Many organisations struggle to yield meaningful outcomes from incident investigations. This paper will demonstrate that we can discover systemic issues by rethinking the ‘Why’ and ‘How’ of investigations, fostering genuine risk reduction. Key points: 1. Shift from ‘prevent recurrence’ to ‘learn and reduce risk’: conventional investigations focus narrowly on recurrence prevention, resulting in narrowly focused investigations with limited ability to identify and reduce organisational risk. Redirecting our objective to learning and improvement opens avenues for broader risk mitigation. 2. Understanding daily work for risk insights: recognise that work is not always completed in full compliance with procedures. Workers will adapt how they complete tasks influenced by the conditions and management systems that exist every day. Understanding the conditions and systems affecting everyday work is vital for uncovering systemic risks. 3. Unearth systemic issues through in-depth exploration: many investigations address surface concerns such as lack of training, incorrect procedures, equipment not being maintained, and missing overarching organisational risks. Systemic, organisation-wide risks are not identified. To discover these systemic issues and create shared learnings, investigators must question management systems, including resourcing, planning and risk management. 4. Design actions for safe failure: common actions like discipline and training fall short and do not reduce risk. Resilience emerges from well-designed controls. Incorporate planning, better resources, interlocks, and guarding to bridge the gap between task execution and risk. 5. Investigation Reports as a Business Case: Investigation reports should not simply state historical information regarding an incident. They are to facilitate decision-making and be presented as a business case to drive change. Organisations can move beyond localised risk and ineffective administrative actions by changing investigation objectives and methods. They can facilitate learning, understand daily work intricacies, identify systemic issues, and enhance the capacity for safety, driving lasting risk reduction.

Optimisation of Blast Design and Execution must be done right the first time. It is too late after the ore is broken. Consequences of poor design and execution negatively affect productivity from load and haul through the primary crusher to processing and hence reduce mine site profits. Harnessing and utilising ‘big data’ from geology, geometallurgy, drill hole location, powder loading, blast timing, blast movement and fragmentation to close the loop in blast design and optimise the blast increases productivity across the entire value chain from Mine to Mill. As the internet of things and artificial intelligence increases the quantity of data available, being able to capture and utilise this data to advantage is a challenge. Recent advances in software and hardware together with the increased power of cloud computing allows engineers to close the blasting loop to increase mine site productivity. Numerous case studies over the years detail significant gains from implementing the innovative technologies available to Mine to Mill projects. The opportunity is available for all members of the multi-discipline teams at each mine site to actively ensure productivity improvements are realised and maintained not only in their area, or silo of responsibility but across the entire mine site. This paper refers to recent case studies which outline how significant improvements in productivity are available to all mining operations to increase mine site profitability and dividends to shareholders.

