Every person in an organisation has what could be called ‘a different take’ on what it means to be a safety leader. The directors will tell you they are ultimately responsible for everyone’s safety and that they can be imprisoned as a consequence of a workplace fatality. Managers, take more of a focused approach and would explain that they are totally responsible for the safety of their teams. And then there are front-line leaders. Those that not only have the responsibility (like directors and managers) but they also have the accountability, and the ability, to influence how safely their teams work. But it is not just physical safety that leaders should be concerned with.

Fatality prevention and zero harm are key focus areas for resource companies. However, they remain an ongoing challenge for high hazard industries, including the mining sector. As one example the latest International Council on Mining and Metals (ICMM) Safety Performance Report shows that, while there is generally a declining fatality trend since the organisation commenced publishing data in 2012, there were still 33 fatalities across member companies in 2022 (ICMM, 2023b). Every fatal incident brings significant pain and suffering to the colleagues, friends and family of those involved, however the failures and causal factors leading to each incident are frequently very familiar to people working in the industry, particularly those tasked with managing risk and safety. In the words of Dame Judith Hackitt, Grenfell Tower Fire Investigator, ‘there are no new accidents – just different people making the same mistakes because they haven’t learned’ (Hackitt, 2022). The recurring themes in terms of the nature of fatal hazards and their associated causes across the mining sector show that, despite improvements evidenced by the favourable declining fatality trends over the long-term (in general and outside of catastrophic multiple fatality events), changes are required to achieve fatality elimination and the aspiration of zero harm. This extended abstract and the accompanying presentation apply a mining frame of reference to the idea that there are no new accidents, examining how this applies within the industry and why the lessons from past failures are not always well understood or applied to prevent the reoccurrence of material unwanted events. Many contemporary mining and resources organisations have extensive internal incident history data and a wide variety of information on fatality incidents is publicly available, including the annual ICMM Safety Performance Report. This extended abstract and the accompanying presentation explore how this existing data can be used effectively to learn from the past and proposes practical ideas regarding strategies for successful knowledge transfer to the workforce as a key component of preventing history repeating and improving safety performance across the industry. As noted by ICMM, ‘sharing lessons from failure is vital to improving health and safety, but it’s not enough’ (ICMM, 2023a).

The Kibali Gold Mine is currently comprised of several open pits, a 3.5 Mtpa underground mine and a 7.5 Mtpa processing plant. Development of the underground operation commenced in December 2012, with initial access established through twin surface declines, followed by the commencement of a blind sink 8 m diameter rock hoist shaft in March 2013. Stope production started in December 2014 with ore and waste trucked to surface. The 755 m deep blind shaft sink was completed in August 2015 together with the cutting and equipping of three main station levels. Two of these levels provide access for the establishment of the underground materials handling system as well as the main dewatering system for the mine. This paper covers the process of determining the optimal location of the Kibali hoisting shaft relative to project-specific critical factors, and then focuses on the design and implementation journey associated with the off-shaft development that essentially encompasses the Material Handling System (MHS) for the mine. The journey starts from the findings of the previous owners Feasibility Study (FS), a subsequent Optimised FS, followed by detailed engineering through to implementation. It was identified during implementation that a significant change in the designed off-shaft layout was required following detailed underground geotechnical and hydrogeological investigations. In addition, the paper provides details on the MHS performance since commissioning to steady state, and on key learnings relating to the overall project process.

Electrostatic separation is a core requirement for the beneficiation of mineral sands suites to provide valuable product. Although the basis of electrostatic separation has been understood for some time, the details required to be able to calculate the metallurgical performance of a separator has not previously been achieved from a mechanistic perspective. Empirical models have had some success but require extensive test work. Traditionally, performance of an electrostatic separator is determined by careful experimental work using the target mineral suite. Although electrostatic separation was invented at the end of the 19th century, very little modification of the basic equipment occurred until relatively recently, with the addition of a plate electrode to aid separation. Previously, the author has presented results, calculated using finite element modelling (FEM), of the electric fields present in an electrostatic separator in various configurations. This paper will present some new mathematical modelling work, which uses the FEM determined electric field configurations but also incorporates some fundamental physical processes that provide insight as to the limitations and opportunities for this important separation technique.

