After multiple tailings dam failures in recent times, the owner organisations have made increased efforts to step up the governance systems to industry best practise in the design, construction and operational management areas. The measures include but are not limited to, site investigation, rigorous peer review (both internal and external), certification through appropriately qualified personnel and improvement in operational management practises. While a lot of focus is on design and operational practices, the construction stage requires adaptation (both in terms of materials and approved design) and immediate decision making on site. Work undertaken in these areas is mostly irreversible and sometimes poses serious threat to the future TSF raise/ expansion planning and may lead to reduced life of the TSF and additional risk to the Owner.This technical paper will discuss some challenges and highlight the importance of appropriate processes to achieve a successful construction regime. The process will include a robust quality control/ quality assurance process, a team of designer-constructor-owner with suitable skills and technical understandings and robust documentation of the entire construction process from day 1 to the completion of construction.CITATION:Kundu, A and Laxman, M, 2018. "Bi- and multilateral banks as well as export credit agencies in the OECD countries, but also numerous commercial banks, last year committed themselves to compliance with the World Bank’s standards. These environmental and social guidelines, also known as the Safeguard Policies, are the strictest international standards in the area of financing of major investment projects. The Safeguard Policies are procedural and quality guidelines by means of which socially and environmentally relevant impacts can be taken into account in the planning and implementation phase of a project. Objectives of the Bank Safeguard Policies are to ensure that environmental and social issues are evaluated in decision-making, to reduce and manage risk of projects and programs, and to provide a mechanism for consultation and disclosure of information. They are often referred to as the "10+1 Safeguard Policies," since ten of the guidelines address environmentally and socially relevant issues, while one guideline is exclusively concerned with public participation ("Public Disclosure"). In the presentation, the transparency of risk management measures and emergency response plans in pipeline projects will be discussed as an indicator of sustainability. The discussion is to be carried out on the basis of the results of an evaluation of several major pipeline projects, which have been financed at least in part by multilateral banks over the past few years. The controversies surrounding some of these projects due environmentally and socially relevant impairments connected with the investments have also been addressed by the Extractive Industry Report of the World Bank. Pipeline construction was the investment type chosen for the examination to be presented here, since pipelines frequently start in remote, unspoiled regions, and their routes often cross various natural zones and areas with various social structures. The question of safety during construction and operation of the line is always central to these projects; not only for the responsible authorities but also for other societal groups involved, such as environmental associations."

The Antamina Mine is located in the mountainous region of the Ancash Department of Peru at an elevation of approximately 4200m.The 120 km main access road crosses the headwaters of numerous high-value trout streams and is adjacent to pristine lakes, some of very high ecological and scenic value. In addition, the mine discharges mine water and tailings supernatant to the Antamina and Ayash watersheds. These streams provide the primary water supply for irrigation by local residents and provide excellent fishery habitat. The rock walls and earthen diversions constructed by historic and current indigenous residents of Peru are world-renowned and local residents continue this tradition, passed down from the Inca Civilization, for both agricultural purposes and water distribution. The key elements of sustainable development – economic, environmental and social development have been incorporated into Antamina’s sustainable development program through integration of indigenous knowledge and capabilities into enhanced storm water, erosion reduction and sediment control systems. Examples of incorporating indigenous technologies and knowledge into cost-effective and environmentally sound environmental controls are:(1) placement of rock walls on steep terrain to reduce slope length and erosion, (2) installation of rock walls (porous rock check dams) within elongated sediment ponds to increase detention time, (3) construction of rock-lined ditches to convey clean water (4)use of rock walls, with or without geotextile, to function as a seep berm, and (5) appropriate selection and usage of native Andean species for protection of canals and sediment ponds, and steep slopes.