Independence Group’s Jaguar Mine treats a polymetallic ore producing a copper concentrate and zinc concentrate. Effective separation of the sulfides requires relatively fine grinding with flotation feed P80 of about 75µm. Zn losses to final tails are greater than 60 per cent and <32 µm. When magnetic conditioning was first installed in 2010 it showed about an 8 per cent reduction of zinc in final tail and on some ores a similar benefit for silver as well. Current testwork is showing about a 12 per cent reduction in zinc and silver in final tail, consistent with the original testwork. Sizing of the final tail during the tests showed that the reduction of zinc and silver losses were only in the <32 µm fraction, as would be expected from an aggregation process. However, the most surprising result was about a 4 per cent reduction in zinc and silver in the plant feed with a >99 per cent statistical certainty. The reduction in zinc and silver in the feed is also only in the <32 µm fraction. Therefore, because the magnetic conditioning of Zn flotation feed cannot impact plant ore feed grade the hypothesis is that the magnetic conditioning is reducing the fine mineral from downstream processes, like filtering and thickening, returning to the plant feed in the recycled process water. This hypothesis is consistent with plant observation of dirty concentrate thickener overflow and measurements of solids in the overflow, as well as the average % Zn grade in cyclone overflow that correlates with the magnetic conditioning ON and OFF days. Many open pit mining operations seek to control blasting-related movement of ore and waste to minimise loss and dilution. Tracking the movement of ore boundaries can result in the maximisation of ore recovery and minimisation of waste sent to the mill, thereby optimising the economic performance of the operation.A novel solution for tracking the ore–waste boundary location before and after a blast uses subsurface markers placed along the boundary with all measurements and scans conducted remotely from the blast block. Upon system activation, user-operated equipment on the surface determines the three-dimensional (3D) location of the subsurface markers.The patented system concept combines two electromagnetic mechanisms, magnetic positioning and radio frequency through-rock communications to enable remote battery-powered markers, buried within the rock to be accurately located in 3D space.Field trials at a quarry have demonstrated the system can comfortably handle over 100 subsurface markers with a complete scan, including set-up, taking less than an hour. Location accuracy for each marker in 3D space is better than 0.5 m at a range up to 50 m from the magnetic source, and the system has an overall operational range in excess of 75 m.CITATION:Blay, K R, Spathis, A T and Leslie, K E, 2015. Achieving occupational health and environmental noise objectives is a major consideration in the planning or operational phase of resource development and industrial projects to meet the needs of the workforce and community and comply with legislative requirements. For material handling systems, ventilation plants, mobile equipment, process plants and fixed infrastructure, noise emissions need to be considered at the design phase. The preferred approach is to provide noise emission technical specifications, but in many cases this is not stipulated. An occupational noise requirement, such as ‘not to exceed 85 dBA at 1 m’, is often provided, but this will not achieve the required occupational or environmental noise objectives and may cause overly stringent requirements when they may not be needed. It should be noted that when developing the technical specification, measuring compliance needs to be taken into account as, in some cases, measuring may be very difficult or even not possible.This paper will discuss the requirements for providing an effective noise specification that ensures the environmental and occupational noise objectives are able to be achieved and that, once installed, compliance can be measured. It discusses planning such as when to set the objective, correlating the appropriate objectives for the environmental and occupational noise requirements, allowing for measurement tolerance, providing a suitable criteria and data format for the suppliers to respond to and methodology to measure compliance. Different phases of a project require different approaches to planning for noise management and these will be discussed in the paper. The hot-dip galvanising industry is the largest consumer of zinc, with over 40 per cent of the world’s total annual production of zinc used for galvanised steel. The dross generated in the hot-dip galvanising coating process is a valuable co-product, since it contains high quantities of recyclable metal. A new method to recover the metal from the industrial galvanising dross was proposed using supergravity separation. During the hot-dip galvanising process, the presence of zinc dross has a serious impact on the surface quality of the hot-dip galvanised sheet, forming zinc dross defects. To reduce the impact of zinc dross, the zinc dross on the surface of the zinc pool needs to be manually or mechanically removed at regular intervals. However, during the dross removal process, a large amount of metal zinc is inevitably carried out, resulting in low zinc utilisation rate and high zinc loss rate. This article proposes an efficient and low-cost technology for the recycling and utilisation of hotdip galvanised dross, which uses super-gravity technology to recover hot-dip galvanised dross online. The metal in the hot-dip galvanised dross returns to the zinc pot in situ, while a small part of the dross remains in the super-gravity equipment, which can reduce the amount of zinc dross by more than 60 per cent and achieve a reduction in zinc loss from 17 per cent to less than 7 per cent.

The cloud transform simulation is a technique used to generate realisations of correlated attributes. The principle of this technique is to partition the scatter diagram between the input attributes into several interval classes and derive for each class a cumulative distribution function (CDF) using only the data pairs of the scatter diagram in that class. The simulation then proceeds like a traditional Monte Carlo simulation but using the derived CDF in each class.The cloud transform simulation has the advantage of being a simple and fast technique that is able to reproduce the uncertainty in the relationship between the two input attributes regardless of whether the relationship is linear or not. However, the technique has three major drawbacks. First, the number of classes has an impact on final simulated results and there is not a robust approach to define such number. Second, simulated results exhibit some saw tooth variations that are unrealistic and third the reproduction of the spatial continuity of the simulated attribute is not guaranteed.This paper presents the use of the quantile regression approach in order to overcome the first two disadvantages of the cloud transform simulation. Details of the technique and how it can be coupled with the cloud transform simulation are presented along with a real case study.