The spatial modelling of geometallurgical domains is an important step in the characterisation of an ore deposit allowing for the quantification of various rock types, their differing rock properties and the overall value of a project. The traditional approach to modelling such features is through the use of deterministic models, where a single estimate of the extents and structure of the ore deposit is mapped out by geologists based on available data and their interpretation of the geological processes. Any uncertainty in the layout of different domains is not accounted for and heterogeneities of chemical and physical properties of the orebody are likely underestimated. Spatial machine learning techniques can be used to derive geometallurgical categories, or classes, from multiscale, multiresolution and high dimensional measured rock properties. Subsequently, geostatistical simulations of these classes can be applied to obtain multiple equiprobable realisations that define the layout of the geometallurgical classes at unknown locations between boreholes, or domains. In-turn these realisations can be used quantify the uncertainties in the location of boundaries between different domains. The development of a workflow for defining geometallurgical domains and later geostatistically simulating them across the Orebody H; a complex stratabound Bedded Iron Ore deposit located in Western Australia’s Pilbara region is demonstrated. This could be used to identify zones of high uncertainty where collection of additional data might help mitigate or minimise risks and in turn improve forecast production performances.

Blastholes fired in a pattern with a constant initiation delay create a frequency in the ground vibration similar to that from a percussion instrument, such as a drum, in the air. A constant firing delay generates a prime or forcing frequency into the ground motion. The forcing frequency may then be modified by ground transmission characteristics into subharmonics of the forcing frequency. The moving vibration source between blastholes also creates a frequency shift because of a Doppler effect. Other wave interactions may form beats. The effects are exacerbated by the accurate firing times of electronic detonators. This paper demonstrates how observations of wavetraces and frequency spectral analysis were used to recognise:Doppler effect and frequency ellipsoidssubharmonics (and sub-subharmonics) of the forcing frequencybeat formation that may double or triple peak particle velocity (PPV) levels.Recognition of beats permitted techniques to be used to control ground vibration from coal overburden blasting to limits deemed to cause no further instability in a slip failure. An understanding of the science involved has practical application elsewhere that has been successfully applied to initiation timings to reduce PPV and adverse human and structure response from other blasting operations.CITATION:Moore, A J, Richards, A B and Laing, T J, 2015. Even though most major operating companies in the minerals industry have Board-level endorsed sustainability principles and objectives, design engineers typically have no mechanism that allows them to implement those objectives in their work. In addition, the existing project management systems do not readily deliver the innovative solutions needed to address key sustainability issues, such as those related to significantly reduced carbon emissions, minimal environmental impacts, and maintaining the societal ‘licence to operate’. SUSOP® (SUStainable OPerations) is an approach for the integration of sustainable development principles into the design of mineral processes, which has been developed through the Co-operative Research Centre for Sustainable Resource Processing (CSRP). Somewhat analogous to the well entrenched HAZOP (Hazard and Operability) studies, SUSOP® is a holistic, systematic and rigorous mechanism for identifying and assessing sustainability opportunities and risks at each stage of project development - from corporate planning, through design and operation, to decommissioning and rehabilitation – without compromising financial rigour. SUSOP® was developed, tested and enhanced through its application to real case studies in the minerals industry. This development path provided significant insights into the process of identifying and evaluating opportunities for improving an operation’s contribution to sustainability and its long-term business case.This paper will discuss the need for a methodical approach for integrating sustainability principles into mineral processing plant design, present the key elements of SUSOP®, and outline its development path through real case studies. In addition, the reasons why such an approach will have growing importance into the future will be raised.

