The main objective of occupational health and safety research is to minimize or eliminate the events that may cause fatal or non-fatal injuries to human workers. A commonly used technique is to devise an “incident prevention plan”, which is more often the product of thorough investigations of past reported incident events. Incident documentation and methodical reporting and systematic and thorough data collection, often help identify the root cause of the incident – in other words, the etiology of the incident. These retrospective analyses of the incidents can efficiently identify the future steps to take to achieve the objective of minimizing occurrence of “events of interest”. This paper introduces a novel technique – “Taxonomic Analysis”, which identifies the root causes of an event (an incident in this case) and can provide future direction for corrective measures to reduce the probability of occurrence of the event. A taxonomic analysis involves systematic and organization of data based on observation, description, and classification. This paper utilizes the rock fall related incident narratives, available with the Mine Safety and Health Administration (MSHA) database (1984-1999), for a taxonomic analysis. The study found that 67.6% of groundfall incidents occurred in supported areas with the remainder (32.4%) in unsupported areas. Some form of rock mass failure appears to cause about 50% of groundfall incidents, although in 39.7% of the overall groundfall incidents or two-thirds of those involving rock mass failure, the exact nature of the rock mass failure could not be determined from the narratives. Human activities factors such as scaling or barring down, drilling or bolting, and setting timbers or cribbing accounted for another 34.5% of the groundfall incidents. Finally, support system failures made up the remaining 15.5%. Results from this taxonomic analysis may suggest focus areas toward which preventive measures and future research activities could concentrate.

Introduction This paper is a descriptive analysis of injuries and fatalities at small surface bituminous coalmining companies in Pennsylvania. The purpose of analyzing a specific industry sector and location is to identify issues that might be missed in a wider analysis. Other studies of small mine safety (e.g., Peters and Fotta, 1994, Randolph and Boldt, 1997) have shown that the relationship between safety and mine size is not a simple one. Some injury types occur at higher rates at small mines, while others occur at lower rates. Consequently, an analysis limited to a specific type of mining can help identify hazards and issues specific to that sector. The results of this analysis will be useful for directing training, intervention, and research efforts towards the most prevalent causal factors and incident types. Procedures and Definitions Data sources The primary sources of data for this analysis were files provided by MSHA containing information collected under Title 30 Code of Federal Regulations, Part 50. These files, commonly referred to as “Part 50 data”, are provided though MSHA’s Program Evaluation and Information Resources directorate. Two primary types of injuries were selected from the Part 50data: Fatalities (MSHA “degree of injury” code 1) and lost-time injuries, otherwise known as nonfatal with days lost (MSHA “degree of injury” codes 2 through 5). Lost-time injuries involve either actual days away from work or days of restricted activity or both. Fatality and lost-time injury incidence rates were calculated using the standard MSHA formula: Number of injuries per 200,000 employee-hours. Additional information on company sizes and mine ownership was provided by the Pennsylvania State University College of Earth and Mineral Sciences. Time span A five-year time span was selected for this study. More than a single year of safety experience is usually needed to capture a broad range of incident types. Many types of injuries, especially fatal injuries, can be extremely rare, and may be missed if too short a time span is selected. However, a time span that extends too far back may begin to suffer historical threats to validity. That is, incidents that occurred several years ago may be caused by hazards that have since been remedied or may be related to mining practices and conditions that no longer exist. A five-year span offers a practical compromise between these two concerns. Consequently, the MSHA Part 50 data files from 1993 through 1997 were used. The 1997 file was in “preliminary” form at the time of the study, so small variations from the final 1997 “close out” file can be expected. Based on past experience, we do not expect the close-out variations to have a significant impact on the findings.