Mine value chain reconciliation (MVC) is a necessary procedure to evaluate a mine’s performance against the assumptions and models used to plan mining and processing operations. Using nomenclature from Parker (2012), F2 reconciliation is the ratio between material received at the mill, compared with material sent to the mill, commonly reported in terms of tonnes, grade and metal. Measurements of material sent to the mill can be estimated in many ways, including: fleet management system (FMS) records, truck counts, or volume reduction. Most time-based reconciliations (for example, month-end reconciliation) use short-term model depletions of in situ rock measured by periodic topographic scans or surveys.

The Mount Isa Mines (MIM) Copper Concentrator has an extensive history of reagent trials, with different reagents coming in and out of favour over the years. The research and use of carbon depressants is particularly prevalent, as carbonaceous pyrite is a major source of concentrate dilution. Years of research led to the use of naphthalene sulphonate due to its excellent carbon depression properties. However, due to rising reagent costs carbon depression ceased in the late 1990s. This led to the loss of knowledge about the use and value of naphthalene sulphonate. Cyanide is used at the Copper Concentrator for the sole purpose of pyrite depression and over the years multiple projects have been devoted to finding alternatives to it. In 2012 the neighbouring MIM Lead-Zinc Concentrator replaced cyanide with naphthalene sulphonate which resulted in cost savings and the elimination of a hazardous reagent. In 2013, plant trials were conducted at the Copper Concentrator in hopes to replicate the outcomes obtained at the Lead-Zinc Concentrator. The results of the trials conducted at the Copper Concentrator showed that naphthalene sulphonate could not replace cyanide and this work was set aside to pursue more promising projects. Further research conducted in 2014 found that mineralogy and a better understanding of the depressing mechanism of naphthalene sulphonate held the key to improved pyrite depression at the Copper Concentrator. The findings showed that Mount Isa copper ore contains two forms of pyrite; euhedral pyrite (cubic pyrite) and framboidal pyrite (carbon rimmed spheres of pyrite), both of which require different reagents to sufficiently depress them. Naphthalene sulphonate was found to be an excellent carbon depressant, suitable for depressing graphite and framboidal pyrite, whilst cyanide specifically targets euhedral pyrite.In 2015 laboratory test work was conducted to examine the use of naphthalene sulphonate in conjunction with cyanide to selectively target both forms of pyrite. This work led to a full plant trial, the results of which showed a significant increase in plant performance, with higher copper concentrate grades and recoveries being achieved. Tailings dam breaches have caused significant life loss, social, economic and environmental damage in recent years. The term "tailings-flow" refers to various forms of tailings outflow movement resulting from the breach of a tailings dam. These flows, comprising a mixture of tailings, water and dam materials, can take different forms, ranging from a debris flood to a flowslide, and can travel long distances and impact large areas. The inundation extents depend on the failure mechanism, tailings dam height, total released volume, sediment concentration (associated with the rheological behaviour of the flow) and downstream conditions (including topography, land cover and presence of a water body). Numerical runout modelling of tailings dam breaches is important for predicting the dynamics and extent of tailings-flows, which are crucial steps in tailings dam consequence analysis and risk management. This paper presents a comparison of numerical simulations of the 1994 Merriespruit tailings dam breach using three numerical models: DAN3D, FLO-2D and MADflow3D. Using compiled site condition information and data from the literature and public domain, model performance was evaluated primarily based on comparisons between the observed and simulated inundation extents. The main purpose of this preliminary model benchmarking exercise was to help us evaluate the capabilities and limitations of each model, and to identify opportunities for future model improvements as part of a broader research initiative known as CanBreach. This exercise also highlights the inherent complexity of tailings-flows and the importance of using judgement to interpret model input and results.

Work health and safety duties around Australia include duties to manage psychosocial health and safety (mental health) of workers engaged by businesses or undertakings and of other persons impacted by the operations of the business or undertaking. Persons conducting a businesses or undertakings must develop systems of work and policies and procedures for ensuring, so far as is reasonably practicable, the safety and health of workers and other persons. Officers have a duty to exercise due diligence and to ensure effective systems are developed, implemented and resourced. The developed systems will only be effective with worker’s co-operation and engagement. The duties placed on workers by work health and safety laws are an important part of the safety matrix of obligations. Supervisors and Leaders are workers and they play a critical role in delivering health and safety outcomes. To deliver these health and safety outcomes with respect to psychosocial safety Supervisors and Leaders need to understand their obligations and have a framework to use to assist them in understanding what workers require and what steps need to be taken to meet their duties. They also need to take steps to assist those workers that they supervise to meet their duties.

