"Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

I N the introductory chapter to "Man and Metals," T. A. Rickard wrote "Five hundred thousand years ago the first footfalls of man's oncoming echoed down the corridors of time." This phrase has always appealed to me, not only for its resonance but because it seems to place in some perspective man's place in the cosmos. As we go about our daily occupations and as we meet here tonight, we must agree that we have come a long way slowly at first through the ages of change but with a quickening pace that has brought to man's know-ledge and use, within the last hundred years, more ideas, more material advantages, more opportunities and more comforts than were ever imagined or dreamed of in those long corridors of time. Looking back to the dawn of his-tory, a mere five thousand years ago, there is one aspect of the birth and growth of civilization that has particular significance for the miner and the metallurgist.

El presente trabajo señala el proyecto de remediación de los relaves mineros realizado en la relavera Ticapampa, ubicado en el Distrito de Ticapampa – Huaráz, región Ancash, con el objetivo de remediar los relaves de TICAPAMPA depositados por más de 30 años y que contaminan al río Santa, fuente principal de irrigación del Valle del Callejón de Huaylas. El proyecto comprende la extracción de contaminantes del pasivo ambiental de Ticapampa a través de la flotación de los sulfuros de fierro, y la obtención de un material inerte (con bajo contenido de sulfuros contaminantes) para la fabricación de productos cerámicos por medio de resinas recientemente desarrolladas.

The year 2010 will mark the 100th anniversary of the beginning of the research efforts by the U.S. government to improve the health and safety of miners working in the United States. This research began in 1910 with the establishment of the U.S. Bureau of Mines (the Bureau) and it has continued during the past 10 decades, even after the closure of the Bureau in 1996. The research on safer mining technology, formerly done by the Bureau, has now been combined with National Institute for Occupational Safety and Health (NIOSH) studies of miners? health to form a unified federal research program to improve the health and safety of miners. This article describes this research and places some of the accomplishments into perspective with other activities in the U.S. Mine Safety and Health Time Line (insert).

The paper describes the search for mineral deposits in an area of 4,500 sq. miles of south-west Tasmania. The area is one of difficult access and the problem of speedy movement and maintenance of field parties was solved by the use of helicopters. The programme has followed a systematic pattern of the aerial photography of the exploration area with the preparation of photQmosaics and topographical base sheets from these mosaics. Geological maps were initially prepared by photogeological interpretation and making use where possible of pre-existing information. The maps produced were later checked by ground traverses. Geophysical maps were obtained by the use of airborne magnetic, electromagnetic and radiometric methods. Geophysical and geological anomalies were checked on the ground by a campaign using ground geophysical equipment in the former case and geological and geochemical work in both instances. Audio Frequency Magnetic and Induced Polarization techniques were used in an area which was covered by several hundred feet of Tertiary sediments.The campaign has reached the stage of the drilling of specific targets.INTRODUCTIONThis paper describes the activities of Lyell-E.Z. Explorations (L.E.E.), an organization which was formed in 1956 by the Mt. Lyell Mining and Railway Co. Ltd. and the Electrolytic Zinc Co. of Australasia Ltd. to explore for mineral deposits in an area of approximately 4,500 sq. miles in southwest and southern Tasmania. To provide the necessary personnel and equipment for this project the exploration resources of both Companies were pooled and augmented and a headquarters was established in Queenstown. The work was directed by a Committee of four made up of officials from both Companies; a review of this structure has recently been presented by Hall (1959).

