Many miners are involved in tasks at the mine site that are similar to those sub-tasks performed in general construction, maintenance and repair (CMR) type work activities. As a result, many of the injuries in the mining industry today can be traced to employees performing these CMR type work activities. Indeed, a recent analysis of a major aggregate company revealed that 65% of all injuries documented originated from performing such CMR activities. It is hypothesized that the number of injuries to miners performing construction, maintenance and repair (CMR) work activities on mine property may be reduced through improved training that focuses on better hazard recognition using the numerous visual and other cues that are available for evaluating workplace health and safety hazards. To this end, researchers at NIOSH’s Pittsburgh Research Laboratory have merged two proven methods of training miners to recognize hazards, stereoscopic (3-D) slides and the concept of degraded images, into this training program. Stereoscopic (3-D) slides have been shown in earlier NIOSH and U.S. Bureau of Mines research to be an effective training aid for improving the ability of miners to recognize various geologic and mining-induced irregularities that can cause groundfalls.3 They realistically portray the natural mine environment, and thus are an excellent training aid for teaching miners to recognize such visual cues. The degraded image concept, originally developed by the military and used for target detection training, describes scenes where the target is partially hidden by cloud cover, dust, rain, natural barriers, buildings, or other obstructions that can camouflage or confuse the target. Research had shown that flight observers who were trained with less than ideal (or degraded) pictures were more successful in subsequent identification of targets than those trained using ideal (or highlighted) pictures of targets. In order to investigate this concept for training miners to recognize hazards, NIOSH researchers developed a prototype hazard recognition training program using such mining scenes. A control program based on traditional highlighted mining scenes was also developed. Both programs were applied to subjects in a field experiment. Results indicated that miners trained with the degraded images did significantly better on a follow-up test of hazard recognition.4 This hazard recognition program teaches trainees to look for hazards in a more realistic manner than that of the control program. This specific training program for CMR activities involves using the degraded image concept and 3-D slides. The module is designed to educate workers on hazard identification techniques and safe work practices. It focuses on teaching miners techniques for recognizing visual (sight) cues, and then uses these cues to evaluate mining conditions. It should be stressed that other senses, such as sound can also play an important role in hazard detection.

To develop a business case that promotes adoption of noise controls by reducing barriers to industry acceptance and enhancing drivers that foster their implementation. Background Over 75% of the U.S. mining workforce is exposed to hazardous noise, putting them at risk of noise-induced hearing loss (NIHL). Consequently, coal miners suffer high rates of hearing impairment that increase to 70%-90% by retirement age. To address the noise problem for coal miners, researchers from the National Institute for Occupational Safety and Health (NIOSH) first determined that the continuous mining machine (CMM) ranked first among machines associated with reported coal noise overexposures. Further studies found that most CMM noise is generated by three component systems on the machine: dust collection, cutting, and conveying. Of these systems, the on-board chain conveyor was found to be a dominant noise source. Noise generated by the conveying system was addressed through the development of a urethane-coated flight bar chain. The coated chain, shown in Figure 1, has demonstrated an 8-hour time-weighted average (TWA) reduction of operator exposure by 3 dB(A). It has since been accepted by the Mine Safety and Health Administration (MSHA) as a "technologically achievable" noise control for CMM operators who are exposed to noise that exceeds the MSHA permissible exposure level. Having demonstrated the noise reduction performance of the coated chain, NIOSH has assembled information into a business case that can be used by operations considering adopting the chain at their worksite.

A revolutionary development in blasting materials has occurred in the past 5 years. Ammonium nitrate, sensitized by the addition of a few percent of No. (class) 2 diesel fuel or other suitable petroleum product, has been adopted as a low-cost, efficient blasting agent2/ by a wide variety of mining and heavy construction operations. Moreover, ready availability of the component supplies throughout the Nation and ease of their formulation into effective blasting agents have made it possible for the blaster himself to pre-pare the mixes at the site of use. Although these field-mixed products have partly or wholly displaced explosives such as dynamites in some applications, there are other blasting operations where the fuel-sensitized ammonium nitrate agents do not appear to be well adapted. Important safety characteristics are inherent in ammonium nitrate-based blasting agents prepared under competent factory control, or when field mixed under equivalent close control--as long as the hazards they present are clearly understood and respected. This publication attempts to summarize knowledge of those hazards and to incorporate practical ways to cope with them. A series of safety recommendations thus is presented. These recommendations cover the preparation, storage, transportation, and use of field-mixed ammonium nitrate blasting agents.

