"INTRODUCTION The Lisbon Valley Mining Co LLC (LVMC) operates the Lisbon Valley Mine located in SE Utah. The mine includes 3 open pits, 150-acre heap leach pad, and solvent extraction electro winning plant (SX/EW). Approximately 25MM tons of ore has been mined since 2005. The ore is stacked on the leach pad to an average height of 55 feet and occurs as a blend of crushed material (minus 2”) and run-of-mine (minus 24”). The host rocks include Dakota and Burro Canyon Sandstones. Copper occurs finely disseminated in this material at an average grade of 0.46%. Minerology is 63% sulfide, 27% oxide, and 10% insoluble. Total inventory mined through 2016 is approximately 230 MM lbs. METALLURGICAL CONSTRAINTS Copper production is constrained by areas of low permeability ore and geochemical issues, including acid consumption. The intrinsic permeability the ore ranges from 10-2 to 1 Darcy. This permeability does not permit sufficient aeration by natural convection, therefore oxygen transport is largely limited to the mass of oxygen that can remain dissolved in leachates. As a result, greater than 50MM lbs of copper remains in inventory and occurs secondary sulfides, including chalcocite, covellite, and bornite. BIOLEACHING FUNDAMENTALS Due to the higher activation energies of secondary sulfides, notably covellite and bornite, a strong oxidant is needed to remove an electron from the metal-sulfide lattice and release the copper cation into the sulfate solution. This process occurs most efficiently using the strong oxidant ferric iron (Fe3+). Ferrous iron can be oxidized to ferric iron by oxygen via chemical oxidation or biologically; which was measured to occur 500,000 times faster when using microbes versus oxygen.1 The microbes that oxidize ferrous iron (Fe2+) to ferric iron are naturally abundant and will develop biomass when appropriate conditions and nutrients present themselves. Most nutrients are provided by the ore/irrigation (P,N,Mg, Ca & Fe2+) while the others (O2 and CO2) are provided by air. In most cases, oxygen transfer is the rate limiting step during copper sulfide leaching. 1 Lacey & Lawson 1970 FORCED AERATION SYSTEM DESIGN This paper describes LVMC’s efforts to bioleach the remaining inventory by forced aeration. Two basic aeration systems were installed and tested including vertical wells and horizontal piping. Vertical System The vertical system was installed using the mine’s blast hole drilling rig. Wells were constructed using 4” high-density polyethylene (HDPE) connected to 4” polyvinyl chloride (PVC) screens similar to conventional bioventing wells. Ore samples were collected at each well location to characterize the spatial distribution of the remaining inventory along with fines (minus 200 sieve), pH (acid consumption), and iron. A block model of each analyte was developed and mapped in 3D."

The 1970s have witnessed the completion and successful start-up of several new non-ferrous metallurgical operations in the Western Hemisphere which utilize large semi-autogenous mills in the grinding circuits. These facilities' are arranged either as single-stage circuits, or in a two-stage configuration n with ball mills. Another major concentrator, designed with an autogenous mill, ball mill, crusher (ABC circuit), is presently under construction and scheduled for start-up late this year. The large primary mills employed in these circuits have high diameter-to-length ratios and are designed to accept a very coarse feed, i.e., the product of a primary gyratory crusher, or run-of-mine ore. This characteristic obviates the need for a secondary-tertiary crushing plant or, in selected cases, even a primary crusher, which is a basic requirement in feed preparation for conventional grinding mills. This paper discusses three such grinding circuits and, presents operating data for two of them: the Island Copper Mine of Utah Mines, Ltd., near Port Hardy, British-Columbia,- and the Pueblo Viejo Cyanide Leach Plant of Rosario' Dominicana, S.A. on the island of Hispaniola. The design criteria for the Cyprus Bagdad Concentrator under construction' at Bagdad, Arizona, are also included. Data pertinent to these circuits are tabulated in the Summary.

