"In the recent past, the mining industry has witnessed a substantial increase in the numbers of fatalities occurring at metal and nonmetal mine sites, but it is unclear why this is occurring. One possible explanation is that workers struggle with identifying worksite hazards and accurately assessing the associated risk. The purpose of this research was to explore this possibility within the mining industry and to more fully understand stone, sand and gravel (SSG) mine workers’ thoughts, understandings and perceptions of worksite hazards and risks. Eight mine workers were interviewed and asked to identify common hazards they come across when doing their jobs and to then discuss their perceptions of the risks associated with those identified hazards. The results of this exploratory study indicate the importance of workers’ jobrelated experience as it applies to hazard identification and risk perception, particularly their knowledge of or familiarity with a task, whether or not they had personal control over that task, and the frequency with which they perform that task. IntroductionAccording to the U.S. Mine Safety and Health Administration (MSHA, 2015a), 37 mine workers were fatally injured in accidents at metal and nonmetal mine sites between October 2013 and January 2015, following a record low of 16 in 2012 and in 2011. In 2014 alone, 29 mine workers were fatally injured at a metal or nonmetal mine site, and 52 percent of these fatalities occurred at a surface stone, sand or gravel (SSG) mine. In January 2015, Joseph A. Main, Assistant Secretary of Labor for Mine Safety and Health wrote to stakeholders in the metal and nonmetal mining community and identified several areas where improvement was necessary to promote the health and safety of mine workers. One area identified was an increased focus on “daily and effective workplace exams to find and fix hazards” (MSHA, 2015b).Hazard detection is challenging in the mining industry, despite the existence of regulation. Mining is a major undertaking involving the use of complex heavy machinery, equipment and processes as well as numerous and diverse worker activities that take place in a dynamic, challenging environment (Scharf et al., 2001). In other industries that are similarly dynamic and unpredictable, such as the construction industry, a large proportion of hazards go undetected (Carter and Smith, 2006). It is critical that miners are able to both recognize worksite hazards and accurately assess the risk associated with the hazards, because their health and safety depend on their deciding whether and how to remove hazards and mitigate risks."

Global practice in hydrometallurgical production of copper cathode by electrowinning is reviewed, based on individual plant operating data for 2018. Data from 29 plants were collected, representing 38% of world cathode copper production. Similar surveys were carried out in 1997, 1999, 2003, 2007, and 2013. This survey includes analysis of historical and current data. Evolving trends in operating conditions and performance, as well as equipment design and process technology choices, are analysed and differences in operating practices with location are assessed. Examples of recent technology and operational choices to increase productivity, improve copper quality, and decrease electrical energy consumption are discussed. Significant project expansions, particularly in Africa, and the consolidation of numerous small Chinese operations are also presented. INTRODUCTION Global trends and operating practices in copper electrowinning (EW) in 2018 are reviewed and evaluated. Similar world and African surveys were carried out in 1997, 1999, 2001, 2002, 2003, 2007, and 2013 (Robinson, Davenport, & Jenkins, 1997; Jenkins, Davenport, Kennedy, & Robinson, 1999; Davenport, Jenkins, Robinson, & Karcas, 2001; Robinson, Garbutt, & Davenport, 2002; Robinson, Davenport, Jenkins, King, & Rasmussen, 2003; Robinson, Davenport, Moats, Karcas, & Demetrio, 2007; Robinson, Sole, Sandoval, Moats, Siegmund, & Davenport, 2013). Contributing operations included sites from the USA and Mexico (9), Chile and Peru (6), Laos (1), Kazakhstan (1), and Africa (9). Despite fewer contributions compared with past experience, this survey nevertheless represents 2018 copper EW cathode production of 1.76 Mt, or about 38% of world production of 4.64 Mt (International Copper Study Group, 2017). This drop in permission to contribute is attributed to the more competitive nature of the industry and unprecedented production pressures; furthermore, several operations in Australia have shut down, while large operations that are currently commissioning or ramping up to full production in Africa and Asia declined to participate. Of the Chinese operations, which represent an increasingly important proportion of global electrowon cathode copper, particularly in Africa, only Tenke Fungurume (Democratic Republic of Congo; DRC) participated in this survey.

