying 4200tph satisfactorily.

JOHN HAMRIC: Our next speakers are Richard Ghent, manager of environmental health and safety for CF Industries, Hardee Phosphate Complex, in Wauchula, Florida. Richard has 20 years' experience with environmental health and safety affairs. He has a bachelor of science degree from the University of South Carolina. With him is Daniel Warnock, who is supervisor of safety and training also at the Hardee Phosphate Complex. Daniel's experience is in safety programs and electrical training. Their presentation is "Regulations Versus Management Practice." RICHARD GHENT: Written procedures give structure to safety programs. At their best, they are brief and understandable to facility personnel and clearly identify roles and responsibilities. At worst, they attempt to recite the entire regulation they are intended to address. Some regulations, such as OHSA's Hazard Communication Standard, require employers to develop, implement, and maintain a written program. As written programs and procedures are developed, some basic questions arise: Is the program designed to achieve compliance with regulatory requirements or does management expect the program to go beyond compliance to some higher level of performance? Management may have difficulty in measuring and communicating performance expectations in safety. A facility with 4 million safe work hours does not suddenly become unsafe the day their safety record is broken. Generally, management expects safety programs to meet regulatory requirements and to protect the employer from enforcement actions and fines from regulatory agencies. Management also expects that safety programs will result in a safer work environment with fewer incidents, accidents, and near misses. But it is difficult to know when a safety program has succeeded. If the program is expected to go beyond compliance, what other standards should be included (e.g., ANSI standards, company management practices, lessons learned from inspections and audits, recommendations from safety committees, etc.) in the

INTRODUCTION The planned 166 kilometer (103 mile) Metro rapid rail transit system in Washington, D.C. is being designed and constructed by the Washington Metropolitan Area Transit Authority. At the present time, 112.6 kilometers (70 miles) of this system is in operation and contains 21 route-kilometers (13 miles) of rock tunneling, and 14.5 route-kilometers (9 miles) of soft ground tunneling. An additional 2.4 kilometers (1.5 miles) of single track tunnels in rock are planned and 6 additional contracts will consist of some 8 kilometers (5 miles) of twin tunnel, resulting in 16 additional linear kilometers (10 miles) of single track tunnel in soft ground. The Authority plans, upon its evaluation and the recommendation of the General Engineering Consultant, to include an alternative design incorporating the convergence confinement method of construction known as the New Austrian Tunneling Method (NATM), for Sections E-5, E-6e and E-8a of the E-Route. The Metro system is constructed in a variety of geological conditions and crosses the boundary of two major geographic provinces. This paper discusses recent Federal Bureau of Mines research on the selective flocculation of various minerals with insoluble crosslinked starch xanthate (ISX). ISX is insoluble, has a shelf life of over a year when kept cold, and has a higher molecular weight due to cross linking than the soluble noncrosslinked starch xanthate used in previous flocculation studies on nonsulfide minerals. ISX readily flocculates a number of sulfide mineral fines but not silicious gangue. Floc formation, floc size, and settling rate are strongly dependent on pH, with the pH dependence differing for each mineral. Bornite has been selectively flocculated from bornite-quartz mixtures, and the bornite-containing flocs were separated from the unflocculated quartz using decantation and washing procedures.

