Serious Injury and Fatality (SIF) prevention remains a priority focus within high-risk industries sector. A major mining operation sought to strengthen their approach to the management of high-risk activities through an in-depth review of all historical incidents that had serious injury or fatality potential (SIFp) events (sometimes known as hi-potential events). The analysis found that the organisation invested a disproportionate level of resources, time and effort on low consequence incidents with a high proportion of SIFp incidents going under the radar. It was found that 76 per cent of SIFp incidents were rated in moderate or lower risk categories and conversely, almost half of those incidents with a ‘serious risk’ rating had little or no potential for a serious outcome. As a result of this project, the operation engaged in a renewed focus on critical risk management and enhanced its operating cadence in critical risk for the workforce. This included bow tie analysis, an update to critical risk standards and increased workforce engagement in the use of critical control verifications in the field. Data analytics with lead and lag indicators continue to provide senior leadership team and risk owners with an understanding of exposure for SIFp and control effectiveness. The audience will take away learnings from a real-world case study that provides data-driven insights of the human, local and organisational factors contributing to incidents with SIF potential. In addition, participants will understand the steps taken to categorise and analyse this incident data and the initiatives currently being implemented by the organisation as a result.

In the humid, subtropical climatic conditions, typical of Papua New Guinean high-lands, auriferous carbonate-quartz lodes and stockwork veins of the Upper Ridges mine at Wau, have weathered in situ forming manganese dioxide-quartz wad flatmakes and veins. Vein assemblages and sulphidized host breccia have been chemically weathered to 20-25m depth, and microscopic grains of electrum (550-600 fine) formerly enclosed in carbonate, sulphide and quartz have dissolved in the oxidizing alkaline fluid. Coarse secondary electrum, crystalline and dendritic, is concentrated by manganese dioxide and at the base of chemical weathering. Fineness is unchanged, eliminating the possibility of gold and silver migration as a chloride complex. Experimental tests on the stability of thiosulphate, and its ability to dissolve gold and silver from colloid, crystal and synthetic alloy confirm that secondary electrum at Upper Ridges is most compatible with precipitation from (Au,Ag)(S203)23-; a complex readily destabilized in the presence of MnO., or with decreasing Eh.

The metallurgical works of the Electrolytic Zinc Company is situated on the Derwent Estuary, approximately five miles north of the city of Hobart and near a relatively dense housing area. Zinc concentrates, consisting chiefly of zinc and sulphur, have been roasted at Risdon for 53 years;- recently at rates of 1, 100 tons per day. The zinc content of the ores is converted to zinc calcine (ZnO), which is a dusty material. The calcine is leached with acid to form zinc sulphate, which is then electrolysed to make zinc. The sulphur is oxidised during roasting to sulphur dioxide, part of which is converted to sulphuric acid and part is dissolved in estuarine water which is then discharged to the Derwent Estuary. Dilution is associated with considerable direct and indirect costs in underground open stoping operations. As shown in previous works, an improved ability to predict dilution enables the economic risks associated with unplanned dilution to be reduced. Depending on the width of the orebody and depth of mining, the economic performance of an open stoping operation can become even more sensitive to dilution. It is shown in several well-documented studies that as little as 1 per cent of additional dilution can impose millions of dollars of lost profits per annum. Optimisation of a stoping operation is therefore largely dependent on the ability of the operation to minimise dilution. This can only be achieved through sensible analysis of past and future performance of mined stopes using the most appropriate recent available technologies and tools. This article presents a new design process that utilises advanced geotechnical numerical modelling and various other analytical tools to assist with a more robust mine design of open stoping operations. The focus of this design process is on geotechnical factors affecting dilution during the life-of-mine. A recent application of this optimisation method for a mining operation is presented in this paper as a case study. Through the application of this method and various other operational improvements, the mine was able to deliver more consistent production and lower operating costs over its life-ofmine outlook and was able to reduce dilution by up to 50 per cent and achieve sustainable record high production rates.

