One area of study in the Coal Mine Injury Analysis project was to illustrate and enhance the ability of mine safety management personnel to apply analytical techniques to injury experience data and to justify requests for funds for safety enhancement actions on a cost/benefit and other quantitative basis. Four volumes of this final project report detail (1) the objectives and scope of the mine safety management research, (2) the results of literature review, and case studies, and (3) models developed for mine safety management. The four volumes are titled as Volume III: Mine Safety Management, Volume IV: Analytical Techniques for Mine Safety Management; Volume V: Mine Safety Management Case Studies and Volume IX: Training Cost Model. This volume deals with the development of a microcomputer based training cost model (TCM) which can be used to analyze training cost implications of various types of training. The model has been developed using Aston Tate's dBASE III plus language. The computer program and a test run of the model are presented in this report.

As the surface reserves are being depleted, more and more stone operators are seeking underground mining options. Stability of underground openings is a major concern for the safety and productivity. This paper relates to a field study conducted at a large underground limestone mine in Central Pennsylvania. Identification of hazards leading to ground failures and assessment of roof fall risks are important in mitigating injuries. Field observations at this mine were integrated with Roof Fall Risk Index (RFRI) mapping software recently developed by West Virginia University. The system is designed to combine field study data related to geologic factors, mining-induced damages, roof profile, and ground water influx to generate RFRI maps. The mine has adopted a proactive ground control program consisting of comprehensive examination of drilling and bolting logs; precision surveying of roof and rib; accurate mapping of cutters, falls, and geologic structures; and installation of extensometers and microseismic monitoring systems. Investment of time and resources has eliminated fatalities and lost-time injuries due to ground fall. This paper presents the geologic settings, mining conditions, ground control issues along with risk management and control techniques adopted at the study mine.

The literature dealing with the electrical conductivity of solid dielectrics, including all its aspects and its relationships to other physical properties of these materials, is extensive and scattered in many books, phamphlets, and periodicals. It is believed that most of the important contributions to this field up to October 1943 have been listed and abstracted in this circular. Also included are some general references on static electricity. Methods of removing static charges are described; methods of and conditions for measurements of resistivity are discussed; equations for the calculation of resistivity are given; and methods of resistance measurements in Situ are reviewed. Data on the variations of resistivity with changing conditions as well as explanations for them have been collected, and three tables showing the resistivity of various dielectrics at different humidities are included. The appendix contains a short development of the mathematical formulation of the charging and discharging currents due to interfacial polarisation. INTRODUCTION Methods for Removing Static Electricity Of the various means employed for preventing or removing accumulations of static electricity in industrial processes, grounding is the most common and most generally effective. It is a precaution readily applied to stationary machines anti may often be utilized to good advantage to rid persons and portable equipment of an acquired charge. There are many situations, however, in which direct low-resistance grounding cannot be used, either because the charged bodies are insulators or are inaccessible, or because contact to a ground lead might give rise to a hazardous spark. If the charged body is a conductor, contact made to a neighboring conductor, even though the latter be insulated from ground and of relatively small capacitance, may also result in the occurrence of an incendive spark. In such instances, static electricity can sometimes be removed most satisfactorily by means of strong atmospheric ionization produced by radioactive material, X-rays, ultraviolet light, or A. This paper presents the results of the geotechnical investigation for the repair of the Fashion Island Boulevard Bridge, Bridge No. 350160, in San Mateo, California. The concrete, precast girder bridge suffered considerable damage during the Loma Prieta earthquake. The existing 36-inch, precast, prestressed concrete piles cracked and spalled at their tops. Based on analyses performed by Biggs Cardosa Associates, structural engineers, the piles did not have sufficient strength and ductility to resist the dead plus seismic loads specified in the Caltrans "Bridge Design Specifications". Additional support for the bridge was needed and the repair strategy consisted of installing large diameter pipe piles at the existing bent and abutment locations. This paper describes the geotechnical engineering analysis and design decisions for the large diameter pile design.

