While a thorough effort was made to cover all available references, a critical review was not attempted nor does this survey pretend to be exhaustive in nature.

The 18th National First-Aid and Mine Rescue Contest was held in the Memorial Auditorium, Buffalo, New Vork, October 5, 6, and 7, 1959. The meet was sponsored by the Federal Bureau of Mines and the Joseph A. Holmes Safety Association, in cooperation with State mine inspection agencies, management and labor of the mineral extractive industries, and other safety minded groups. The Arrangements Committee, as authorized by the General Committee, made a11 the necessary arrangements for conducting the 1959 contest. Members of the Arrangements Committee were: Harry Gandy, Jr. (chairman), National Coal Association; James B. Benson, Southern Coal Producers' Association; Charles Ferguson, United Mine Workers of America; George C. "INTRODUCTION There is urgent need for a satisfactory permissible multiple-shot blasting unit capable of firing 10 detonators in series and safe for use in underground mining where a. gas-ignition hazard may exist.. As there is at present no entirely satisfactory permissible unit of either the battery or generator type, tile described capacitor-battery type has been developed. Consideration of the Bureau of Nines approval schedule pertaining to the generator and magne to-type units and of the electrical characteristics of the unit described in this report indicate that this unit should meet Bureau of Mines permissible requirements.This report gives a detailed description of the design and operation and results of tests to determine safety of operation in gassy atmospheres.DESIGN AND OPERATION OF UNITOperation of the vibrator-type multiple-shot blasting unit is based upon utilization of the discharge energy from a capacitor charged to a given. voltage and having capacitance adequate to store enough energy to initiate 10 detonators in series. The primary source of energy is a 4-volt cap lamp battery such as is carried for illumination on the person of underground workers in nearly all mines. As this battery is already available and is recharged daily it is considered to be a reliable and convenient source of energy for a blasting unit. Figure 1 is a schematic diagram of the circuit for an improved unit, and figures 2 and 3 are views of the experimental unit. The battery potential is applied through a radio-type synchronous vibrator (8) to an 8-volt center-tapped primary of a small transformer (9) having a high-voltage center-tapped secondary. The vibrator introduces an alternating low-voltage potential into the primary and mechanically produces full-wave, rectified, pulsating, direct current from the secondary. A buffing condenser (11) across the secondary gives the proper wave form for long vibrator life: a resistor (12) its in series with this condenser across the total secondary to protect the transformer in case of• failure .of this capacitor. A voltage divider (10) with an adjustable slider permits accurate adjustment of the potential applied to the high-capacity firing condenser (5)."

The Bureau of Mines has conducted considerable research to improve life support technology for underground mining applications. This proceedings volume presents several new developments that may help increase the chances of mine workers in surviving underground disasters. Several papers address the performance of present self-contained self-rescuers (SCSR's) and provide proposed guidelines for the design and testing of a second-generation SCSR. Improvements In the safety and effectiveness of mine rescue and recovery operations are described, including the design of a low-profile rescue breathing apparatus and a rescue team helmet.

On-the-job training is a common means of training or retraining workers and the practice may seem simple and straightforward. Doing it effectively however requires more thought and preparation than simply having someone follow an experienced worker around and watch what they're doing. Over the years, various techniques and practices have evolved that pass on the skills and knowledge of a trainer or coach in a manner that leads an inexperienced trainee to really absorb the information being taught. On-the-job training thus becomes less a haphazard show-and-tell and more a real transfer of information. The goal of this document is to describe how to develop or manage an on-the-job training program so that information is passed on from trainer/coach to trainee efficiently and effectively. By efficient is meant the best use of time and resources of all those involved in the training process, and by effective is meant that the trainee truly learns the skills and internalizes the knowledge needed to perform the job well. The first part of the document discusses topics to think about when developing a formal on- the-job training program. A structure based on coaching is suggested. Then a workshop designed to prepare trainers as coaches is described. Workshop materials include an instructor’s guide and trainee workbook. These materials can be used to train coaches and to assist trainees as they go through the learning process. Content related to specific jobs can be added to these materials to create targeted on-the job-training manuals. Structuring a formal on-the-job training program It is likely that some form of on-the-job training (OJT) is being conducted at your workplace. The formality of that training can range from a structured program to an informal “follow Joe around” method. If your program is completely unstructured, it may not be meeting current training needs. Worse, it may perpetuate bad habits and create “trained” employees who are not given the same information or evaluated to the same standards. Wiehagen et al. comment that, “Success in using unplanned OJT is usually dependent on the luck of the draw, that is, whether the informal trainer is competent at the task he or she is teaching, is motivated to teach, can organize the job into logical components, and knows something about good practices in teaching and evaluating” (2002, p. 27). It is only through planning a structured OJT program that consistency can be created and maintained. At the least, all trainees who have successfully completed such a program can be expected to perform at an established level—they will know what they must do to be successful.