This paper presents a waste rock treatment and rehabilitation program conducted at a metaliferous mine site in northern New South Wales from 2000–2013. It updates findings from 2005 for Program #1 (which examined acidity, leachable metals and tree growth in a control and three treated areas of waste rock) and includes new results for Program #2 (which examined acidity, plant uptake of metals and trace elements and tree growth in a control and four treated areas compared to undisturbed background levels) and Program?#3 (which examined the treatment and revegetation of submerged mine tailings and an exposed tailings beach).Program #1 showed low soil pH in the control area remained unchanged from years 1–14, and soil pH in the limed and capped areas was initially increased but fell back to starting levels within 12 months and remained low for 14 years. In contrast, soil pH in the Terra B area resulted in soil high pH immediately after treatment and increased in years six and 14 without further treatment. Tree height and density in 2005 and again in 2013 indicated acidity in the control, limed and capped areas suppressed tree height and density; however, in the Terra B area, revegetation in 2013 was comparable to undisturbed forest. Outcomes in the control, biosolids and three Terra B areas in Program #2 were similar to those observed in Program #1.Program #3 showed the submerged and exposed tailings initially had low pH and high concentrations of total and leachable metals before treatment, but these became non-leachable after treatment with Terra B. Revegetation was achieved on the tailings beach, with almost uniform colonisation of bulrush and common reed, along with bottlebrush and golden wattle; a variety of reptiles have also been observed on the revegetated tailings beach, along with migrating ducks, spoonbills and grebes.CITATION:Fergusson, L, Blair, D and Scutts, T, ?2014. "Heat-shrinkable cable accessories are solving problems in the mining industry. This paper describes a range of products used by the industry, including low-voltage splices, high-voltage splices to 8.7 k V, cable repair sleeves, mining cable terminations used on system voltages up to 25 kV and motor connection kits up to 8 k V. Technical procedures and designs are illustrated.IntroductionThe development of high-performance heat-shrinkable materials slightly over 20 years ago proved to be one of the major technological breakthroughs in power cable accessories. Heat-shrinkable products were introduced initially for use as low-volt age cable accessories in the aircraft and electronics industry. A program of continuous development and product introduction now enables the manufacturer to offer a broad range of products for mining, industrial and utility electrical systems.With the advent of atomic energy, an important discovery was made-exposure of some plastic materials to high-energy electrons can cause the permanent crosslinking, or intermolecular joining, of adjacent molecules. Once the material has been crosslinked, it will not melt or flow at any temperature.The unique properties of heat-shrinkable products also result from the exposure of special thermoplastic formulations to high-energy electrons. Because of crosslinking, these products have perfect elastic memory. The heat-shrinkable products are supplied in expanded condition. When heated, they will shrink and tightly cover the object over which they have been placed. 'The functional and operating demands of mine cable accessories exceeded the classic electronic applications of heat shrinkable materials. These demands required a very sophisticated program to develop materials and adhesive systems that could endure the operational environment associated with the mining industry,Ease of assembly, low installed cost and operational reliability are among the main reasons for the acceptance of heat-shrinkable mine cable accessories in electrically powered mines. The transport of fluid, heat ,and particles (bubbles and solid inclusions) in flowing molten steel is investigated in steel refining ladles, the continuous casting tundish, continuous casting mold and strand, and steel ingot casting processes. The two-equation k-?? model is used to simulate the turbulence. Multiphase fluid flow is numerically simulated with a Lagrangian-Eulerian approach, an Eularian-Eulerian approach and the Volume Of Fluid (VOF) method. The simulation can predict inclusion trajectories, inclusion removal fraction, free surface waves and other phenomena, which can be used to optimize these important metallurgical operations.

The Juneau Gold Belt, centered on Juneau and approximately 120 miles long by 10 miles wide, stretches from Berners Bay on the north to Windham Bay on the south (fig. D-l). Access within the Juneau Gold Belt is provided by the Glacier, Thane, and North Douglas Highways. The Glacier Highway extends northwest from Juneau for approximately 40 miles to Echo Cove, and the other two roads end 5.5 miles southeast and 12.6 miles west from Juneau, respectively. None of these roads connect with other highway systems and access to other areas is restricted to airplanes, the State ferry system, and commercial barge lines. BUREAU ACTIVITIES The majority of lode gold produced in Alaska came from the Juneau Gold Belt portion of the Juneau Mining District. To evaluate the economic potential of the Gold Belt, the Bureau located 173 mines and prospects within the area. A total of 263 ad its and shafts were found and 56,650 feet of underground workings were mapped. The Bureau collected and analyzed 1,557 samples during the study. In addition, considerable detailed geologic map-ping was done in the vicinity of the mines and prospects from which ore control models were developed.

