In May of 1980, a mine operated by Belmoral Mines Ltd experienced a failed crown pillar, resulting in the death of eight miners and putting sixteen others in harm?s way. The Ferderber mine was located in the Val d?Or region of the province of Quebec in Canada, and had been under development for 19 months and production for 9 months, since the opening of the portal. The crown pillar separating the 1?7 level from overlying overburden ultimately failed, permitting an estimated 1.5 million cubic feet of liquefied sediment to gain entry into the mine. This paper back-analyses the cave-in by applying decision error theory, in an examination of the organizational culture and the collective decisions contributing to the cave-in disaster. A case will be made for a greater understanding of mining methodology, geomechanics and risk assessment when operating in such challenging geologic conditions. Boundary condition and decision (BCD) analysis will be introduced as an innovative new method for the analysis of mine accidents and incidents. It will also be shown that BCD analysis can be applied to any event in an enterprise for which standards, norms or legislation exists. A cognitive profile will be presented that provides insight into the safety ethos existing within the Ferderber mine organization at the time of the tragedy.

Geographical information systems (GISs) provide sophisticated functionalities t1tat can be used by the personnel in many aspects of the mining industry to visualize, analyse, and manipulate spatial and tabular data. This paper presents an experimental case study for Hartebeestfontein Gold Mine. It shows how data from different computer systems can be brought together into a GIS by use of the Arc/Info and ArcView programs.

"Ivanhoe Mines’ Kamoa copper project is a recently discovered high-grade copper sulphide deposit in the Katanga Province of the Democratic Republic of Congo. The mineralization identified to date within the resource is typical of sedimentary-hosted stratiform copper deposits and comprises three distinct units: supergene, mixed, and hypogene mineralization. Since 2010, various metallurgical test work campaigns have been conducted on drill core from different areas within the deposit in line with resource expansion. The most recent bench-scale flotation test work has shown positive repeatable results. The work was conducted on a composite sample of drill core from the southern part of the Kamoa resource. The sample is representative of the first four to five years of planned production, when Ivanhoe Mines intends to produce clean, highgrade copper flotation concentrate. A detailed test work roadmap developed from first principles for the campaign led to an optimum flow sheet for treating the Kamoa mineralization. Copper recoveries of 88.3%, at a concentrate grade of 39.0% copper, were achieved using a composite sample — an improvement on the previously published 85.9% life-of-mine average copper recovery indicated in the November 2013 Kamoa Preliminary Economic Assessment. IntroductionThe Kamoa project is a newly discovered, very large, stratiform copper deposit with adjacent prospective exploration areas within the Central African Copperbelt, approximately 25 km west of the town of Kolwezi in the Democratic Republic of the Congo (DRC) and about 270 kilometres west of the provincial capital of Lubumbashi. Ivanhoe Mines holds its 95% interest in the Kamoa copper project through a subsidiary company, Kamoa Copper SA Limited SPRL, and DRC government owns 5%. The project location is indicated in Figure 1.In January 2013, a new independent mineral resource estimate ranked Kamoa as Africa's largest high-grade copper discovery and the world's largest undeveloped highgrade copper discovery. At this time, Ivanhoe Mines had estimated discovered Indicated Mineral Resources of 739 Mt grading 2.67% copper, containing 43.5 billion pounds of copper, and Inferred Mineral Resources of 227 Mt grading 1.96% copper, containing 9.8 billion pounds of copper, as demonstrated in the Preliminary Economic Assessment (PEA) (AMC, 2013). A 1% copper cut-off grade and a minimum vertical mining thickness of 3 m were applied in each classification."

'Gee Whiz', a nationwide schools' science and technology competition, is being screened on SATY1 on Friday evenings at 18:00. Twenty-seven schools from al over South Africa are taking part in the series, which will run for 13 weeks. The participating schools were selected on the basis of their performance in the National Youth Science Olympiad and in Mintek's Minsa science competition, which has been organized on an annual basis since 1988.