Problems caused by water discharge from surface mine ventilation shafts can be significant and are usually unforseen. The discharged water is often hyper-saline or acidic and can cause environmental degradation over a wide radius. It can also lead to recurring power pole fires and primary fan breakdowns. A range of solutions to the water discharge problem has been implemented in many Australian mines. In most cases the solutions have been ad hoc, with the overall cost and effectiveness varying tremendously. This paper reviews and contrasts the practicality and effectiveness of the various available remedial options.

The Otago Goldfield has produced approximately 8 million ounces of alluvial gold and over 2.5 million ounces of hard rock gold and is currently home to New ZealandÆs largest producing gold mine. In the eyes of explorers this production history makes the goldfield a good place to explore.   Modern exploration in the Otago Goldfield commenced in the late 1970s with the exploration for tungsten. However, a rapid rise of the gold price in the early 1980s saw the focus shift to the exploration for gold which lead to the discovery of the Macraes Gold Project in 1984, which since it came into production in 1990 has been New ZealandÆs largest producing gold mine. With the exception of the Macraes Gold Project, exploration continued in the Otago Goldfield for a further 20 years without further success.   In 2006, Glass Earth (NZ) Limited (Glass Earth) reviewed all the exploration technologies applied to the Otago region and available geological data, including the 1996 OceanaGold Helicopter Electro-Magnetic (HEM) data, and concluded that combined magnetics and HEM were able to delineate the Hyde Macraes Shear Zone (HMSZ) which hosts the Macraes Gold Project. Unlike previous explorers who explored the Otago region in a piecemeal manner, Glass Earth opted to collect a new, region-wide dataset which could be integrated with all previous collected geological information to come up with new exploration concepts and targets. As part of this process, Glass Earth applied in mid 2006 for New ZealandÆs largest on-shore gold Prospecting Permit, PP39-322 which was subsequently granted in November 2006. In mid January 2007, Glass Earth commenced acquisition of approximately 50 000 line kilometres of HEM data, making the Otago regional aerial geophysical survey (survey) the largest aerial geophysical survey ever undertaken in New Zealand.   This paper will report on the progress of the Otago Regional aerial geophysical survey and outline some of the preliminary findings from what is still a work in progress.

Multiphase slags form the basis of the reduction pathway for manganese ferroalloy production. The present work aims to understand the flow of slag through the coke bed based on experimentation in controlled conditions. Synthetic slags of different basicities were evaluated for their phase composition, viscosity, and flow. The two-phase slag will have two zones: a liquid slag zone and a multiphase zone. The multiphase zone will consist mostly of a solid phase with some liquid slag between the solid particles. The multiphase part will hence have a high viscosity. The liquid zone will flow into the coke bed while the multiphase area will stay. With increasing reduction, the multiphase area will decrease in size until the solid phase is gone and the whole slag will flow into the coke bed. The effective viscosity of slag will decrease with the lowering of the solid oxide phase. The results from flow experiments confirmed that the segregation of phases occurs when slag flows through the coke bed. It is observed that the flow is dependent both on the size of the void and the viscosity of slag.

Much practical experience has been gathered in the last 30 years of application of high-pressure grinding rolls (HPGR) integrated with ball milling in size reduction of fine iron ore concentrates. The company Vale S.A, in Complexo de Tubarão (Brazil), was one of the pioneers applying the technology prior to pelletisation with an outstanding size reduction energy efficiency in the circuit and a significant increment in the specific surface area of the product. Recent studies by the authors demonstrated benefits of modelling and simulation to improve the performance of HPGRs in this particular application, with the model being able to describe HPGR performance under different operating conditions and under variations of feed size distribution. Despite these important advances, this modelling approach has only been used offline and under steady-state conditions. The present work applies the modified Torres and Casali model proposed by the authors in pseudodynamic simulations. The ability of the model to predict the characteristics of the product in real-time is evaluated using data available online for the pellet feed preparation circuit. Results demonstrated the model capabilities to map the physical operation and give a realistic representation of the process. Additionally, the model is demonstrated to be able to support the pellet feed production by providing extended real-time information of the process, making it a useful tool for improvement of the operational strategies and process stability.