The connection between mineralogy and metallurgical performance in a plant was recognised long ago (Gaudin, 1939; Petruk, 1976; Petruk and Hughson, 1977; Cabri, 1981; Petruk and Schnaar, 1981; Kinloch, 1982; Peyerl, 1983, Baum et al 1989 for example). The modern toolbox that we now routinely use for flowsheet development took decades to develop (Lotter et al., 2002; Baum and Lotter, 2004; Lotter, 2011 for example). Many workers contributed considerable time and effort to this overall mission of connecting sampling, mineralogy and mineral processing to integrate the flowsheet development activities. Certain key milestones marked this journey, such as Gy’s treatise on the sampling of particulate materials in 1979, which was later added to by François-Bongarçon, Pitard, Minnitt, and Holmes for example. The development of QEM*SEM by Gottlieb et al. of the MLA by Gu et al. constituted a huge leap forward in accuracy and mineral measurement capacity, as well as a reduction in standard errors of mineral characterisation. The advancement of the microprobe to an automated platform made a significant difference to detailed compositional mineralogy. This led to a strong position from which to draw meaningful processing implications from the mineralogical data. Reproducible flotation testing and reliable plant survey material for characterisation was dealt with by, amongst others, High Confidence Flotation Testing ( Lotter, 1995: Runge, 2010, and Bradshaw, 1997); and Statistical Benchmark Surveying (Lotter, 2005), with earlier prototypes which were steadily improved (Lotter, 1994; Restarick, 1976). The sterling work of Napier-Munn in engaging sound statistical methods with mineral processing led to the successful design and use of methods that could identify and quantify small recovery gains in plant scale trials. This history and present capability are reviewed, including leverage into asset management.

"The asbestos mining industry in the Eastern Townships of Quebec has early recognized the undesirable side-effects associated with mineral production, such as air pollution, noise and unsightly waste dumps. In order to efficiently achieve the industry's aim of eliminating or minimizing these objectionable features of mining, all of the asbestos producers in Quebec interchange information, knowledge and design dealing with the environmental control measures through the Environmental Committee of the Quebec Asbestos Mining Association.The paper describes what has been done and what is planned for the future in the various problem areas, such as control of dust emitted during the open-pit drilling operation, control of dryer stack emissions, control of the environment in ore, fibre and tailings handling operations, rehabilitation of the countryside covered by waste dumps, and many others."

The success of any blast is dependent on the accuracy of the blast design and the field controls used on the shot. This point cannot be over emphasized because when the geometry is not measured and/or field controls are not properly implemented, it can often have devastating effects on the final blast results. Thus drilling accuracy, tiont row burden control, explosive confinement, energy distribution, explosive selection, timing and structural geology become critical parameters in the design of successful blasts. Understanding the actual dynamics of the blast and why certain results were achieved is the fast step in optimizing a blast design. As stated by R. Frank Chiappetta, “Of all the blast monitoring systems available today, high speed videography still remains as the best stand alone instrumentation system for blast diagnostics.” Over the past years I have analyzed hundreds of blasts using either high-speed lowcams (500 frames per second) cameras or standard digital (30 frames per second). These analyses confirm, without a doubt, that shot geometry, in combination with structural geology, is the most important factor in overall blast results. Drilling accuracy is the single most important component in efficient blasting. Having equal mass associated with an equivalent energy is the basis in which blasts are designed. When drilling inaccuracy or borehole deviation occurs, not only is the energy unevenly distributed, but the basis of all design calculations becomes invalid. Over the past several years, quarry and mining operations have employed the use of boretracking measuring devices to measure borehole deviation and hole toe locations. In cases of operations using top hammer drills, it was discovered that the amount of deviation was significant and far greater than they believed they had. Set up error, improper drilling techniques and bit wear were some of the contributing factors. Through video anaysis of boretracked blasts, blast results were directly linked to drilling ..accuracy. The comparisons here took into account blast design, energy factors and hole sequencing.