This study forms a part of Bureau of Mines continuing research on thermal fragmentation of rocks. The potential of internal heating techniques for rock fragmentation is discussed. A concept of rock fragmentation with rapid inter¬nal heating of a localized inner volume of rock is advanced The energy required for rock fragmentation is made available by creating a required vol¬ume of localized heating. The fragmentation of rock using the proposed method is analyzed and qualitatively compared with the results of experiments with Charcoal granite, Dresser basalt, and Sioux Quartzite. High frequency dielec¬tric heating below 100 MHz is used in the study as an example to examine the working of the concept. The selection of operating conditions for the experi¬mental heating is explained. Fragmentation of Charcoal granite and Dresser basalt test rock blocks is achieved for localized heating below 550° C. The heated volume required for fragmentation is observed to be less than 2 percent of the test block volume and the electrical energy consumption is in the order of 3.5 to 6 5 kwhr/m3 The method offers a possibility for in situ fragmenta¬tion of rocks, favorable for internal heating. The method is highly dependent on rock properties governing internal heat generation The efficiency of dielectric heating in the experimental study is observed to increase with increasing dielectric loss of the rock material, ranging from about 10 percent in Sioux. This report describes a new application of electroamalgam metallurgy in recovering all the zinc as refined metal from galvanizers' sal skimmings. Pure zinc was recovered by electrolysis of skimmings leach liquors in amalgam cells. The following information was gained as a result of literature studies and subsequent laboratory investigations. 1. Reports in the literature indicate that pure zinc can be recovered from chloride solutions with the aid of amalgam cells. From Europe it was reported that amalgam cells could be used to recover pure zinc and chlorine from leach liquor obtained from pyrite cinders following a chloridizing roast.

This document is the final report of a study aimed at defining the relative contributions of various sources of respirable airborne dust on longwall faces. In order to define the contribution of various primary sources, continuous dust level data were recorded on five longwalls for an average of five shifts at each. By analysis of these data with records of face activity, it was possible to determine the overall contribution of each of the major elements of the longwall system to overall respirable dust exposure at various face locations. Average levels were determined for roof support generated dust, conveyor and intake dust and for dust levels during various shearer activities. When average levels are compared between mines, a significant degree of consistency is noted. The next phase of the study was aimed at differentiating the various dust sources associated with the shearer. To this end 'fly cuts' varying from 6 in. to 30 in. were closely monitored as were dust levels at operators' positions over the various segments of longwall cycle. These data indicated that the clearance ring of the shearer drum as a disproportionately heavy contributor to shearer dust make. A statistical analysis of the effect of shearer haulage speed on dust make revealed that high haulage speeds are associated with low specific dust make.

The performance of ten anemometers of several types has been measured at low air speeds. Multiple calibration runs provided sufficient data for measuring random error about the mean calibration curve in addition to the difference between indicated and true speeds. Measurements were also performed of the minimum operating speeds of the anemometers.

Virtually from the date of its discovery in December 1928 the Oklahoma City oil and gas field, Oklahoma County, Okla., has attracted national and international attention. The history of the field has been associated closely with rules and regulations pertaining to production control and proration practices. Early development was rapid, and Oklahoma City soon attained the distinction of being the first major field in which the producing wells were over a mile in depth. Before the structural features of the reservoir had been disclosed every outpost well was considered a "wildcat", so complex appeared the underground geology. The wells not only were deep and the reservoir pressures high, but the production from the early wells was a high A.P.I. gravity oil, accompanied by large volumes of gas. Several "blow-outs" and fired occurred during the early development, and the close proximity of these now unusual events to a large city gave the field much publicity, both local and national. Later, when its production threatened to demoralize the market for oil and its products, the Oklahoma. City field again attracted wide publicity. The then little understood geology of the underground reservoir and the diversity of ownership of the land overlying the producing structure made orderly development and production difficult to enforce. Proration was enforced with difficulty, and the rate of production from the field continued to increase until, on August 4, 1931, martial law was declared in all prorated fields of the State, and most of the wells in the Oklahoma City field wore shut in. On October 9, 1931, permission was granted to reopen and produce the wells in the Oklahoma City field. The order permitting production of the wells provided that during the period beginning October 10 and extending to October 20, 1931, all wells in the field that had not produced more oil than specified under the earlier proration orders and that had been shut in for the required 65-day period were permitted to produce 5 percent of their last potential during that 10-day period.

