[Certain chapters of the tentative document entitled, "Test Requirements for instruments & Apparatus to be Considered for MESA Intrinsically Safe Certification" are evaluated and supporting data are presented on certain key issues with the purpose of helping the regulatory agency in improving the above document. The subjects discussed in detail are: hot wire ignition with W and BeCu wires; the most sensitive methane-air :and propane air ratio; the ignition limits for inductive-resistive circuits in 8.3°; methane-air mixtures in the 24-OV, 100 mH-2 [ ]H range, the resistive ignition limit as a special case when L ? 0; the capacitive-resistive ignition limit representing a four ,dimensional problem is analyzed and discussed in general and the variation of the ignition limit using Vo and/or r2 as a parameter are determined experimentally. The ignition limit is also determined for the [r1 ,~ r2] = 0 case ("capacitor alone") and for the most dangerous capacitive resistive circuit combination. The ignition limits are below the limits proposed in the document. A survey of the different factors influencing the ignition is made as to their suitability to provide a constant safety factor. It is shown that a constant safety factor cannot be achieved. The best method for applying a safety factor appears to be the increase of initial pressure or use of other gas mixture. The revision of the safety factor concept and ignition limit curves is recommended.]

The area around Big Bell was actively explored and sporadically mined for gold from 1904 onwards. The lease was named Big Bell in 1913. The property became established as a major mining centre with the development of the Big Bell underground mine in 1936. Mining operations commenced in late 1937 and ceased in January 1955, after milling 5.56 million tonnes of ore and producing nearly 21 tonnes of gold. In 1969 Australian Consolidated Minerals Ltd (A.C.M.) acquired the deposit then explored it alone and in joint venture until the current joint venture agreement with a subsidiary of Placer Pacific Limited (PPL) was negotiated in 1984. Following exploration and evaluation of near surface mineralisation, and a reassessment of the remnant pillars underground, the concept of a large open pit in conjunction with an underground mine evolved. The feasibility study was completed in October 1987, and the decision to go ahead was formally announced on 2nd December 1987. Placer Dome Technical Services Ltd were contracted to be the Project Managers, commencing construction on site early in January 1988 and handing the plant over to the operators 13 months later. The capital cost of the project at handover was estimated to be $133 million. The operating team commenced pre-production stripping of the open pit on 1st May 1988. By 31st January 1989 almost 16.0 million tonnes had been stripped and 350,000 tonnes of ore stockpiled.

Geosynthetic clay liner (GCL) is a kind of waterproofing material used widely in engineering. The waterproof mechanism is understood in terms of bentonite particles becoming a water-obstructcolloid layer after they absorb water and swell. The swell stress, however, has not been determined directly till now. In our experiment, the swell stresses of the GCL under saturated and water absorbing conditions are measured directly using a custom-made instrument. The results show that (1) the instrument designed by the authors performed satisfactorily and the test results are reproducible; and, (2) The trend line of swell stress variation with time can be divided into three segments. The first segment is characterized by a quick increase of the swell force in the first 0?50 hours. The swell stress increases by 7.00×10-4?1.00×10-3 Mpa / hour. The second segment shows a slow increase of the swell stress during the 50?1730 hours. The swell force increases by 7.54×10-6?2.02×10-5Mpa / hour. The third segment is characterized by little variation in the swell stress after 1730 hours. In this segment, the average value of the swell stress measurements is 0.0719Mpa and the maximum value is 0.0729Mpa. It is suggested that the swell stress is mainly raised by water entering the pores among montmorillonite particles and the interstitial layers in individual montmorillonite crystals, leading to an increase in volume and manifested as a stress on the surrounding materials.