Introduction Hydrogen peroxide is known to the mining industry as a powerful oxidant. able to attack and to destroy cyanide, the most important leaching reagent (Knorre and Griffiths, 1984; Griffiths, 1988; Griffiths, 1988). [ ] The most recent development, however, is the use of H2O2 as an oxidant in the cyanidation of precious metals. This application may appear to be a contradiction to the above mentioned detoxification process. This can, indeed, be the case if certain conditions are not considered carefully. Degussa has developed a new process to enable the application of H2O, in this field without having any detrimental effect on the cyanide. The incentive to seek an oxygen supplier like H2O2 instead of air was the fact that one of the most important conditions for the fast dissolution of gold is the presence of physically dissolved oxygen in the pulp (Kydryk and Kellog, 1954; Warstell, 1987). [ ] Complete gold recovery within a reasonable leaching time may only be achieved if the pulp is saturated with dissolved oxygen. Gold losses in leach operations mainly occur due to the following reasons: In some cases, the saturation of oxygen in the pulp cannot be achieved by aeration due to the difficulty to transfer oxygen from the gaseous into the liquid phase. This happens, for example, in pulps with a high viscosity. In such cases, even violent aeration only results in a low concentration of dissolved oxygen in the pulp. In any case, the starting phase of leaching requires the highest amount of oxygen. Therefore, it is especially important to raise the dissolved oxygen level to saturation in the starting phase. Peroxide assisted leach process (PAL) The intention of the process is to introduce oxygen in a liquid form. As a result of intensive research work, it could be shown that hydrogen peroxide can be used as a source of oxygen in the leaching of gold ores. To avoid side reactions, the following conditions must be applied: Hydrogen peroxide must be diluted before it is added to the pulp. The dosage of H2O, must be properly regulated based on the oxygen level of the pulp. An important part of the continuous O, control is a special oxygen electrode, which resists the extremely abrasive action of the pulp.

In developing nations, novel mineral processing methods like bacterial leaching are gaining prominence. The principles and practices for isolation and preservation of cultures used in bacterial leaching are discussed with special reference to cultures used in leaching copper from Indian ores. In all, 145 potentially useful microorganisms such as iron- and sulfur-oxidizing bacteria were isolated from various ecosystems of the copper mining areas in the country. The major organisms isolated by enrichment culture technique belonged to T. ferrooxidans and T. thiooxidans species. In addition, thermophilic cultures like Sulfolobus sp. and thermophilic thiobacilli were also isolated. The process of copper leaching was scaled up to the 300-kg level in PVC columns. Since the preservation of efficient cultures in active state was vitally important, a new method was devised that preserved the activity of thiobacilli for a period of more than two years. The novel method of storage in sterile chalcopyrite ore at 8°C can be considered an important advance.

Bong Mining Co. Ltd. in Liberia, West Africa, operates an iron ore open pit some 60 miles northeast of Monrovia. The production is at present about 40,000 t/d (metric tons/day) from which about 18,500 t/d concentrate with 65.5% Fe is produced. The ore waste ratio is about 1: 1.5 to 1:2. The technical layout and the equipment are very similar to those in many modern open-pit operations as described by Jacobs and Thomson (1967). As described by Lersch in detail (1966), the deposit is a very irregular one. The quality of the ore changes within very short distances with regard to both physical conditions (hardness) and chemical properties. Very soft and very hard ore as well as high-grade and low-grade ore are found adjacent to each other. In addition, there are enclosures of sterile rocks. For the purpose of identification and production scheduling, 12 different types of ore are distinguished.

The Office of Coal Research Program has been outlined in our Annual Report for a number of years and reported on by various members of the OCR staff as well as by our contractors. The program is national in scope and is intended to develop new and improved uses for our Nation's coal resources and to maintain coal in a favorable economic position, vis-a-vis, the other energy forms. A program has been developed that includes conversion of coal to gas, gasoline and other petroleum-type products, and to electricity. These projects are only one facet of the OCR program albeit an important one. In many of the conversion projects we have underway, original development work has been done with bituminous and sub-bituminous coal. It is the intention of this paper to present a general outline as to how lignite might fit into one or more of these projects as a raw material. Our national lignite reserves are vast in scope and this has been recognized in the OCR program by development of a project specifically for lignite and by including lignite as a test raw material in many other projects. Project Lignite is intended to develop the Cop Acceptor Process for conversion of lignite to a high B.t.u. gas. In this effort we have included the design, construction, and operation of a lignite gasification plant at Rapid City, South Dakota. The work is being conducted by the Consolidation Coal Company and is an outgrowth of their laboratory development of this process. The first portion of our effort included a detailed economic study of a somewhat arbitrary grouping of States designated Market Area A that could be expected to use gas manufactured in a lignite gasification plant. The area included the Dakotas, Minnesota, Nebraska, and Iowa. This geographical area could readily be served if a process could be developed for converting North Dakota's lignite reserves, estimated at 351 billion tons, to high B.t.u. gas. Our study showed that Market Area A had a potential commercial- industrial market of one trillion cubic feet of gas a year. To meet this demand, we believe that part of the load can be obtained from commercial lignite-to-gas plants. At the time of our original study, we concluded that gas could be delivered to the city gate in Market Area A at about 45¢. As a result of price escalation in the interim, our estimate today might be slightly higher, but not significantly so, since some parts of the process appear better today than they did at the time of the original projection.

