Health and safety indicators help mine sites predict the likelihood of an event, advance initiatives to control risks, and track progress. Although useful to encourage individuals within the mining companies to work together to identify such indicators, executing risk assessments comes with challenges. Specifically, varying or inaccurate perceptions of risk, in addition to trust and buy-in of a risk management system, contribute to inconsistent levels of participation in risk programs. This paper focuses on one trona mine’s experience in the development and implementation of a field-level risk assessment program to help its organization understand and manage risk to an acceptable level. Through a transformational process of ongoing leadership development, support and communication, Solvay Green River fostered a culture grounded in risk assessment, safety interactions and hazard correction. The application of consistent risk assessment tools was critical to create a participatory workforce that not only talks about safety but actively identifies factors that contribute to hazards and potential incidents. In this paper, reflecting on the mine’s previous process of risk-assessment implementation provides examples of likely barriers that sites may encounter when trying to document and manage risks, as well as a variety of mini case examples that showcase how the organization worked through these barriers to facilitate the identification of leading indicators to ultimately reduce incidents.

Diagnostic: tests were performed on a production ST-5A Load-Haul-Dump machine manufactured by Wagner Mining Machinery Company to evaluate and rank order noise sources. The diagnostic evaluation indicated that the major noise sources were the transmission, the engine, and the cooling fan. This phase was followed by implementation of recommended noise control treatments, which included vibration isolation of the transmission, a transmission enclosure, an engine enclosure, and modifications of the inlet to the cooling fan. The final step in the program was an underground operational period which began on 31 August 1976 at the National Gypsum Mine in Shoals, Indiana. The noise control treatments successfully reduced the LHD operator noise exposure by 7 dB(A), thus increasing its allowable operation time by a factor of 2.7. Actual measurements made underground using noise dosimeters have indicated that, during, a typical shift, the noise control treatments have reduced the fractional exposure to an allowable level. The report discusses the mechanical aspects of the LHD, its noise sources, the treatments applied, the final levels achieved, and field experience with the noise-controlled machine.

Quartz veins from the Martha Hill epithermal gold-silver deposit, Waihi, commonly exhibit crustiform banded textures. Visual differences observed between bands are primarily the result of a 1 to 1500 ¦m variation in quartz grain size. Quartz band textures are dominated by anhedral interlocking quartz mosaics, commonly associated with minor adularia and sulphide phases; these textures are interpreted to be products of amorphous silica crystallisation. The occurrence of rhombic adularia suggests that prolonged boiling influenced parent fluids. In contrast, the presence of coarsely crystalline bands, made up of subhedral to euhedral quartz grains, infers primary crystal growth from solution; evidenced by the occurrence of growth-zoned quartz within these bands. There is a narrow range of 3.3ë in d18Oquartz between maximum and minimum isotopic values for the deposit. Significant overlap in d18Oquartz values are observed between the Martha and Welcome lodes. However, statistically significant and commonly systematic differences do occur between individual bands analysed in most samples. Temperature fluctuations may be the dominant factor responsible for observed inter-band variation.

"Often mineralogy dictates the physical properties of a material rather than chemistry. This in turn affects how that material behaves in a process plant, and whether optimum conditions for refining one mineral are different to those for refining another. For example how does refining copper from chalcopyrite differ to refining copper from bornite?Minerals are naturally occurring inorganic solids that have a definite chemical composition and an ordered atomic (crystalline) structure. X-ray diffraction is a common and highly developed method to identify and quantify minerals. X-rays are fired onto a material and the mineral is identified by the angles where the diffracted X-rays are in phase.Most X-ray analyzers are laboratory based. A sample is prepared and analyzed by a detector that travels in an arc to collect the diffraction pattern. COSMA has a curved detector which picks up the complete x-ray pattern instantaneously. Therefore, material is collected and transported through the machine where is it is analyzed continuously.COSMA measures about 0.5 tons of material per day, while laboratory analyzers measure a few grams at a time. Real time data trends are produced allowing for improved process evaluation and automatic control. The COSMA on-line system has taken X-ray diffraction out of the laboratory into the field.There are a number of systems in operation in cement production. It is recognized that on-line mineral analysis has a role in many other processes. This paper presents the technology and invites further discussion should the technology be beneficial to members of the audience.INTRODUCTIONThis article describes COSMA, a system that provides direct and continuous on-line mineral analysis using X-ray diffraction in the field. While X-ray diffraction is a common technique used in mineral analysis, the time from sampling to analysis can be hours and plant operators typically do not get the data in time to control a process or respond to an upset. COSMA provides rapid measurement, analysis and feedback of data, in real time. This provides powerful information that can be used to optimize operation, thereby reducing costs and increasing quality."