SYNOPSIS Certain disadvantages associated with the use of Anfex have been raised on previous occasions. This interim investigation shows that by loading at lower pressures the sensitivity to initiation of Anfex can be improved. Associated with this is a reduction in velocity of detonation and charge density which should lead to more desirable fragmentation. The effect of lower loading pressure on other factors is also considered. INTRODUCTION Discussion at the Symposium on the use of Anfex in Underground Mining, conducted by the South African Institute of Mining and Metallurgy during May 1968 showed that there was considerable uncertainty and diversity of opinion on the characteristics of Anfex and its method of application. Some of the disadvantages associated with the use of Anfex were: Inadequate sensitivity to initiation; Excessive fragmentation of the rock and damage to the hanging and footwalls; Wastage as a result of blow-back; Poor water resistance. As a result of this, further investigations on the properties of Anfex have been carried out and, although these have not reached finality, it is considered that a progress review may be of interest and assistance to the mining industry. EXPERIMENTAL TECHNIQUE The work was carried out on a laboratory-scale where, for test purposes, the explosive was loaded into 1 in. (nominal) internal diameter steel pipes unless otherwise stated. Charging was conducted with a 1 in. (nominal) Schutte-Koerting eductor having a 0.2 in. internal diameter venturi nozzle, the Anfex being fed into the eductor from a conical hopper fitted directly above it. A 5 ft long copper loading tube with outside and internal diameters of 3/4 in. and 5/8 in., respectively, was fitted to the discharge end of the eductor. The remote end of the loading tube was inserted into the test pipe so that it was about 6 to 8 in. from the closed end of the pipe. The loading apparatus was withdrawn as the pipe filled, maintaining approximately the same stand-off distance throughout the operation. REVIEW Sensitivity to initiation Sensitivity of Anfex to initiation was found to deteriorate when the dynamic pressure of the air to the eductor was increased. The addition of binder to full plant tailings significantly affects the rheology of a backfill mixture and, consequently, the pressure gradients in the pipeline. The binder increases the yield stress and apparent viscosity of the mixture, and the transition velocity between the laminar and turbulent flow regimes. In a test conducted at a slurry relative density of 1,72 with binder addition, the mixture also exhibited time-dependent behaviour owing to the hydration of the binder.

The effect of high solids concentration and coarse particle sizes (d50 ranges 90?300 µm) on the viscosity of a suspension of water and silica sand was investigated. The experiments were designed to include conditions that have been tested by other authors and those encountered in the operation of tumbling mills. The rheological data was measured using a tube rheometer with operating pseudo shear rates of up to 1500 s-1. The rheograms obtained depicted dilatant behaviour. The Otswald-de Waele, Bingham, Herschel-Buckley and Casson models were fitted to the experimental data. The Herschel-Buckely model, which approximates the yield point and gives an indication of the shear thinning or shear thickening behaviour of the suspension, was found to provide the best description of the flow curves for all slurries. The Herschel-Buckely model was then used to determine the apparent viscosity of all the tests. The apparent viscosity and yield stress increased with solids concentration and particle size at the different pseudo shear rates. The increase in slurry viscosity with solids concentration was attributed to increased frequency of particle-particle interactions. The increase in slurry viscosity with particle size was attributed to increased inertial effects. At low shear rates the sand slurry exhibited shear thinning behaviour for all solids concentrations. At higher shear rates above 1000 s-1the high solids concentration suspensions exhibited a transition from shear thinning to a shear thickening behaviour.

The AK6 kimberlite is situated 25 km south of the Debswana Orapa Mine in Botswana and was discovered by De Beers geologists in 1969 during the follow-up of geophysical targets in the Orapa area. The kimberlite was not extensively pursued at the time as the initial bulk sampling indicated it to be of limited size and low grade, factors largely contributed to by the basalt breccia capping. Completion of high resolution integrated geophysical techniques and drill bulk sampling to depth recovered 97 tons of kimberlite during 2003 and 2004, which led to the increased size and grade estimates. Bulk sampling by Large Diameter Drilling (LDD, 23 inch diameter) commenced in 2005; 13 holes were drilled to a cumulative depth of 3,699 m and 689 carats of diamonds were recovered. In July 2006 the De Beers Mineral Resource Classification Committee classified these Phase I LOO results at a High Inferred level with an average grade of 24 carats per hundred tonnes (cpht) at a bottom cut-off of +1 mm, and a modeled average diamond value of 150 dollars per carat. A second phase of LDO drilling was initiated in 2006, and bulk sampling by trenching commenced in 2007 in order to deliver a resource estimate at indicated level. An Indicated Resource of 11.1 million carats at an average grade of 22 cpht was declared for the deposit mining lease application lodged in 2007.