"INTRODUCTION The Congress theme ""Improvement of Mine Productivity and Overall Economics by Modern Technology"" is probably beat: addressed by the subtheme allocated to this Plenary Session. It is appropriate that .Theme 1 emphasizes the simple point that effective use of geotechnical information is the key to improving mine productivity. In essence, this means that: (1) geological and geomechanics, (rock mechanics) data must be collected in sufficient quantity and of high quality; (ii) data interpretation must be performed specifically for purposes of engineering design; (iii) innovative design approaches should be used to bring about Improvements in productivity and safety.While, each of these three aspects .may: seem obvious to any mining engineer it is a fact that the above sequence is seldom followed and as a result, mining engineering not infrequently lags, behind other branches of engineering when it comes to design creativity, efficiency and safety.. . After all, mining is still-one of the most hazardous occupations and we are gathered here to discuss, at the highest world professional forum, how to use more effectively the modern technology. This however, does not mean that mining engineers are less capable than other engineers or that their achievements are any lesser; far from it! Mining engineers are the cream of the engineering society on whose efforts depends the welfare of mankind. Moreover, they have produced minerals and coal evergrowing quantities against evergrowing difficulties. Yet, we must and can become even better because we are not making full use of the modern technology and rock mechanics knowledge already at our disposal, and. we are not being innovative enough to the full extent of .our capabilities."

In May l942, the Bureau of Mines started exploring for manganese on the Black Poch manganese property, Esmeralda County, Nev., to determine possible reserves of manganese in the deposits. The work was done by the Bureau in cooperation with the Federal Geological Survey and comprised a study of the geology, mapping the area, and exploring by trenching, test pitting, and sampling. All ore bodies were examined and sampled, and the three most important areas were explored by trenching and test pitting. This report notes the location, ownership, and history of the deposits; explains the physical features of the district; mentions briefly mine workings and metallurgy; and describes the geology and ore bodies. The Bureau erected a temporary camp, repaired 5 miles of temporary road, constructed 1,000 feet of new road, dug 3,173 foot of trenches with a vertical crone section of 4 x 10 feet and 91 test pits with a cross section 3 x 6 feet, averaging about 6 feet deep, and cut 134 samples.

Recent employment figures (taken from the Mine Safety and Health Administration's database) indicate that the number of independent contractor' employees working in the coal mining industry has doubled over the past ten years. As their numbers increase, they become a more visible segment of the industry. [ ] Increased exposure to mining hazards has also brought an increase in the number of fatalities involving independent contractor workers. One gauge that is traditionally used to evaluate the overall safety of an entity is the "number of fatalities." Over the five-year period from 1985-89, 27 independent contractor employees were fatally injured while working on coal mine property. This number increased to 45 fatalities during the subsequent five-year period from 1990-94. Nearly half of these fatalities (47%) were due to accidents involving powered haulage (See Table); 13% were electrical accidents; and 9% were machinery accidents. Combined, these three accident classifications accounted for 69% of all coal nine independent contractor fatalities. Information taken from fatality data is an important piece of the safety puzzle; however, other variables need to he examined in order to better define contributing factors associated with accidents. More specifically, information on trends in fatal and nonfatal accident and injury data is necessary before deciding what approach should be used to improve safety. In order to implement appropriate safety strategies, it is important for safety practitioners to examine all available lost time accident and injury information. The following coal industry data sites specific and present trends of independent contractor employees working in underground locations at coal mines, surface areas of underground coal mines, surface coal mines, and preparation plants. The data compares employment and accidents for time periods at the beginning and the end of a ten-year span from 1985 to 1994. Because the number of incidents involving independent contractors in certain accident categories during any one year may be small, three-year totals are used; the first three years (1985-87) of the ten-year period are compared with the last three years (1992-94). [ ]