As stated by the Congress in the preamble of the act establishing the Office of Coal Research, its purpose is, "To encourage and stimulate the production and conservation of coal in the United States through research and development by authorizing the Secretary of the Interior to contract for coal research, and for other: purposes." Thus, while the overall responsibility for air pollution control from all sources rests primarily with the Department of Health, Education and Welfare, it is obvious that OCR could not perform its mandate of, increasing the utilization of our Nation's greatest energy resource if it ignored one of the greatest practical obstacles to achieving that objective. Therefore, air pollution abatement and prevention is one of the practical considerations taken into account in the selection of the OCR program. And as the severity of the air pollution problem imposes new restrictions on the use of coal, the importance of this criteria in the selection of OCR projects increases. The work done by OCR in air pollution abatement does not conflict with nor does it duplicate the program developed by HEW's National Center for Air Pollution Control. Technical members of the OCR staff maintain close formal and informal liaison with their counterparts in NCAPC. One formal agreement has been executed whereby the HEW agency has transferred funds to OCR for a practical study of the potential effectiveness of air pollution control when coal is burned in a fluidized bed comprised of inert alkaline earth material. A flexible expert system for a personal computer has been developed to take countermeasures for prevention of emergency in underground coal mines. The system makes a diagram to estimate maximum airflow rate of a specified branch such as coal face or advancing face, etc. under the conditions of opening regulators set in any two different branches, by computer simulation. The system approximates the ventilating condition underground by using both measured values and safety factor derived from the rules in the system. The probability of happening gas explosion, spontaneous combustion and mine fire underground is shown on a display and countermeasures in a data base in the system are selected according to a confidence factor. When it is recognized in the system that either safety factor or confidence factor of countermeasure has a small value, the system sends information relating to reasoning in the system to registered human experts by computer communi¬cation and facsimile. The replies in file transfer from the experts are stored in the system and reconsultations for estimation of underground condition are started automatically. As the replies in facsimile are shown on the display, user inputs new rules and, values for reconsultation. It is shown that the system is available to take better countermeasures as the condition' of a network in coal mine is changed by mining day by day.

So far my consulting in industrial minerals has been confined principally to carbonate rocks,-salt, coking coal, and glass sand. Much of this work has been for companies either too small for a full-time geological staff or unaware of the value of employing one or more qualified geologists permanently. It gave me considerable personal satisfaction to see several of the latter companies employ one or two full-time geologists after the completion of a consulting contract with them. One possible reason why some companies lack the wisdom to hire geologists is that they are dominated by engineers who still subscribe to the old idea that engineers are self-sufficient in all fields. They could have learned this from their professors. I once knew a Dean of Engineering who got purple in the face when a faculty committee was discussing the financing of a broad gauged engineering project and suggestions were made that assistance be obtained from the Schools of Business and Law and from some of the science departments tie pounded the table and shouted "Engineers are trained to handle all this!" In teaching engineering geology some years ago I used to tell the class repeatedly that the course would not make them geologists, that the purpose was to teach them