The trend in gas well testing has been to rely more on the early-time flow data of drawdown and buildup tests than on stabilized flow tests. The stabilized testing methods often are not adequate for complicated modern gas reservoirs because many of these reservoirs have extremely low permeabilities and the transient flow period of a well test can last for days or weeks. It has become common to augment the flow capacity of gas wells by hydraulically fracturing their producing formations. In deep reservoirs the induced fractures are generally vertical and tend to follow a single plane of weakness. The presence of a vertical fracture at the wellbore complicates the transient flow behavior of a low permeability gas well. The flow is further complicated when turbulence occurs near the wellbore. Russell and Truittl published transient drawdown solutions for vertically-fractured liquid wells. They developed methods of drawdown and buildup testing utilizing these solutions, which were based on numerical simulation. Clark2 applied the basic Russell-Truitt solutions to analyze fractured water injection wells by falloff tests. Field examples were given to substantiate the method of analysis. The Russell-Truitt solutions and well testing methods can be extended to gas well flow. Millheim and Cichowicz3 presented the drawdown equations for *Now with Scientific Software Corp., Denver. ideal gas flow, including the effects of formation damage and turbulence. Two actual field cases were presented, both of which exhibited extensive fractures and turbulent flow. For the pressure range of these well tests, the use of ideal gas equations was acceptable. These field cases were significant in being the first published data showing the occurrence of turbulence in fractured gas wells. We, also, observed such cases in practice. Our purpose here is to extend the theory of fractured gas well testing to the flow of real gases. To determine the effects of wellbore storage and turbulence on well test interpretation, we developed a finite-difference model to simulate well test conditions. Since gas wells usually are widely spaced and have high compressibility, the emphasis has been put on the early transient behavior before the effects of the outer boundary are noticeable at the wellbore. The Mathematical Model The geometry of our mathematical model is similar to that used by Prats.4 The well is centered in a circular uniform reservoir. A vertical fracture of infinite flow capacity penetrates the formation and passes through the wellbore. The wellbore itself is not important. Fig. 1 shows a sketch of the problem. The purpose of this report is to chronicle the interaction between Anglo Platinum and Sandvik Tamrock during the development and application of Long Hole drilling as a viable, practical method of narrow reef mining. Appendix I identifies the people from Anglo Platinum and Sandvik Tamrock who participated in the project. Experiments in long hole drilling and blasting were conducted in a number of mines during the late 1950s and early 1960s. Some of the advantages envisaged were: ? The face would require no daily examination ? No barring down or immediate face support required ? No sockets to clean ? Only a few well drilled holes rather than large numbers of short blast holes ? Avoidance of poorly-marked holes, inaccurate drilling and improper charging ? Better supervision and better face advance. Holes 50 mm in diameter were drilled parallel to the stope face by pneumatic rock drills from advance strike gullies. Holes were charged with amon gelignite cartridges. All holes were traced with detonating cord to ensure propagation of the explosive along the entire hole length. Rock was blasted onto a rolling scatter pile. A number of reasons have been cited for the failure of this mining method. The two most important were: ? problems in controlling hole deviation which was exacerbated by the fractured ground ? difficulty of charging up holes in fractured ground, due to relative movement of rocks along fracture surfaces over the hole length. The idea of drilling a few long holes parallel to the face rather than a large number of short blast holes perpendicular to the face was an attractive option to mechanize narrow reef mining. This particular project started in 1998. Initial trial The Union Section of Anglo Platinum purchased a Solo H 606 RA long hole drilling rig from Sandvik Tamrock in 1997. In order to evaluate long hole drilling as a basis for face mechanization. The Solo series of drill rig was selected because it combines high drilling performance and good drilling accuracy with high versatility with a compact size. The mining method proposed by a consultant to Anglo Platinum was based on the current trackless