"Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

Geostatistics was initially developed in the mining sector, but has been extended to other geoscience applications, including forestry, environmental science, soil science, and petroleum science and engineering. This paper presents methods, workflows, and pitfalls in using geostatistics for hydrocarbon resource modelling and evaluation. Examples are presented of indicator variogram analysis of categorical variables, lithofacies modelling by sequential indicator simulation and hierarchical workflow, porosity modelling by kriging and stochastic simulation, collocated cokriging for integrating seismic data, and collocated cosimulation for modelling porosity and permeability relationships. These methods together form a systematic approach that can be effectively used for modelling natural resources.

This report presents the results of a study of the overturning Stability of Front End Loaders and the development of an Obstacle Detector to warn of Obstacles in the blind regions of the vehicle. The contributing factors have been identified, the FEL stability characteristics have been determined and six overturn warning indicators were fabricated and two were field tested in a mine operation over an extended time period. A simplified overturn indicator, based on the principle of sensing wheel load, was designed, constructed and field tested. The principle was demonstrated to be sound, however, the method of sensing wheel load must be revised. An obstacle detector capable of detecting a man sized target at ranges of two feet to one hundred feet was designed. A "Breadboard" device utilizing existing microwave devices was constructed and tested successfully.

The viscosities of CaO-SiO2-Al2O3-MgO-FeO slags were measured under conditions ofCaO/SiO2=1.15–1.6, 10–13 percent Al2O3, 5–10 percent MgO and 5–20 percent FeO. Slag viscosity decreased with increasing FeO content at a fixed basicity (CaO/SiO2) of slag. Slag viscosity at low FeO (<7.5 percent FeO) exhibited a minimum value by increasing MgO content in slag. Viscosity decreased with increasing slag basicity up to 1.3 while it increased as slag basicity increased from 1.3 to 1.5. Thus, it was proposed that the driving force for the decreases of slag viscosity would be an increase in depolymerization of silicate network at C/S=1.3, while the viscous behaviour at C/S>1.3 would be increased with increasing the chemical potential of primary solid phase e.g. dicalcium silicate. A thermodynamic approach for the activity of primary solid phase in molten slags resulted in a reasonable relationship between viscosity and slag components. Therefore, it was confirmed that slag viscosity in basic slags system (C/S>1.3) could be determined by the chemical potential of dicalcium silicate. Selection of tunneling method, specially the use of shielded machines for application in deep rock tunnels can be heavily impacted by presence of ground convergence and squeezing conditions. Tunnel convergence is a function of ground conditions (i.e., rock mass behavior, groundwater, in-situ stresses), size of the opening, and TBM operational parameters. The case study of Ghomroud water conveyance tunnel project in Central Iran, which is under construction using a double shield TBM, is used to examine the effect of rock parameters on tunnel convergence and hence on the need for over excavation and shield lubrication to avoid problems with the shield getting stuck. Although it is impractical to perform a continuous assessment of rock mass conditions within the shield or in the lined tunnel, results of field observations and subsequent studies show that the amount of the tunnel convergence has direct correlation with the percentage of fines and large rock fragments in the muck and TBM thrust/torque/ROP. These relationships have been examined and will be presented in this paper.