"The Bankei-Kitanosawa tunnel that is located in Sapporo, Hokkaido, Japan includes a 450m long section near the arrival point where mudstone which can liquate out natural hazardous materials such as arsenic and selenium exceeding the tolerance stipulated by country as well as weathered mudstone and unconsolidated sedimentary rocks with small overburden and the competence factor less than 1.0 are present. The mucks containing hazardous materials were disposed into the waterproof branch tunnels created from the inside of the main tunnel. The full face early ring closure method was employed to ensure the stability of the tunnel in the low strength rock masses having no stand-up time. As a result, the waterproof tunnels branching off the main tunnel can manage water tightness so as to ensure safety and relief against environmental risks of the hazardous materials. It is also demonstrated that the full face early ring closure method enable construction of the tunnel with a small overburden in the low strength rock masses. In order to overcome the restrictions, the hazardous materials were designed to be disposed into large cross section waterproof tunnels which were newly designed and constructed in high strength and hazardous material free Andesite. The lining for the tunnels was composed of with shotcrete. In addition, full face early ring closure (Sato et al., 2013) was employed to stabilize the tunnel so as to enable safe tunnel excavation in low strength rock masses where a ground arch is deemed unachievable. This paper presents the safe disposal method of mucks which are likely to liquate out natural hazardous materials as well as the full face early ring closure method and its mechanical characteristics."

Within recent years portable seismic refraction units have found their place in the industry for shallow investigations, Completely portable, relatively inexpensive and of proven accuracy, the portable refraction seismograph is an electronic tool in an electronic age. The age of electronic instrumentation has seen the development of seismic reflection and refraction equipment for deep exploration, especially in the petroleum industry. Large, expensive units, normally truck-mounted and utilizing explosives, are being used successfully for this type of sub- surface investigation. The portable seismograph utilizes the same theory for shallow work as that applied to deep refraction seismology.

"It is now possible to use 3D X-ray tomography analysis for plant-site characterization of coarse particles at a sampling rate of about 3 kg/min for particles which range in size from 150 mm to 1 mm at a voxel resolution of about 150 microns. This is quite a significant advance in X-ray tomography technology and should be useful for plant-site coarse particle characterization (size, shape, composition, density, texture, grain exposure, mineral liberation) with a response time of minutes after receiving the sample. Applications in the mineral industries are discussed including the coal industry, aggregate industry, metal mining and the processing of industrial minerals. INTRODUCTION Plant-site characterization of coarse particles at coal preparation plants, crushed stone/aggregate plants, and mineral processing plants is needed to improve plant capacity, product quality, and/or process efficiency. Not much on-line coarse particle characterization is done at these plants. Some limited information regarding particle size is obtained on-line from reflected light images using systems such as the OPSA, Split, and WipFrag analyzers. Even laboratory analysis of coarse particles has been difficult because of the large amount of sample required to insure statistical significance of the results. During the past decades laboratory 3D characterization of particles from energy and mineral resources by X-ray tomography has been demonstrated and used to good advantage [1, 2]. 3D X-ray tomographic analysis had its origin in the medical services industry; it is used to provide internal images of the human body and now has also been developed for non-medical applications [3]. Figure 1 shows a schematic diagram for the cone-beam geometry micro-CT system. In our case X-rays from a microfocus X-ray generator are partially attenuated by the sample (a packed bed of particles) that is made to rotate in equal steps in a full circle about a single axis of the particle sample. At each rotational position, the surviving X-ray photons are detected by a planar two-dimensional array (image intensifier) large enough to contain the shadow of the specimen. These two dimensional projection images are collected using conventional video technology. The video signal for each projection is then converted to a two-dimensional digital array by an image processing system. Finally, a three-dimensional image is reconstructed from the collected set of projection images. With the availability of automated high speed X-ray tomography [4] it now seems possible to do plant-site particle characterization at a sampling rate of about 3 kg/min for particles which range in size from 150 mm to 1 mm at a voxel resolution of about 150 microns. The U.S. National Institute for Occupational Safety and Health (NIOSH) Office of Mine Safety and Health Research (OMSHR) conducted a study of mining industry electrical injuries reported to the U.S. Mine Safety and Health Administration (MSHA) for the years 2000 to 2009. The findings of that study are detailed in this paper, and serve to characterize the circumstances surrounding electrical injuries and identify causal factors. The study included three tasks: 1) a direct review of mining industry occupational injury data compiled by MSHA, 2) interpretation of the narrative descriptions available for the injuries (from MSHA data) and 3) a separate examination of fatal electrical injuries. Eight-hundred sixty-five electrical injuries were reported during the 10-year period studied, with 39 of those being fatalities. This makes electrical injuries disproportionately fatal with respect to most other types of injuries in mining. Electrical injury rates were higher in coal mining than noncoal mining and, within the coal sector, rates were higher in underground operations than in surface operations. Of the 865 total cases, electrical and machine maintenance or repair activities were involved in 580 (69%), and electricians and mechanics were injured in 362 cases (42%). Of the 39 fatal electrical injuries, 27 (69%) involved electrical maintenance or repair work, and in 21 of these 27 cases, the failure to de-energize, lock-out and tag the circuit was the cause or a contributing factor. Also, contractor employees had a much greater chance of an electrical injury being fatal than did mine operator employees. The top three root causes for fatal electrical injuries were 1) no or inadequate lock-out and tagging, 2) failure of power system components and 3) contact of overhead electrical power lines by mobile equipment.

