Gas wells have often intersected mining resources, but unconventional shale well drilling has more recently challenged mines to balance the risk of interaction due to higher pressures and larger quantities of gas. Full extraction mining, such as the longwall method, induces ground movement that may influence the casings if wells are drilled within the mining area. The possibility of a casing shear would lead to the risk of unplanned gas migration into the mine. This has the potential to quickly overcome the ventilation system and reach the explosive range which, if ignited, can have catastrophic consequences on the health and safety of underground workers. The utilization of network software to model mine ventilation is a common practice in the mining industry as a predictive tool for planning and monitoring and can assist with a well breach scenario over a large mining area. This work focuses on a well breach in between two longwall panels after second panel mining with gas entering primarily through the gobs of the adjacent longwall panels. Applying the network software Ventsim, a gob zone and ventilation network were created to better understand the distribution of gas within the mine and the limitations and effectiveness of a ventilation system. The model shows both a transient flow simulation and steady state concentrations throughout the mine ventilation system. Using a standard Pittsburgh coal seam longwall ventilation scheme, we find that the system can dilute a significant inflow of up to 700 cfm of methane.

Urban quarries offer one of the greatest opportunities for profit in non-metallic mining with limited financial risk IF properly evaluated, planned and developed to meet market needs. Location, with respect to market, is the prime consideration. Geologic considerations and quality of rock are nearly as important. Cost factors and return on investment (ROI) are the principal financial considerations. Risk of capital is determined by weighing all factors that should be considered and how well judgements are based on information acquired. It is a simple, true axiom of business that an investment must be founded on amount of capital required with its expected return plus profit, expertise required to operate and time involved. Urban quarries generally offer excellent stone product market opportunities. Costs of land with mineral containment and processing operations are much greater than in rural areas. Environmental problems have become increasingly severe. Competition from other quarries in the market area is usually directly proportional to the profit attractiveness. Risk of capital (initial, developmental and operational) can be evaluated and reduced only by thorough analysis of the entire situation. Time and money expended on planning and preliminary investigations probably represent the most productive investment of available company resources.

Since the decade of the 1990s is nearly upon us, I have focused on what I see are the likely trends featured in new large underground coal mines 10 years from now. I do this from the vantage point of having been involved in the installation of large mines for more than twenty years. Little of the equipment resources we have available today was unknown when I entered the coal business in 1960. There were continuous miners, longwalls and diesel-powered equipment in Europe, monitoring systems, computers, heavy media separation systems, froth flotation and conveyors then as there are now. A substantial evolutionary process has occurred with these and other mining machines and processes. Some evolved only to die out, such as boring-type continuous miners; others, such as longwall systems, flourished. My intuition tells me that since there have been largely evolutionary rather than revolutionary developments in the near past, the same is true of the future, at least for the next 10 years. In other words, the elements of what are in the future are with US now. Some, such as computer-based applications, may evolve beyond our wildest dreams -but I doubt it.

Continuous monitoring of carbon monoxide and other fire-related parameters by means of an atmospheric monitoring system (AMS) has been used by the mining industry for early fire detection in underground mines. The National Institute for Occupational Safety and Health (NIOSH) initiated a project to integrate real-time AMS sensor data with NIOSH’s mine fire simulation program, MFIRE 3.0, to simulate and predict the spread of smoke that would provide assistance to mine fire emergency response personnel. Determining the heat release rate of a fire using the monitored sensor data was a critical component of the successful completion of this project. NIOSH researchers developed a direct method to calculate the heat release rate when a fire is within close range of sensors. However, this method is only applicable to the case where a fire occurs in AMS-monitored airways. This paper presents an improved method for determining the fire heat release rate for complicated scenarios where a fire is distant from sensors and airflow splits and merges are present. The method was validated using a fullscale diesel fuel fire test conducted in the Safety Research Coal Mine at the Pittsburgh Mining Research Division and can help mine operators and safety personnel make informed decisions during a fire emergency.

In the 1980's the Canadian surface mining industry was confronted by declining competitiveness in the face of low cost foreign producers and alternative materials. Low metal prices undermined profits for many mining companies. Restructuring, rationalization, capacity reduction, technology transfer, as well as favourable currency rates and higher commodity prices, helped revitalize the industry by the close of the decade. Now in the 1990's, however, the need to improve competitiveness for profitability has re-emerged in greater intensity. In the current climate it is considered that there are three strategies available to industry to ensure profitability: continued rationalization and restructuring; optimization of operating practice; and technology development and transfer.