Why does a continuous casting mould powder, which works successfully on a particular caster with respect to semi quality, fail to perform on a caster of a similar design utilizing liquid steel from a similar route for an identical product without some tuning of its make-up and composition? In casting sequences of the same steel grade, why do some apparent random heats produce poor surface quality with respect to cracking when otherwise good surface quality is realized? The answer to these questions lies in the interaction of the mould powder, the casting hardware, and the casting process variables. Using, as examples, data collected from within Corus UK and during collaborative work on the assessment of mould powder performance and its effects on surface quality, the interaction of the mould powder, the casting hardware and the casting process variables is explored. The steelmaking route and its associated alumina load, unexpected changes to slag chemistry, infiltration of the slag into the mould strand gap, and the nature of the slag film are all shown to affect powder performance. Copper refineries are constantly looking for methods to upgrade their plants to reduce refining costs and to improve the purity of their electrolytic cathodes at the same time that the purity of the anodes they refine is being lowered by the rehabilitation of reverberatory furnace smelters to one of the direct smelting processes. The comments given below are based on the answers to the questionnaire returned bf the 36 copper refineries that represent 66% of the western world s refining capacity. Eleven refineries employ some form of uniform weight anade casting, e.g., Outokumpu, Contilanode, Mitsubishi, Lurgi, or Hazelett. This type of anode casting presents the best possibility for improving cathode purity. The cathode press of Phelps Dadge can improve current efficiencies and cathode quality. The press is used on first-run cathodes, after 48-60 hours, on half-cells at a time without interruption of the cell current. One refinery interrupts current for sever.al hours to allow the anode slime to settle before the, cathode pressing. The cathode press is now used at eight refineries. Apart from periodic reverse current (PRC), it is not expected that current densities will be increased. Thirty refineries, using direct current, average 231 amps/M, with a low of 178 and a high of 350. Five refineries use PRC at an average of 338 amps/M, with a low of 290 and a high of 380. Current efficiencies are good at an average of 94%, with a low of 88% and a high of 99% for the 34 refineries that reported. It must be remembered that the cycle efficiency with PRC is about 93%., so that the effective current efficiency with PRC would be only .94 X .93 or 87% However, PRC should be considered seriously for refinery expansions because it can save about 25% on capital costs.

The present study was carried out to investigate the impact of slag refractory lining reactions on the formation of inclusions during ladle treatment of steel. The experiments were conducted on an industrial scale in the ladle at Uddeholm Tooling AB in Hagfors, Sweden. The inclusion chemistry and population during ladle treatment were studied along with the composition of the ladle glaze, taken from the ladle lining. The inclusions in the steel were classified into four groups according to the Swedish standard SS 111116. SEM/EDS analyses were carried out to identify the phases present in both the inclusions and the ladle glaze. The number of inclusions in the steel before deoxidation was found to increase with the ladle age, i.e. the number of times the ladle had been in use. A similar increase was also found after vacuum degassing and before casting. A great portion of inclusions before casting was found to be supplied by ladle glaze. This observation was further confirmed and explained by thermodynamic analysis. The present results show that ladle glaze is a major source of inclusions in the ladle at Uddeholm Tooling. In studies conducted by the Bureau of Mines, two test areas were hydraulically stimulated and exposed by mining: One in the Pittsburgh coalbed at the Vesta No. 5 mine, Washington County, Pa.; and one in the Illinois No. 6 coalbed at the Inland mine, Jefferson County, Ill. Induced fractures were contained totally within the coalbed with no adverse effect on the stability of associated rock strata. Induced fractures were vertical and were propagated normal to the direction of least residual tectonic stress. Gas production was increased fivefold to twentyfold by hydraulic stimulation.