The development of a cyanidation route for the treatment of gold-plant gravity concentrates is described. This work was carried out as a natural consequence of earlier improvements to the recovery of gold in the gravity sections of existing gold plants. Concern about the environmental aspects of the conventional amalgamation process was further motivation in the development of an alternative procedure. It was found that the partial pressure of oxygen in solution was the most critical reactionrate determining factor. The rate of gold dissolution was shown to be controlled by the reaction rate below approximately 25°C and by diffusion above that temperature. Thus, the efficiency of pulp agitation and oxygen partial pressure permitted a high degree of control over the rate of gold dissolution. The use of oxygen rather than air significantly increased the dissolution rate of both gold and silver. The solubilities of sodium, calcium, and potassium aurocyanide complexes under simulated reaction conditions were studied, and it was established that the values for the sodium and calcium complexes were limited, requiring the use of lower pulp densities and higher reaction temperatures for the treatment of very high-grade concentrates. The presence of excessive amounts of tramp iron in such concentrates of up to 25 per cent by mass was found to result in poor gold dissolution owing to the cementation of gold under certain reaction conditions. The maintenance of high oxygen partial pressures, together with high cyanide concentrations, high pH levels, and the presence of calcium in the system, all aided in passivating the tramp iron. Copper, gold, and iron were shown to be the main cyanide-consuming elements. Optimum gold dissolution was obtained in a closed bench-scale reactor incorporating a flotation-machine type of agitator for effective pulp agitation and oxygenation, the oxygen being recirculated. The reactor was designed to operate under a very small positive pressure. After reaction times of 2 to 6 hours at temperatures in the region of 30°C, both the gold and the silver dissolution averaged 99,S per cent. In the case of very high-grade concentrates, it was shown that gold could be floated in the same reactor by drawing off the flotation concentrate through a port in the reactor. A flotation recovery of 87,4 per cent of the gold in 3,9 per cent by mass was obtained. The recovery of osmiridium from cyanide-leach residues and the electrowinning of gold from the pregnant liquors were also considered as processing aspects in the treatment of gold-plant belt concentrates. Final table tailings with a gold concentration of 10to 12 gft were obtained. The results of the investigation strongly suggested that an alternative cyanidation route for the treatment of gold-plant concentrates was technically feasible, provided the system included effective agitation and oxygenation. In the case of lower-grade concentrates, cyanidation followed by the electrowinning of gold appears to be the simplest route, while, for high-grade concentrates, flotation and smelting of the concentrate followed by cyanidation of the flotation tailings are suggested. The use of oxygen rather than air for sparging the pulp would depend largely on the grade of material to be treated. A preliminary comparison of the costs related to the amalgamation and cyanidation processes for the recovery of gold from such concentrates indicated very similar capital and operating costs. It was concluded that other considerations, e.g. toxicity, safety, and lower labour requirements, were more important than such small cost differences. Furthermore, the 'opening' of the gold-plant gravity circuit, which would be made possible by a cyanidation route, would further reduce the operating costs because of the subsequent decrease in the tonnage to be treated.

Using a very large number of chip-sample values from a mined-out area of the Hartebeestfontein mine and accepting these as equivalent to borehole values (also areas of 1 km by 1 km as equivalent to 'mines'), the authors apply a series of borehole-valuation procedures to these 'mines', Large undeveloped sections of existing mines are also regarded as the equivalent of such 'mines'. These procedures start with the orthodox arithmetic mean and then introduce additional information in the form of the Iog normal frequency distribution model for Sichel's 't' estimator. Knowledge of the average log-variance within mines leads to the tH estimator. The mean grade of the 'family' of mines to which a new mine belongs is then introduced via regression or elementary kriging. Next, further information on the macro spatial structure of the grade is used, together with the grades of adjacent existing mines in ordinary and simple macro-kriging procedures; finally, via the distribution model, the grade is seen as a function of the logs of the grades and their variances. These are introduced as spatial variables, and both are estimated by macro-Iog normal ordinary and simple kriging procedures with grades and variances from adjacent mines'. Comparisons are also made between macro-kriging on lognormal transformed values versus untransformed values. Conclusions are drawn on the relative importance of the various items of additional information used, on the procedures followed, and on the significance of the advantages gained