GENERAL DESCRIPTION The mines are located at Broken Hill in the western part of New South Wales, Australia, 930 km west¬northwest of Sydney. Mining Methods Where pillars must be left between stopes, extraction of a block of ore between two or more levels is carried out in stages: 1) Stopes are mined overhand by horizontal cut¬and-fill stoping, or by long-hole stoping. The various methods of horizontal cut-and-fill stoping used are desig¬nated as: -Loader cut-and-fill stoping with horizontal blast¬holes and diesel load-haul-dump (LHD) units. -Open stoping with horizontal blastholes and com¬pressed air or electric scrapers, or with diesel or electric LHD units. Square-set timber stoping, where ground condition precludes open stoping. The backs are sup¬ported with timber square sets. Pattern rockbolting of the backs may be used as an intermediate stage between open stoping and square-set stoping. 2) Vertical pillars are mined by undercut-and-fill stoping or by vertical crater retreat stoping. 3) Where a horizontal pillar has been formed below the level above the primary stope, this level pillar, usually 6.1 to 8.2 m in depth, may be mined by over¬hand square-set stoping or by undercut-and-fill. Long-hole stoping is used at the Zinc Corp. (ZC) and New Broken Hill Consolidated (NBHC) to mine parts of the zinc-rich A and B lodes. Drilling is carried out from drill drives and, in some cases, sills. Hole sizes used are 165 mm (61/2 in.), 110 mm (4'h in.), and 65 mm (21/2 in.) nominal diam with hole lengths being limited to 50, 30, and 20 m respectively. Ammonium nitrate-fuel oil (ANFO) and water gel explosives are used. Broken ore is loaded from drawpoints at the bottom of the stope using rubber-tired diesel-driven LHD units which deliver ore to an orepass. Annual production at Zinc Corp. and NBHC accord¬ing to mining method is given in Table 1. At Zinc Corp. mining is being carried out at depths varying between 200 and 832 m. At NBHC it ranges from 528 to 915 m below the surface. The average travel time to the stope is 30 to 45 min. GENERAL ORE BODY REQUIREMENTS Size, Shape, and Dip There are six separate lodes which carry minable ore in the mines of Zinc Corp. and New Broken Hill Consolidated. The lowest of these and the main ore bodies presently being mined at Zinc Corp. are the lead lodes, consisting of a siliceous fluoritic ore body (No. 3 lens) and overlying it a calcite rhodonitic ore body (No. 2 lens). Each possesses characteristic ratios of lead, silver, and zinc, although the grade within each lens is variable, in places being as high as 30% Pb. There are also low¬grade portions which consist chiefly of poorly mineral¬ized rhodonitic material. No. 3 lens terminates within the Zinc Corp. leases, while No. 2 lens continues into the NBHC leases where it continues to provide a significant proportion of pro¬duction from the mine. Overlying the No. 2 lens are the various zinc-rich ore bodies. In ascending sequence these are lower No. 1 lens, upper No. 1 lens, A lode, and B lode. A typical assay for the small No. 1 lens group ore bodies is 8% lead, 50 g/t silver, and 20% zinc. The `A' lode is a large low-grade ore body in which the higher grade sections assay 4% lead, 30 g/t silver, and 10% zinc; larger quantities of lower grade ma¬terial also occur. B lode, the main zinc lode, typically assays 5% lead, 30 g/t silver, and 17% zinc. B lode increases in size as it extends south, and in NBHC is a major ore body, contributing about 52% of the production. All the lodes pitch south, the average pitch being 0.52 rad (30°) in the Zinc Corp. leases and rather flatter than this in the NBHC leases (see Figs. 1-3). The ground is all competent rock. Production requirements at Zinc Corp. are 900 000 t and 1 200 000 at NBHC.

Spokane Research Center personnel have been investigating the use of seismic tomography to monitor the behavior of a rock mass, de¬tect hazardous ground conditions and assess the mechanical integrity of a rock mass affected by mining. Seismic tomography can be a valuable tool for determining relative stress in deep, >1,220-m (>4,000¬ft), underground pillars. If high¬stress areas are detected, they can be destressed prior to development or they can be avoided High-stress ar¬eas can be monitored with succes¬sive seismic surveys to determine if stress decreases to a level where de¬velopment can be initiated safely. There are several benefits to using seismic tomography to identify high stress in deep underground pillars. The technique is reliable, cost¬effective, efficient and noninvasive. Also, investigators can monitor large rock masses, as well as moni¬tor pillars during the mining cycle. By identifying areas of high stress, engineers will be able to assure that miners are working in a safer envi¬ronment.

Two Sun Juan Basin studies were completed through the USGS National Coal Availability program. The study areas are in the northwest Bisti field (Fruitland Formation) and in the eastern Standing Rock field (Menefee Formation). In the Bisti and Standing Rock areas, the original coal resource5 are estimated at 4.66 and 3.1 7 Gt (5.14 and 3.49 billion st), respectively. The Bisti coal resources are greatly reduced by the Bisti-De-Nu- Zin wilderness area, which contains an estimated 1.13 Gt (1.25 billion st) of coal. Technological restrictions remove 32.5% from Standing Rock's original resource. Available resources are 60%. 2.79 Gt (3.07 billion st), of the Bisti original resource and 65%, 2.07 Gt (2.28 billion st), of the original Standing Rock resource.

The N20 to N25 Tunnel (basic) segment was the second tunnel contract awarded for construction by The PB/MK Team. This project embodied 3.9 km (2.4 miles) of tunnel and was located within the northwest quadrant of the 86.9 km (54 mile) long main collider ring. The contract required the excavation and initial support of a 54.3 m (178 vlf) deep ventilation shaft, a connecting horseshoe shaped Adit and TBM starter tunnel and 3,865 m (12,680 If) long main collider tunnel with a 4.3 m (14 ft) minimum finished diameter. This paper discusses the construction methods, equipment utilized and the production achieved in the construction of the ventilation shaft, connecting adits and the main collider tunnel.