Substantial schedule advantages can be made on projects if cross-passage construction can occur at the same time as tunnel boring machine (TBM) operation with little or no impact on TBM logistics. However, this approach can prove difficult to achieve with many of the more cost-effective means of supporting the segmental lining during cross-passage break-out. This paper discusses the constraints and shortcomings of conventional approaches, and describes how a cost-effective solution involving a shotcrete shell and opening constructed inside the segmental lining was designed and constructed to deliver cost and time savings on Seattle University Link contract U220.The paper explains the spatial and schedule constraints on the TBM tunnelling and cross-passage construction, and how a shotcrete solution was arrived at in preference to traditional approaches. As the solution was novel the design had to be robust, and the paper explains how this was achieved through a combination of 3D finite element modelling and closed form solutions. Following a description of the trials to verify that the structure could be demolished post-cross-passage completion, the paper describes how the shell solution was constructed, used and demolished safely to leave the lining surface in a clean and undamaged state fit for tunnel operation.CITATION:Harding, A, Chappell, M, Burdick, M and Krulc, M, 2014. The behaviour of tailings under cyclic loading has an impact on the stability and deformation of tailings dams and a good understanding of the cyclic behaviour of the tailings under earthquakes is important for tailings engineers and tailings operators for the design and safe operation of tailings dams. This paper presents an investigation into the cyclic behaviour of tailings using both laboratory cyclic direct simple shear (DSS) testing and numerical modelling. The laboratory testing involved cyclic DSS tests on gold tailings and zinc-copper tailings under different loadings and loading frequencies. The gold tailings specimen tested at a higher loading frequency and a higher cyclic stress ratio experienced spontaneous liquefaction during cyclic shearing and the zinc-copper tailings specimen tested at a lower loading frequency and a similar cyclic stress ratio exhibited ductile behaviour and liquefied due to cyclic mobility. The liquefaction behaviour of the tailings was modelled using an advanced constitutive model (UBCSAND), which was developed specifically to model soil liquefaction and strain softening under cyclic loading. The constitutive soil model presents the nonlinear soil behaviour under dynamic loading using a hyperbolic stress-strain relation. Dynamic analyses were carried out using a finite difference program FLAC. The cyclic stress-induced excess pore water pressures measured in the laboratory were modelled using numerical simulations of the laboratory tests under different loading conditions. The results of the non-linear dynamic analyses demonstrated the capability of numerical modelling of cyclic shearing induced excess pore water pressure and the onset of the spontaneous liquefaction. The laboratory investigation and numerical modelling technique provided the key properties and parameters for the dynamic analyses to assess the stability and deformation of tailings dams under design earthquake events. Various Australian and international regulatory bodies (Environmental Protection Authority, 2015) require an early assessment of the geochemistry of waste rock. This usually includes estimates of the locations and volumes of potentially acid forming (PAF) materials and requires sampling of the waste rock zones as well as submitting the samples for a series of tests. We know that the required number of samples will vary depending on rock type, mean sulfur levels and spatial variability of sulfur as well as total volume. FerrAus Ltd's Robertson Range and Davidson Creek Projects are located in the Hamersley Province about 100 km east of Newman, Western Australia. Significant iron mineralisation defined by an aggressive exploration program within the Marra Mamba Iron Formation is shallowly north dipping at Davidson Creek, and mainly shallowly east dipping at Robertson Range, forming part of a regional scale anticline with the granitic and greenstone basement of the Sylvania Inlier as its core. With minor exceptions, the stratigraphy is comparable with that seen in the main Hamersley Basin. Granite and greenstone basement rocks of the Sylvania Inlier are unconformably overlain by shales and volcanic rocks of the Fortescue Group. The overlying Hamersley Group is locally represented by interbedded chert, banded iron-formation (BIF) and shales of the Marra Mamba Iron Formation (Nammuldi, MacLeod and Mount Newman Members), and dolomites, cherts, BIF and shales of the Wittenoom Formation. Iron enrichment occurs predominantly in the Mount Newman Member, and to a lesser extent in the West Angela Member of the Wittenoom Formation. Enrichment varies from 20 - 30 m in thickness in elongate bodies up to 3 km in strike length, and generally thins down dip. At Davidson Creek iron mineralisation is a haematite-goethite-ochreous goethite assemblage, while at Robertson Range this assemblage is consistently overprinted by a later, goethitic alteration related to 'hardcap' weathering. Near surface hardcap alteration in both project areas comprises more siliceous, aluminous and vitreous goethite. Initial exploration completed by Rio Tinto Exploration between 1999 and 2001 indicated the potential for about 20 Mt of iron mineralisation at Robertson Range. Since 2005 a detailed exploration program undertaken by Australian Manganese Pty Ltd (a wholly owned subsidiary of FerrAus Ltd) has defined a global indicated and inferred resource totaling 164.4 Mt at 58.6 per cent Fe (ASX: FRS 17 April 2009).