The study highlights the importance of prompt fire location determination for effective firefighting strategies and reducing injury risks. The ML model was trained with simulated data and achieved an accuracy score of 0.920, which improved to 0.962 by grouping closely connected airways.

"A packaged cooling system is being developed for refuge alternatives (RAs) that will meet U.S. Mine Safety and Health Administration (MSHA) qualification guidelines for deployment in coal mines. This system will allow full occupancy ratings of deployed refuge alternatives to be maintained by keeping the apparent temperature inside the RA from exceeding 35° C (95° F).Research investigations sponsored by the National Institute for Occupational Safety and Health (NIOSH) have shown that heat buildup caused by occupants in mobile RAs will result in apparent temperatures that exceed the 35° C (95°F) limit established in 30 CFR 7.504.Figure 1 shows the temperature rise in an inflatable RA having an occupancy rating of 23 people at an 27° C (81°F) mine air temperature. This plot shows the need for either heat mitigation or occupancy derating in warmer mines.The Refuge Alternative Environmental Control System (RAECS) will be designed to provide enough heat mitigation to eliminate occupancy derating due to heat buildup over the 96-hour refuge period.This article describes testing of a prototype cooling system that uses proven vapor compression technology powered by batteries to keep the RA interior below the 35° C (95° F) limit for a minimum of 96 hours. Testing of this prototype cooling system was conducted in December of 2015 and January of 2016 at the NIOSH test mine in Bruceton, PA. December testing was conducted at the prevailing mine ambient temperature — approximately 15.5° C (60° F). The January testing was done with the mine ambient temperature in the immediate vicinity of the RA artificially elevated to 29° C (85° F). Realistic heat gain from RA occupants was provided by human simulators developed by NIOSH. These simulators add sensible heat corresponding to human metabolic processes and latent heat generated by respiration and perspiration. Heat generated by CO2 scrubbers was also added. Results of cooling system performance using these simulated loads at the two ambient conditions are described. A comparison of the apparent temperature in the RA using two different control techniques is also presented. This work documents the ability of the cooling system to meet the established apparent temperature limit at the full occupancy rating of a six person RA. A new cooling system intended for use with existing RA deployments and having greater cooling capacity is currently in development and will be described.Expendable desiccant modules were also used to provide supplemental water vapor removal and their role in mitigating cooling load is addressed."

High Pressure Grinding Roll (HPGR) technology has been evaluated to replace tertiary cone crushing at Coeur Rochester operations (Coeur Rochester, Inc.) as an opportunity to improve silver recovery. In 2016 and 2017, onsite and third-party pilot programs were initiated to evaluate the impact of metallurgical recovery and heap leach performance of crushed product generated by an HPGR. In 2018, Forte Dynamics evaluated the HPGR laboratory metallurgical data from Coeur Rochester and all third parties to develop field-scale leach curves based on new crushing scenarios. The laboratory recovery test work data was consolidated and scaled to field conditions to determine projected heap leach performance. Based on the metallurgical testing conducted, the replacement of current tertiary cone crushers with HPGR technology has the potential to increase Coeur Rochester’s silver recovery rate by 8%, from 61% to 69%, and shorten the overall timing of silver recoveries from 20 years to just over two years. INTRODUCTION The Rochester mine, and associated valley fill heap leach facilities is an open-pit silver and gold mine located in Pershing County, Nevada, approximately 26 miles northeast of the city of Lovelock. In 2017, Rochester produced approximately 4.7M troy oz silver (Ag) and 51k troy oz gold (Au). Ore mined at Rochester is either fed directly to the primary X-Pit crusher, crushed at an in-pit crusher system, or placed directly onto a heap leach pad as run-of-mine (ROM). Both crushing circuits are comprised of three stages of crushing to produce a nominal 3/8” ore. Primary crushing consists of jaw crushing while secondary and tertiary crushing are composed of varying cone crushers. Historical recovery rates of crushed Rochester ore average 61.4% Ag and 95.9% Au over a period of 20 years [1]. These rates were derived from third party verification of in-house metallurgical test results, crusher production and heap leach production results from 1986 through 2004 in a private report for Rochester produced by KD Engineering Co., Inc. Recovery rates are regularly verified by compositing daily samples of ore delivered to each leach pad from the X-pit crusher, in-pit crusher, and/or ROM. A proportionate split of each sample is used to generate monthly composites for metallurgical analysis utilizing column leaching and bottle roll leaching. These tests are run in a manner analogous with production conditions and deliver test results that provide expected heap leach performance. As the Rochester oxide deposit has been mineralogically and metallurgically consistent, the gold and silver recovery trends have also been consistent over the life of the project. In 2016 and 2017, onsite and third-party pilot programs were initiated to evaluate the impact of HPGR technology to metallurgical recovery and heap leach performance of crushed product in an open circuit configuration, as a potential replacement for the currently in place tertiary cone crushers. During these years, HPGR was evaluated in three phases of test work.