I. INTRODUCTION. WHEN so much has been written upon a subject on which so little definite information is available as upon this, some reason must needs be assigned for adding to the volume of literature. A sufficient reason is found in the probable great future importance of China as a producer of mineral wealth. The present undeveloped state of mining in the Empire is due to many causes, among which the most important are the relatively simple needs of the population, the lack of transportation-facilities, the inelastic regulations governing the industry, and the superstitious reluctance of the people to make excavations which might disturb the spirits of the earth and air, or of ancestors. Iron is the metal most in demand, yet the needs of the population, until recently, only made necessary a production of approximately 0.5 lb. of iron per capita per year. The present annual production of iron for domestic consumption, in the United States, is nearly 600 lb. per capita. The curious state of development of transportation in this interesting country has been a greater handicap upon the mining industry than upon any other. The point upon which it hinges is the absence of a road-system. Except for a few military roads, now almost impassable, there are no roads in China; that is to say, there is no land which is set aside as a right of way, belonging to the commonwealth. Throughout the agricultural districts, generally speaking, the entire area is in private ownership, and the lines of travel are between fields. This report is volume 10 of an eleven-volume final report for a Grant completed by The Pennsylvania State University for the Bureau of Mines to develop a dynamic general purpose computer simulation model for underground coal mining. The simulator developed under the Grant has a wide range of possible applications in the design and evaluation of underground coal mining systems from the standpoints of health and safety, productivity, and mine environment. The Master Design Simulator consists of nine free-standing function-oriented subsystem models linked together into an integrated, comprehensive model of an entire mining system. The subsystems comprising the Master Design Simulator include geology and reserves, methane generation, roof support, subsidence, production, rail haulage, cost, water generation, and ventilation. This volume of the final report describes the Ventilation subsystem of the Master Design Simulator. The Ventilation subsystem has the capabilities to handle: (1) branches in parallel or series, (2) fans in parallel or series, (3) multiple fans, (4) methane, compressed air, or other injections into the system, (5) leakages and fixed quantities, and (6) calculations of absolute pressure at nodes. The program uses the Kirchoff's Junction and mesh laws, and the Hardy-Cross iterative technique to converge to the desired accuracy in solving fox the head quantity. A case study is included in the report illustrating the use of the model. The appendices of the report constitute a user's manual for the Ventilation subsystem, and include a program listing.

The common approach of designing blasts on a trial and error basis is quickly coming to an end. When utilizing the full scale blast environment, trial and error can quickly become cost prohibitive and leaves much to be desired in terms of true blast diagnostics. In these situations testing is usually only continued until a perceived improvement is achieved and when the gross undesirables in blast results are removed. This does not preclude, however, that further improvements of up to 100% or more can not be obtained.