Wells producing from the Wilcox zone were allowed to produce 4 1/2 percent of their potential5/ from November 1, 1931, to January 1, 1932; Arbuckle limestone wells were allowed to produce 250 barrels of oil daily; and Simpson sandstone wells which had a potential of over 2,000 barrels per day or which made 3 percent or more water were allowed to produce 200 barrels of oil per day.

This summary of State laws on explosives was compiled primarily to ascertain what subjects relating to their control have been acted upon by each State legislature and, in general, how they have been treated. It does not include any legislation passed after 1942, or any proclamations, regulations, or administrative orders. It does not purport or attempt to be authoritative. If any problem arises involving interpretation of the statutes of any particular State, the original source must be consulted for the exact words used by the legislature. This summary, however, will be useful in determining whether relevant statutes exist, and in locating them quickly with a minimum of effort. It will be of value also in comparing statutes in one State with similar statutes in other States to determine what construction has been placed upon them by the courts of these jurisdictions or to contrast the varying provisions of many States upon a similar subject. To make these comparisons easier, the statutory provisions for each State have been grouped under several major topics in outline form. Although this sometimes has resulted in disruption of the continuity of individual acts, the increased general clarity and availability of comparable sections have more than justified the procedure followed.

The vapor pressure-temperature relationships for saturated pure liquid nitrogen are calculated by the Strobridge equation using a digital computer. This information is presented in two tables: first) in pressure increments of 0.1 psi from 1.8 to 492.9 psia) and second) in temperature increments of 0.1° from 63.15° to 126.26° K. These tables should facilitate the calibration of temperature-measuring devices over this temperature range.

The literature dealing with the electrical conductivity of solid dielectrics, including all its aspects and its relationships to other physical properties of these materials, is extensive and scattered in many books, phamphlets, and periodicals. It is believed that most of the important contributions to this field up to October 1943 have been listed and abstracted in this circular. Also included are some general references on static electricity. Methods of removing static charges are described; methods of and conditions for measurements of resistivity are discussed; equations for the calculation of resistivity are given; and methods of resistance measurements in Situ are reviewed. Data on the variations of resistivity with changing conditions as well as explanations for them have been collected, and three tables showing the resistivity of various dielectrics at different humidities are included. The appendix contains a short development of the mathematical formulation of the charging and discharging currents due to interfacial polarisation. INTRODUCTION Methods for Removing Static Electricity Of the various means employed for preventing or removing accumulations of static electricity in industrial processes, grounding is the most common and most generally effective. It is a precaution readily applied to stationary machines anti may often be utilized to good advantage to rid persons and portable equipment of an acquired charge. There are many situations, however, in which direct low-resistance grounding cannot be used, either because the charged bodies are insulators or are inaccessible, or because contact to a ground lead might give rise to a hazardous spark. If the charged body is a conductor, contact made to a neighboring conductor, even though the latter be insulated from ground and of relatively small capacitance, may also result in the occurrence of an incendive spark. In such instances, static electricity can sometimes be removed most satisfactorily by means of strong atmospheric ionization produced by radioactive material, X-rays, ultraviolet light, or A. A series of explosive detonation experiments was conducted in NIOSH’s Bruceton and Lake Lynn Experimental Mines to evaluate low level (<6 psig) detonation wave propagation behavior in single and multiple entry configurations. Entry cross-sectional area varied from 5 to140 m2 and explosive weights of Pentolite ranged from 0.24 to 2.4 kg. Behavior of the detonation wave was evaluated through recording of the pressure as a function of time and distance from the explosive detonation in single and multiple entry interconnecting tunnels. Peak pressure from detonation of an explosive in an underground mine maintains magnitude for a greater distance than is the case on the surface and is shown to follow the simple scaling rela-tionship P = 702.8(W/Vt)0.514(0.65)n, where P is the peak detonation pressure in kPa, W is the weight of the explosive charge in kg, Vt is the total volume in m3, and n is the number of splits, n = 0, 1, 2, …n. The total energy, or impulse, I, generated by these detonations can be expressed as I = 1.34(W/A)0.64n, where W is the weight of the explosive charge in kg, A is the area of the main tunnel in m2, and n is the number of side en-tries, n = 0, 1, 2, …n.