"Introductory Statement:In melting the nonferrous metals and alloys for foundry and rolling-mill work, even with the best practice, there is always a loss, due principally to oxidation. Melting losses are a serious source of financial loss in brass and bronze practice, but they are especially serious in the case of aluminum and its light alloys because of the relatively high price of aluminum. The indications are that the melting losses in many aluminum foundries of the United States are at times high, and in some plants (especially where certain furnaces are used) they are entirely too high when compared with the average practice. The fact that the oxidation losses in melting aluminum and its light alloys are high is well known, but it is doubtful whether the factors governing the most suitable methods of melting for a minimum of dross are equally well known. The metal losses involved in melting are worthy of especial attention now because of the rapidly growing importance of the aluminum industry.Factors in the operation of furnaces which affect metal and fuel losses:The essential requirements in the commercial melting of any metal or alloy are that the melting losses should be low, the melting should be rapid, and the melting cost should be low. Certainly it may not be possible to obtain these requirements in any furnace selected at random, and it follows that the design and the operation of furnaces for melting different metals are subjects that warrant the most extensive study and tests. As a general rule, in the industrial operation of furnaces, run on aluminum and aluminum alloys, an effort is made to keep the dross and oxidation losses as low as possible. In attempting to hold dross losses low, numerous factors involved in metal melting may be overlooked, or at least disregarded, and the net practical result may be that the furnace is exceedingly wasteful of fuel. In the case of aluminum and its light alloys, the effect of increasing the temperature of melting, at least in an atmosphere containing free oxygen and nitrogen, is to increase the net melting loss."

"There seems to be some confusion between the terms carbon black and lamp black, although in American lampblack is generally understood to be a soot formed by the smudge process. In this process oil, coal tar, resin or some solid, or carbonaceous substance is burned in an insufficient quantity of air, while on the other hand carbon black is a product resulting from the incomplete combustion of gas and is deposited by actual contact of flame upon a metallic surface.The various carbons, such as gas retort coke, oil retort coke, graphite, carbon black, lampblack, vine black, wood-pulp,black, willow charcoal, and blacks made from refuse material such as leather have different characteristics as to flocculence strength of color, and physical structure. These blacks are made by one of the following methods:(1) Formation by direct contact of a flame upon a depositing surface.(2) Production by combustion of an oil, tar, etc., in an inadequate supply of air; the soot is allowed to slowly settle out on the floors and the walls of the collecting chambers.(3) Carbonization of solids and subsequent reduction to a state of small subdivision.(4) Production by heating carbonaceous vapors or gases to a decomposition temperature by external heating with or without air in the forming chamber. This is usually referred to as cracking or thermal decomposition."

"IntroductionOn May 27, 1927, a coal-dust explosion occurred at the Palagua No. 5 mine of the Victor American Fuel Company, Delagua, Colorado, resulting in the loss of even lives and in and in considerable damage to the main slope and to several overcasts the explosion did not extend into the mine working where 125 additional men were employed but escaped uninjured. It is believed that the saving of these 125 men and the evidence of large property loss were due in a large measure to coal-dusting and of other sanity measures taken by the coal company management. A number of features connected with the explosion and mine safety practice seem worth the attention of the mining public and grateful acknowledgment is made to the Victor American Fuel Company for its aid in assembling the data herein contained."