"Recent underground coal mine incidents have again raised the issue of: under what circumstanceis it safe for personnel to re-enter the mine? Mine rescue guidelines make recommendations aboutthe limits in terms of the circumstances, lives at risk, no lives at risk and training exercises. Theseare based upon percentages of the upper and lower explosive limits. Many factors infl uencesett ing limits for safe re-entry. These include: the representativeness of the sampling location(s),the rapidity of gett ing an underground sample analysed and the rate of change of the atmospherebeing monitored. Value judgements based upon these parameters will then infl uence the sett ing ofa trigger point based upon per cent explosibility. This raises the question of how the percentage ofthe lower or upper explosive limit is established. It is easy to defi ne this when the sampling pointdata sit on the fresh air limit line. However - what value is used when the sample sits somewherein the body of an explosibility diagram? This paper explores the various options and illustratesvarious pitfalls and problems that are faced in making such a determination. These include howthe data are represented and the assumed knowledge of those doing the interpretation."

The rapid production and accumulation of mineral processing wastes in the environment can present serious threats to environmental quality and human health. Phytostabilisation with native plant ecosystems is, in many cases, a preferred option for rehabilitation of mine waste storage facilities. However, much of the literature has focused on species selection in short-term experiments to test their suitability under the often extreme chemical conditions in the wastes without addressing the fundamental requirements – ie hydrogeochemical stabilisation and functional root zones. As a result, successful examples of phytostabilisation of mine wastes such as base metal tailings have been rare, especially if success is defined as meeting the completion criteria of non-polluting and sustainable land system at rehabilitated sites. Conventional capping options to isolate reactive tailings/wastes by using physical break and/or sealing layers are often not reliable in the long term (particularly under semi-arid climatic conditions) and, in many cases, they may not be feasible in the field due to the large areas to be covered and the lack of adequate suitable materials at mine sites. By using sulfidic tailings as an example, we have proposed a new paradigm for phytostabilising mine wastes – ecologically engineered pedogenesis in tailings involving accelerated physical, chemical and biological weathering of tailings minerals to achieve hydrogeochemical stabilisation and the development of soil functions, soil horizons and functional root zones.Ecologically engineering functional root zones firstly requires controlling geochemical reactivity in the wastes (ie hydrogeochemical stabilisation), which can cause polluting effects in the root zone and plant toxicity. By means of ecological engineering, in situ weathering of primary minerals (eg pyrites) in tailings may be accelerated by adding functional materials (eg functionalised biochars and organic matter) in the waste slurry during the process of deposition. This will remove or diminish key risk factors (eg potential acidity in tailings and elevated soluble heavy metals) in the solid phase, before placing growth media for reconstructing root zones and transplanting/sowing target plant species. However, much of the acidity, soluble metals/metalloids and other salts would be transported out of the solid phase into the wastewater, which can be addressed by on-site water treatment facilities during mine operation. Modern gold production from the Gympie Goldfield commenced in 1995 with the first production from the Monkland Mine. The primary ore target was the 2.5 m wide Inglewood Fault ore zone, which was initially mined using traditional narrow-vein shrink stoping. This manuallyintensive mining method allowed the mine to re-open with minimal capital cost, however the trade off was low productivity and high operating costs. As the mine has developed, access through 100-year-old, timber-lined, small shafts, and the high cost of delineating reserves, has restricted modernisation of the operations. In early-2000 a stockwork lode was discovered adjacent to the existing workings with an economic width of 40 m. In order to maximise the benefit of such an orebody, mining needed to be approached in a new way. This paper summarises the many changes implemented at the Monkland Mine, including the necessary cultural change, implementing long-hole stoping, de-bottlenecking and the application of mechanisation. The result has been a significant improvement in safety performance, higher productivity and a more secure future for all those involved in the project.