mining method practised by Union Section. The trackless mining method is better described as a trackless development and transport method. Reef strike drives were installed approximately 40 metres apart on dip using trackless equipment. Reef was mined using hand-held pneumatic rock drills and scraper winches and the excavation was supported by sticks and crush pillars at the top of the panel. Rock scraped from the panel was loaded by Lad Haul Dumps (LHDs), trammed to an inclined haulage, and loaded into trucks for transport to the surface. It was intended that the Solo would drill from the bottom advance strike drive to the top advance strike drive. Blasted rock would still have to be scraped from the panel. It is also difficult to understand how well the support would have survived. In the initial motivation it was stated that drilling accuracy was of the utmost importance. Previous work by Sandvik Tamrock had shown that, in other parts of the world, 20 metre long holes could be drilled with a deviation not exceeding 100 mm provided that: ? holes were collared with low thrust pressure ? operating percussion pressure was kept low -often not ? exceeding 10 MPa ? drilling was to be with tubes, not rods. In early 1998 initial trials with the Solo H 606 RA were inconclusive; attempts were made to drill 40 metre long holes from one advance strike drive to another. Difficulty was experienced in measuring hole straightness and accuracy, there was no survey of the holes, and no blasting was done to expose hole barrels. A long hole mining system was then defined, with a corresponding face mining system, which led to the design of a test programme for the Solo drilling rig. Earlier experience at Telfer The proposed face mining system was based on a mining method that is practised at Telfer Gold Mine in Western Australia. Telfer has a series of domed reefs that dip at angles of less than 20° to more than 40°. The ore bodies are all thin, with widths from 0.6 metres to 0.8 metres. Figure 1 shows the face layout. Experience at Telfer indicated the following: ? The spacing and size of the dip pillars should be based on the rock mechanics of Union Section ? The length of the face is determined by the ability to correctly position the blast holes. Positioning of blast holes is a function of hole straightness and hole direction. Blast holes in Telfer were drilled 64 mm diameter with retrac bits from Sandvik. Extension steel was T38 by 1.8 metre lengths. A guide rod was fitted immediately behind the bit. To achieve greater accuracy' operators drill at relatively low percussion and thrust pressures. Results show that very good accuracy is achieved in 20 metre stopes and only fair accuracy in longer stopes. The approximately 2800 Ma old Ventersdorp Contact Reef is a unique Witwatersrand orebody, from which about 6% of the world?s gold production is derived. A major portion of this production is mined at depths exceeding 2500 m and deepest levels of mining are at about 3600 m. The orebody?s hangingwall consists of volcanic rocks. This differs from the older Witwatersrand orebodies, which have quartzitic hangingwalls. The thickness of the Ventersdorp Contact Reef is generally about 1.20 m, but is highly variable, at an average dip of about 18°. A pronounced unconformity beneath this orebody is also expressed by varying footwall rock type characteristics and frequent rolling of the reef plane. These rolls follow topographic variations of the palaeo-surface. More geological than rock mechanics studies were performed on the Ventersdorp Contact Reef prior to 1992. Recently, however, a multidisciplinary team comprising various rock engineering and geological disciplines has delineated geotechnical areas by a novel technique. Geotechnical areas are defined by distinct footwall/hangingwall rock assemblages. The presence of three footwall and two hangingwall rock types results in the definition of six geotechnical areas across a distance exceeding 60 km, including about 10 gold mines. Distinct rock mass behaviour has been documented in three of the six geotechnically defined areas. Different rock mass behaviour is expressed by features such as mining-induced fracturing, closure rates, fall of ground characteristics and seismic character. Some geological features override the rock mass behaviour as defined for the individual geotechnical areas. These are undulations of the orebody or the frequency, characteristics and attitude of major discontinuities. It is concluded that the concept of geotechnically subdividing orebodies is not only important for facies delineation and the associated grade, but also needs to be considered for safety aspects, in determining factors such as the appropriate support to be employed.