"INTRODUCTION Barrick Goldstrike underground mine is located in Northern Nevada on the Carlin trend. The underground mine produces approximately 1.5 to 1.8 million tons a year and approximately 400,000 ounces of gold a year. The mine is comprised of two main mining methods, underhand drift and fill and long hole stopeing. The mass blast project stated in 2012 as Value in Use project between Barrick, Southwest Energy and Orica, the goal of this project is to be able to blast an entire drilled stope in one blasting event instead of taking multiple blasts. This will improve productivity by eliminating intermediate blasts, reducing additional blast time, eliminate drill hole cleaning due to the previous blast. Project implantation started in the fourth quarter of 2013 and mass blasting has been common practice throughout all of 2014. STOPPING PRACTICE AT GOLDSTRIKE BEFORE MASS BLAST PROJECT The production from the Goldstrike mine is approximately 70% from stopes. The timely completion of all stoping blocks is imperative to maintaining the plan, delays in the execution of any particular block can have detrimental ripple down effects as neighbouring stoping blocks become delayed. Before the implementation of this project the standard practice for Goldstrike was to: 1. Drill the entire stope 2. Blast a bottom shot, normally between 10 feet and 20 feet off the bottom of the slot rings. This would be mucked out before the next shot 3. Blast a pull through shot, this included the raise and one or two production rings. Again mucking this out before the next shot 4. Blast production rings, one or two more blasts depending on stope size. In most cases between each of these blasts the holes in the other rings of the stope would have to be cleaned. This meant either cleaning manually using air and water, or mechanically cleaning with a bench drill. In many cases this resulted in delays in completing the stoping block."

INTRODUCTION Davy McKee have developed and patented a combined mixer settler (CMS) which has undergone extensive trials and has now been installed in two commercial uranium plants in South Africa. Substantial capital and operating cost savings can be realized over conventional mixer settler in uranium extraction, especially from dilute feedstocks containing approximately 200 ppm U308. Pilot plant work has also been undertaken on other metal extraction systems and the results are very encouraging in most areas of interest. CONVENTIONAL MIXER SETTLERS The conventional plant in use at many uranium recovery facilities consists typically of a mixer box in which the aqueous and organic phases are contacted and a settler in which the two phases separate. The mixer box must allow sufficient residence time for the mass transfer kinetics to be satisfied for the particular species involved. The settlers are usually rectangular and take the form of shallow tanks of large cross sectional area.

As national and international health and safety management system (HSMS) standards are voluntarily accepted or regulated into practice, organizations are making an effort to modify and integrate strategic elements of a connected management system into their daily risk management practices. In high-risk industries such as mining, that effort takes on added importance. The mining industry has long recognized the importance of a more integrated approach to recognizing and responding to sitespecific risks, encouraging the adoption of a risk-based management framework. Recently, the U.S. National Mining Association led the development of an industry-specific HSMS built on the strategic frameworks of ANSI: Z10, OHSAS 18001, The American Chemistry Council’s Responsible Care, and ILO-OSH 2001. All of these standards provide strategic guidance and focus on how to incorporate a plan-do-check-act cycle into the identification, management and evaluation of worksite risks. This paper details an exploratory study into whether practices associated with executing a risk-based management framework are visible through the actions of an organization’s site-level management of health and safety risks. The results of this study show ways that site-level leaders manage day-to-day risk at their operations that can be characterized according to practices associated with a risk-based management framework. Having tangible operational examples of day-to-day risk management can serve as a starting point for evaluating field-level risk assessment efforts and their alignment to overall company efforts at effective risk mitigation through a HSMS or other processes.