"In Europe, and therefore in Hungary opening a new mine is harder and harder. Depletion of mines, unfavourable legal and economic changes, and growing public concerns are the main reasons. Social and economic interests require total utilization of mineral raw materials of the operating mines, and furthermore utilization of waste heaps of the former mines. There are 5-6000 waste heaps of the operating and the closed mines in Hungary. 2000 of these waste heaps have already been estimated, and researches show that total amount of waste heaps can be calculated to 1 billion tones. These researches left out of consideration waste heaps exploited and placed in deep lignite mining. In several projects and researches we analysed processing of waste heap and possibilities to use its components. A processing technology for utilizing waste heaps has been developed based on laboratory and pilot plant tests. Great amount of useful materials of quarrying waste heaps can be utilized with this technology. Tests were also done for processing waste heap of a former ferrous metals mining. The developed processing technology contributes to use these waste heaps as aggregate for cement industry, as dielectric and aggregate for construction. The essay presents in details main characteristics of waste heaps, technologies already used for processing waste heaps of quarrying and ferrous metals mining, mass balances and qualitative parameters of the end products."

The trend in gas well testing has been to rely more on the early-time flow data of drawdown and buildup tests than on stabilized flow tests. The stabilized testing methods often are not adequate for complicated modern gas reservoirs because many of these reservoirs have extremely low permeabilities and the transient flow period of a well test can last for days or weeks. It has become common to augment the flow capacity of gas wells by hydraulically fracturing their producing formations. In deep reservoirs the induced fractures are generally vertical and tend to follow a single plane of weakness. The presence of a vertical fracture at the wellbore complicates the transient flow behavior of a low permeability gas well. The flow is further complicated when turbulence occurs near the wellbore. Russell and Truittl published transient drawdown solutions for vertically-fractured liquid wells. They developed methods of drawdown and buildup testing utilizing these solutions, which were based on numerical simulation. Clark2 applied the basic Russell-Truitt solutions to analyze fractured water injection wells by falloff tests. Field examples were given to substantiate the method of analysis. The Russell-Truitt solutions and well testing methods can be extended to gas well flow. Millheim and Cichowicz3 presented the drawdown equations for *Now with Scientific Software Corp., Denver. ideal gas flow, including the effects of formation damage and turbulence. Two actual field cases were presented, both of which exhibited extensive fractures and turbulent flow. For the pressure range of these well tests, the use of ideal gas equations was acceptable. These field cases were significant in being the first published data showing the occurrence of turbulence in fractured gas wells. We, also, observed such cases in practice. Our purpose here is to extend the theory of fractured gas well testing to the flow of real gases. To determine the effects of wellbore storage and turbulence on well test interpretation, we developed a finite-difference model to simulate well test conditions. Since gas wells usually are widely spaced and have high compressibility, the emphasis has been put on the early transient behavior before the effects of the outer boundary are noticeable at the wellbore. The Mathematical Model The geometry of our mathematical model is similar to that used by Prats.4 The well is centered in a circular uniform reservoir. A vertical fracture of infinite flow capacity penetrates the formation and passes through the wellbore. The wellbore itself is not important. Fig. 1 shows a sketch of the problem. Over several decades in Southern Africa the advance rates when sinking vertical shafts have declined significantly. The upside has been a substantial improvement in shaft bottom conditions and a commensurately improved safety performance, with an increasing number of shafts being sunk without fatalities and disabling injuries. This paper briefly reviews historical sinking rates and matches changed sinking productivity to the introduction of new sinking methods that have been adopted to eliminate/mitigate safety risks. Meeting the challenge of returning to historical advance rates whilst progressing in the quest for zero harm by utilizing three dimensional animated computer modelling is described. Making use of commercial modelling software makes it easier to evaluate different methods, sequencing, in shaft barrel ergonomics, and stage and shaft bottom layouts before procuring project plant and equipment. Benefits accrue not only in better communicating methods and configurations for detailed planning and design, but also during crew training. The traditional engineering ?as-built? drawing practice has been extended to the concept of an ?as executed? sinking model which seeks to capture operational experience and learning for future best practice. The paper concludes with some of the organizational culture challenges being overcome as this non traditional approach to shaft sinking optimization is being introduced.