With prevailing assumptions that plentiful domestic coal reserves will be available for meeting our Nation’s expectations for on future low emission, coal-fired power plants, major government funding in Clean Coal Technology, and Carbon Sequestration Programs continues to be aggressive. However, after more than a decade of cutbacks in funding for domestic coal-related programs and personnel, there is a growing concern that adequate quantification and qualification of our Nation’s coal reserves is increasingly being compromised. This paper will define the growing concerns and dichotomies that continue to increase, and address the decreasing number of fossil fuel mapping and assessment programs by Federal and State programs.

ALTHOUGH nickel was in use in alloys long before the Christian era, the metal was not discovered until 1751, when Cronstedt recognized it in niccolite from Sweden. The Chinese apparently used a nickel alloy, known as "pahfong" thousands of years ago and Persian coins dating farther back than 200 B.C. consist of a copper-nickel alloy similar to some of those now in use in coins. The pure metal was first prepared about 1775 and the metal was refined for commercial purposes some years later. The nickel industry did not attain much prominence until the ores in New Caledonia were discovered. Exports of ore from that country began in 1875 and since that date there has been an ever-increasing world output of nickel and a great expansion in the uses of the metal except during short periods of commercial and industrial depression. Previous to 1875, for a number of years, Norway was the chief producer, although the metal was mined also in Sweden, Finland, Italy, Germany, Austria, France, China, the United States and other countries. With the dis-covery of nickel deposits in the Sudbury district, Ontario, a new factor was introduced into the nickel industry and Sudbury has since become the overwhelmingly dominant feature in the world's nickel markets. Although nickel was first discovered in Canada as early as 1848 near the north shore of Lake Huron and nickel-copper sulfides were found in 1856 by surveyors working close to the present location of the important mine at Creighton, in the Sudbury field, it was not until 188.3 that any importance was attached to this field. In that year copper sulfide attracted attention where the main line of the Canadian Pacific railroad was being projected across Canada to the Pacific coast. Mining began near this discovery in 1889 but in the meantime operations had begun in 1886 in other parts of the field and by 1890 about 100,000 tons of ore had been raised. In the Sudbury field, as in Norway and at the old Lancaster Gap mine in Pennsylvania, the ore was first mined for its copper content and difficulty in the refining of the copper led to the discovery of the important proportions of nickel in the ore. A good thing is becoming a bad thing. Rock mass classification systems, that are so excellent for communications between engineers and geologists, and that can be valuable in categorizing project experience, are emasculating engineering geology and rock mechanics. Some engineering geologists have been sucked into thinking that Q and RMR values are all that is needed for engineering purposes, and seem to have put aside what can be learned from structural geology and geomorphology. Many rock mechanics engineers seem to have forgotten the scientific method. This paper attempts to redress the situation by showing how mechanics can be used in rock engineering to design with a similar rigour to that used in the fields of structural engineering, hydraulics and soil mechanics. It also attempts to remind practitioners of what can be achieved by skilled engineering geology.

The Brightwater conveyance system includes 26 km of tunnels, a 130 mgd pump station, 2 km of micro tunnels, and a 2.4 km outfall. The tunnels extend from Woodinville to Puget Sound through varying soils in 1 to 7 bars of hydrostatic pressure and depths to 185 m. The tunnel diameters range from 4 to 6 m and include numerous lining systems. This paper highlights major shaft and tunnel design, including dual cellshafts, multiple pipe configuration tunnels, polymer concrete segments, and the use of a one pass lining designed to convey high pressure effluent.

Ground characterized by hard boulders embedded in soft matrix represents one of the most challenging conditions for TBMs, as the cutterhead must cope with the non-optimal conditions of cutting hard blocks while simultaneously excavating through soft, soil-like matrix material. Dowden and Robinson (2001) and Hunt and Mazhar (2004) summarize several case histories relating to the problems of advancing a TBM through boulder-laden ground and provide guidance with respect to appropriate site investigations and baselining strategies. Some rock types constituting boulder-laden ground include: ?Sedimentary Rocks?cemented alluvial channel deposits, colluvium/talus, landslide debris, and glacial till. ?Igneous Rocks?agglomerates, lahars, and pyroclastic deposits. ?Metamorphic Rocks?tectonic mélange and cataclastic fault zones. ?Decomposed Rocks?corestone bearing saprolites. Disc cutters are commonly utilized to cut the hardblocks; however, as compared to the understanding of disc cutter behavior in hard continuous rocks, little is known regarding fundamental cutting mechanisms in highly heterogeneous bouldery ground conditions. To advance this understanding, a suite of full scale tests have been performed using a linear cutting machine at the Colorado School of Mines (CSM). This paper summarizes the observed cutting mechanisms and their practical implications for TBM excavations through boulder-laden ground.