The Porgera gold project is now in its seventh year of operation as one of the world's prime gold mining projects in one of the most underdeveloped and remote regions of the world. The unique gold deposit was development in stages to its present size of operation with an underground mine of 2000tpd, an open pit operation moving 210,000tpd and a complex metallurgical plant treating an average of 16,500tpd of ore. During the past years the operation had to address an array of issues relating to the "development" of the region and this is still ongoing. Development means different things to different people and though the parties that signed the different agreements relating to the Mining Development Contract were aware of this, none has real- ised the extent to which this is true. The developing company (a joint venture between Placer Pacific as the Manager, Goldfields Porgera, Highlands Gold and the Government of PNG), provided funds for risky exploration and the capital for development of the project. Its shareholders ex- pect an economic and viable mine operation from the deposit to ensure them returns on their investment. The local people gave up their land, their isolated way of living and accepted to be out of their comfort zone for some time in anticipation of a better lifestyle and economic benefits in support of that lifestyle. The PNG Government has a leg in both camps and expects both outcomes at the same time. The Government is also the regulator and expects compliance on top of that. If development in general is to be successful, all parties must substantially satisfy all expectations. It is evident that the outcome of some elements of development are reasonably defined (i.e. develop a viable mine to exploit the deposit), whilst other elements are moving targets (i.e. a better lifestyle). The seeds for conflict are thus abundantly present and to resolve them requires the will of all parties in all areas to come to a solution. This path to problem solving is many times torturous as each party comes from a different culture and background and has a different understanding of the parameters in the equation.