VERSLAG- REPORT No. 1365 STUDIES OF INCIPIENT FUSION IN THE SYSTEM CHROMITE- MgO- Al2O3-SiO2-C 5th January, 1972 Investigator: N. A. Barcza SYNOPSIS A fundamental study was made of twelve chromite ores by use of a thermobalance under atmospheric and reducing conditions. It was found that their thermal behaviour under atmospheric conditions can be related to the mineralogical composition of the chromite spinel and the gangue. The overall mechanism of reduction with solid carbon over the temperature range of 1 200 to 1 500°C was shown to be diffusion of carbon through the reaction product. The effects of additions of fluxes and variations in particle size on the kinetics and mechanism of reduction were determined. It was shown that carbon monoxide does not reduce chromite ore up to temperatures of 1 550 °C, and it can therefore be eliminated as the diffusing reducing species. However, the reaction product of reduction, namely a mixed carbide of iron and chromium, was shown to be an effective reducing agent for chromitc ore, while also facilitating the transport of carbon. VERSLAG - REPORT No. 1380 AN ELECTROCHEMICAL MODEL FOR THE LEACHING OF URANIUM DIOXIDE 10th January, 1972 Investigators: C. R. S. Needes, M. J. Nicol SYNOPSIS A correlation between the rate of leaching of UO2 and the electrochemical properties of the oxidant concerned has been derived. This correlation is found to be borne out by experimental results. Certain effects observed in UO2 leaching and hitherto unexplained have been clarified in terms of an electrochemical mechanism. Experiments have shown that the mixed potential of a UO2 electrode gives a measure of the rate of dissolution of UO2. Its significance to commercial practice is discussed. CONCLUSIONS (I) The dissolution of UO2 in the presence of a number of oxidants can be explained in terms of an electrochemical mechanism. (2) The mixed potential of the UO2 electrode is a measure of the rate of UO2 leaching. This principle could be used in the control of industrial leaching operations. (3) A simple treatment has provided a mathematical basis for a fundamental correlation between the rate of U02 dissolution and the chemical properties of the oxidants concerned. This relation has been found to agree well with experimental results. (4) A number of phenomena occurring during the leaching of UO2, hitherto not explained in a satisfactory manner, have now been clarified in terms of an electrochemical mechanism.

High Pressure Grinding Rolls (HPGR) are mostly used prior to pelletising processes to increase the specific surface area (SSA) of iron ore concentrate. For expressing SSA, the Blaine index is utilised as one of the most important dimensional properties of iron ore pellet feed. ISO 21283 is a standard test method that explains the manual of Blaine air-permeability apparatus for determination of this parameter. In this standard, the disaggregation method is based on passing the minus of 1 mm samples from the 0.150 mm sieve. The disaggregation process has an effective role on the results of Blaine airpermeability test especially for regrinding HPGR products. In this research, firstly the Blaine index of samples was measured without disaggregation. Following this, disaggregation was done by passing the minus of 1 mm samples through three sieves with different dimensions, 0.150, 0.090 and 0.045 mm, separately, and the Blaine index was calculated for each one. Finally, lab-mill with rubber balls, called Jar mill, was investigated as another option. Screen analysis and image analysis with SEM were performed to evaluate the effects of the sample's disaggregation on the size distribution and agglomeration of samples. The results showed that the disaggregation method with sieve size 0.150 mm, according to ISO 21283, was not applicable for HPGR products because the particles were still stuck together. In addition, the disaggregation method with the sieve size 0.045 mm or with Jar mill offered reasonable results. "In platinum group metals (PGMs) smelting, the slag-to-matte ratios are very high (approximately 4:1 to 9:1). The slag temperature therefore has a strong influence on matte temperature, which is a continual concern due to the low matte melting temperature and its resultant high superheat. Maintaining a stable slag temperature is therefore a critical safety and operational issue. This project aims to improve the understanding of the slag bath temperature drivers so that a more effective slag bath temperature control strategy can be implemented.With the aim of developing a multiphysics model of the furnace, extensive data analysis was performed to develop an in-depth understanding of the process before the start of modelling work. In this work, the effect of fluctuating furnace feed composition on slag temperature was examined. The results are based on equilibrium calculations using FactSage, implemented with ChemApp.A method was developed, using an equilibrium calculation at 25°C, to convert the available chemical assays to approximate mineralogical assays in order to determine the feed enthalpies. Actual 2013 furnace feed data was used and the average specific energy requirement to produce slag and matte at 1600°C was calculated to be 671 kWh/t concentrate. The furnace energy input was then kept constant, and it was found that the slag temperature varies significantly as a function of the feed composition. These results were compared to actual slag temperatures measured on site. It was found that close management of the furnace energy balance is important for maintaining a stable slag temperature."