Gilsonite is a lustrous black material that can be easily crushed into a chocolate-colored powder. It is a solid but relatively soft, lightweight hydrocarbon that visually resembles the volcanic glass obsidian. It was once thought to be a type of coal, but because it is thermoplastic it melts rather than bums. Gilsonite is found in commercial quantities only in the Uinta Basin of eastern Utah and western Colorado, where it forms vertical, nearly parallel veins of varying depth, width, and length. Gilsonite from different veins has different properties, most notably variations in softening point and solubility. These differences result in its use in a variety of industrial applications. Gilsonite&apos;s earliest commercial uses were in varnishes, buggy paints, and beer vat linings. Eventually it came to be used in more than 160 products ranging from inks to explosives. It is exported from the United States to most industrialized countries throughout the world. Today, due to competition from synthetics and other hydrocarbon products, its use has narrowed to several specific industries. Although sold mostly as a commodity, current research is focused on applications for gilsonite as a value added product. Historical Overview The discovery of gold in California and the dawn of the mining era in Nevada, Colorado, and Utah around 1850 brought a wave of prospectors to the western United States. One of those pioneers, Samuel H. Gilson, is generally credited with a major role in the discovery of gilsonite. The material now known as gilsonite was discovered several times from the late 1860s through the mid-1880s. It was William P. Blake who, after receiving samples of the material, wrote a paper in 1885 describing "a new variety of asphaltum from the Uinta Mountains." He termed the material uintaite. Although Sam Gilson was not the first to discover the mineral, he was an-inventor who used gilsonite in products that he patented. It was his conviction that the substance was of great value, which caused some to refer to it as gilsonite. The term Gilsonite became the registered trademark of the American Gilsonite Co., and it continues to be in use today. When the first company was formed to mine this substance in 1886 it was named the Gilsonite Manufacturing Co. This firm acquired many gilsonite mining claims and marketed a varnish made from the material. It was the unusual properties of gilsonite varnish that attracted the attention of Adolphus Busch of the Anheuser-Busch Brewing Co. In 1889 Anheuser-Busch bought out Gilson&apos;s interests and formed the Gilson Asphaltum Co. Busch had been importing asphalt from Sicily for use in lining beer barrels and saw gilsonite as being more readily available and cheaper. Although gilsonite did not work well in this application, Busch did acquire a large number of mining claims in the Uinta Basin. In 1903 the parent firm of Gilson Asphaltum financed the building of an 85-km railroad from near Grand Junction, CO, to Dragon, UT, to haul gilsonite ore to market. Underground mining was initially centered near Dragon and then later at Bonanza, UT, as different veins were mined. The railway was abandoned in 1937 in favor of truck transportation. In 1938 Gilson Asphaltum changed its name to Barber Asphalt Co., the predecessor of Barber Oil Corp. In 1946 Barber Oil teamed with Standard Oil of California to form a partnership, American Gilsonite Co., which today has its operational headquarters at Bonanza, UT. Barber Oil investigated many new uses for gilsonite, including the making of petroleum derivatives such as gasoline. This work led to the construction of a 116-km pipeline to carry slurried gilsonite to a refinery in Colorado for the production of gasoline, fuel oil, and coke. Pipeline operations began in 1957 but stopped when the economics of competing with petroleum became unfavorable, and the refinery was sold in 1973. In 1981 Standard Oil (later Chevron) became the sole owner of American Gilsonite and remained so until 1991, when it sold its interest to Stratford Enterprises Co. of Tulsa, OK. The companies have since been merged and now operate under the name American Gilsonite Co. with headquarters in Salt Lake City, UT. American Gilsonite continues to be the world&apos;s largest miner and supplier of the mineral. The American Asphalt Association was formed in 1902 by former stockholders in Gilson Asphaltum. This company mined various gilsonite claims until it was dissolved in 1953 and its properties were sold to American Gilsonite and Gordon S. Ziegler and Associates. The Utah Gilsonite Co. was established in 1920 and mined several claims until 1952, when its holdings were sold to Gordon Ziegler. Several other gilsonite mining ventures have been pursued in Utah, although they were significantly smaller in scale than those described above. There are currently three companies which mine gilsonite resin in Utah. They are American Gilsonite Co., Ziegler Chemical & Mineral Corp., and Lexco.

Extensive site investigations revealed that abandoned shallow mine workings exist under a site planned for construction of a major new facility. Since the mine voids were susceptible to collapse, they were stabilized by grouting. After completion of the mine remediation, three types of deep foundations (micro-piles, auger-cast piles, and drilled piers) were considered for this site. Micro-piles spread the load via the bond between the pile and the bedrock. Without significant length socketed in rock, the axial load of auger-cast piles and drilled piers has to be concentrated on end bearing. Although spreading the load is preferable in this environment, economic consideration dictated the choice of drilled piers for construction.

The Mining Foundation of the Southwest is a non-profit organization. It educates the public about the mineral industries and reports outstanding accomplishments in the fields of mining and the earth sciences. This year’s inductee, guest of honor and featured speaker at the banquet will be Pierre Lassonde, president of Newmont Mining and resident of the World Gold Council. He is one of Canada’s leading gold analysts.