PURPOSE: Evaluate practical technological advances in hearing protective devices for use in different mining environments. RESEARCH SUMMARY: Previous NIOSH research shows that miners develop hearing loss much more quickly than nonoccupationally noise-exposed individuals. Also, miners experience a greater severity of hearing loss than would be expected for nonoccupational noise-exposed persons of the same age and sex. As described in the National Occupational Research Agenda, problems created by occupational hearing loss include (1) the reduced quality of life due to social isolation and unrelenting tinnitus, (2) impaired communication with family members, the public, and coworkers,(3) a diminished ability to monitor the work environment (e.g., warning signals, equipment sounds, etc.), (4) lost productivity and increased accidents resulting from impaired communication and isolation, and (5) expenses for workers’ compensation and hearing aids. Although the elimination of hazardous noise is the ideal long-term solution to the noise problem, miners will likely need to rely on personal hearing protection in certain situations for quite some time. However, it is well known that noise attenuation provided by hearing protectors as they are worn during everyday use in the workplace is usually worse than when tested under laboratory conditions. Additionally, workers may refuse to wear their hearing protection for any number of reasons. Therefore, this project was initiated to evaluate practical technological advances in hearing protectors, with an emphasis toward optimizing the balance between effectiveness, comfort, overprotection, and communication ability. Currently, the necessary in-house laboratory facilities and research techniques are being finalized. Individual experiments are planned that involve analysis of new hearing protector technology under both laboratory and field conditions. The laboratory components are to consist of con-trolled studies using human subjects and/or an anthropometrically correct artificial test fixture to evaluate the adequacy/effectiveness of various hearing protective devices. Field studies are planned to determine the actual usefulness and practicality of the hearing protectors as used in diferent mining environments. Recommendations can then be offered to the mining community regarding correct earplug/earmuff usage that maximizes recognition of hazard/warning sounds while adequately protecting the miners’ hearing.

This Bureau of Mines report discusses uranium in situ leach mining in the United States to acquaint the public with an overview of this emerging mining technology. This report is not intended to be a technical discussion of the subject matter, but rather should be used as a reference source for information on in situ leaching. A list of in situ leaching publications is included as well as engineering data tables for almost all of the active pilot-scale and commercial uranium in situ leaching operators. These tables represent a first attempt at consolidating operational data in one source, on a regional basis. Additional information is given that discusses the current Bureau of Mines uranium in situ leaching research program. Also included is a listing of various State and Federal permitting agencies, and a summary of the current uranium in situ leaching operators. Finally, a glossary of terms has been added, listing some of the more common terms used in uranium in situ leach mining.

This report (Software Safety 2.2) is the third in a nine-part series of recommendations addressing the functional safety of processor-controlled mining equipment. It is part of a risk-based system safety process encompassing hardware, software, humans, and the operating environment for the equipment’s life cycle. Figure 1 shows a safety framework containing these recommendations. The reports in this series address the various life cycle stages of inception, design, approval and certification, commissioning, operation, maintenance, and decommissioning. These recommendations were developed as a joint project between the National Institute for Occupational Safety and Health and the Mine Safety and Health Administration. They are intended or use by mining companies, original equipment manufacturers, and aftermarket suppliers to these mining companies. Users of these reports are expected to consider the set in total during the design cycle. • 1.0 Safety Introduction.—This is an introductory report for the general mining industry. It provides basic system/software safety concepts, discusses the need for mining to address the functional safety of programmable electronics (PE), and includes the benefits of implementing a system/software safety program. • 2.1 System Safety and 2.2 Software Safety.—These reports draw heavily from International Electrotechnical Commission (IEC) standard IEC 61508 [IEC 1998a,b,c,d,e,f,g] and other standards. The scope is “surface and underground safety-related mining systems employing embedded, networked, and non networked programmable electronics.” System safety seeks to design safety into all phases of the entire system. Software is a subsystem; thus, software safety is a part of the system’s safety .• 3.0 Safety File.—This report contains the documentation that demonstrates the level of safety built into the system and identifies limitations for the system’s use and operation. In essence, it is a “proof of safety” that the system and its operation meets the appropriate level of safety for the intended application. It starts from the beginning of the design, is maintained during the full lifecycle of the system, and provides administrative support for the safety program of the full system. • 4.0 Safety Assessment.—The independent assessment of the Safety File is addressed. It establishes consistent methods to determine the completeness and suitability of safety evidence and justifications. This assessment could be conducted by an independent third party. • 5.0 Safety Framework Guidance.—It is intended to supplement the safety framework reports with guidance providing users with additional information. The purpose is to assist users in applying the concepts presented. In other words, the safety framework is what needs to be don and the guidance is the how it can be done. The guidance information reinforces the concepts, describes various methodologies that can be used, and gives examples and references. It also gives information on the benefits and drawbacks of various methodologies. The guidance reports are not intended to promote a single methodology or to be an exhaustive treaty of the subject material. They provide information and references so that the user can more intelligently choose and implement the appropriate methodologies given the user’s application and capabilities. [ ]