ARE we a "have" or a "have-not" nation in our domestic supply of metals and minerals? Impinging on the ears of a people weary of war and faced with the problems of reconversion to peace, the import of this question has built up with the momentum of an avalanche. Stemming from statistics cited by Pehrson and accelerated by the pessimistic and possibly biased statements of Ickes, hardly an issue of any technical periodical dealing with mining has appeared during the past year but within whose pages was to be found some comment, some discussion of the question: Are we a "have" or a "have-not" nation? We have proved commercial reserves of high-grade iron ore in the Lake Superior region sufficient for 22 years, based upon the prewar rate of consumption; commercial copper reserves to last only 34 years, one fourth being inferred ore; zinc sufficient for nineteen years, over half being inferred; lead for twelve years, two thirds of which is inferred; mercury for only three years. So read the figures. Although admitting that these figures may be extended by new discoveries, by the use of marginal and submarginal ore, and by further technological advances, are we nevertheless to assume complete pessimism in the acceptance of the meaning of these statistics; are we to expect a rapidly increasing dependence upon foreign sources of mineral supply; are we to believe that we are at or near the peak of the "metal age" as has been held by Theodore J. Hoover? Or is some more optimistic viewpoint possible?

The Santa Rita mine is located in north eastern part of Bahia in Brazil and is owned and operated by Mirabela Mineração do Brasil Ltda. Typical SABC circuit was designed as the preferred option for the 4.0 mtpa (500tph) to produce a P80 of 125um for flotation. Although the SAG was originally designed to operate with conventional radial pulp lifters, later it was fitted with Outotec?s Turbo Pulp Lifters ? TPL (Latchireddi, 2005). The milling circuit commenced commissioning on the 24th of October 2009 and the milling circuit achieved the nameplate capacity within days. with small and relatively simple measures to rectify. Since commissioning, the SAG mill has been working in fully autogenous grinding (FAG) mode with no balls (in contrast to the estimated 10% ball charge) and has been consistently treated up to 650tph. This paper examines the comminution design process, the operating philosophies, start-up experiences, post commissioning optimization and future development works. An evaluation of mill performance relative to the design criteria for the project life to date is also included.

Worldwide, promulgation and enforcement of health and safety laws and regulations have been the primary means of ensuring minimum and uniform standards of health and safety in the mining industry. In U.S. coal mines, a new era was ushered-in in 1969 for the control of health hazards from respirable coal mine dust. Since then significant progress has been made on all aspects of the respirable dust problem-its definition, monitoring, reduction, and control. In the intervening years, our ability to monitor, analyze, and characterize the mine atmospheric environment and the health of miners has increased many fold. Also, advancements in production technology have far outdistanced enhancements in environmental control technology. Therefore, questions are continuing to be raised on the adequacy of the characterization and control measures with regard to both the airborne respirable dust in mines and the health effects of respirable dust. Late in 1995, the Secretary of Labor appointed an Advisory Committee on the Elimination of Pneumoconiosis Amona Coal " Mine Workers (Dust Advisory Committee) to review the progress towards elimination of coal worker's pneumoconiosis (CWP) and silicosis, and in the program to control respirable coal mine dust in underground and surface mines, and make recommendations including improved standards and other appropriate measures for the control of exposure to airborne coal mine dust. The Committee's report was released in October, 1996. In this paper, a summary of the recommendations of the Committee is presented. A brief discussion of the background, and the issues and findings leading to the recommendations is also presented. The Committee concluded that the progress made towards making mines safer from the health hazards of respirable coal mine dust is substantial but that it is not sufficient to achieve the intent of the 1969 Coal Act. The Committee's recommendations encompass a wide range of issues addressing permissible exposure limits (PELS), sampling, dust control plans, hazard and medical surveillances, role of miners, training, and research and development.