Recent research efforts have shown have shown selective solvation extraction of a gold cyanide ion-pair complex from alkaline cyanide solution is possible with organic phosphorus esters. These alkyl phosphorus esters such as tributyl phosphate (TBP) and di-n-butyl n-butyl phosphonate (DBBP) can selectively load gold to rather high levels, even in excess of 30 gpl. However, traditional stripping techniques are not particularly effective. In this regard, gold recovery by direct electrolysis of the organic phase has been found to be an interesting possibility. Further, the rate of direct electrodeposition from the organic phase can be improved significantly by using an aqueous/organic mixture. Direct electrodeposition of sheet gold from loaded DBBP was successfully accomplished at a current density of 8x10 4 amp/cm2 and 55°C from such a two-phase mixture with the aqueous phase as the continuous phase. No appreciable degradation the DBBP was found after 5 cycles of extraction and, electrodeposition. Finally, the applicability of the process strategy for an industrial situation was demonstrated by the successful treatment of waste gold cyanide electroplating solutions.

A four-mile length of interceptor sewer tunnel in Manhattan has finished sizes ranging from 10'6" to 14'6" horseshoe. Mixed face conditions were anticipated in three areas and encountered in five. All were short but since the tunnel lies well below the water table and failures would involve important utilities, major structures and human lives, great care had to be exercized in choosing a construc¬tion method. Chemical consolidation was selected as the best approach with compressed air working as a backup system. Initially grouting was accomplished from within the tunnel, but treatment from the surface was more successful and was later adopted. The soils consisted largely of impermeable weak silts and it was found possible to inhibit access of water to this material so that it could be controlled by breasting within the tunnel without loss of stability. It is concluded that in the interlayered permeable and impermeable soils encountered, chemical grouting would successfully stabilize the working face. INTRODUCTION An interceptor sewer is currently under construction along the entire length of the west side of Manhattan. When complete, the tunnel will intercept and divert to a treatment plant (also under construction) a daily flow in excess of 200 million gallons per day. Webster's dictionary nearly equates portland cement with its current primary definition of cement. Although such equation may be a triumph of common usage, the confusion between the terms cement and concrete unfortunately is very widespread. Full definition of cement properly would extend to many substances, but for purposes of this chapter cement is restricted to varieties of portland cement including portland-pozzolan cement. Among the usually recited historical notes concerning cement is that it was developed by the Romans. Their use of cement in the great structures of Rome, and even in the far comers of their Empire, such as Hadrian's Wall in the north of England, testifies to the antiquity of a major cement industry. From English sources we know of John Smeaton's carefully proportioned hydraulic cement mixed in 1756 and of Joseph Aspdin's patented cement to which, in 1824, he gave the name portland cement. (He claimed that his cement mortar looked like the famous dimension limestone quarried from the Isle of Portland on England's south coast.) The year 1971 marked the centennial of portland cement manufacture in the United States. A fair historical account of the many real contributors to the cement industry since 1756 unfortunately would be too extensive for this chapter. Development of rotary kiln practice and burning at higher temperatures clearly was the major move into today's technology. Cement manufacture is the processing of selected and prepared mineral raw materials to produce the synthetic, mineral mixture (clinker) that can be ground to a powder having the specified chemical composition and physical properties of cement. Cement making incorporates a number of distinct steps, but it is characterized by the key pyroprocessing (or burning) step, which brings about the necessary changes in the raw materials. This pyroprocessing is a chemical process. Conversion of large volumes of mineral raw materials into specification-controlled cement at relatively low cost is the objective. Statistics on the cement industry are of uneven quality. The US Bureau of Mines (USBM) is the principal collector of available data. These statistics can be only as good as the quality of information supplied by the reporting companies. The data perhaps most often questioned relates to productive capacity of the industry. The questions submitted by USBM are valid, nevertheless, and the information gained from them, as good as is available, is what is generally used. The Portland Cement Association also is a source of much quality information. In analyzing cement industry production rates, it is well to keep in mind that measurement of raw materials requires the reader to recognize that approximately 1.6 t of dry raw materials are required to make 1 t of clinker. This number can vary depending on the amount of kiln dust wasted. Also that to produce 1 t of portland cement required .94 to .96 t of clinker, the remainder being gypsum or anhydrite. A ton of masonry cement or pozzolanic cement re- quires considerably less clinker.