"The current economic challenges faced by the mining industry led to a review of options for reducing the cost of ventilation control devices (VCDs). The VCDs at the Mount Isa Mines site operated by Glencore in Queensland typically experienced significant damage on the operating levels and required expensive replacements. Ventilation controls can become damaged by the effects of stope firings. This paper discusses the nature of blast wave effects, giving consideration to distance from the firing and changes in mine geometry. The time taken to adjust the airflow on the operating levels with the current VCDs resulted in the loss of productivity opportunities. The mine site engaged an equipment supplier to collaborate on the development of a high-strength regulator to control the required airflows within its underground workings.The strengthened regulator proved to be cost-effective and provided additional benefits, including ease of use, simplicity of installation and speed of change in the control of available ventilating airflow. An added advantage of the re-engineered regulator compared to contemporary VCDs is its flexibility, which includes the ability to easily move mobile equipment into different ventilation districts where required without the need to remove the VCD from its location. Permanent rock bolts used to support rock tunnels are required to perform for the design life of the structure as a minimum. Quality of installation is critical to the performance over the life of the rock bolt.This paper will present the findings from an in investigation into the integrity of rock bolts installed in an underground hydroelectric power station cavern and access tunnel. The 25?mm diameter rock bolts were installed 50 years ago, tensioned and post cement grouted. Due to a major machine upgrade the whole facility was subject to an asset survey. The owners are responsible for managing the risks associated with the failure of these rock bolts and ensuring the structural integrity of the tunnels and caverns is maintained to continue operations, protect machinery, assure production and more importantly maintain a safe working environment for all employees and visitors.To help manage risk a proactive approach is desired. To this end a test schedule using a non-destructive testing (NDT) system was carried out by Independent Integrity Testing Pty Ltd (i2t). The system used in this application was RokTel™. The system operates by applying a small axial impact to the rock bolt end and measuring the resultant acceleration response. The rock bolt characteristics affect the acceleration response.The data output from the system is analysed principally using Fourier Analysis (FFT) and sinusoidal waves. The energy generated in the bar, the rate of damping and changes in frequency are all assessed. Testing was carried out on 70 rock bolts within the tunnel and cavern. A selection were then over cored, photographed and documented. The data gathered from the testing is then correlated with physical evidence to establish a calibration for the remainder of the rock bolts and assess if this is a viable method for identifying defective elements, and what degree of accuracy can be achieved. In 1983 the New South Wales (NSW) Government began to construct a 35 km freight rail line to provide additional rail capacity to Port Kembla in the Illawarra region, south of Sydney, through a direct connection between the Unanderra to Moss Vale line at Dombarton to the Main South rail line at Maldon in the Southern Highlands. The project area lies within the carefully managed Sydney Water catchment and is also underlain by previous coal mining activities.Construction of the line halted in 1988 prior to its completion after the NSW Government reassessed the demand case and found that the project was not as viable as originally forecast. However, by this stage the line was partially constructed, including the majority of civil earthworks, approach structures to the main river crossings and establishing the portals for the Avon tunnel as a precursor to major tunnelling works.In 2012, the Commonwealth Government funded Transport for NSW to undertake the necessary engineering and environmental preconstruction works. Most importantly here, the 1980s rail link was predicated upon the use of electrically powered locomotives, whereas the locomotive fleet in operation today in NSW is diesel powered. This fact has a significant impact upon the approach and development of the Avon tunnel section, as the diesel powered locomotives combined with the length and grade of the tunnel alignment produces a unique set of conditions requiring specialist engineering design and systems to ensure safe and functional operation of the railway through the tunnel section.Given the above, the Avon tunnel will need to be supported by a ventilation and smoke control design based on the heat rejection from the locomotive consist, consideration of ambient air quality compliance, comparison against in-tunnel air quality goals, and providing safety systems in line with the fire and life safety strategy it requires.This paper explores the current development of both the tunnel civil works responding to the geography, geology, rail functional requirements, as well as the tunnel ventilation systems being revised to suit the diesel locomotives.Further, this paper describes the importance of combining the design outcomes with the procurement methodology in order to achieve the project objectives, a functional outcome and value for money across the project life.CITATION:Baker, T and Bagis, A, 2014. AngloGold Ashanti Australia Ltd operates an underground mine below a large open pit at the Sunrise Dam Gold Mine near Laverton in Western Australia. Operations are based on an extensive mineralised zone which has very complex geometry, has a high ‘nugget effect’, and is difficult to interpret and model. Initial underground ore was produced from relatively narrow orebodies dipping from 15º - 70º. Open stopes aimed to selectively extract high-grade ore, using methods including jumbo stripping and longhole drilling and blasting. The complex geology makes it difficult to accurately locate development and stopes in the positions required for profitable production. In narrow stopes, rock mass damage and overbreak dilute ore grades and increase the cost per ounce of gold poured. In flat orebodies, several different techniques have been used to recover blasted ore but overall productivity is low. Thus selective mining can also be slow, costly and recovers only a portion of the available mineral resources. Large longhole open stopes with cemented paste backfill are used to extract multiple zones of mineralisation from the Cosmo and GQ orebodies. Bulk emulsion explosives and electronic detonators are used in all large stope blasts to reduce the risk of misfires and to accurately control blasthole initiation sequences and delay timing. Larger stopes improve productivity and significantly reduce direct mining costs, enabling the use of lower economic cut-off grades which increase the overall recovery of mineral resources. This paper reviews the evolution of open stoping at Sunrise Dam, the various mining methods and drilling and blasting practices used, the results achieved, and the lessons learned.