In situ, or in-place leaching is concerned with the extraction of metals from natural mineral disseminations without removal from their geologic location which, in many cases, amounts to the reversal of the deposition process accompanied by a presumed increase in velocity of the processes, if the project is to be profitable. Solution mining processes involve removal of minerals such as sulfur, common salt, and potash, generally leaving cavernous voids, in contrast to in situ leaching which term is applied to the selective dissolution of a mineral from its gangue without sensible disturbance of the formation. Where such a process can be operated, the advantages are manifold and have been so recognized by miners from the earliest days of mining. Recent technological developments have encouraged a reassessment of the status of in situ leaching, and it now appears possible to extend the application of the method from a few isolated special situations to many uranium and copper deposits. Anaconda's interest in the techniques stems from long experience with vat and heap leaching, 'dump leaching, and natural leach activities associated with several established mines. Increasing activity through-out the industry has been noted, particularly in leaching low grade pillars and the peripheral zones of operating mines, often by the introduction of leach solutions via boreholes drilled from surface. Reactivation of dormant mines] using similar techniques has been successful. Over the long term, environmental improvement activity will lead to excess sulfuric acid production, even to a disposal problem. The cost of leach solutions and reagents must be reexamined in this light. The zinc-lead-silver mineral resource at Lady Loretta ranks amongst the worldÆs richest when measured in terms of in situ metal grades. However, the project remains undeveloped 28 years after its discovery in 1969. The primary obstacle to development has been the fine and complex grain structure of its ores, which make flotation concentration inefficient even with fine grinding. The size and remote location of the deposit have also hindered exploitation. BUKA planned to directly fume the volatile target metals from the ore when it acquired the project. Intensive, post-acquisition modeling showed the innovative processing route to be uneconomic, notwithstanding high metals recovery and low waste content in fume product. Progressive modifications and supplements to the underlying fuming process enhanced economics and resulted in a new mine-to-metal processing route for zinc-lead-silver ores. The proprietary, three-stage BUKA Zinc Process offers technical, economic and environmental advantages. It is expected to find widespread application in the treatment of ores and residues. However, neither continued R&D nor application to Lady LorettaÆs currently delineated resource can be justified under prevailing market conditions. The objectives and results of each step in the process development sequence are outlined with reference to their impact on the commercial viability of the Lady Loretta project.

The natural cementation strength of retorted oil shale is the focus of this investigation. The optimum retorting temperature and residence time to obtain natural cementation strength in retorted Green River oil shale from western Colorado were determined as follows: [Optimum Conditions for Natural Cementation Maximum Residence Obtained Retorting Time at Compressive Mode of Temperature Temperature Strength' Retorting °F °C hr psikg/cm-2 Direct 1,520 822 2.0 270 19.0 Indirect 1,530 832 2.9 325 22.8] 1 Scattered samples were tested at strengths as high as 463 psi (32.6 k /cm 2) (Run 8, indirect mode) and 406 psi (28.5 kg/cm2) (Run 48, direct mode), but these high strengths were not confirmed by nearby tests. Minus 0.5-inch (1.3-cm) Green River oil shale was heated in a differential retort under conditions simulating potential full-scale retorting processes. The variables investigated were retorting temperature, residence time, retorting mode, and shale grade. Samples of the retorted Green River oil shale were moistened to optimum plus 2% (25%) moisture, compacted to 95% Proctor density, sealed and cured at 100°F (38° C) for up to 40 days. Lower curing temperatures, which are likely in the field, would probably result in significantly slower development of cementation strength, and might lower the ultimate strength developed. However, these effects were not studied in this project. The cured samples were tested for compressive strength, and correlative equations were developed for the increase of compressive strength with curing time as a function of retorting temperatures and residence time. Petrographic analysis indicated that the cementing mechanisms may be the interlocking growth of acicular hydrated calcium aluminum sulfate crystals between the contacts of retorted shale fragments. Samples of the retorted shale were also tested for optimum Proctor density and moisture. Permeability and quality of the leachate of shale from typical runs were determined. The effect of use of retort rather than tap water on compressive strength and leachate after curing was determined. The retorted shale effectively filtered several mineral constituents from the retort water.