The National Steel Pellet Company is owned by National Steel Corporation, managed by the M. A. Hanna Company and located near the town of Keewatin in northern Minnesota. Annually, the concentrator processes 15.24-Mt (15.0 million It) crude taconite with eight 8.2-m diameter by 5.5-m long (27 ft x 18 ft) wet semi-autogenous (SAG) mills to produce 4.57-Mt (4.50 million It) of iron ore pellets. To improve operating costs and primary mill grinding circuit efficiencies, a task force on liners was formed with the operation, maintenance and engineering staff at N.S.P.C. along with the local supplier of mill liners. Results of the task force recommendations since 1980 along with improvements made in the primary circuit operations, are presented. Mill Liner Design and Metallurgy Changes The primary mill liner task force was formed in 1977. Its function was to improve the operating costs and milling efficiencies of the ten 8.2-m x 5.5-m (27 ft x 18 ft) wet SAG mills that were operating at the time. The SAG mills are driven by two 3500 HP motors drawing a total of 5450 kW at 76% of critical speed.

Although the first published optimum pit limit design algorithm by Lerchs-Grossmann dates back to 1965, the majority of the open-pit mines in the U.S. has not as yet adopted such a rigorous optimization algorithm. Instead, the floating cone approach initially developed by Kennecott Copper in 1961 is still used today for various reasons. However, shortcomings with the floating cone algorithm, particularly for precious metal deposits with erratic mineral distributions, have provided a renewed interest in optimizing algorithms.

"Powerful mining systems typically generate high-level noise that can damage the hearing ability of miners. Engineering noise controls are the most desirable and effective control for overexposure to noise. However, the effects of these noise controls on the actual hearing status of workers are not easily measured. A tool that can provide guidance in assigning workers to jobs based on the noise levels to which they will be exposed is highly desirable. Therefore, the Office of Mine Safety and Health Research (OMSHR) of the National Institute for Occupational Safety and Health (NIOSH) developed a useful tool to estimate the hearing loss due to occupational noise exposure in a systematic way and evaluate the effectiveness of developed engineering controls. This computer program is based on the ISO 1999 standard and can be used to estimate the loss of hearing ability caused by occupational noise exposures. In this paper, the functionalities of this software are discussed and several case studies related to mining machinery are presented to demonstrate the functionalities of this software. INTRODUCTIONPeople regularly exposed to noise can develop hearing loss at different levels of severity depending on a number of factors. The resultant hearing loss can affect one’s ability to understand speech, learning, acoustic signal perception, or appreciation of music. Hearing loss caused by overexposure to noise has long been recognized in literature. However, it still does not get serious attention by those working in noisy environments because the loss of hearing ability is a gradual process and workers are often unaware until the damage becomes an apparent problem. In the United States, occupational hearing loss is one of the most common work-related illnesses. Depending on the definition of exposure and impairment, in 2012 the National Institute for Occupational Safety and Health (NIOSH) estimated that there are between 5 and 30 million workers in the U.S. who are exposed to noise levels at work that put them at risk of hearing loss.4 In the mining industry, noise-induced hearing loss (NIHL) is particularly severe due to the fact that heavy-duty machinery is widely used to improve production efficiency, and this machinery can generate much higher-level noise than the permissible level defined by regulation. As a result, the mining sector has the highest prevalence of hazardous workplace with high noise exposures (76%) among all industrial sectors."

INTRODUCTION This method is a variation of cut-and-fill mining where either the wall rock or the vein is too unconsolidated to permit safe mining by conventional cut-and-fill methods. Basic development consists of driving a 2.4 x 2.7-m (8 x 9-ft) haulage heading laterally in the footwall side of the vein. Crosscuts are driven over to the vein at 91-m (300-ft) intervals. Then the vein, at each inter¬section, is mined out to a distance of about 9 m (30 ft ) above the track level. After the installation of an ore chute, scrams or intermediate drifts are driven in both directions along the vein. Mining then starts imme¬diately below the level bounding the upper limit of the stope. A cut varying between 3 and 4.2 m (10 and 14 ft) in depth and mined for a distance of 45.7 m (150 ft )along the strike is taken from the orepass in each direction (Fig. 1). In this mined-out void, the floor of the cut is equipped with 203 x 203-mm (8 x 8-in.) caps hitched into the wall with steel saddle hitches installed on 1.5-m (5-ft) centers. Double wood stringers, 152.4 x 152.4 mm (6 x 6 in.), are then installed on 1.2-m (4-ft) centers longitudinally down the length of the stope. Reinforc¬ing wire mesh is also placed, overlying each mat of timber. Directly above the stringers, stulls are installed from hanging wall to footwall to hold the timber in place during the sand-filling operation. Then a cement¬sand mixture in a ratio of about 1:10 weight is poured to a depth of about 2.4 m (8 ft). The entire cycle is then repeated down to the scram drift. Another variation of timbering is illustrated in Fig. 2. There is a vein width of approximately 5.5 m (18 ft) with the same unconsolidated ground conditions. The primary difference between handling this situation and the previously described undercut-and-fill method is that standard timbered sets are installed in each excavated stoping void rather than stulls. Once again the basic method is to work down the ore block from an upper timbered drift to a lower timbered drift. Cuts are taken from a timbered raise with a chute on the stope side. Each piece of ground excavated is essentially two sets high, 5.5 m (18 ft), by two sets wide, 5.5 m (18 ft), by the length of the cut along the strike of the vein. The timbered sets are 2.4 x 2.4 x 2.4 m (8 x 8 x 8 ft). The net result is, therefore, two sets of timber on a top floor separated by doubled-up 76-mm (3-in.) lagging from