The following review covers published information on pressing of metal powders at room temperature. Only those operations are considered which occur between the time the powder is filled into the cavity and the compact is ejected from the die. Thus, this review does not concern hot pressing, coining or sizing feed or flow problems, commercial presses nor designs. It was also necessary to disregard to a large extent the effect of pressing on subsequent operations and on properties after sintering. 'The pressing operation is considered independently in its function to compact loose metal powder into useful shapes which can subsequently be handled. There are some comments by the writer in connection with a few of the findings quoted in the literature in order to stimulate thought and discussion. The selection of references quoted in the text is in some cases arbitrary although an attempt was made to favor papers containing novel ideas, techniques or results which mag stimulate future research. In order to enable the reader to check further into the subjects mentioned, the original, rather extensive, bibliography is appended even though not all the references are touched on in the printed version of this review. The equipment used by various workers, for example a die design created by Bridgman,22 is very interesting. 'The die insert has a conical external surface. The vertical pressure exerted on the punch transmits itself to the die which in turn is pushed harder into its tapered retainer. Thus, the higher the pressure exerted on the punch, the higher also the pressure exerted inwards on the die inserts. Inasmuch as this pressure counteracts the internal pressure from the compact the two stresses balance each other so that it is possible to obtain tremendous pressures without breaking the die. The die collar was made from an oil hardening tool steel, heat treated to a tensile strength of some 300,000 psi. The die was closed at the bottom by a plug made from special tool steels. Bridgman found that there is no steel which will withstand a com-pressive stress of 700,000 psi without continuous slow flow or immediate fracture. Therefore, the only material suitable for use in the pressing punch was sintered carbide. In view of the ingenious design it was possible to use special tool steel as die material. Jones22 raises the question whether this method of pressing has practical application in powder metallurgy. Although these tremendous pressures cannot readily be utilized without many special precautions, the tapered die and retainer idea is believed to be useful even for lower pressures. In response to recent mining disasters and new mine rescue team legislation, NIOSH researchers conducted meetings across the United States with mine emergency response experts to investigate current needs and issues. Some of the issues include emergency response preparedness, mine rescue contests, real-life training capabilities, and training facilities. Many new teams are being formed that must be trained for mine rescue team competitions as well as be ready to respond to a variety of mine emergencies including a fire or explosion, a massive roof collapse, mine inundations, or vertical shaft rescue situations. Therefore, it is important that these teams are adequately prepared and trained. This paper presents a summary of domestic and international coal mine rescue training facilities and identifies those that provide unique, real-life, and/or state-of-the-art training. Research findings from this report will be used in further NIOSH investigations to improve coal mine rescue training. A 21st Century worker is one who works less with his physical power and more with mental ability. Such a worker is more skilled than a labourer and is consequently rewarded accordingly. A21st Century supervisor is one who ensures that workers are constantly supported so that they can maximize safety and productivity. A 21st Century manager is one who constantly searches for ways to maximize returns for the various stakeholders. Given the well-known constraints in narrow reef mining, all of the above is meaningless unless a mining method is found which provides conditions which support the 21st Century initiatives. [ ] The identified mining method is a continuous non-explosive stope breaking system using rocksplitter technology. Rocksplitting is defined as: The fragmentation of rock through high energy pulse wave transmission in a fluid-filled hole or hydro fracturing. Shotholes are effectively filled with a water-based geland 30 grams of explosive after which the hole is closed off with a ?rocksplitting gun?.

Fletcher Challenge Limited through its subsidiary Grampian Mining Co Limited has carried out an ore evaluation of one of New Zealand's largest known deposits of ilmenite located at Barrytown on the West Coast of the South Island, New Zealand. The ilmenite is located in a beach sand environment which has previously been mined for gold. Two analytical techniques were developed to quantify the resource. Considerable difficulty was experienced in this phase of the evaluation due to the presence of major inclusions within the ilmenite grains. Wet and dry concentration circuits, utilising spiral, wet magnet, high tension and plate electrostatic separators have been designed to produce ilmenite from the ore. Studies to utilise the titanium content of the Barrytown ilmenite have been made. Recent work has been directed towards a titanium dioxide pigment process.