To reduce operator exposure to drilling noise from roof bolting machines. Background Noise-induced hearing loss (NIHL) is a prevalent occupational illness in the United States. Studies indicate that 70%?90% of miners have NIHL significant enough to be classified as a hearing disability. Mine Safety and Health Administration studies indicated that roof bolting machine operators are among those overexposed to noise. Field studies support the premise that, on average, drilling is the loudest noise source to which a roof bolting machine operator is typically exposed and contributes greatly to the operator?s noise exposure. The National Institute for Occupational Safety and Health (NIOSH) has further determined that the drill steel and chuck radiate a significant amount of noise during drilling. To address this noise hazard, NIOSH developed a collapsible drill steel enclosure (CDSE) to encapsulate the drill steel during the drilling portion of the roof bolting machine duty cycle. In addition, NIOSH is collaborating with J. H. Fletcher & Co. in the continued development of a CDSE.

Graphite in the Suffern deposits near Hillburn, N. Y., consists of flakes ranging in size from fines to one-half inch in diameter. The Bureau of Mines investigated the Suffern deposits in the spring of 1943. Gneiss outcrops were examined, and five trenches excavated and sampled. The graphite is not confined to any definite zone but occurs in small, local pockets. Eleven samples were collected and combined for laboratory tests. Tests by the Bureau of Mines indicate that the graphite ore can be beneficiated by stage crushing, "skin flotation" of coarse flakes, and wet flotation of minus 10-mesh flakes. Over 90, percent of the graphite was recovered.

The DuBois fluorite property in the southwestern part of Hardin County, Ill., has been active intermittently since 1904 and has produced an estimated total of 1,000 to 2,000 tons. The persistence of the vein and the stratigraphy suggested extensions of the ore body formerly mined. The Bureau of Mines investigated the property by core drilling early in 1944. Four holes were bored which totaled 1,106 feet and extended along a strike length of 1,150 feet. The data obtained in the course of the investigation are presented in this report.

"The tungsten area in southern Mecklenburg County, Virginia, was studied by the Bureau of Mines during the period August 1944 to March 1945. The explorations were an expansion of the work done by the Bureau in 1943 in the tungsten area of Vance County, North Carolina, which joins Mecklenburg County on the south.3/Outcrops of quartz veins in the area were searched for and examined, and the only Promising showing found (a vein on the Gladys N. Taylor tract) was explored by trenching, test pitting, and diamond drilling (fig. 1).Mecklenburg County, Virginia, is in the southeastern part of the State, bordering on North Carolina,U. S. Highways 1, 15, and 58 traverse the County, and State and County roads provide ready access to most parts of the area.In 1942, Joseph and Richard Hamme discovered tungsten-bearing quartz veins in Vance County, Forth Carolina, and started mining and milling the ore. The first concentrates were shipped in July 1943. In August 1943, the Hamme holdings were acquired by Haile Mines, Inc., who continued the mining operations.The Bureau of Mines began exploratory work in North Carolina in June 1943. Reconnaissance work showed that the mineralized belt extends north-ward into Virginia. The Kimball tract, which joins the Taylor property on the north, was explored in 1943 by trenching and drilling by the R. G. Lassiter interests of Raleigh, Y. C., with disappointing results.The topography of the district is mature and characterized by wide, shallow valleys and low, rounded ridges. The main ridges trend northeast. Most of the area ranges from 300 feet to 400 feet above sea level.The relief apparently is controlled by resistant quartz lenses and the associated hard, silicified zones. The enclosing schists and granites are usually deeply weathered. Consequently, most of the quartz veins are found on the ridges."