Federal regulations require that refuge alternatives (RAs) be located within 305 m (1,000 ft) of the working face and spaced at one-hour travel distances in the outby area in underground coal mines, in the event that miners cannot escape during a disaster. The Mine Safety and Health Administration mandates that RAs provide safe shelter and livable conditions for a minimum of 96 hours while maintaining the apparent temperature below 35 °C (95 °F). The U.S. National Institute for Occupational Safety and Health used a validated thermal simulation model to examine the mechanisms of heat loss from an RA to the ambient mine and the effect of mine strata composition on the final internal dry bulb temperature (DBT) for a mobile tent-type RA. The results of these studies show that 51 percent of the heat loss from the RA to the ambient mine is due to radiation and 31 percent to conduction. Three mine width and height configurations and four mine strata compositions were examined. The final DBT inside the RA after 96 hours varied by less than 1 °C (1.8 °F) for the three mine width/height configurations and by less than 2 °C (3.6 °F) for the four mine strata compositions.

ABSTRACTThis paper describes the main testing procedures that should be carried out to evaluate the capacity and capability of the service brakes on the large-size diesel-electric trucks now in common use at open-pit mining operations. Brake testing programs in general are described, and then consideration is given to different techniques of service brake testing, brake performance evaluation, braking energy and typical performance characteristics.IntroductionThe first diesel-electric truck with a capacity of about 85 tons was introduced in 1963 by Unit Rig and Equipment Company of Tulsa in cooperation with General Electric Company, who successfully developed and supplied the most important component- motorized wheels. Since then, its popularity and numbers have continued to grow, and now more than 3700 such units are in operation in mines around the world. In Canada alone, their number is more than 700, including about 150 in the Province of Newfoundland and Labrador. Similarly, their size has also grown from that initial capacity of 85 tons to 350 tons.Although the introduction of the electric trucks proved to be invariably economical, their indiscriminate use was not without severe consequences. Many runaways and even fatalities occurred when the dynamic retarder failed while the truck was travelling loaded down severe grades. The service brakes in such cases proved incapable of stopping runaway trucks, mainly due to their limited capability and the fading caused by excessive heat produced during the act of braking itself. This eventually led the various governments, the open-pit operators and the manufacturers to seriously question the braking capability as well as function of both service brakes and dynamic retarders. "Paper presented at the conference Economic geology in Europe and beyond II, models for mineral deposits in sedimentary basins, held in Keyworth, UK, 13-14 April 1994. The Mississippi Valley-type deposits occur in massive, homogeneous dolostones from which diagenesis has obliterated all primary sedimentary structures. The mineralisation forms irregular, lens-shaped stratabound bodies, mostly in the lower part of the dolostones. They are composed exclusively of sphalerite, galena, pyrite, marcasite and barite, with white sparry dolomite a common cementing phase. Mineral zoning can be recognised from pyrite and marcasite in the east to barite in the west. Sphalerite, the most common sulphide, occurs basically as colloform and banded crusts and galena as isolated cubic or skeletal crystals, mainly in the centre of the banded botryoidal sphalerite. The combined observations of the deposits' geology, geochemistry and ore petrology suggest a three-stage genetic model: carbonate sedimentation in a shallow-water environment; dolomitisation, probably after compaction, by fluids of connate origin and lateral evolution; and mineralisation by migrating solutions, perhaps related to oil-field brines, that originated in deep zones of the basin during diagenesis of the basinal mudrocks in the NE"

The liquid-level gage described in this report is an instrument designed to locate liquid levels in wells rapidly and accurately and, with auxiliary equipment described, to follow moving liquid levels automatically. The depth at which the instrument makes contact with the liquid in a well is indicated at the surface by a depth meter on the cable to which the instrument is attached. When the gage is used to follow a surging moving level, the liquid depth is indicated continuously at the surface. This report contains a description and illustrations of the gage and outlines the operating procedures. The instrument is 1-3/4 inches in diameter and approximately 6 feet in length and may be operated through 2-inch tubing. The gage consists essentially of a float that moves upward with respect to the rest of the assembly, causing a change in the flow of electric current from the surface when the liquid level is reached. The instrument has been tested under a variety of conditions and has been found to be sensitive to changes in liquid level of 1/2 inch. Its accuracy is limited principally by the accuracy of the wire-line depth indicator used. It is not affected by pressure or temperature, and it can be run in open hole or under pressure through a lubricator. It employs only one electric circuit and can be run on an Amergraph single-conductor cable (type 1-H-0) ,