The 3.5 km long Ashbridges Bay Outfall in Toronto, Ontario, Canada was a challenging drive set below Lake Ontario. After a remote machine acceptance due to the global pandemic, an 8 m diameter Single Shield machine launched in March 2021 from an 85 m deep shaft and began its bore in shale with limestone, siltstone and sandstone. During excavation, the TBM and its crew bored a city-wide record of 30 rings in one day, or 47 m of advance. This paper will cover the unique project, from TBM acceptance through to launch, tunneling in difficult conditions, and completion in 2022.

"The mining industry suffers from a number of incorrect perceptions, one of which is that it is an outdated industry that has not changed for hundreds of years. But the truth of the matter is that mining is a vibrant, highly complex industry that not only employs highly skilled and educated professionals but is also often on the cutting edge of technology. It is an industry with projects that are ever-evolving and among the most challenging engineering feats in the world.Take for instance the U.S. 53 Highway Relocation project in Virginia, MN. While more of a civil engineering job than a traditional mining project, it is still a complex engineering challenge taking place in a mining environment that has called on the skills of eight state agencies, three federal agencies, a host of contractors and many consultants, including many from the local mining industry in the Iron Range of Minnesota.In 1960, the Minnesota Department of Transportation (MnDOT) constructed U.S, Highway 53 on a private easement owned by US Steel that cut across a massive taconite deposit. MnDOT constructed the highway with the agreement that if the highway needed to be moved to access the taconite deposit before 1987, US Steel would foot the bill. And if the move was required after 1987, the responsibility would fall to MnDOT. In 2010, MnDOT received the notice that the highway would have to move as property owner United Taconite planned to access the ore, setting in motion one of the greatest engineering challenges, and the highest risk project, in the history of the states’ transportation department.Pat Huston, U.S. Highway 53 Relocation project manager, opened the 90th SME Minnesota Section Conference in Duluth, MN on April 19 with a presentation about the project that includes 5 km (3.2 miles) of fourlane highway and a new 345-m (1,132-ft) bridge that will be the tallest in Minnesota when it is completed later this year.“This project was not business as usual,” Huston told the packed crowd. “Highway 53 connects Canada and the Iron Range to Duluth and Lake Superior. Deposits of metals in only slightly disturbed strata have been attributed to sedimentation processes contemporary with deposition of the strata. Recently this origin has been extended' to a greater variety' of deposits and local enrichment has been attributed to selective remobilization of metallic minerals in the host stratum. Effects attending metallization and deformation, and exact age' of the metallization are compiled to establish the sequence of events in ore deposit histories in order to clarify importance of remobilization within a formation relative to introduction from an outside source into an fav¬orable host. The pertinence respectively of deposit; origin and geometry to the work of the mining geologist is reviewed. Dwarf faunas in the Clinton iron ore, Tully pyrite bed and Rammelsberg deposit, indicate abnormal metal ions in the sedimentary environment. Slight¬ly disturbed strata are host rocks in the Swedish Kolm, Timna, Kupferschieffer, and White Pine deposit; the latter two have ore only in conjunction with tectonic structures.