Nowadays, with such a focus on environmental protection, surface mining activities have come under pressure to ensure their activities do not adversely affect the local surroundings. As part of the extractive process, blasting activities are an integral part of a mining company’s operational cycle. Consequently achieving beneficial blast results while complying with environmental requirements has become a key focus. This especially is the case when firing blasts within close proximity of residents where additional control measures need to be implemented to control vibration. One case where this occurs in Indonesia is in the Paringin Pit, one of the operational areas of the company's. The pit has complex geological conditions and is surrounded by several residential areas. Between 2014 to 2017 there were 12 cases of ground vibration exceedance due to blasting. As part of a continuous improvement project the company's targeted minimizing the vibration exceedance in this area. A project was undertaken to research and develop a method to confidently predict vibration. It was decided that the scaled distance formulas were not adequate to predict vibration when using non-electric detonators, as accurate wave propagation could not be achieved. The problem is how to manage of complex geology condition related with wave propagation in ground vibration control. Geological condition mapping with specific spatial range can and combine with Signature Hole Analysis be a solution to make wave propagation easier to accommodate in ground vibration control. With this new approach geological condition mapping with specific spatial range was produced that would minimize vibration while ensuring beneficial blast results. Introducing the new prediction produced nil cases of vibration exceedance in 2018 and beyond and proved that the Signature Hole Analysis in specific spatial range of geological condition is the preferred method to optimize blast result and maintain vibration control.

Gwynfynydd is a small, high-grade black shale-hosted vein-gold deposit. The Chidlaw Link Zone is the only area being mined and contains 24 000 t ore grading 15.2 g/t gold. Within the Link Zone a complex vein structure occurs consisting of a mother vein up to 5 m thick and narrow (less than 0.5 m) leader veins. The former shows minor variations in dip, strike and thickness, whereas the latter show marked changes in all three. The leader veins are hosted within the footwall zone of the mother vein and both are composed of quartz with galena, sphalerite, chalcopyrite, pyrite and minor arsenopyrite. The vein system is transected by a series of cross-vein and intra-vein dip-slip faults. Pockets of gold occur at vein intersections or branches and within the complex footwall zone and are often related to variations in vein attitude and/or geometry. The Chidlaw Link Zone displays a complex, erratic distribution of gold mineralisation and a multi-stage history of structural activity, fluid flow and mineral deposition

This report describes methods and techniques employed by The Glidden Co. of Cleveland, Ohio, in explorating an ilmenite-bearing sand deposit in Ocean County, New Jersey, during 1960 to determine ore reserve potential and the feasibility of mining and establishing a milling plant on the property for the production of ilmenite concentrate. The property covered approximately 1,250 acres. Comprehensive sampling by water-jet drilling indicated the deposit averaged 20 feet in thickness and contained 4.5 percent heavy minerals, of which 80 to 85 percent was ilmenite and leucoxene. Average depth of the water table as determined by auger boring was 16.8 feet. Pilot plant tests indicated that circular concentrators in combination with spiral concentrators would recover at least 80 percent of the heavy mineral content of the sands. It was estimated that sufficient ore reserve tonnage had been developed to supply the company's Baltimore chemicals division, pigments department, with a suitable grade ilmenite concentrate for a period of 20 years at the present rate of pigments production. The property was designated as Lakehurst mine and plans are in progress for mining development and plant construction.