The Cawse nickel-cobalt operation is situated 50 km NW of Kalgoorlie in Western Australia, at latitude30o22'30'' south and longitude 121o08'30'' east on the Kalgoorlie 1:250 000 geological map sheet (SH 51-9) (see Figure 1). The project is owned and operated by Centaur Nickel Pty Ltd (CTR) and was the first in Australia to commercially produce nickel metal and cobalt sulphide through pressure acid leach (PAL) technology. Cawse has a total resource base (all categories of resources + reserves) of 213 million tonnes Mt at 0.7 per cent Ni and 0.04 per cent Co, comprising 127 Mt at Cawse Central which is 100 per cent owned by CTR and 86 Mt at Cawse Extended where CTR has a 80 per cent equity. Cawse Extended is located immediately to the northwest of the Cawse Central leases. All current reserves (proved and probable) of 30 Mt at 1.0 per cent nickel are contained within Cawse Central. In addition the Siberia project area 20 kilometres to the NW has a resource of 63 Mt at 0.7 per cent Ni. Cawse is a dry laterite deposit, with nickel and cobalt hosted by iron and manganese oxides. Cawse Central mineralisation extends over a ten kilometre strike length, with three pits currently being mined. The Bunyip pit has the premier nickel and cobalt grades within Cawse Central. There are seven dominant lithology types forming mappable units within current pit exposures. Four of these units are host to nickel and cobalt mineralisation. Ore types are defined as Upgrade, Grind and Non-upgrade, depending on their physical characteristics with respect to the treatment route. Mining commenced in February 1998. To the end of calender 1999 the operation has produced 2100 tonnes nickel plate and 470 tonnes of cobalt as a cobalt sulphide product. Future production will target 9000 tonnes per annum Ni metal and up to 2000 tonnes per annum Co, based on processing 500 000 tonnes of ore per annum.

Increased environmental regulations and pressures have created changing and new demands for tailings management strategies in a variety of mining applications. Critical among these items is the disposal of tailings as the pursuit of increased yields and lower costs have driven the application of different process technologies. With a number of technologies having a niche application focus, the belt filter press has been utilised for decades as a cost-effective and versatile option for dewatering different types of mineral tailings and other waste process streams. One of the critical advantages of the belt filter press is the technology’s ability to adapt and work in numerous plant designs, operating conditions and applications. As permitting and disposal options have become more stringent, plant designers, operators and equipment suppliers must develop comprehensive and site-specific economic, design and process evaluation techniques to ensure proper equipment selection and system design for belt filter presses or any other dewatering technologies. This paper will explore these techniques in relation to belt filter presses (with reference to other technologies) in a variety of different applications, along with other considerations to ensure a successful tailings dewatering system and disposal design. Summaries of equipment performance characteristics, operating and maintenance strategies and optimisation techniques will also be addressed in conjunction with the belt filter press’s adaptability to different plant designs and the resulting tailings characteristics.CITATION:Fenzel, M S, 2015. On 15 August 1998 the first section of the PUTRA LRT system was opened to the public as a free service prior to official commencement of revenue service on 1 September 1998. It is a remarkable achievement to open a highly advanced medium capacity urban railway scheme in four years after the outline concept began. Even more remarkable is the implementation of the next stage which is the underground section planned to be operational in mid-1999. The decision to build the first underground railway in any major urban centre is a technical challenge. The PUTRA LRT System can be likened to Hong Kong when the initial MTR system began in the 1970s and Singapore MRT in the 1980s. Prior to the PUTRA railway very little bored tunnelling had been undertaken in Kuala Lumpur. A few shallow sewer tunnels had been built but the main experience of tunnelling in Malaysia had been for water, hydroelectric and road tunnels usually through granite formations. The ground conditions in Kuala Lumpur were known mainly from experience of piled foundations and occasional deep excavations. The suitability and likely difficulties to be experienced during bored tunnelling could only be theoretically predicted. In both Hong Kong and Singapore, government owned corporations were set up to implement the scheme, but in Kuala Lumpur the whole project has been undertaken by a private company PUTRA, a wholly owned subsidiary of Renong Berhad.