The difficulties encountered in justifying replacements for over-age mobile mine equipment at the Gay Mine are common to most mining operations. More critical management reviews, resulting from ever rising capital expenses, increased such difficulties. These increased difficulties are especially apparent to those who are primarily operators and are not statisticians, mathematicians, or computer oriented. The Gay Mine is located 17 miles east of the town of Fort Hall in southern Idaho. This open pit operation annually supplies the J. R. Simplot Company and the FMC Corporation with 1.5 MM - 2.0 MM tons of phosphate ore. Stripping ratios average approximately three bank cubic yards of waste per ton of ore mined. The mobile mine equipment fleet consists of ten 24 yard scrapers; twenty-one 30-50 ton end dump trucks; four 32 42 yard shovels; fifteen dozers in the D-8 and D-9 classes; and three road patrols. Equipment replacement is based on the results of evaluations made on individual units. Authorization of capital expenditures for equipment replacement is dependent upon a favorable profitability index (true rate of return on investment) plus a favorable payout time. The system is based on cash flows and requires the determination of (1) the net cash outflows in the form of capital expenses plus (2) the expected cash inflows resulting from reduced expenses and depreciation tax credits. After adjustments for taxes and the time value of money, the net amounts are used to calculate the required indices. The basic elements for the system related to the replacement of mobile mine equipment include: (1) the determination of dollar penalties to be expected from the continued use of aging equipment; (2) the selection of equipment that will provide the greatest return on the investment dollars; (3) the quantification of the expected savings; and (4) the calculations of the prescribed indices. Most statistical measures of mineral exploration continued downward in 1986. Prospects in 1987, however, appear to have brightened for exploration as the overall economic health of the mineral industry improved. The Society of Economic Geologists (SEG) survey of US and Canadian firms documents the marked decline in US mineral exploration since the early 1980's. From 1981 through 1986, the total exploration expenditure by US firms declined by more than 73% (Fig. 1). Exploration expenditures by US firms declined by 66%%. US based professional exploration staffs were slashed by 70% (Fig. 2). Also, many firms have written off some reserves because of weak markets or have not delineated new reserves to replace production. The Canadian government reported that between 1981 and 1986 Canadian molybdenum re¬serves had declined by 30%, lead and zinc reserves by 20%, and nickel and copper reserves by 15%. The SEG survey also shows the immediate impact of the decline in the dollar on US exploration expenditures abroad. Between 1981 and 1986 the share of the exploration dollar spent by US firms outside of the country declined by nearly one-half and exploration acreage abroad declined by 69%. The downward trend of the last six years may finally be near the end as a leaner and more efficient minerals industry starts to gear up for a period of increased activity. An industry survey reported to Mine Development Bimonthly (Metals Economics Group) indicates that mining firms were expected to increase exploration expenditures in 1987 by at least 10% above 1986 levels. Worldwide expenditures for 1987 by companies covered by the report were expected to top $800 million. It is the first time since 1984 that an increase in exploration expenditures was expected. Also, the US Bureau of Land Management reported a 30% increase in new mining claims for fiscal year 1987 over fiscal year 1986. The bulk of the increase in claims was in states having intense gold exploration, particularly Washington and Oregon. Much of the optimism is undoubtedly due to the increases in metal prices that occurred in 1987. Some of these increases include gold, average price up about 20%; copper, average price up 24%; aluminum, average price up 26%; and nickel, average spot price up 20%. The SEG survey shows precious and base metals accounting for 89% of the exploration expenditures in the US in 1986, up 52% from 1981. The US Bureau of Mines estimates that 80% of US exploration activity in 1986 was directed entirely at gold (1986 Minerals Yearbook). Worldwide exploration plans indicate the continued dominance of gold exploration. New programs were announced for Ghana, Malaysia, and Indonesia. Major new exploration programs were also launched for lead-zinc sulfides in Morocco, platinum and palladium in northwest Brazil, rare earth minerals in Greenland, diamonds in Indonesia, and heavy minerals off-shore Australia. Introduction Some unique opportunities exist for industrial minerals in British Columbia and Alberta. The region's diversified geology, particularly in British Columbia, offers favorable environments for a rich variety of mineral resources. Many minerals have been produced for years. For some, however, the level of production is far less than the potential. Others have had production in the past, but are not currently produced. Still others are known to exist and may have been actively prospected. But they are not yet developed. In addition, a number of speculative possibilities have recently come to light. The potential significance is not fully confirmed, but is suggested in showings in favorable geologic settings. Location and geography British Columbia and Alberta are the westernmost provinces in Canada (Fig. 1). The region overlaps two major physiographic divisions of the country, the Cordillera and the Interior Plains. It also takes in a small segment of the Canadian Shield on the east. The entire territory is greater than 1.6 Mkm2 (618,000 sq miles). Most of British Columbia lies within the Cordillera, a mountainous belt some 800 km (500 miles) wide. The belt is made up of three main physiographic components, the Eastern, Interior, and Western subdivisions. The Eastern subdivision embraces the rugged, linear ranges of the Rocky Mountains, elevations between 2600 and 3300 m (8530 and 10,800 ft). They form a 100-km (62-mile) wide band straddling the BC-Alberta border and extending northwest across northern British Columbia. The Interior subdivision includes several ancient mountain ranges but is dominated by an intermountane plateau area of low relief (average elevation 1200 m or 3900 ft). The Western subdivision is dominated by the Coast Mountains (elevations between 1800 and 4000 m or 5900 and 13,123 ft). These form a band about 175 km (110 miles) wide into which are sculptured the spectacular fiords of the BC coastline. Alberta lies largely within the Interior Plains, a prairie region of low to moderate relief (elevations between 200 and 1200 m or 650 and 3940 ft) formed of nearly horizontal bedrock strata. The Plains surface rises gradually to the southwest toward the deformed belt of the Rocky Mountains, where the bedrock strata are upturned, folded, and faulted. To the east, the Plains merge with the Canadian Shield, which exposes the Precambrian crystalline basement of western Canada. The total population of the BC-Alberta region is about five million people. Minerals, forestry, and agriculture are the primary industries here. It has well developed infrastructures to service these industries. British Columbia is third among the Canadian provinces in metal mining - $1.1 billion in 1985 - and first in forest products. It is also second in hydro-electric power development. Alberta leads all provinces in total mineral production, largely due to its extensively developed petroleum and natural gas resources - $26.2 billion in 1985.