"Electron spin resonance (ESR) measurements show that grinding of quartz particles in air produces silicon-based (Si- and SiO-) radicals which decay with aging in air. ESR spin trapping measurements provide evidence for the generation of hydroxyl and possibly superoxide radicals from a suspension of fresh quartz particles. The hydroxyl radical generation potential of the fresh quartz particles decreases on storing in ambient air and on the addition of catalase, superoxide dismutase, desferoxamine, or DMSO. Silica-induced lipid peroxidation also decreases on storing the fresh particles in ambient air. These findings suggest that oxygenated radicals play a role in the biochemical mechanism of pneumoconiosis in general and acute silicosis in particular.INTRODUCTIONThis communication summarizes our electron spin resonance (ESR) investigation of the formation of oxygenated radicals by freshly crushed quartz particles in a cell-free aqueous medium, and its potential role in the biochemical mechanism of acute silicosis. This study was undertaken because the mechanisms by which the quartz particles exert their toxic action on cells and the process(es) by which these actions progress to fibrogenesis are still not well understood. 1-3 It is generally believed, however, that the action of quartz particles on the cell membrane is the starting point of the silicotic process. 1,4 Since the first step must involve surface reactions, characterization of the chemical structure of the surface of quartz particles has been the subject of several recent studies!-' For example, we reported that mechanical crushing of quartz under normal atmosphere generates silicon-based (Si• and SiO•) radicals which decay with time, and that these radicals are associated with a higher cytotoxicity of fresh quartz dust as compared to that of an aged dust from the same stock. 6,8 Independently Fubini and coworkers' have also reported on the formation of Si• and SiO• -type of radicals from quartz particles crushed under atmospheric conditions. Gulumian and van Wyk9 have reported that quartz particles react with hydrogen peroxide (H202) to generate hydroxyl (•OH) radicals. These authors9 suggested that this process might contribute to quartz's pathogenicity but the mechanism of the -OH radical formation was not clarified. Earlier, Gabor and Anca10 had suggested that the cytotoxicity of quartz particles might be associated with the generation of some factor or factors possessing the properties of a free radical and hence capable of promoting the peroxidative chain cleavage of polyunsaturated fatty acid moieties of the phospholipids in the cell membrane. More recently, Weitzman and Graceffa 11 reported that asbestos is able to catalyze the generation of -OH radicals from H2O2. Their later work12 indicated that lipid peroxidation might be one of the mechanisms of tissue injury by asbestos."

"INTRODUCTION Preliminary reports by Bureau of Mines engineers 3/ and by the Federal Geological Survey 4/ suggested that exploration would discover substantial reserves of minable ore in the Contact mine, Grant County, New Mexico. During February and March 1943, a sampling program was conducted on this property under the direction of the author. This was art of a general survey of the manganese districts of southwestern New Mexico.In its program of exploration of mineral deposits, the Bureau of Mines has as its primary objective the more effective utilization of our mineral resources, to the end that they make the greatest possible contribution to national security and economy. It is the policy of the Bureau to publish the facts developed by each exploratory project as practicable soon as after its conclusion. The Branch, Lowell B. Moon, chief, conducts preliminary examinations, performs the actual exploratory work, and prepares the final report. The Metallurgical Branch, R. G. Knickerbocer, chief, analyzes samples and performs beneficiation tests. Both these branches are under the supervision of Dr. S. Dean, assistant director. With respect to this report, special acknowledgment is due to C. H. Johnson, district engineer for New Mexico, and E. D. Gardner, regional engineer, and his staff, of the Central Region of the Bureau of Mines at the time of this work in 1943; also to Kenworthy, metallurgist at the Rolla. A novel method of extinguishing a mine fire in a stowed or gob-filled area by employing liquid carbon dioxide under high pressure eras used recently with success at the President (Prasident) coal mine, Bochum, Germany, as reported by P. Cabolet in Gluckauf (Apr. 24, 1937, p. 393) The use of carbon dioxide gas as a fire-fighting agent in surface plants and in ships is now general practice in the United. States, but when it has been tried for extinguishing fires in mines in large areas more or less confined by stoppings it has not been successful. Examples of the latter method of attempting to fight mine fires are given in Bureau of Mines Bulletin 229, Fifty-Nine Coal-Mine Fires; How They Were Fought and That They Teach, by G. S. Rice, J. VT. Paul, and 14. VT. von Bernewitz (1927). The reason for its nonsuccess may be either that the stoppings are not tight or that the roof and pillars are more or less fissured and enough air passes into the area to sustain a slow fire, whether of spontaneous origin or produced by gas explosion or other source of ignition.