Prior to 1995, the smelting process at the Kennecott Utah Copper Corporation (KUCC) smelter consisted of Noranda reactors and Pierce-Smith converters. In the consideration of expansion opportunities, the existing process did not lend itself to a program geared towards processing the total output of KUCC mining and milling operations. During 1989 and 1990, a number of smelting options were evaluated. The final selection was the Outokumpu flash smelting process, coupled with the new Kennecott-Outokumpu flash converting technology. Facilities were designed and installed to process 1.0 million tonnes per year of copper concentrate, with the ability to expand to 1.2 million tonnes per year. Following the shutdown of the smelter facility utilizing the Noranda technology in May 1995, the flash smelting furnace commenced operation in June with the flash converting furnace startup following a few weeks later. The smelter has achieved design production targets and environmental goals. Current operating rates are at or above design rates for concentrate and matte processing. Emissions are lower than the permit levels and equate to less than 3.5 kg of S02 per tonne of copper. This paper outlines the design, construction, startup and the first two years of operation of the modernized KUCC smelter.

This paper presents the results of laboratory and pilot plant investigations for the development of efficient methods of beneficiating hematite ores from the Krivoi Rog, Kursk Magnetic Anomoly, Olenogorsk, and other localities. The tests included gravity, magnetic (in a high intensity field), flota¬tion, and magnetizing roasting methods. It was possible to develop efficient beneficiation methods depending on the mineral composition of the ore and degree of dissemination of ore and gangue minerals. The paper shows the relationship between the iron silicate and quartz content of the ore and the technologic indices of the various beneficiation methods. Some of the conclusions based on these investigations and the technical and economic computations related to the cost of one ton of pig iron produced from concentrates obtained by the various beneficiation methods are: 1. If the hematite ore is not disseminated uniformly, it is expedient to use a combined gravity and flotation process. Ore particles up to 50 am. in size are beneficiated by gravity (heavy suspension, jigging, and others) and yield a lump concentrate. The middlings from this process are recrushed and floated; 2. Finely disseminated hematite ores, containing small quantities (less than 5 percent) of iron silicates, are most effectively beneficiated by flotation. Anionic (sulphate soap, tall oil emulsion with various foam depressors) as well as cationic collectors may be used. Depending on the mineral and chemical composition of the ore, it is possible to obtain concentrates containing up to 65 percent iron with an extraction up to 94 percent; 3. Finely disseminated, mixed hematite-magnetite ores may be beneficiated by flotation or a magnetic separation-flotation process; 4. Finely disseminated hematite ores, containing large quantities of iron silicates and earthy hematite ores are most efficiently beneficiated by the magnetizing roasting method. This method will yield concentrates containing up to 68 percent iron with extractions up to 90 percent. Although beneficiation by the magnetizing roasting process is costly, it will nonetheless, when employed on this type of ore, yield the least expensive pig iron.

Oxidized coals are difficult to float. Therefore, in this paper, a special flotation procedure was investigated for upgrading this type of coal. The procedure is called direct contact flotation. It relies on mixing dry coal either with pure (anhydrous) flotation reagent(s) or microemulsion. By applying the new direct contact flotation procedure, a coal yield reaching > 95% using anhydrous flotation reagents was achieved, but with relatively high consumption of reagents (up to 30 g/kg). This high reagent consumption may be attributed to the fines included in the flotation feed (100% less than 0.5 mm, with 40% less than 75 µm). On the other hand, the clean coal yield was found to be approximately 88% when mixing dry oxidized coal with microemulsion droplets from dodecane and dodecyl tetra (ethylene glycol) ether (C12E4) in water at a total reagent dosage of 10 g/kg. The direct contact of coal either with unhydrous reagents or microemulsion also provided a high flotation selectivity.