In 1852, six barrels of iron ore were loaded aboard the steamer Baltimore at Marquette, MI, after a trip by mule wagon from the nearby Cleveland Iron Mining Co. Shipped to Sault Ste. Marie, they were portaged around rapids and reloaded onto another ship for the final journey to the steel mills of Cleveland. In 1855, the same company-the predecessor of Cleveland-Cliffs Iron Co.-chartered the brigantine Columbia to carry the first iron-ore cargo through the newly opened Soo locks. In 1981, about 2,190 iron-ore shipments in the form of taconite pellets were transported by water from US Great Lakes-ports. The total volume during the 284-day shipping season was 63 Mt (62 million It). Counting Canadian ship¬ments, almost 76 Mt (75 million It) of iron ore were moved on the lakes last year. The five Great Lakes-Superior, Huron, Michigan, Erie, and Ontario-contain 20% of the world's fresh water. The area surrounding the lakes has 26% of the nation's populations, 47% of its steel production, and over 80% of the raw materials required to make steel. That figure includes 71% of the country's iron ore and 40% of its limestone. Only the availability of low-cost water transportation of raw materials has kept the heartland steel producers competitive. Iron ore shipped on the lakes originates in the Lake Superior district's Mesabi and Marquette iron ranges. Major mines and their principal partners are Cleveland-Cliffs Iron Co.'s Empire, Republic, and Tilden; Armco and Republic's Reserve Mining Co.; Oglebay Norton Co.'s Eveleth; Hanna Mining Co.'s National and Butler; Inland Steel Co.'s Minorca; Pickands Mather & Co.'s Erie and Hibbing, the latter operated in conjunction with Bethlehem Steel Corp.; and US Steel Corp.'s Minntac. Pelletized ore products at these facilities is shipped through seven principal ports on the upper lakes: Duluth, Two Harbors, Silver Bay, and Taconite Harbor, MN; Superior, WI, and Marquette, MI, on Lake Superior; and Escanaba, MI, on Lake Michigan. In 1981, tonnage volumes ranged from 13.3 Mt (13.1 million It) shipped from Duluth, to Marquette's total of 5.4 Mt (5.3 million It). The ore docks, with two exceptions, are operated by railroads, most of which are owned by mining companies. Table 1 illustrates US iron-ore shipments in 1981. In the lower lakes, ore is discharged at dock facilities close to the great steel-producing centers of Chicago, Detroit, Cleveland, Pittsburgh, and Buffalo. Between the mines and mills, lakes and connecting waterways form one of the country's most effective transportation systems. Each year, vessels on the Great Lakes transport about 181 Mt (200 million st) of bulk industrial and agricultural materials-a greater quantity at a lower cost than can be achieved by any other transportation mode. Introduction History. Vat leaching operations began in October 1953. Since that time, about 4.2 million tpy of oxide ore averaging 0.72% copper have been treated. Economic conditions, improved operating methods, and increased production facilities have allowed the ore grade to be reduced over the years. The average ore grade was 0.84% Cu during the 1950s, 0.68% in the 1960s, and was 0.50%, or below, during the 1970s. By 1961 some 35 million tons of oxide ore had been removed from the mine, exposing the underlying sulfide ore body. A 5,000¬ tpd concentrator was constructed and put into operation to treat the sulfide ore in November 1961.1 The original crushing plants were used for both oxide and sulfide ores, but by adding a third crushing shift, six days per week, the total ore treated was raised from 12,000¬17,000 tpd. Dwindling oxide ore reserves prompted expansion of the concen¬trator to 13,500 tpd in 19672 so that exhaustion of known reserves of both ores would occur at nearly the same time. Again, no additional crushing facilities were added during the expansion. It was anticipated that the treatment rate of oxide ore would be reduced from 12,000 to about 9,000 tpd. Instead, the addition of two crushing shifts per week and improved operating procedures have increased the treatment rate to 26,500 tpd-13,000 tpd to the leaching plant and 13,500 tpd of sulfide ore treated in the concentrator. Geology. The Yerington ore body is a disseminated porphyry copper deposit where a quartz monzonite porphyry intrusive contains the major part of the ore values, with some copper mineralization found in the adjacent granodiorite and other rocks. The geology of the Yerington Mine was aptly described by J. R. Wilson (Mining Congress Journal, June 1963). The ore body is one of few porphyries where geologic conditions were optimum for the formation of an important concentration of oxidized ore with minimum economic effect of secondary enrichment. The principal mineral in the oxidized zone is chrysocolla, which irreg¬ularly occurs dispersed throughout the rock and as narrow seams along fissures. The grade of the ore in the oxidized zone is generally the same as in the primary sulfide below, although some massive concentrations do occur as vein-like fissures and as filling of breccia zones. Cuprite, tenorite, melaconite, malachite, and azurite occur but are not abundant. Lying between the chrysocolla horizon and the primary sulfide is a transition zone whose limits are irregular but seldom with vertical range exceeding 50 ft. Chalcocite, cuprite, melaconite, native copper, and chrysocolla occur superimposed on the primary sulfides. Chalcocite replacement of chalcopyrite, as well as secondary chalcocite, do occur. The primary sulfide minerals are chalcopyrite and pyrite, with minor amounts of bornite and covellite. There are no appreciable gold or silver values, and molybdenite has rarely been noted.