The explosives demolition of a post-stressed, steel reinforced, concrete road bridge in Australia’s capital city, Canberra. The bridge was 50 m from the new replacement bridge, 320 m from community structures and within five kilometres of the national parliament buildings. The job required multiple overlays of risk assessment, public relations, safe work procedures, specialised blast design, environmental predictions and flyrock controls. Project stakeholders included Worksafe, Roads ACT, bridge owners, consulting structural engineers, community groups and road users. The bridge precincts, including a major road underneath, had to remain operational to peak traffic in the days leading up to the demolition. The break-up and removal of debris had to be executed in just two days from Friday to have the road beneath re-opened for Monday morning city-bound traffic.In Canberra, all explosives demolitions are now viewed with particular caution after a demolition in 1997, during which a young girl was killed. Any approval requires that Explosives Regulations, AS2187 2006 and Worksafe processes are satisfied, and a convincing case presented for blasting as a safe and viable option.Our design used specialised shaped cutting and blasting charges to sever in situ concrete beams and tendons and to shatter components of the stressed concrete bridge assembly. We include comments on structural engineering reports dealing with pre-weakening mechanisms. The benefit of pre-weakening is that it reduces the amount of explosives needed to safely complete the job and facilitates quick removal of debris after the shot. The complementary skills of the team, each one an expert in his field, resulted in a highly successful demolition, praised by authorities as an outstanding example of controlled use of explosives.

The Peru based tin mining company, Minsur S.A., commissioned Ausmelt Limited to design, supervise construction and commission a tin smelter and refinery for production of refined tin metal. The plant is located 240km south of Lima, near Pisco, Peru and is operated by Funsur S.A., the wholly owned subsidiary of Minsur S.A.. Selection of the Ausmelt Technology followed a series of crucible scale test work and pilot plant trials conducted at Ausmelt's facility in Dandenong, Australia and two feasibility studies undertaken by Ausmelt in 1991 and 1993. Ausrnelt began the plant design in 1994 with the construction phase following in 1995. Commissioning of the plant commenced early in 1996 with a target throughput of 30,000 tonnes per annum of concentrates. In mid 1998 oxygen enrichment of the Ausmelt lance air was introduced to expand the capacity to the throughput of 40,000 tonnes per annum of concentrates. The process route involves the use of an Ausmelt furnace for primary concentrate smelting, followed by a conventional tin pyro-refining circuit to produce high grade tin suitable for sale. Further work continues between Ausmelt and Funsur to investigate the use of an alternative, more economical, fuel source and the installation of a second Ausmelt furnace. The paper reviews the project to date with details of ongoing operations and developments at the Minsur Tin Smelter.