This case study reviews the Charters Towers vein systems for potential open-pittable bulk tonnage resources in' stockworks, horsetails or multiple vein systems. The review summarises district evaluation undertaken over the last 18. months before specific drill evaluations.Historical mining from Charters Towers includes in excess of six million ounces at head grades of one ounce per ton. A review and structural studies show most veins as simple, tight systems without significant horsetails or multiple sheeted systems. In addition the normal gold content of the pyritic selvages ('formation') is low and disseminated gold is virtually absent. Shoots are structurally controlled on vein/vein and vein/contact intersections. Historically high production vein systems are hosted by .,.t o of the younger granodiorite phases.Geometrically most veins dip shallowly (from 45¦ to as low as 200) in a north or easterly quadrant. Flat dips permit open cut exposure of considerable vein area at a favourable stripping ratio.

To effectively plan for closure, we must have an end-point in mind. To undertake meaningful rehabilitation, a post-mine land use (or uses) must be known. However, it is becoming more apparent that the end-point or “goal-posts” are shifting - and may continue to do so throughout the life of a mine. As well as being safe, stable and not causing environmental harm, post-mine land uses should always be sustainable, achievable within the given land capability, and they should involve stakeholders in their design and development.The majority of mines in Australia, and around the World, were historically expected to return disturbed land to either native ecosystem or agriculture/forestry practices. However, examples of highly successful final land uses suggest that today’s focus on closure includes not only minimising future liability and impact but also – and importantly – a move towards the creation of economic opportunity (Murphy and Heyes, 2016).CITATION:Durbin, J, 2018. In the mining industry, crusher-related incidents have led to 15 reported deaths in Australia, New Zealand, and the USA between 1998 and 2007 (NSW Government Resources Regulator, 2024). The majority of these fatalities occurred during routine crusher maintenance tasks. A survey of quarry and mine site maintenance staff was completed to ascertain what they thought of such safety device to understand if beneficial and if so reasons if unused. Overall, findings show, staff perceived risks differently, cost, familiarity, device awareness and not front of mind, influenced adoption. This underscores the importance of asking the right questions involving maintenance and safety staff to lead to more proactive risk assessment.

One of the regular tasks of geotechnical and mining engineers is the measurement and management of deformation in underground excavations. Although several methods are used to do so, the incumbent methodologies for monitoring deformation and ground support serviceability are limited to the use of visual observations or measurements at discrete points, resulting in a qualitative assessment only. The introduction of LiDAR technology allows a quantitative assessment across the entire excavation volume and its rock surface, by collecting a full three-dimensional image of the entirety of an excavation such as an underground drive, decline, or a tunnel. Comparisons between epochs of complete 3D data coverage allows for change detection over time that doesn’t feature otherwise typical omissions. The implementation of regular scanning has shown to be highly advantageous for mines with swelling or squeezing ground, but also at mines with rapid deformation. Despite the LiDAR potential, its adoption has been slow. A key reason for that is that working with point clouds is foreign to most geotechnical engineers. Point clouds have long been the realm of surveyors, but to fully adopt their use in geomechanics requires retraining, upskilling, and devotion by the individuals and the company. Sites are already stretched, so often there are insufficient resources to be applied to bringing the new technology into use. The solution to this hurdle is to partner with subject matter experts to achieve the desired high-quality outcomes without the commitment of time by geotechnical personnel. This paper presents a new solution of fully automated raw LiDAR point cloud data processing that not only enables geotechnical engineers to avoid having to learn unrelated skills by allowing for deformation monitoring without associated training or expertise, but it also provides instant reporting results as well as a sophisticated database with features otherwise not available. All data processing time and effort is completely replaced by an automated process which at the same time opens up the opportunity to monitor many more excavation volumes at a higher frequency than is possible with conventional tools and methodology.