"Microcel™ sparging systems were installed on one column of the molybdenum cleaner circuit and one column of the copper first cleaner circuit at Antamina. The Microcel™ system is known to generate finer bubbles than jetting-type spargers, and smaller bubbles have been associated with higher recoveries in flotation. Alternating tests were carried out on the molybdenum column, using either jetting-type spargers or the Microcel™ system. Experimental conditions of gas flow rate, wash water rate, and froth depth were tested. Use of Microcel™ decreased the bubble sizes from 2.6-2.9 mm to 1.5-2.0 mm. In addition, Microcel™ improved both unit molybdenum recovery and unit molybdenum concentrate grade. In the copper first cleaner circuit, side by side tests were carried out that compared the Microcel™ column with a parallel columns fitted with jetting-type spargers. Best results were obtained at manifold pressures of 260-290 kPa, where bubble sizes of 1.6-2.6 mm were observed. In either copper or molybdenum flotation, the Microcel™ system required only 50-60% of the gas flow used by jetting-type spargers.INTRODUCTIONAntamina is a copper-zinc mine located in the Peruvian Andes. Compañía Minera Antamina is owned by BHP Billiton (33.75%), Xstrata (33.75%), Teck Cominco (22.5%), and Mitsubishi (10%). The mine was commissioned in 2001 with a capital investment of US$2.3 billion, and has a nominal 100,000 t/d mill capacity. Mining, milling, flotation, and tailings impoundment take place at the mine site, which sits at an altitude of 4,300 m above sea level. The deposit is a polymetallic skarn ore body with intrusive phases of quartz-monzonite porphyry into Cretaceous aged limestone [Evans et al, 2005]. Ore body thickness is 100 to 150 m and has the shape of a flat-lying crescent. Copper is present mainly as chalcopyrite with a minor bornite component. Zinc is present as sphalerite. Molybdenum is present as molybdenite. Some lead, bismuth, and silver are associated with the sulphides. The deposit is currently classified into six main ore types [Dechert, 2006], grouped into two categories: copper and copper-zinc. The mill produces four concentrate types: copper, molybdenum, zinc, and lead-bismuth. Copper, zinc, and lead-bismuth concentrates are transported by pipeline down to the port of Huarmey. Molybdenum concentrates are bagged and trucked to port."

Henry Walker Eltin (HWE) are the mining contractors for Gloucester Coal at Stratford mine, which is in an environmentally sensitive area in the Avon Valley, NSW, Australia. Normal blasting at Stratford is done in a “stand up” or “box cut” mode to suit the truck and shovel operations. Recently an opportunity of doing a throw blast became available when part of a ramp had to be blasted to accommodate changes to the pit design while not damaging the remainder of the ramp. The objective was to maximise throw into the void in order to minimise excavation costs by reducing the amount of overburden to be moved by the excavation fleet. It was necessary that the best result be achieved while staying within the vibration and airblast limits at a neighbouring property and nearby township. Both objectives were achieved through the application of Orica’s knowledge of blast design, modelling and i-konTM electronic detonators.

In the course of, investigation of domestic manganese-mineral occurrences, examining engineers of the Bureau of Mines collected 2-ton lots of ore from properties in the Cleveland-district, Idaho. As only one of the ores was high enough in grade for direct marketing to-producers of ferromanganese, ore-dressing concentration methods were investigated to determine their amenability to treatment. High-grade ore. (38.7 percent Mn) from the Idaho Manganese Mining Co. was taken from a replacement deposit in limestone consisting of several horizontal beds 1 to 5 feet thick, about 1 mile northeast of Cleveland. The manganese minerals occur as an intimate intergrowth of manganese oxides, containing minor amounts of limonite, ?quartz, and calcite as rather fine inclusions. However, the major contaminants are claylike products of rock decomposition, which can be eliminated by thorough washing of the ore at relatively coarse sizes. The Hot Spot property is 1 mile south of Cleveland. The ore occurs as a replacement deposit of medium-grade- ore in limestone, .The sample taken by the Bureau of Mines assayed 26.0 percent Mn. The manganese minerals consist of nodules, principally pyrolusite with some psilomelane and manganite embedded in wad.&apos; The low-grade wad, containing manganese and siliceous minerals in intimate association, was easily rejected by washing.