With the growing demand for energy, explora¬tion for uranium deposits has dramatically increased in the past few years. Since the early ages of uranium exploration, natural gamma ray logs has proved fast and convenient in locating and evaluating mineralized bodies. However, natural gamma ray logging is an indirect measure of the uranium content and in some circonstances, the radiometric signal can be 'altered in a very significant way. To check the presence of any distortion, statistical comparision of radiometric counts and chemical assay values for the same samples is generally performed. Of course in such studies, one inevitably mixes samples where the radiometric signal is really proportional to the uranium content and samples where this signal is influenced by many other sources or distorted before being recorded. Nevertheless, the average tendency leads to correction factors which are then applied to every radiometric reading in the deposit or portion of the deposit where the statistical comparison is made. To account for the "average" character of these correction factors, a parameter of uncertainty like a standard deviation of possible errors is associated to each corrected value. This parameter can be latter used in an ore reserve estimation when the weights assigned to samples should depend on the precision of their grade. The purpose of this paper is to present a new regression technique which can be used without any restrictive hypothesis like a normal or lognormal distribution. Different standard ways which can be used to statistically compare radiometric and chemical assay values for the same sample and to derive a correction factor for the latter will first be reviewed, after a general discussion of disequilibrium problems. The discussion and the examples will be restricted to the case of natural gamma ray logging in drill holes but the methods presented in this paper can also be applied to data from the radiometric probing of a working face or a truck load in an operating mine.

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."