"In mechanized mining, poor equipment efficiency (availability, utilization, productivity, and quality) can endanger the success of the operation. This case study will show how an initiative to improve equipment performance developed into a comprehensive turnaround plan for the mine that placed it in the forefront of performance achievement.As part of a company-wide review process, poor overall equipment effectiveness (OEE) was identified as a major reason for the mine not achieving its targets. A project to improve the OEE identified eight improvement areas (elements) that contributed significantly to the poor performances. Measurement metrics were determined for these elements, followed by determination of baseline and target (improved) key performance indicators (KPIs). Cost savings associated with the improved efficiencies were calculated and tracked throughout the project. The mine team determined the specific actions required to achieve the target KPI in each element. These were individually developed and managed like mini-projectswith allocated responsibilities for delivery. The paper will indicate how this OEE improvement initiative triggered an improvement in almost all sections of the mine. Soon after launch, the initiative gathered momentum as the KPIs starting to improve. A visible tracking system exists at the mine and each employee can see the improvements and feel the success. The original eight elements were extended by five more, and the mini-projects grew as participants saw the success of the initiative.This paper concludes that through management and worker involvement, visible measurement and controls, and carefully chosen improvement elements, the mine was turned around. It is now achieving and exceeding its targets, and employee relations and motivation as well as safety have improved considerably. All of these achievements are reflected in the bottom line."

With gold first discovered at Carrick in 1864, exploration and mining on the Carrick Goldfield in Central Otago, New Zealand, has now been ongoing for more than 130 years. Recorded historical hard rock production from up to 52 different mining claims from 1864 to about 1921 is 24,047 ounces (oz) of gold.   Since 1980 Amoco Minerals New Zealand Limited, CRA Exploration Pty. Limited, Amax Gold Mines New Zealand Limited and Summit Gold (New Zealand) Limited have conducted modem systematic exploration campaigns targeted at locating shear controlled gold bearing quartz lodes and stockwork deposits. More than NZ $1million has been spent by these companies on aerial photography, geological mapping, stream sediment surveys, soil and rock chip geochemistry as well as reverse circulation (RC) percussion and diamond drilling.   RC drilling by Summit Resources in 1997 has identified a 1,500 m long zone of low-grade gold mineralisation at Lower Carrick, with an average grade of 1.00 g/t Au over a 14 m average thickness. This gold mineralisation, hosted within a low angle shear zone, has analogies with the Hyde-Macraes Shear Zone in eastern Otago. Recent exploration and drilling at Carrick has revealed the potential to discover large (>0.5 million ounce (Moz)) low-grade bulk shear controlled quartz vein and stockwork gold deposits similar to those now being mined at Macraes.

Water use and wastewater management is becoming a focus for many industries around Australia and the rest of the world. Good quality water is increasingly being recognised as a precious resource, and regulations on the bulk storage and discharge of waste streams is becoming more stringent in order to protect the environment, existing water reserves and the communities that use these resources. Components such as heavy metals, arsenic, cyanide, and sulfate from acid mine drainage are some that occur at mining and metallurgical processing sites. Removal of such components can allow safe re-use of these streams, reducing the new water requirements of the process, or else allowing acceptable discharge to the environment. Re-use or discharge of treated water eliminates or reduces the risk associated with storage of large volumes of contaminated water on site.Technologies where a contaminant is selectively precipitated and removed as a sludge are used for many of these treatments. Selection of the best treatment option for wastewater must depend on what is suitable to each site, and consider not only the nature of the contaminant, but also other aspects, including the location and situation of the site, local regulations, cost of reagents, the sensitivity of the local environment and waste sludge disposal options. Blast vibration in the highwall from a cast blast in open pit mines or in the hanging walls from a stope blast in underground mines is a common concern. Vibration in such cases is a typical near-field situation where the distance from a blasthole to a point of interest varies significantly compared to the distance between the blast as a whole and the point of interest. Most existing blast vibration models are designed for far-field vibration using a single seed wave to represent all blastholes within a blast and do not simulate wave shape change over the distance.