The coal seam gas (CSG) industry is expanding rapidly in the Surat and Bowen basins. CSG production involves pumping water from the coal seams to release the gas adsorbed to coal particles. The reduction in water pressure in the coal seams will cause some reductions in water pressure in overlying and underlying aquifers because there will always be some interconnectivity between the formations. The Surat Basin is a sub-basin of the Great Artesian Basin, which contains aquifers of high economic, environmental and cultural value. The Condamine Alluvium is also an important water resource, which overlies parts of the eastern margin of the Surat Basin.Under the Queensland (Qld) regulatory framework, an area of intensive CSG development involving multiple CSG operators can be declared to be a cumulative management area. For such areas the Office of Groundwater Impact Assessment develops assessments of future impacts on aquifer water pressures and develops and manages arrangements. The results are published in underground water impact reports.The Surat underground water impact report (QWC, 2012) was approved in December 2012. The development of novel technologies capable of real-time, high-accuracy, automated analysis of gold, other precious metals and mineralogy, both in ore samples and directly on process streams is reported.X-ray fluorescence (XRF) analysis is widely used for process stream monitoring of base metals. However, detection limits are generally a few parts-per-million (ppm) or larger, precluding the use of this method for monitoring precious metals. Complex interferences with other elements also limit accuracy. An XRF slurry analyser that uses advanced data-analysis techniques to resolve overlapping fluorescence lines, achieving detection limits in the parts-per-billion (ppb) range has been developed. This allows feed, concentrate and tailings streams in precious metal applications to be monitored in real-time. An extended trial of our technology in a platinum group element concentrator and laboratory tests on gold process slurries in the 0.1 - 2.0 ppm concentration range is discussed.Analysis of process stream mineralogy can provide useful information to supplement conventional elemental data. An on-stream X-ray diffraction analysis technique that provide real-time mineral and elemental measurement of process slurry streams has been developed. Both X-ray techniques can be combined into a single on-stream analyser to provide simultaneous elemental and mineralogical information. The successful trial of a combined XRDF (X-ray diffraction and fluorescence) slurry analyser is also reported.It is widely recognised that rapid, accurate analysis of grade is an essential prerequisite for optimising the efficiency of exploration, mining and processing of metals ores. The low grades of precious metals ores, such as gold and the platinum group elements, have led to fire assay becoming the predominant analysis technique. Significant drawbacks of fire assay include long turn-around times, limited absolute accuracy, high costs and requirement for skilled operating staff. For rapid measurement of gold samples, a technique that allows the gold content of large samples to be determined absolutely with high accuracy in less than two minutes using activation with high-energy X-rays has been developed. The method is non-destructive, has detection limits in the low ppb range, is capable of measuring slurries, ground or crushed solids with minimal preparation and can be made fully automatic. There is much debate on the Competent Persons qualifying criteria for providing classified resource estimates for public reporting. The CRIRSCO family of codes including the JORC Code, SAMREC, PERC and NI43–101, require Competent Persons to have at least five years ‘relevant’ experience and belong to professional bodies. These codes provide further, albeit vague, guidelines to assist individuals with their personal decision to take on the mantle of ‘Competent Person’. There is a risk, however, that these qualifying criteria can result in ill-prepared Competent Persons whose actions and recommendations would undermine their own reputation and that of the organisations that employ them; the integrity of the profession and the principles based reporting systems; as well as the stability of stock markets that rely on these estimates.Based on doctoral study on the development of professional competency within the mining industry, this paper presents an evaluation of the suitability of the current qualifying criteria as well as an analysis of alternative criteria. The research included a series of interviews with industry experts and an online survey of resource geologists. Semi-structured interviews provided a sense of experts’ expectations and interpretation of the current definition of ‘Competent Person’. The comprehensive online survey included a self-assessment, a series of situational questions designed to evaluate responses to typical resource geology scenarios and a set of open-ended questions relating to factors contributing to competency development.A summary of the data analysis, interpretation of the results and a set of recommendations for revised ‘Competent Person’ qualifying criteria are presented. Mass Blasts are generally required at the completion of longhole stoping cycles to maximise the recovery of ore from a specific orebody. During the course of mining, the practice is to leave pillars of ore to support the opening left by extracted ore and to mitigate stresses from working areas of the stope. These pillars usually contain ore that has been included in the resource evaluation and is highly valuable to the mining operation. Due to their stressed nature, the removal of pillars by conventional blasting techniques, through small blasts, can result in pillars being reduced beyond their critical size and ultimately failing, rendering the ore unrecoverable. The AurionGold Kanowna Belle Gold Mine operates a Mine to Mill philosophy. This philosopy requires the fine tuning of blasting to provide the best possibe ore feed at the highest grade for mill productivity. This means that ore to the mill must be well fragmented from blasting that provides high extraction rates from draw points, with minimal dilution from overbreak or fall off, and sized to maximise SAG mill throughput. The AT04 Mass Blast contained significant ore reserves which were required to meet gold production targets. KBGB needed the AT04 Mass Blast to perform as well as if not better than conventional stope blasts to maintain mill production. This paper describes the circumstances, planning philosophy, techniques, including the use of Electronic Initiation Systems and Bulk Emulsion Explosives, and results that provided a highly successful outcome.

MacPhersons Reward gold deposit is in a post-kinematic tonalite intrusion which is a setting different from many other deposits of the Coolgardie goldfield that are hosted in early Archaean tholeiitic sills or meta-sedimentary rocks. Gold mineralisation at MacPhersons Reward consists of auriferous quartz veins that are best developed in the tonalite but extend to the adjacent komatiitic rocks and along the lithological contact zones. The gold-related alteration halo is dominated by biotite-chlorite and pyrrhotite. Unlike many other Coolgardie deposits, the As concentrations are low.