This paper discusses the behaviour of large-span tunnels and caverns in hard rock in a major underground storage project. In-situ stresses were measured using hydraulic fracturing during site investigation and 3-D overcoring during construction, with both showing a horizontal stress ratio of 2-3 times the vertical stress. Rock reinforcement design was based on the Q-system combined with numerical modelling for special design cases. Tunnel design and performance were checked and monitored by in-situ instrumentation during rock excavation, including measurements of deformation, convergence, and load on rock bolts. Deformation measurements using borehole extensometers included one borehole drilled into the virgin rock before rock excavation, which provided a direct and very insightful comparison with the various deformation measurements made during rock excavation. These measurements coupled with numerical calculations have proven that the high horizontal stresses to be very favourable to the stability of the large openings.

The mining industry is gradually realizing that safety education and safety inspection are economic factors that may not be overlooked as a means of reducing operating costs . Many mine operators , however , have failed to realize the benefits of safety , and the efforts of the more progressive mining companies are offset by the carelessness of neighboring companies when the accident records of a community or a State or the nation as a whole are considered . This tendency is indicated by the data collected by the safety division of the United States Bureau of Mines . During the fiscal year ended June 30, 1930 , relatively definite information: was secureä concerning 34 mine explosions , 24 mine fires , and 29 miscellaneous accidents ; these accidents are by no means all of the explosions , fires , and miscellaneous accidents which occurred in the mines of the United States during the period; but the data on the fires and explosions constitute a record of all of those types of accidents that were called specifically to the attention of the safety division , and the miscellaneous accidents include those on which definite , detailed data were obtained by the safety division .

This paper presents a case history of an engineered, monitored, and tested program that provided an effective and satisfactory basis for the completion of pile foundations for Twin Dolphins 1 at La Peninsula Condominiums located in the Isles of Capri in southwest Florida, U.S.A. An 8-story building (withl-story parking underneath) along with recreational facilities (such as a swimming pool and tennis courts) had been planned for final construction. Following an evaluation of various foundation alternatives, a precast prestressed concrete (PPC) pile-supported system was selected. The building foundations were designed for pile capacities of 445kN (50 tons) and 890kN (100 tons) in compression and 133kN (15 tons) in uplift. A test pile program was performed to provide site-specific pile driving data, verify design assumptions, determine production pile lengths, and evaluate pile driving criteria. A total of 24, 356 mm (14-in.) square, 19.8 m (65 ft) PPC concrete piles were driven and dynamically load tested utilizing a Pile Driving Analyzer (PDA) within the building limits. Dynamic monitoring during probe pile installation and restriking was performed to estimate the ultimate capacity of the piles and to evaluate driving stresses during installation. CAPWAP analyses were preformed on PDA data to better estimate pile capacity, resistance distribution, and predict pile freeze (increase in pile resistance with time) and thus optimized the production pile lengths. Upon completion of probe pile installation, 12 of the probe piles were tested using the Pile Echo Tester (PET) function of the Pile Integrity Sonic Analyzer (PISA). PET testing was performed to evaluate the integrity of piles which had exhibited high driving stresses during PDA testing. Information gathered during the probe pile driving program was effectively utilized as part of the overall quality control program to install a total of 539 PPC piles (including 24 probe piles) within the building limits at the project site to depths ranging from 9.1 to 19.8 m (30 to 65 ft) below the existing ground elevation as part of the foundation support system. Prior to installation of probe or production piles, a termination criterion was established using 1-dimensional wave equation analyses in addition to the design pile embedment requirement. The predicted pile capacities compared very well with the capacities from the CAPWAP analysis for probe piles. Use of geotechnical instrumentation and PDA assisted technology enabled each foundation support element to be constructed to its optimum capacity and at optimum cost. Furthermore, by using PDA the consultant was able to quantify a difficult to measure geotechnical phenomenon termed "pile freeze" which eliminated costly overdriving and unnecessary splicing of piles.