‘Every mine is a pilot’ is an attitude which encourages the mining paradox of optimistic mine plans and equipment under performance; propped up by an environment lacking accountability. The premise that ‘every mine is a pilot’, appears to underlie the development and operating approach to most mines. It is the hypothesis of this paper that many mining companies, development teams and their managers largely ignore what is done elsewhere. Anecdotally, most planners don’t know whether the selected rates are high or low (rates based on theoretical models tend to be high – certainly higher than what most companies can achieve). Mines then consistently operate the equipment at rates well below targets; often more than 20 per cent below. Further to this paradox, mine performance data demonstrates the average mine is currently operating their equipment at levels of output not much more than half the output of those in the top ten per cent. On the whole mines don’t know the potential of their equipment and therefore don’t put appropriate plans in place to do better. After all, when ‘every mine is a pilot’ there is little reason to look to one’s peers for an indication of potential. This paper will outline why every mine is not a pilot and how mines do value some knowledge, but not as much as they should. This paper proposes a two-pronged approach to address this issue in the area of equipment performance. This approach is to:1. engage in better forecasting of equipment rates and better subsequent performance through benchmarking against industry standards2. provide conditions for more accountability.These parameters are linked and proper implementation of them will address the losses which many shareholders and financiers have incurred when investing in companies with unrealistic mining plans.CITATION:Lumley, G I, 2016. There have recently been executives at major metals companies who have recommended that Greenfield Projects not be pursued. Greenfield includes not only new geographical locations, but also new technologies and new types of feed materials. In the middle part of the 20th Century, two such projects were successfully commercialized to recover nickel from Cuban lateritic ores. One project (at Nicaro) used the Caron ammonia-ammonium carbonate leach and the other (at Moa Bay) a high pressure acid leach. How both of these projects dealt with these Greenfield issues will be reviewed, based on the original reports that described the research and development of the plants. This study will include a review of the critical ore characterization and sorting issues, as well as the extractive metallurgical, product recovery and equipment selection and sizing problems and how they were solved at each stage of project development, from laboratory through one or more pilot scale plants to start up and operation. In one case, a major revision of the plant was made after several years of operation to correct operating, equipment selection and materials of construction problems that hindered economical operation. These descriptions will provide a guide through the complicated and critical issues that in recent times have caused many companies to give up on similar development. Even with the more advanced analytical and computational tools available to today’s mineralogists, engineers and metallurgists, the basic concepts with which these projects successfully addressed their sets of problems are still required. In addition, upper management, now perhaps further removed from hands-on experience than was common then, will see how complicated problems require intense and detailed effort to avoid failure in the field and capital blowouts. Build it and we will make it run, kicking the can down the road and borrowing operating and design concepts incorrectly on technological issues, or nine year olds on a sugar high making financial decisions are not philosophies by which to succeed with Greenfield Projects.

I cannot help but be reminded, as I stand here today, of my boyhood years and heritage, both of which are inextricably tied to this very valley and are, in a: bay, apropos of the theme of this Keynote Session. I remember the scarcity of people. For example, 48, 000 in Phoenix and 436, 000 in the State in 1930 and 65,000 in Phoenix and 499, 000 in the State in 1940. Today Phoenix boasts a population of over 500, 000 and the State a population of 1, 500,000. I remember the' lack of facilities, goods and services, including gas and electric power. I recall stories of my grandfather who had the freighting or teaming contract to haul all the supplies and generating equipment, by wagon mind you, up the old Apache Trail to Roosevelt Dam. . .a three day trip from the nearest railhead which, by the way, was not Phoenix, for it did not merit a railhead on those days, but the village of Maricopa approximately 25 miles south of Phoenix. I remember chopping firewood, heating wash water on an open fire in a No. 3 wash tub at the rear of the house and doing my school homework by the light of a kerosene lamp just 25 to 35 years ago within what is now a large metropolitan area