During August, two eastern Kentucky coal miners died in a roof fall during retreat mining operations. They had just finished extracting one pillar block and were moving to the next one when the intersection they were standing in collapsed. Regrettably, this was not an isolated incident. Since the end of 2000, there have been 28 ground fall fatalities in U.S. coal mines. Of these, 10 occurred during pillar recovery operations. Although pillar recovery has been associated with about one third of recent ground fall fatalities, it accounts for only about 10% of U.S. underground coal production. Statistically, a coal miner engaged in pillar recovery is several times more likely to be killed in a ground fall than a miner on an advancing section of a longwall. Pillar recovery is dangerous because it creates an inherently unstable situation. Once the pillars are extracted, the roof is expected to cave. Safe pillar recovery does not mean preventing roof collapse, it means ensuring that it only occurs after the miners have completed their work and have left the area. Fortunately, there are a number of proven techniques that can greatty reduce the hazards of pillar recovery. These include better mine planning, improved roof support, and safer work procedures. The goal of this article is to describe the most significant "risk factors" associated with pillar recovery, and prescribe control techniques that, taken together, can reduce the overall risk to miners. The conclusions are based on extensive studies conducted by the National Institute for Occupational Safety and Health (NIOSH) and the Mine Safety and Health Administration (MSHA) that have included detailed analysis of every fatal retreat mining incident since 1992 (25 incidents resulting in 30 fatalities). Risk reduction strategies for pillar recovery can be divided into three main groups: Global Stability: Prevention of section- wide pillar failure; Local Stability: Prevention of roof falls in the working area; and Work procedures and worker location: Minimizing exposure to hazardous areas.

For many years there have been small scale tests available for evaluating the toxic fumes production by cap-sensitive explosives (DOT Class 1.1), but these could not be used with blasting agents due to the large charge sizes and heavy confinement required for proper detonation. Considering the extensive use of blasting agent in construction and mining, there is a need to determine the quantities of toxic fumes generated by blasting agents. At the International Society of Explosive Engineers Twenty Third Annual Conference on Explosives and Blasting Technique in 1997, the authors reported on a facility for detonating large (4.54 kg), confined blasting agent charges in a controlled volume that had been constructed at the National Institute for Occupational Safety and Health’s Pittsburgh Research Lab’s Experimental Mine. Since 1997, this facility has been used to collect data on toxic fumes produced by the detonation of various ammonium nitrate/fuel oil (ANFO) mixtures and several cap-sensitive explosives. ANFO composition ranging from 1 to 10 percent (pct) fuel oil have been studied. As expected from previous studies, with an increase in fuel oil content the carbon monoxide production increases, while nitric oxide and nitrogen dioxide production decrease. The detonation velocity varies from 3,000 to 4,000 m/sec for the 1to 10 pct range of fuel oil content, suggesting that ANFO mixes with improper fuel oil content may appear to detonate properly, while their fume production differs significantly from optimum. The study also considers such factors as degree of confinement, water contamination, and aluminum content on blasting agent fume production. Results indicate that water contamination of the ANFO has little effect on carbon monoxide production, but causes significant increase in nitric oxide and nitrogen dioxide production. Decreasing confinement from Schedule 80 steel pipe to 0.4-mm thick sheet metal also has little effect on carbon monoxide production, but significantly increases nitric oxide and nitrogen dioxide production. Adding5 and 10 pct aluminum to the ANFO had no clear effect on carbon monoxide, nitric oxide, or nitrogen dioxide production.