"In early 2000, CANMET-MMSL was engaged by the Ontario Ministry of Labour (OMOL) to conduct an international study of the practices, in several key mining jurisdictions, of determining the criteria that are used to decide when the wire ropes used in mine hoisting are to be retired from service. Subsequently, the author visited key officials and institutions in Canada, the United States, the Republic of South Africa, the United Kingdom, Germany, and Poland. Material from the Australian province of New South Wales and Hungary was also included.The report that resulted included, among other sections: (1) the history of wire ropes and trends in manufacturing technology; (2) a review of mine hoisting practices; (3) the requirements regarding safety factors; (4) a review of the various operations and their potential future development; (5) the criteria for rope replacement; (6) potential future developments that could take place in both hoisting and rope inspection practices in Canada; and (7) recommendations.Subsequently, the report was issued by OMOL, and as a CD-ROM by CANMETMMSL. In this paper the author presents the key findings and the recommendations made in the study."

Within the famous Harrods department store, a new lift shaft has been constructed, servicing a depth of 22m below ground level as well as the existing 6 storeys of the store. The shaft will connect with a new goods tunnel which is to be excavated beneath the shop, as well as new floor space above ground level which has resulted from the infilling of a previously existing lightwell within the store. The construction of the deep shaft while maintaining uninterrupted use of the store by the Client led to a number of innovative solutions being undertaken. Empirical assessments were made of settlements anticipated from the construction process, which took place within a congested environment of existing foundations. Various elements were designed and installed to mitigate the effects of the inevitable ground movement on the surrounding overlying structure, which supported sensitive high quality finishes and mechanical and electrical installations. The reinforced concrete lining of the shaft was contained within a cofferdam of minipiles. The minipile placement accuracy (1 in 150) was vital as it could not interfere with the subsequent tunnelling works. As the scheme progressed this became even more critical since the minipiles were needed as support for parts of the permanent works. This paper describes:- the high quality placement and control of the minipiled cofferdam, the methods employed in assessing ground movements and the techniques used to mitigate against the effects of these movements.

The U.S. Bureau of Mines investigated the Sheep Creek prospect in the north-central Alaska Range to evaluate the Pb, Zn, Ag, and Sn resources. The studies included field investigations, mineralogical characterization, and concentration tests. Mineralization at Sheep Creek consists of disseminated, banded, and massive pyrite, sphalerite, and galena. Tin occurs as grains of cassiterite (Sn02) in the quartz-mica matrix and within pyrite and sphalerite grains, and Ag occurs in solid solution within the galena. Three bulk surface samples and two sets of core samples were collected at the Sheep Creek prospect for this study. Gravity concentration yielded products with up to 30 pct Sn, and the best result was a product that recovered 52 pct of the Sn at a grade of 22 pct Sn. Bulk sulfide flotation yielded products that recovered up to 89 pct of the Zn, 73 pct of the Pb, and 75 pct of the Ag, but grades were relatively low; selective flotation would significantly increase the grades.

Installation of initial tunnel support is often the most uncertain element of work in the design of, and a contractor bidding on, construction of a rock tunnel. The risks the construction contractor and owner assume are delineated to a great extent in the technical specifications portion of the contract documents. It is intended that this paper identify current practices for specifying initial tunnel support and the impacts each case has on the assignment of risks associated with providing the initial tunnel support. Examples of recently completed tunnel designs are given.

Generating short- and long-term mine production schedules in isolation and independently from each other means that only a local optimum can be achieved when each scheduling phase is carried out. The globally optimal solution however, can be achieved when integrating scheduling phases and accounting for the interaction between short-term and long-term activities simultaneously. This paper addresses the task of integrating short- and long-term production plans by combining the short-term objective of minimising deviation to targeted mill feed grade with the long-term objective of maximising net present value (NPV) into a single mathematical model. A case study is presented which compares the present values of the same stope data set scheduled via separate long and short-term models and than by the proposed integrated model which applies a predetermined penalty for each tonne of metal deviation to target for the respective ore tonnage being processed. This allows an operation to obtain a globally optimal scheduling regime when taking into consideration the cost and loss of value resulting from fluctuations in grade to the mill over the short-term.