The Fort Knox Mine is located in Alaska’s interior, where the average ambient air temperatures range from -24oC in January to 16oC in July. The mill processes a free milling gold ore utilizing both a gravity recovery circuit and a conventional cyanide leach/carbon-in-pulp circuits. Mathematical models have been developed to accurately predict the impact of leach circuit slurry temperature on gold leach, carbon adsorption and cyanide destruction kinetics. Additionally, an energy balance approach has been used to model the seasonal variations in slurry temperatures throughout the Fort Knox mill. The energy balance model, combined with the kinetics models previously developed to predict plant performance, was used to justify a tailing thickener installation at the Fort Knox Mine. This paper describes the development of the energy balance model and its prediction of plant performance assuming a tailings wash thickener to reduce heat loss from the mill. Also presented is an analysis of the post project plant performance that validates the model and shows the mine produced an additional 596 kg (19,155 ounces) of gold, while reducing mill reagent costs by $6,302,000 during the first 38 months of operation.

Laboratory leaching experiments were conducted to ascertain the effectiveness of acidic solutions of thiourea, CS(NH2)2, for dissolving gold from finely disseminated gold ores. The ore tested was from the Chipmunk ore body of the Northumberland mining district in Nye County, Nevada, USA. Extractions of up to 90% were attained from the ore. Extraction was shown to be dependent on thiourea concentration, ferric sulfate oxidant concentration, acid concentration, and leaching time. Optimum conditions were an ambient temperature leach near pH 1.0 utilizing 100 to 200 kg/ton thiourea and 50 kg/ton ferric sulfate for 8 to 24 hours.

In 1994, Gold Mines of Australia acquired the rights to the historic Mt. Lyell copper mine, and commenced a feasibility study into expanding production. The ore was shown in laboratory tests to be amenable to single-stage, fully autogenous grinding (FAG) of underground primary crushed ore, and this route offered the maximum cost savings. Flotation performance was expected to be enhanced. However, batch flotation tests done on pilot-scale autogenously ground ore showed an unexplainable recovery loss of between 1% and 3% Cu. A second pilot plant trial was set up, with the ground product feeding banks of continuous flotation cells. Single-stage FAG, FAG/pebble and FAG/ball circuits were tested. Results confirmed a loss of about 3% recovery with single-stage FAG milling. The FAG/ball circuit eliminated the loss. Analyses showed that autogenous milling probably produced more chalcopyrite/pyrite composites and that recovery of these composites was lower than with ball milling. In size fractions below about 53 microns, where chalcopyrite is well liberated, copper recovery was less affected by milling method and flotation selectivity. It was concluded that for the Mt. Lyell situation, a FAG/ball circuit was preferable to single-stage FAG.

This paper describes the methods of mining and washing brown iron are in 1957 at the Sawyer mine, leased by Wayne County Mining Co., and the Fore mine, leased by Four Mining Co., both in southern Missouri; it is one of a series prepared by the Federal Bureau of Mines on mining and milling methods and costs in various districts of the United States. The Sawyer mine is in Wayne County, about 10 miles north-northeast of Williamsville (fig. 1); the Fore mine is in Howell County, about 20 miles southwest of West Plains (fig. 2). Both are open-pit mines from which the crude are was upgraded by washing and hand-sorting to produce are of grade and quality suitable for sale to operators of iron and steel furnaces. The operations described are typical of many others that produce or have produced marketable brown iron are from small sedimentary deposits, several hundred of which outcrop on the crests or hill sides of southern Missouri (fig. 3). The 1957 production of marketable brown iron are by 40 separate operations in the southern part of the State was 281,427 wet long tons, valued at $1,637,243. Production from the Sawyer mine that year was 17,000 dry long tons of washed are, and from the Fore mine, 20,816 dry long tons.

"0 ne of the incentives that led to the development of non-electric shock tube initiation systems was the desire within the industry to improve detonator safety. And to the extent that nonelectric initiators are immune to the hazards of stray current and radio frequency energy found on normal blast sites, an advancement in safety has been accomplished. However, as a cautionary note, it is important to realize that there are still many hazards associated with any initiation system. While some of these hazards may differ from those associated with electric detonators, others are identical to those associated with any type of initiation system. For instance, contrary to what some uninformedblasters may believe, lightning remains a hazard when using nonelectric initiation as well as electric initiation."