"INTRODUCTION A layer of soot on the heating surface of a boiler decreases efficiency because it increases resistance to the flow of heat, with the result that the heat transfer to the water is lower and the temperature of the flue higher than they would be if the surface remained clean. The percentage of reduction in heat transfer depends on the position and area of the sooted. The percentage of reduction in heat transfer depends on the position and area of the sooted surfaces, the thickness of the most, until the percentage of the total heat absorbed by those surfaces when clean; thus it will vary with the type of boiler and in particular with the ratio of the area of the convection heating surface to the total area. Medium-size and large boilers usually are equipped with soot blowers, and tests have been published giving the increase in efficiency resulting from removal of the soot. Fuel savings up to,8 percent have been reported, but 4 to 5 percent is probably more conservatives. However, these savings may not be due wholly to a reduction the thermal resistance of the surfaces but may be due partly to improvement in the combustion because of better draft or more uniform flow of the gases. In addition, the deposits on the surface of boilers include as a clinker, and as the size of the boiler and the rate of burning increase very little or none will be soot.There are no corresponding test data on small boilers operated at low rates of burning. In stall boilers, deposits are much thicker, and it might be expected that the reduction in efficiency would be greater; in fact, statements have appeared suggesting that the decrease may be as high as 50 percent. This investigation was undertaken because the Bureau frequently receives requests for authoritative information on the effect of soot in small and other heating boilers."

"The fire boss in coal mining and the shift boss in metal mining, though held responsible to a large extent for safety of underground employees and of the mine, are, it seems to me, not always vested with sufficient authority nor given compensation commensurate with their duties. The metal-mine shift boss, although his position as regards his relations between employer and employee, is frequently an unenviable one, has as a rule much more authority than the fire boss and is not expected to perform the menial tasks required of the latter by many coal-mining companies.In most coal-mining States the coal-mine foreman and the fire boss are re¬quired to possess a certificate of competency obtained by passing an examination. The foreman is legally responsible for the safety of the miners, as in most instances the superintendent is not required to pass an examination. Although the foreman is held legally responsible for the safety of the miners, the law does not, in general, compel the superintendent or other higher officials to delegate sufficient authority to the foreman to enable him to put his wishes or judgment into effect; and this condition has tended in many instances towards perpetuation of unsafe conditions.The fire boss is held legally responsible for conditions in his ""beat"", but he is generally given only such authority as the mine foreman sees fit to delegate, frequently very little, Moreover, the fire boss is generally required to cover his ""beat"" within two hours before the entrance of the underground workers. Only too often his territory is so great that in order to reach all the working faces and chalk the required date at the face, he must travel at a rapid walk or even run. When such haste is necessary to cover the required ground, it is manifestly im¬possible for the fire boss to make a careful inspection or examination of the roof, timbering, wiring, gas, etc., at each place, and doubtless in his hurry many unsafe conditions escape his notice."

"The results of a two—year research study into the relationship between quartz dust levels in host material and in respirable dust in continuous mining sections are presented in this paper. The experimental design and sampling procedures for underground data collection are outlined. The sample analysis procedures for quartz determination, the ¬analysis results and conclusions of the study are discussed.The results of the study indicate that the quartz contents of the airborne dusts are generally higher than those in the host material and bulk samples; and the quartz content increases with decreasing size of the airborne dust. The source for quartz appears to be the roof and the floor materials rather than the coal seam itself. It also appears that the material containing quartz may be breaking into a different size distribution than coal.INTRODUCTION Without doubt, the potential for permanent and severe health damage exists due to respirable dust in the work environment. The attention directed at the reduction of respirable airborne dust to levels below those specified in the law and the achievements to date in meeting these standards are but a reflection of the recognition by many parties government, industry, and labor - of this potential and the need to reduce it.The ambient dust standard in underground U.S. coal mines (2 mg/s3) has a qualifier (Code of Federal Regulations, Title 30, Part 71.101) to determine an applicable dust standard when a respirable dust sample contains more than 5% quartz. This standard is frequently referred to as the ""reduced standard."" The reasoning behind the reduced standard is the relative toxicity of the quartz dust compared to that of pure coal dust. As the quartz levels in samples increase, the applicable standards become more stringent and maintaining the workings in compliance becomes increasingly difficult.The number of underground work locations operating under the reduced standards has shown a dramatic increase since 1980 (Figure 1). For comparison purposes, there were no mining entities operating on reduced standards prior to 1977. The emergence of this problem, which affects the health and safety standards on one hand and creates severe compliance problems on the other, has been rapid."