The Thompson Mill has a capacity of 15,000 T ID of ore and produces two grades of nickel concentrate, copper concentrate and occasionally graphite concentrate. Up to July of 1985 we processed two distinct ore types: T-1 ore, averaging 2. 8% Ni and . 2% Cu and Pipe Open Pit ore, averaging . 8% Ni associated with peridotite and serpentine. Presently our ores from T-1 mine and Thompson Open Pit are of the same type and are processed together. (Figure 1)! n October, 1982, we decided to install an Outokumpu Courier 300 on-stream analyser on an initial 6 streams. The equipment was delivered to the mill on May 2, 1983 and the first analyses were generated by June 17th. The calibrations were 35% complete by the mid-July summer shutdown and were completed in the fall. 1. 2 Analyser Slurry Feed System:The key to successful on-stream analysis is delivery of the sample streams to the analyser.Our system uses pressure-thief sampling for collection. (Figure 2) Initially we used thief samplers modelled after the Outokumpu design, with turbulence bars upstream of the collection point. These units have been replaced with simple laterals as they have worn out. Our thief samplers are mounted vertically on pump discharges and we believe sufficient turbulence exists to ensure representative sampling.

The objective of this research was to devise and test an apparatus to rapidly evaluate the cohesive and frictional strength of coal in situ, in order to increase the safety and percentage of coal recovery from underground coal mines. During the first year device was designed utilizing the bore-hole shear test principle from soil shear strength testing: Two opposing corrugated plates are forced outward to exert a controlled contact pressure against the sides of a previously bared hole, and then the plates are very slowly pulled along the axis of the hole to cause shearing of coal in contact with the plates. Results with the device are expressed in terms of a friction angle[_] and a cohesion c. The [ ] angles obtained from an Iowa coal arc in close agreement with and fall between results from triaxial and direct shear laboratory tests on the same coal. The measured cohesion was found to be much less than from the comparable laboratory tests. It may be increased by pre-stressing and other techniques to embed the shear plate teeth for the first shear test point.

Laminated rock can delaminate and fail under certain stress states. Delamination-caused roof fall and cutter roof failure is a common failure mechanism in roadways in Pittsburgh coal seam due to the existence of laminated shale roof. Two-dimensional numerical models were developed at laboratory scale based on the bonded particle method in order to simulate the delamination with particle flow code. In these models, an assembly of bonded particles and parallel weak planes represents the laminated rock. This model calibrated the micro-parameters of the bonded particlematerial and weak planes with laboratory data. The mechanism of fracturing and delamination in a laminated specimen was investigated using unconfined and confined compression test and unsupported roof compression test. The numerical results show that, under a constant horizontal stress, delamination occurswhen the stress is much lower than its compressive strength. The confined compressive test indicates that delamination is restrained by applying confining stress since delamination-caused failure does not occur during any stage of the confined compressive test. Unsupported roof compression tests showed propagation of cutter roof failure with initiation of cracks and delamination at the intersections of the roof and ribs. An outer failed zone and inner failed zone form separately. The separated zones connect and form an opening in the roof. The presented numerical results provide additional insight into process of delamination.

The paper highlights development of tunneling methodology in the UK during the years 1990-2000 using the example of two major tunneling contracts where there solution of difficult tunneling problems during construction has benefited the industry as a whole. The paper deals with specific difficulties on two Contracts. The London Water Ring Main 80 kms (50 miles) long tunnel average internal diameter 2.54 m (9 ft4 in) and the Brighton Stormwater Tunnel 5.2 km (3.2 miles) long tunnel with an internal diameter of 6 m (19 ft 8 in). The results of human endeavour and technological innovations will demonstrate the giant strides made in tunneling methodology in the last decade of the 20th Century.