As part of the Bureau of Mines health and safety research program, a short-range locator system, capable of locating miners who have become trapped following a mine accident, has been devised, The system consists of a lightweight, low-power radio transmitter incorporated into a miner&apos;s cap lamp battery and a directional receiver carried by rescuers. Prototype transmitters have been constructed, and the propagation of their signals has been investigated, The detectability of the signal in underground coal mines has also been investigated, This research indicates that a short-range location system would serve as an economical and practical means of locating miners from within the mine following an accident,

The objective of this contract is to design, build, surface test, and evaluate a breadboard prototype of the Variable Wall Mining System so that data are available for the design of a production machine. The Variable Wall Mining System (VWMS) consists of a series of side cutting augers, connected end to end, and distributed across a panel. Supplying rotary power at the end of the string and anchorage from conventional longwall roof supports, the augers are sequentially thrust into the face and then lifted to the roof to extract the coal in a continuous manner. The augers not only cut but also transport the coal. The effort described in this report summarizes the operation characteristics of the VWMS, the design methodology used to develop various working models, the surface testing of a ½ -scale version of the VWMS, prediction of power requirements and performance potential of a full-scale system, and a critique of the VWMS concept, conclusions, and recommendations.

That the maintenance of proper air conditions is a most important factor in the control of occupational hazards is shown by the large amount of material presented each year through the technical journal's on the effects on workers of injurious dusts, gases, and variations in temperature and humidity.. SUMMARY OF RECENT LITERATURE ON EFFECTS ON WORKERS OF EXPOSURE TO DUSTS The subject of the control of dust and dust diseases continues to be a matter of serious concern in most of the industrial countries of the world, as indicated by the following summary of reports published in a number of foreign countries as well as in the United States. Australia. The Workmen's Compensation Act of 1920 for New South Wales3 provided for the promulgation of schemes for the compensation of workers dis- abled by silica or other dust. In 1924 the prevalence of silicosis and tuberculosis among Sydney stonemasons, quarrymen, sewer miners and rockchoppers was investigated by a Technical Committee of Inquiry, appointed upon the recommendation of the New South Wales Board of Trade. The findings of this committee were published by the Government in 1925. No scheme was promulgated by the Government and in 1926 the Act was amended limiting its scope to workmen disabled by disease caused by silica-dust. This was promulgated and gazetted on September 16, 1927, and applies to men working in sandstone as stonemasons, quarrymen, rockchoppers, and sewer miners in the County of Cumberland.

"The purpose of this paper are (1) to present a broad picture of the recognized methods of measuring the various forms of atmospheric pollution; (2) to evaluate the significance of such measurements; and (3) to suggest suitable methods for obtaining comparative information on the concentration of contaminants in city air.The atmosphere (the gaseous envelope surrounding the earth) consists of a mixture of gases. These gases and their usual proportions are:4/The volume of water vapor in the atmosphere ranges from almost zero (in very cold weather) to about 5 percent arid averages about 1.2 percent.Strictly speaking, any other substance in the atmosphere or an excess of any of the substances mentioned (with the possible exception of water) is a contaminant. Needless to say, there are many contaminants in the atmosphere.In accordance with their physical state, contaminants may be classified as gaseous or particulate, and the latter may be classified as solid or liquid matter. Particulate matter, especially when dispersed in some medium such as air or some other gas, sometimes is referred to as dispersoid, and the complete system of gas and dispersoid is referred to as aerosol.All substances on the earth are potential sources of atmospheric contaminants. Combustible materials (particularly fuel for heating and power), sea water, earth or soil, metals, and animals (including man) are some of the important sources of contaminants.Ordinarily gases are produced by chemical processes, whereas dispersoids are produced by both chemical and physical processes. There are two general dispersoid-producing processes: (1) Condensation, in which molecules join to form larger units or particles, and (2) disintegration, in which large units are reduced to the particulate range (which for the purpose of this article may be considered to be from 100 microns, or 1/254 inch, down to molecular size) by such processes as crushing, grinding, abrading, and atomizing.“Condensoid"" is a convenient term for dispersoids formed by condensation, and ""disintegrasoid"" for those formed by disintegration. Solid condensoids are sometimes called ""fume,"" solid disintegrasoids ""dust,"" liquid condensoids ""fog,"" and liquid disintegrasoids ""mist.""5/"