The primary mine planning focus during early study phases of future mining projects is Life of Asset ore production and grade quality, this will always be the situation. While waste schedule optimisation is generally considered in the latter study phases nearing execution for a limited number of schedule periods only and not a true optimisation considering all attributes such as waste destination location, design, cycle time and closure. This can be particularly applicable in large multi-pit bulk mining operations where one approach to meet Life of Asset ore tonnes and grade targets has been to maintain pit development flexibility resulting in large open pit areas and substantial expit Overburden Storage Area (OSA). Seam gases are the most important atmos- phere contaminants in Australian collieries, their importance increasing with depth; C02 as well as CH4 may be important in intruded envir- onments. The generally high rank of the coal, with low moisture content, makes for abnormal dust production. The other contaminants are introduced and subject to better control, in- cluding products of spontaneous combustion. Due to market situations the generally lower rank and higher sulphur coals more prone to self heating are mined less. Future mining at greater depths should see greater problems with atmosphere contaminants.

Paste fill has been used successfully at Henty Gold Mine for the past five years. It has allowed 100 per cent extraction of the orebody, stabilised poor ground and enabled wide sections and offsets to be mined safely. However, there are a number of inefficiencies in the current paste fill system. These include the low output from the drum filter, the addition of aggregate to the paste to compensate and the trucking of paste from the plant to the portal. These inefficiencies have increased the operating cost of the paste fill system and caused a number of operational challenges such as blockages and increased pipe wear. These challenges have been partly overcome by taking ownership of the paste fill process, improved quality control and better communication. Henty is currently developing two new orebodies, the Mt Julia and the Darwin and has an opportunity to build an efficient paste fill operation. This is likely to be a high density paste system without aggregate, either located above the new orebodies or located at the plant and pumped to the new site.

The gravity-induced stirred mill such as the TowerMill is an energy-efficient grinding solution used in secondary, tertiary, and regrinding duties in the mineral processing concentrator. The interest in this technology is emerging, with more than 300 mills installed globally in various grinding duties and commodities for the last 30 years. Although the technology is matured, there are still opportunities for improvement and the development of new knowledge to improve its circuit energy efficiency and reduce water consumption. Recent studies show that a combination of stress intensity, ore hardness, slurry density, and mill feed configuration can be used to evaluate the performance of the grinding circuit. A recent pilot-scale TowerMill test work at various stress intensities shows that a lower operating work index and size-specific energy were achieved at higher agitator tip speed and using medium size grinding media. These outcomes were confirmed through several site trials. A smaller 17 mm grinding media has reduced the operating work index and size-specific energy by 18 per cent and 11 per cent, respectively, compared to the 20 mm in a magnetite concentrator. Moreover, the conversion from top feed to bottom feed configuration has reduced the operating work index and size-specific energy by 13 per cent and 37 per cent, respectively. The circulating load was reduced by 34–38 per cent, leading to a reduction in the dilution water consumption in the hydrocyclone feed sump by 20 per cent. A variable frequency drive offers flexibility to change the stress intensities on the TowerMill, where a 35 per cent reduction in specific energy was achieved when the ore became soft. This paper explores the potential design and operating strategies that can be implemented to enhance the energy efficiency, reduce water consumption and reduce CO2 emissions in the circuit and finally evaluate its impact on the total cost of ownership.