"Ore sorting has been quite successful in providing a separation means in the rock size range of 1 to 6 inches; a range where mineral dressers do not have many options. The industrial minerals and recycling worlds have embraced this technology. However, ore sorting has not found wide application in the base or precious metals mining industry. One reason for this is thought to be the reliance of most sensors on rock surface characteristics, not very useful in case of sulphides, non-existing in case of gold. Microwave heating, however, is specific for sulphides and graphite (or carbonaceous matter) embedded in the rocks; washing or even drying does not appear necessary. Separation can be done afterwards by already existing ore sorting technology using infrared sensors. Test results on six Nevada gold ore samples are presented in detail. Some simple test procedures are also presented for those wishing to explore the microwave potential of sulphide sorting in their own operations.INTRODUCTIONMineral processing in the base metals industry most often involves upgrading of the ore, concentrating the valuable mineral fraction by classic methods, such as flotation. Since the liberation of minerals in the ore occurs mostly at or below the 100 micron level, the industry has been living with fault lines, so to speak, between blast hole spacing (10,000 mm), run-of-mine ore size (100 mm) and beneficiation size (0.1 – 0.01 mm). The acceptance of ore sorting will inevitably lead to the revisiting of the following questions: “Exactly how much below ore grade rock ends up in the mill because the geologists or the miners cannot define the resource boundaries more precisely? How can the economics of dilution be improved? How many ore grade rocks are lost with the overburden?”"

The use of phosphate depressants in the cationic flotation of silica from francolite in the Florida phosphate industry is rarely practiced. The occasional use of gelatinized starch derivatives and pH adjustments to assure a slightly alkaline flotation feed slurry pH have been used in some processing plants to improve selectivity. This paper presents a preliminary laboratory comparison of several potential phosphate depressants for use with the flotation of siliceous phosphates. It was found that starch is ideal for floating fine (-35 mesh) silica from phosphate, while sodium tripolyphosphate may be the best for coarser (+35 mesh) feeds.

The amphibolite-facies metabasaltic rocks of the Mine Mafic Package at Plutonic Gold Mine, Marymia Inlier, Western Australia, contain an estimated total endowment of 12.2 Moz of Au, which includes 4.71 Moz of past production and 2.77 Moz of reserves and resources; the remainder comprises lower-grade inventory. Over 65?000 multi-element analyses of whole-rock core and face channel samples from the underground workings of the deposit have been collected by portable X-ray fluorescence (pXRF) using a handheld Innov-X Omega pXRF unit. The analyses were performed on pulverised samples through paper bags, and were corrected using a pulverised, matrix-matched, reference standard. This method of data correction compares favourably with conventional XRF analyses performed on the same samples.A preliminary study based on pXRF analyses from a small number of drill holes found that a geochemical stratigraphy was evident in the pXRF data, and that this stratigraphy strongly controlled the location of Au mineralisation. The present study incorporates a much larger pXRF data set that samples a greater volume of diamond drill core and face channel samples and presents the data in a three-dimensional (3D) framework.This data set allows an investigation of the mineralogy of Au mineralisation with varying geochemical associations across the deposit. Historically, there has been a strong relationship identified between As and Au, which has caused issues with metallurgical performance, rendering some areas of the deposit only marginally economic. Utilising seamless data integration of the pXRF data set allows for recognition of the different styles of Au mineralisation using elemental concentrations and ratios from the multi-element analyses (Au:As, Au:As:Cu, As:Cu vs. K:V), and to visualise the distribution of these different mineralogical associations in 3D. The results of the 3D modelling and geochemical characterisation of the different styles of Au mineralisation are currently the focus of an extensive metallurgical testing program, the preliminary results of which are presented here.

SAFEX International, as the name suggests, is a global organisation, which has the fundamental objective of improving the safety of operations concerned with the manufacture, storage, transport, and use of commercial and military explosives and pyrotechnic products throughout the world. It is a nonprofit making organisation of manufacturers of explosives from all parts of the world, founded in 1954 with the aim of exchanging experiences within the explosives industry. Every third year SAFEX organises a Congress during which members present papers on selected themes and discuss reports of significant incidents that occurred since the previous Congress. Admission is for SAFEX members only, thereby creating a safe environment for discussing problems “within the family”. The XV Congress will celebrate the Association’s 50th anniversary in Geneva where it was founded. An overview of the Congress indicates its structure and covers aspects such as the training event, technical seminars and the themes for the plenary sessions and associated workshops. The paper also highlights the special event for the chief executives of member companies and the review of incidents that occurred in the industry since the last Congress. The Congress concludes with a social programme for delegates and their partners on the last day.