This chapter analyses the parameters influencing initial design of communication systems, for new mines and upgrading existing systems. Paragraph 5.2 outlines those variables that must be taken into account during the design stages of a new wired phone system. Recommended features, general requirements, and how they can be implemented are treated in this section. Paragraph 5.3 describes ways of improving or extending the range of trolley carrier phone systems and pager phone systems already installed in the mine. 5.2 New Phone System Design The task of designing an adequate communication, control, and monitoring system for an underground mine must be addressed on a system basis. In addition to insuring that effective voice communication is established, any new system should take into account present and future requirements of remote control and monitoring functions. Chapter 4 illustrated the drastic savings in response time that can be realized when remote control and monitoring are integrated into the overall communication system. The importance of including control and monitoring in the overall design plan for any system cannot be overemphasized. Because each mine is unique, and thus usually has its own special operating characteristics and communication requirements, there is no such thing as "the one best system" to meet the requirements of all mines. The optimum communication, control, and monitoring system for a mine must be one that has been tailored to meet the special requirements of that particular mine. Factors that must be considered during system design include- a. Type of mine and mining methods (low- or high-seam coal, deep hardrock mine, stope caving, longwall, room and pillar, etc.). b. Maximum number of working sections. c. Expected mine growth rate and eventual maximum size. d. Haulage methods (tracked trolley, diesel, belt, etc.). e. Underground power distribution system (dc, ac, or both). f. Features desired (two-way radio paging, private line capability for emergency use, etc.). g. Redundant or backup systems for use during outages of the normal system. Although no two mines are alike, the following items have been established as the main characteristics desired for any underground communication system: 1. Multiple Communication Paths to Outside--the objective here is to give all telephones a second method of communicating with the surface. 2. Audible Emergency Signaling--the communication system provides the main means of alerting miners during emergencies. The system should include means to broadcast distinct audible signals for emergency signaling. Initiation of these signals should probably be controlled from. a central outside point, such as a surface control room. 3. Emergency Override--provisions should be included to permit any conversation to be overridden with emergency communication. 4. Selective Area Page--as mines grow larger it is apparent that the entire telephone system paging mode need not be activated each time a call is initiated. When the general area of a

A system for measuring amplitude probability distributions (APD’s) of electromagnetic noise in coal mines is described and typical APD’s from an underground coal mine are presented. The APD is a basic statistic required for the design and analysis of communication systems, especially those intended for use in noisy environments, and where neither overdesign nor underdesign is acceptable. The rms and average field strengths are obtained by integration of the APD, and examples are shown at several frequencies. All field strength levels are given in absolute units. Selected frequencies cover the range from 10 kHz to 32 MHz.

Illinois has the largest reported bituminous coal resources of any state and is one of largest coal-producing states in the nation. Coal-bearing rocks underlie about 65 percent of Illinois, including all or part of 86 of its 102 counties. An estimated 18 1 billion tons of coal resources are still in the ground. Using present mining methods, more than 30 billion tons of coal are classified as recoverable. This represents almost one-eighth of the total recoverable reserves of all types of coal in the United States. The state's surface mining regulatory program was conditionally approved June 1,1982. It received full approval October 25,1988. The Abandoned Mine Land Program was approved June 1, 1982, and the AML emergency authority June 11, 1984.