The study commenced in October 2012, to understand the basic ecology of the spotted-tailed quoll (Dasyurus maculatus) and to confirm whether the species utilise previously mined land on the Mt Owen mine site that was rehabilitated in 1996 to 2011, as part of their home range. The spotted-tailed quoll is a widely distributed marsupial carnivore that is listed as rare or threatened in every state in which it occurs. The species is a forest-dependent marsupial carnivore that occupies a wide range of habitat types (Long and Nelson, 2004).The first part of the study involved trapping a spotted-tailed quoll and fitting the individual with a combined global positioning system / ultra high frequency (GPS/UHF) radio-transmitter collar (Telemetry Solution – Quantum 4000E). The benefit of the GPS/UHF collar is that it records locations (or fixes) at intervals programmed by the user. The UHF component allows the researcher to locate the individual at a frequent interval, and the Quantum 4000E also has a remote download function, enabling the GPS data stored in the collar to be wirelessly transmitted between the collar and researcher. The benefit of this new technology is that the animal does not need to be repeatedly captured to retrieve the GPS data.Daytime searches are also undertaken using the UHF receiver to locate the individual and locations of daytime denning and latrine sites. The location of each den site is recorded using a hand-held GPS. Data downloaded from the GPS collar is then imported and mapped using computer software program ARCGIS (geographic information system), to overlay the data points onto aerial photography.Results demonstrate that threatened species will utilise rehabilitation areas post mining. The data presented will be used to develop a greater understanding of threatened species ecology in and around rehabilitated areas, which will assist in the planning and design of native forest rehabilitation areas.CITATION:York, P, 2014. With traditional grade control processes, geological models are created by geologists at the completion of the data collection stage and wireframe construction is closed off at a set date to allow for time to complete the estimation process. Any issues identified at earlier stages or the addition of new data requires the estimation process to be started again. Alternative interpretations are explored only if time allows and often in a limited fashion. An alternative workflow has been developed at the Waihi Underground Gold Mine utilising Leapfrog Edge and Seequent Central. Geological and grade control models are created, dynamically linked, and saved as separate branches. A project geologist is able to work on the grade control model and a mine geologist on the geological model simultaneously. This allows for a non-linear modelling workflow with continual review and feedback. The introduction of new data can occur right up to the model due date, with data collection, validation, alternate interpretation, and review happening incrementally. Models are categorised and stored in Seequent Central according to project status. An approved version of the geological model and grade control estimate are uploaded at the time of release and transparently capture all data used at a point in time, as well as the review process. Dynamically updating models for grade control has reduced turnaround time while increasing accuracy and quality. Estimates of individual domains are vastly improved from being able to rapidly evaluate the effects of model parameter changes in real time and 3D. Development and stope design directly benefit from these streamlined processes, with higher quality, timely production occurring as a result.

The mineralogy of ten trihydrate bauxites and their leached residues from bomb digestion tests at 140, 180 and 2400C was investigated and the resultsòcorrelated with analytical data. It is shown that increased recovery of alumina at high temperatures is due to. the dehydroxylation of goethiteòrather than the presence of alumina monohydrates. The effects of aluminous goethite -on the interpretation of "available alumina". results-and normative analysis are discussed and it is concluded that,'cautionòmust be exercised in interpreting the results of bomb digestion tests in terms of alumina hvdrates.

The development of robust, economical transport infrastructure has been identified as a key driver to sovereign economic growth. Notwithstanding the value of undeveloped mineral resources, unless they can be brought to market they are of little value. Within a global context, the continent of Africa probably faces one of the most significant transport driven economic development programs as countries strive to unlock their much sought after resources.The UN’s Africa Review Report on Transport (United Nations Economic and Social Council, 2009) provides an excellent context for the issue facing African countries and the potential for investment in transport to solve many of the intrinsic problems they are confronted with. With little exception, economic development in Africa is very likely to be driven ‘off the back’ of transport development – and more specifically from heavy-haul rail as a means by which economic wealth can be derived from export of the continent’s vast natural and mineral resources to the world. The report found:transport is a key driver of economic development and poverty reduction;road transport is the most dominant mode in Africa;the ‘Cost to Market’ of African goods is hampered by the reliance upon road transport;the cost of road transport is not just economic – 19 per cent of the world’s road fatalities were recorded on African roads;African railways are seen to be disjointed and under-utilised;Africa’s ports are key to export trade - yet only six ports (three in Egypt, and three in South Africa) handle about 50 per cent of the continent’s total container traffic;the cost of transport is increasing making African trade even less competitive; andsignificant negative impacts of Africa’s reliance on road transport – health, and greenhouse emissions.We understand that mining the resource is only part of the solution – it has to be put to market at a competitive landed cost.Most African countries face huge costs associated with transport development and ongoing operation – in many cases beyond their available resources.The Australian rail industry has faced and solved many of these challenges.