Samples (15 to 20 g) of pre-calcined Mamatwan manganese ore or pure Mn 304 smaller than 48 mesh were reacted at temperatures in the range 1000 to 1300°C with samples of spectrographically pure graphite smaller than 325 mesh. Samples (1 g) of ore or pure Mn02 smaller than 48 mesh were reacted at temperatures of 1000 and 1200°C with pure graphite fines in argon, with graphite in carbon, .'1onoxide, and with carbon monoxide alone. In each case, the loss of mass as a function of time was recorded with a hermobalance. The rate and degree of reduction were found to increase with additions of carbon and with increasing temperature up to 1300oC, when they reached their maximum. The shape of the reduction curves, together with the results obtained from examination of the products of reaction by microscopy and X-ray diffraction, were used as the basis for a postulated reduction mechanism. Some details are given of an initial kinetic model that was used to verify the proposed mechanism. It was found that the reduction of Mamatwan ore by graphite at temperatures in the range 1000 to 1300°C occurs in two stages: (i) an initial stage during which the higher oxides of manganese are rapidly reduced to MnO with concurrent formation of CaMn2O4 and (ii) a longer stage during which CaMn2O4 and MnO are reduced to carbides by carbon dissolved in the ferromanganese phase.

South Africa is the third-largest supplier of coal on the world's export markets after Australia and the USA, and the second-largest supplier of bituminous steam coal following Australia. There are many Similarities between the coal industries of South Africa and those of Australia and the USA, in contrast to the coal industries in Europe. For South Africa to maintain a competitive position on the export markets, it must continue to compete against these two countries. It is therefore important that the performance of the loca1 coal industry should against that of the Australian and US coal industries. Although financial performance is the major criterion, financial information is not easily obtained, and the paper therefore uses employee productivity in the form of tonnes per man year as a means of comparison. The paper reviews recent trends in the level of production and numbers of employees prior to review1ng the productivity. The influence on productivity of some mining methods is highlighted. It is realized that published information (information available within the public domain) can contain anomalies, but every effort is made to compare data of a similar type. Notwithstanding possible inaccuracies, it is shown that South African coal-De productivity lags far behind that of Australia and the USA. The significance of the difference is that the South African coal industry is able to apply overseas practices and procedures that have been shown to give improved performance, and that the increased use of more productive extraction techniques has limited application.

Computerisation of all aspects of these fields of endeavour has come to be seen as essential for complete integration with other mining orientated disciplines. Release 1 .l. of the Anglo American system of mineral estates management has been tested and is in the process of being implemented at several Anglo American Gold Division mines.

A short review is given of the effects of corrosion fatigue on the materials used in pressurized water-reactor vessels. Selected topics such as testing procedures, test data, and mechanisms in simulated environments are discussed. The main fact that emerges is that most of the data on the growth of fatigue cracks reside below the ASME X1 1980 High-R 'Wet' design-code lines, Le. they fall within that specification. Finally, some recent advances in the understanding of the corrosion-fatigue behaviour of such materials in pressurized water reactors are highlighted.