The Mogalakwena North Concentrator of Anglo Platinum Operations was built with the capability to allow for future projected expansion. Several research groups, currently working with Anglo American on collaborative research over a range of projects, were brought in to review the existing operation, with a view to releasing some specific bottlenecks and providing specialist design input to the potential plant expansion.Based on a thorough review of the high pressure grinding rolls (HPGR) and crushing circuit, a wide range of de-bottlenecking opportunities were identified. This was followed by a series of surveys to provide performance measurements and to model the circuit in detail. From this effort a full simulation of the plant has been developed, including updated models of the HPGR, crusher, bins and screens. These models have been implemented in the researchersÆ Model Development Kit (MDK) version of JKSimMet, and in a prototype dynamic simulator based on the Matlab Simulink software.The observations, measurements and simulations have provided guidance and quantified the benefits of some proposed circuit modifications. Based on these findings a number of upgrades are being planned, with the aim of ensuring full utilisation of the installed equipment. A considerable part of the sulphuric acid requirements of the Aluminum Company of Canada's Arvida smelter comes from the burning of 180-200 tons per day of zinc '"'sulphide concentrates in a 22-ft.-diam-eter fluidized bed roaster. The plant and its operation were fully de-scribed in a publication that appeared about ten year ago (1). A simplified flowsheet of the roasting and gas-cleaning sections of the plant is given in Figure 1. The wet zinc sulphide concentrates are un-loaded into a pug mixer, by means of a Pay loader, and mixed with water in a mixing tank to make a slurry of 80 per cent solids. The slurry is transferred to a .feed tank and then introduced into the roaster at a point four feet above the constriction plate, which is the height of the settled bed of zinc calcines. In this work the formation and environmental stabilization potential of arsenate-containing fluoroapatites is considered. In particular in this research various compounds were precipitated from synthetic solutions containing fluoride, arsenate/phosphate and calcium by varying different parameters-temperature, Ca:As ratio, pH, and P:As ratio, and aging time. The results obtained helped determine the operating window that should be used for the formation of highly crystalline fluoroapatites, in particular svabite (Ca5(AsO4)3F). It was found that svabite has a specific pH range of precipitation, with lower pH yielding CaF2 and calcium arsenate compounds such as CaHAsO4•H2O. At higher pH, a mixture of svabite and johnbaumite (Ca5(AsO4)3OH) precipitated, yielding a less stable material. This range of precipitation varied with the concentration of arsenic in the starting solution, and widened with higher initial concentrations of Ca2+, AsO4 3- and F-. Furthermore, svabite/fluoroapatite solid solutions were formed by using the same procedure as svabite but replacing some of the arsenate in solution by phosphate. The increase in phosphate content yielded a gradual change in morphology and crystallinity. Work on stability evaluation is currently under way.

Modern underground coal mines can be very large, having a total connected load in excess of l5 000 hp. These mines generally have many miles of high-power conveyor belts and 15 or more miles of high-voltage power cables at distribution voltages of 12.47, 13.2, 13.8, or 14.4 kV. The shielded cables used in mine power distribution systems have a significant level of capacitance, on the order of 110 pF/ft. This level of capacitance, in an extensive power distribution system at today's voltage levels, can cause significant charging currents during a ground fault. This paper addresses the potential detrimental effects of capacitance charging currents during line-to-ground faults in mine power distribution systems. A representative mine power system is modeled, and simulations with faults at various locations are conducted to evaluate the effects of this capacitance on the level of fault current and relay selectivity. This paper also includes results of capacitance measurements made on mine power feeder cables used to validate the simulation model.

"Multiple-point simulation (MPS) methods have been developed over the last decade as a means of reproducing complex geological patterns while generating stochastic simulations. Some geological spatial configurations are complex, such as the spatial geometries and patterns of diamondbearing kimberlite pipes and their internal facies controlling diamond quality and distribution.Two MPS methods were tested for modelling the geology of a diamond pipe located at the Ekati mine, NT, Canada. These are the single normal equation simulation algorithm SNESIM, which captures different patterns from a training image (TI), and the filter simulation algorithm FILTERSIM, which classifies the patterns founded on the TI. Both methods were tested in the stochastic simulation of a four-category geology model: crater, diatreme, xenoliths, and host rock. Soft information about the location of host rock was also used. The validation of the simulation results shows a reasonable reproduction of the geometry and data proportions for all geological units considered; the validation of spatial statistics, however, shows that although simulated realizations from both methods reasonably reproduce the fourth-order spatial statistics of the TI, they do not reproduce well the same spatial statistics of the available data (when this differs from the TI). An interesting observation is that SNESIM better imitated the shape of the pipe, while FILTERSIM yielded a better reproduction of the xenolith bodies. IntroductionThe simulation of geological units poses several technical problems that challenge the capabilities of available geostatistical simulation approaches and their ability to deal with spatial complexity of categorical variables. The spatial geometry of diamondbearing pipes and their internal facies controlling diamond distributions are a specific and well-known difficult modelling problem. The geometry of a kimberlite pipe is typically like an inverted cone (Figure 1); the wider part is known as the crater and is located near the surface, and the narrower, deeper part is known as the diatreme. Xenolith bodies can also be found inside the pipe. A very important contribution to the geological modelling of the contours of the pipe is the measure of uncertainty about the location of the contact between the kimberlite pipe and the host rock."