This paper is one of a series being prepared by the United States Bureau of Mines describing quarrying methods and costs at stone quarries throughout the United States. These papers are designed to disseminate technical information regarding the methods used. The cost tabulations represent operating expenditures only and not total production costs. It is recognized that publication of total production costs might in many instances cause embarrassment to individual producers as well as to the industry as a whole. On the other hand, operating costs are essential to the technical discussion and study of the methods employed. The attention of the reader is specifically called to this differentiation in order that no misunderstanding of the scrope of the cost tabulations shall ensue. This circular describes four modern change houses of the Lake Superior iron - ore district , all conceived and constructed since 1937. The oldest change house described herein was constructed and occupied in 1938 , yet it embodies the modern improvement of air - conditioning . The purpose of this report is to describe and illustrate improvements with regard to health and safety features combined in the construction , arrangement , ventilation , heating ,lighting , and other features , with the aid of photographs , sketches , and diagrams . The modern change house is a fire - resistant structure in which intermingling of men in mine clothes and men in street clothes , hazardous dust , heat and humidity , crowding , poor lighting , slippery and muddy floors , and inadequate facilities for clothes and for bathing have been largely eliminated . The miner coming off shift at a modern mine no longer faces the elements when he gets off the cage . He travels through a tunnel to the change house , where he is cooled gradually by room temperatures as he passes from the mineclothes section and shower room to the street - clothes room . He is assured of dry clothes when he is ready for them . No longer is he sent out of the change house into a muddy yard to drive away as best he can . The modern change house has an improved parking lot and graveled or concreted walkways from it to the change house entrances .

Researchers at the U.S. Bureau of Mines (USBM) have developed adaptive process control systems in which genetic algorithms (GA&apos;s) are used to augment fuzzy logic controllers (FLC&apos;s). GA&apos;s are search algorithms that rapidly locate near-optimum solutions to a wide spectrum of problems by loosely modeling the search procedures of natural genetics. FLC&apos;s are rule-based systems that efficiently manipulate a problem environment by modeling the "rule-of-thumb" strategy used in human decision-making. Together, GA&apos;s and FLC&apos;s include all of the capabilities necessary to produce powerful, efficient, and robust adaptive control systems. To perform efficiently, such control systems require a control element to manipulate the problem environment, an analysis element to recognize changes in the problem environment, and an adaptive element to adjust to the changes in the problem environment. The control system also employs a computer simulation of the problem environment. Details of an overall adaptive control system are discussed. A specific laboratory acid-base pH system is used to demonstrate the ideas presented; all results are from the physical laboratory system and not from a computer simulation.

This report describes the feasibility of mining the oil shale deposits of the Piceance Creek basin in Colorado by an integrated open pit system. The study is based upon a comprehensive analysis of pertinent environmental, economic, and engineering factors. Ten mining cases are investigated, and the mine design and cost estimates for each case are presented in this report. Nine of the cases represent three cutoff grades, defining oil shale ore as equal to and above 15 gallons per ton (gpt), 20 gpt, and 25 gpt at three ore production levels of 500,000 tons per day (TPD), 1,250,000 TPD, and 2,500,000 TPD. The tenth case is an alternate design of the 500,000 TPD production rate at the above 15 gpt cutoff grade. The report consists of Volume I, a technical and cost comparison of the ten cases; Volume II-Part I, a detailed explanation of the estimating methods used; and Volume II-Part 2, a more detailed discussion of the technical and cost features of the ten cases than is presented in Volume I.