Foundry sands used to make molds and cores have been very important to foundrymen since metal casting began hundreds of years ago. They are now used in two basic ways, either in an uncured green state, grain-bonded with clay, or in a set cured state achieved with sand that, mixed with resins or oils, is cured by baking or by chemical reaction. The term sand, as generally used in foundry applications, may best be defined as material composed of granular particles of mineral matter, ranging in size from 0.5 to 2 mm in diameter. The particulate nature of the material may be the result of natural disaggregation or result from crushing and screening of rock or ceramic materials; crushed and screened products are sometimes referred to as "manufactured sand." Based on their origin, the raw materials for foundry sands will vary in grain shape, grain composition, relative surface, grain size, and grain distribution patterns. These properties, in addition to their chemical analyses, sintering point and expansion characteristics play an important part in the choice of sands used as the base molding or core aggregate in metal casting. Sands with different origins may be blended to produce specific compositions and grain size distributions. Early raw material requirements based on silica sand were simple, but as sand/core/mold technology became more sophisticated and binder technology continued to be developed, the physical properties of foundry sands became more critical. Silica sand is composed of quartz, along with, in most cases, small amounts of feldspar, mica, clay, and other common minerals. Although silica sand is still by far the most widely used base material in production of molds and cores used in metal casting, other natural mineral sands have their own unique characteristics and fill an important niche in special applications. As a result, the term sand, as applied to raw materials used in metal casting, has been logically extended to include granular materials composed of a group of minerals other than quartz. With increased use in advanced foundry technology, natural sands composed predominantly of zircon, chromite, staurolite, or aluminum silicate minerals and of granularized chromite or olivine, have become more than just alternate materials and are now commonly classified as sands when the grains are sand-sized. Foundry sand may therefore be considered under two categories: silica sands and non-silica sands.

Batch leaching of Ohio #1, Ohio #4A and Ohio #5 coals was conducted with the thermophilic. sulfur, oxidizing microorganism: Sulfolobus acidocaldarius. Four different size fractions of each coal, with median sizes of 508, 200, 90, and 49 microns, were subjected for 10 days to leaching in a 5% coal-water slurry at 72°C and a pH of 2. Release rates and extent of sulfate and iron I1 and I11 ions from the coals during the leaching process and distribution of sulfur forms (sulfate, pyritic and organic) in the coal samples before and after treatment were determined. Data indicated that Ohio #1 coal, with an initial sulfur content of 0.6%, showed no significant changes with leaching in total sulfur content or forms of sulfur. The Ohio #4A coal, with initial total sulfur content near 5%, yielded product, for sizes below 200 microns with 90% reduction of pyritic sulfur while the extent of organic sulfur removal varied with coal particle size. Results similar to those for Ohio #4A were observed on the Ohio #5 coal, which had an initial total sulfur content of 3% and a final sulfur content near 1%. The findings were interpreted in terms of basic biological, chemical and physical mechanisms.

The physico-chemical interactions among minerals such as olivine, chromite and serpentine; the most commonly present ions in solution such as Ca and Mg; and flotation reagents such as EDTA and oleic acid were investigated. The study was conducted via release tests, electrophoretic measurements and microflotation of selected minerals coming from the Kukes deposit in Albania. The results were confirmed in beneficiation tests on two ores, one from the Kukes (Kalimash) deposit and the other from the Rizo deposit in Greece. It was demonstrated that, when Na-oleate is used as collector, chromite, olivine and serpentine can be separated at pH 2.5 and/or pH 5.5 when employing ethylenediamine tetraacetate (EDTA) on previously deslimed material. This approach ensures flotation concentrates with a Cr203 content better than 50% with a recovery of more than 75%.