With the activities of the Petroleum and Natural Gas Division geared chiefly to problems directly connected with the war effort , the emphasis of the work under oil and gas development and production was to make engineering studies of selected reservoirs for the Petroleum Administration for War , to determine the extractable quantities of components in them, and to study the characteristics of these components for use in the manufacture of aviation gasoline and other products . The needs of the Petroleum Administration for War and the operators for specific data from gas - condensate fields were fulfilled , and , in addition , engineering data along lines of a forward - looking program of research on reservoir behavior were collected , which should be of great value in the postwar period . It is planned to publish many of these data as soon as conditions permit . The work of the Secondary Recovery Section had as its objectives in- . creasing the rate of production of crude petroleum and , which is more important , preventing the premature abandonment of wells in " stripper " and marginal According to the National Survey of Stripper Wells by the National Stripper Well Association in cooperation with the Interstate Oil Compact Commission , a total of 67,460 stripper wells was abandoned from January 1 , 1935 , to July 1 , 1942 , inclusive , with an estimated loss in proved reserves of approximately 216 million barrels . This survey also indicates that over 1.2 billion barrels of oil would be recovered by primary production from more than 290,000 stripper wells then in operation , and an additional 2.5 billion barrels of oil could be recovered from them by secondary production methods . The work of the Chemistry and Refining Section was oriented towards the optimum utilization of crude oils in the manufacture of aviation fuels for spark-ignition and jet - propulsion engines . The work involved intensive studies of superfractionation , hydrocarbon analysis , and the engine testing of fuels . Particular attention was given to the effect of sulfur compounds in the crude oils and in the fuels made from them . The research on thermodynamics is concerned with the experimental determination of precise and accurate values of the fundamental thermodynamic constants of pure hydrocarbons and related compounds . The data obtained in these investigations will be utilized for devising and improving chemical processes in which petroleum hydrocarbons are the raw materials . Without such data , much time and expensive material and equipment may be wasted in attempts to obtain reactions under conditions that the laws of thermodynamics rule out of consideration . Thermodynamic data are useful and required in the petroleum industry for calculations of heat balances in the design of furnaces , fractionating towers , and heat- transfer equipment . It is of utmost importance , also , to know the heat requirements of cracking and other chemical reactions so that heat may be supplied or dissipated in the proper proportions . These needs for thermodynamic data are being recognized more and more by petroleum -refining engineers . The responsibility for the production of helium for the Navy , the Army , other governmental agencies and non-Federal users is vested in the Bureau of Mines by an Act of Congress ( 50 Stat . 885 ; 50 U.S.c. 161-166 ) . The Helium Subdivision of the Petroleum and Natural Gas Division continued to meet all demands for helium .

THE high copper price during the war stimulated the capacity to produce far beyond a possible normal consumption. The curves in Fig. 1 show this. The line YZ indicates, what the world production and consumption of primary copper nor-mally would have been, had not the 3 war upset the rhythm of industry. Granting that the maximum world production of 3,150,000,000 lb. in 1917 was obtained by crowding both the equipment and the grade of ore mined, yet in many instances addi-tional capital was invested, upon which the producer will be driven to earn at least a reasonable interest. During the period of deflation, or C liquidation, which we have entered, the consumption of copper will nat-urally be below the normal. Fig. 1 shows the actual consumption ex-pressed-in per cent. of normal : 1919, 96 per cent.; 1920, 94 per cent.; 1921, 70 per cent. The copper industry, consequently, faces: (1) Potential over-production capacity; I (2) Diminished consumption. To deny the existence of these factors would be illogical. To minimize the menace of this potential producing capacity might lead to regret. All past attempts at artificial control have not proved very successful. Obviously the remedy is to increase, if possible, the consumption of copper to a point where these factors cease to be dangerous. In ultimate analysis, this means: Produce an article so cheap and so good that no one can afford to use substitutes. The problem can be solved by: (1) Re-ducing price; (2) Improving product offered.

The frequency of mine fires during recent years, with an apparent increase in recent months, has focused attention on the necessity of reducing this hazard by every means possible. Investigation reports are available on 51 mine fires of electrical origin between August 15, 1949, and August 16, 1954. Nine other fires in the same period were suspected as being of electrical origin. Every mine fire is a potential hazard to underground workmen; it is remarkable that only six lives were lost as a result of these fires, since the injury list was very high. Modern mining methods, with increased power demands and extended distribution systems underground, involve greater electrical hazards and possibilities of costly production delays as a result of inadequate electrical service. Improvements in electric-power-distribution facilities in many underground coal mines is desirable. The purpose of this paper is to show the need for this improvement as revealed by studies and investigations of electric-power-distribution systems representative of those in a large number of coal mines.