From the most remote periods of civilization stone has been used to perpetuate the memory of individuals or to immortalize their noble achievements. Ancient memorials ranged from simple piles of stone (cairns), or single markers, to great obelisks and pyramids or magnificent mausoleums. The Taj Mahal at Agra, India, built of white marble by Shah Jehan in memory of the Empress Mumtaz Mahal, is one of the most beautiful and costly memorials ever built. It was erected between 1632 and 1650 at an estimated cost of $50,000,000 or more and is today an object of unusual interest for travelers. Other magnificent memorials both ancient and modern are to be found in many lands. No sharp line can be drawn between buildings and monuments, because the larger types of memorials such as the Bok Singing Tower, Lincoln and Jefferson Memorials, and mausoleums of various types have features in common with buildings. However, great care must be exercised in selecting a site for a quarry that will produce acceptable monumental stone because appearance as well as quality is of prime importance. The deposit must be of uniform texture and attractive color and must be notably free of knots, hairlines, impurities, incipient seams, or other defects. Many memorials are of polished stone, and spots or streaks that might be virtually Invisible on a natural rock face will appear distinctly on a polished surface. Hence, stone to be polished must be selected with unusual care. However, less flawless stone may be used for hammered surfaces.

Tunnel-driving is recognized as a hazardous occupation; adequate support of roof and sides for preventing accidents from falling material is usually one of the most important safety problems. Where extraordinary conditions of sand and water are found, this problem becomes even more difficult. Tunneling at the Tioga No.2 mine was stopped when a 22-foot bed of water-saturated, fine gray sand was encountered. The tunnel, designed to house an ore-conveyor belt, was being driven from both ends. Driving was extremely difficult at both the upper and lower faces when the sand horizon was reached. A distance of 178 feet remained between the faces. Heavy pressures deformed the steel-supporting rings at the upper face, and there was an inrush of mud and sand at the lower face.

The concept of data availability must be addressed, because there are perceptible changes from year to year, and the availability of accurately reported, precisely defined statistics and nonstatistical information provides the only sound basis for analytical studies required for government policy planning or industry market analysis. As the countries of Eastern Europe and the former U.S.S.R. undergo the transition from centrally planned economies to market economies, there has been a perceptible increase in information availability for rather obvious reasons. Any industrial operation seeking economic assistance from Government and/or private sources outside the country in which the facility is sited must provide some appraisal of its operations at the time it seeks assistance, as well as some information on previous performance to serve as tools in evaluating its potential under the proposed development scheme for which it seeks financial assistance. Additionally, there was growing governmental recognition that concealment of much of the national mineral production, trade, and consumption data for purposes of national military and/or economic security probably never was very successful, never was really necessary for most commodities, and certainly proved unnecessary with the end of the so-called cold war. This report describes three cases of carbon monoxide (CO) poisoning in a manhole, including one fatality, from CO migrating through soil after nearby use of explosives. A municipal sewer project involved installation of new pipes and manholes. Work of the general construction contractor was interrupted when a subcontractor detonated 265 pounds of nitroglycerin-based explosive 40-60 feet south of the manhole to break up underlying rock. A construction worker who descended into the manhole 45 minutes after the explosion collapsed within minutes, and two co-workers descended into the manhole to rescue him. One rescuer retrieved the unconscious worker, and the other rescuer died in the manhole. All workers had elevated carboxyhemoglobin levels. In the subsequent NIOSH investigation, air monitoring was conducted with real-time instruments, and air samples were collected in Tedlar bags. Laboratory analyses of the bag samples collected near the bottom showed 1905 parts per million (ppm) CO, 19.5% oxygen, and 3% carbon dioxide. Direct reading instruments showed progressively higher concentrations as the sensor was lowered into the manhole. Subsequent chamber tests on sample explosive yielded 27 liters CO per kilogram detonated. Based on this value, the surface blast at the construction site may have produced about 3,250 liters (114.8 cubic feet) CO. The CO in this incident most likely was released from the nearby explosion and migrated through soil and fractured rock into the manhole. The blasting and construction industries should be made aware of this previously unrecognized CO exposure hazard associated with surface blasting. The extent of CO exposure from explosives used in construction is not known, and additional information on the extent of CO exposure must be collected. In addition, confined space entry procedures (including monitoring confined space atmospheres before entry) should be observed; CO monitoring of confined spaces in the presence of blasting can prevent future incidents such as this one.