In 1984 and 1986, at the request of the U.S. Forest Service, the U.S. Bureau of Mines conducted a mineral appraisal of the 315,000-acre Lemhi Range study area. This area is located in Lemhi and Custer Counties, about 25 mi south of Salmon, Idaho, and is in the Northern Rocky Mountain physiographic province, Topography is generally rugged with elevations ranging from 5,600 ft to about 11,350 ft above sea level. Since the 1880's, approximately 500 mining claims have been located within the study area; about 400 were current at the time of this investigation. Also, part or all of 12 patented mining claims are in the study area. During Bureau field work, forty-four mineralized sites were examined. The sites had been explored by 80 adits, 14 shafts, and about 140 pits and trenches. At least five properties in the study area have had significant production; the adjacent Mohawk, Grace Phelan, and Snowbird claims had a combined total of 136,000 oz of silver and 67,000 lb of lead; production at the Brown Bull was about 14,800 oz of silver and 115,000 lb of lead; the Portland mine yielded an unrecorded amount of silver. The Hilltop mine, located a few hundred feet outside the study area, produced 125,000 oz of silver, 2.7 million lb of lead, 87,000 lb of zinc, and 62,000 lb of copper from mineralized zones that might extend into the study area. More than 10,000 lb of copper were produced at the Blue Jay mine from workings located about 1,000 ft outside the study area. The Bureau of Mines investigation did not reveal the presence of identified resources in the study area. However, several areas contain mineralized sites and are known occurrences. Some of these warrant additional work to further delineate the mineralization and possibly determine identified resources.

The mechanisms of corrosion and corrosion fatigue damages to sucker rods are examined from a fundamental electrochemical viewpoint, and the relationships of sucker rod microstructures to these damaging effects are defined. The common corrosive agents in crude oil production and storage are discussed as to types of attacks that they cause. The variations of fatigue strength with alloy content, surface preparation, and stress ranges are analyzed. Finally, conclusions are drawn as to how microstructures largely influence and control sucker rod performance. INTRODUCTION Greater than 90 per cent of the crude oil produced by artificial lift methods is produced through sucker rod pumping systems. In the USA each year more than $14 million is spent for sucker rods in new and old pumping installations. It is important to make this investment a wise one. Essentially, the sucker rod system is a long series of steel rods, operating under alternating stress loadings and under mechanical vibrations, which connect the surface power source with the subsurface pump. These rods often operate in a variety of corrosive media, including water, brines, carbon dioxide, hydrogen sul-fide, and air. Basically, this paper is concerned with the problem of the behavior of certain steels in various corrosive media under high and dynamic stress patterns. The main objective of this paper is to show how, once the stress effects and the corrosive environments are defined, one can still realize structural control and beneficial influence upon corrosion and corrosion fatigue. FUNDAMENTAL DEFINITIONS AND ANALYSIS In the corrosion and stress behavior of this solid state mixture of iron and iron carbide called steel, one must analyze upon what major factors does its service life depend and of what, exactly, is steel composed. The former question can be referred to two external factors and two internal factors. The two major external factors are: 1. the nature of the stress patterns — size and range of stresses present 2. the nature of the corrosion — erosion environment — the corrodent and/or wear involved The two fundamental internal factors that govern the strength and useful life of a steel both in normal duty and in heavy-duty corrosion service are: 1. the nature of the steel volume properties 2. the nature of the steel surface properties For the latter question normal steel is defined as a solid mixture of iron and iron carbide particles, generally with certain accidental and intentional elements present. (The amount of iron carbide present is determined by the carbon content of the steel.)