The order-disorder transition in Ag-Mg alloys in the range 17 to 26 at. pct Mg was investigated and some thermodynamic, electrical and mechanical properties of ordered Ag-Mg alloys were measured. A modification of the phase diagram is proposed on the basis of measured transition temperatures. The stability of the ordered structure is analyzed in terms of the quasichemical theory. Kinetic aspects of the order-disorder transition were also investigated. The energies of ordering at 273°K of Ag-Mg alloys were measured by liquid metal solution calorimetry. The electrical resistivities and the tensile Properties of the ordered alloys were measured. The exothermic energy of ordering increased with magnesium concentration up to the composition Ag3Mg; it is discussed in terms of the quasichemical theory. Pronounced order hardening was observed and can be explained by several strengthening mechanisms, which are particularly effective because of a change in crystal structure associated with the ordering transition. IN silver-rich solid solutions containing approximately 17 to 26 at. pct Mg, long- range ordering may occur during appropriate therma1 treatments. Clare-brough and Nicholas1 first detected such long-range order in the solid solution of composition Ag3Mg and suggested that the unit cell of the superlattice contained 256 atoms. x-ray2 and electron diffraction3 investigations confirmed the occurrence of long-range order, but indicated a shifted Ll2-type structure. On the basis of recent X-ray measurements the crystal structure of ordered Ag3Mg has been confirmed as type D023.4 The present paper is concerned with the order-disorder transition in silver-rich Ag-Mg solid solutions and some thermodynamic, electrical, and mechanical properties of the ordered alloys. The effects of short-range order on the properties of silver-rich solid solutions containing up to 26 at. pct Mg are discussed in a concurrent paper.5 1) EXPERIMENTAL PROCEDURES 1.1) Preparation of Specimens. Specimens of Ag-Mg alloys containing from 18 to 26 at. pct Mg were prepared in the form of 1.0-mm-diam wires as described in Section 1.1 of Ref. 5. Disordered specimens were prepared by annealing at 773°K for 1 hr and quenching into iced brine. Ordered specimens were prepared by annealing at 773°K for 1 hr and slowly cooling to room temperature over a period of 15 days. The specimens were stored at 78°K. 1.2) Resistivity Measurements. Electrical resistivi- ties were measured by a potentiometric method.' Equilibrium values were obtained at several temperatures. The kinetics of ordering were investigated by following the time-dependent changes of the resistivity of initially disordered specimens during annealing. The specimens were enclosed in copper capsules and immersed in salt pots; the heating up was accelerated by injecting preheated helium into the capsules. 1.3) Calorimetry. The energies of ordering of the alloys were measured in a tin solution calorimeter as the difference between the heat effects of additions of completely ordered and disordered specimens of the same composition. The procedure and the method of calculation have been described.6 Magnesium was used for thermal compensation in most calorimetric runs in order to improve the accuracy.677 1.4) Mechanical Tests. Tensile tests were carried out with wire specimens at room temperature as described in Section 1.5 of Ref. 5. Microhardness measurements were also made.? 2) RESULTS AND DISCUSSION The characteristics of the order-disorder transition of silver-rich Ag-Mg solid solutions will be discussed first. Some thermodynamic, electrical, and mechanical properties of the ordered alloys will then be considered and compared with the corresponding properties of dis-disordered alloys. 2.1) The Order-Disorder Transition. 2.1.1) Thermodynamic and Structural Aspects. The transition temperatures Tt were determined from discontinuities in the slope of the equilibrium resistivity vs temperature curves. Normalized curves for three compositions are shown in Fig. 1. The transition temperature increases with the magnesium concentration and reaches a maximum at the stoichiometric composition Ag3Mg. The transition temperature of the alloy Ag3Mg was measured as 665° ± 2°K and compares with published values of 660°,1 663°,8 and 66°k.3 In the slopes of the resistivity vs temperature curves of alloys containing 22.2 and 22.5 at. pct Mg, discontinuities were observed at two temperatures. Such upper and lower discontinuities indicate a two-phase field. A modified form of the published phase diagram9 is shown in Fig. 2. A two-phase field was found only on the low-magnesium side of the composition Ag3Mg. On the high-magnesium side, the existence of a two-phase field could not be established because of insufficient resolution, but such a field must be present. This part of the phase diagram can be made complete by a eutec-toid-type reaction a = (a' + ß). The existence of a boundary between the two-phase fields (a + ß) and (a' + ß) is also in accord with published lattice parameters.3 The crystal structure of ordered Ag3Mg (type Do23)

The Block 37 Chicago Transit Authority (CTA) Tunnel Construction Project was originally designed by STV to connect Blue Line subway in North Dearborn Street and Red Line subway in State Street in the downtown Chicago Area. The project was proposed using tunnel mining techniques combined with steel pipe jacking and fiber glass rebar soil reinforcement. Weidlinger Associates, Inc. (WAI) and Kiewit Construction Company proposed an alternate design using open cut construction technology. WAI designed a complex temporary excavation support system using Soldier Pile Tremie Concrete (SPTC) walls and Soldier Pile Lagging walls supported by cross lot/corner bracing elements. Below the top of the existing tunnel, a top-down construction technique was utilized. A steel temporary traffic decking system at top of the excavation site served as the daily street traffic function as well as the first level of bracing for the support of excavation. The project encountered great challenges of soft clay conditions in downtown Chicago, numerous existing utilities in the excavation site and the requirements of protection for a number of historical buildings nearby as well as the protection of the city streets. The construction was successfully completed at both State Street/Washington Street and Dearborn Street/Randolph Street. The monitoring system showed that ground movement and adjacent building settlement was kept to a minimum during construction.

O coque desempenha três principais funções no Alto-Forno sendo elas: física, térmica e química, em se tratando do papel físico, a função estrutural é de primordial importância, pois não pode ser assumida por outro combustível, com isto as indústrias avaliam a resistência mecânica a frio para garantir uma marcha estável no reator, porém os ensaios padronizados não são suficientes para avaliar a resistência do coque. Neste contexto este trabalho desenvolveu uma metodologia para avaliar de forma individual a resistência mecânica a compressão do coque nas granulometrias típicas do coque metalúrgico e como os principais defeitos, poros e trincas governam a resistência do material. Foram realizados testes de compressão, análise de microestrutura, microtextura, inspeção visual de trincas, MEV. Os resultados das análises realizadas no coque mostraram que a tensão média de ruptura aumenta com o aumento da granulometria.

The objectives of this investigation were to determine the strain propagation laws for a strain pulse in salt and to compare the seismic effects from the detonation of different explosives in salt. The investigation showed that a strain propagation law of the type, e = K(R/W 1/3 ) n, and a strain energy propagation law of the type, Ea/W 1/3 = K(R/WI/3)b , are valid in salt. A change in the regression law coefficient, n , was noted with strain level. A similar change in b was inferred because strain energy is proportional to the square of strain amplitude. Particle velocity data may be used interchangeably with strain data under the test conditions.

[Polychlorinated biphenyls (PCR's) have been widely utilised as fire-resistant dielectric coolants; in electrical equipment used in mining applicants, including transformers, capacitors, electric motors, and electromagnets. In addition, PCB's have been used in hydraulic fluids and heat-transfer fluids and are present in many oil-filled transformers. The U.S. Environmental Protection Agency (EPA) recently banned the manufacture of PCB's and equipment using PCB's, and imposed strict requirements on the continued use and disposal of existing PCB equipment. This manual discusses the EPA requirements, suggests ways to decrease the risks resulting from continued use of PCR equipment, and surveys the non-PCB equipment that is available as replacements for the PCB electrical equipment presently used in mines.]

Introduction During the past 10 years, energy costs in mining have risen from about 4% to as much as 25% of total production cost. When combined with declining ore grades, these costs exert inordinate stress on mine productivity. Industrial engineering techniques have traditionally been aimed at improving manufacturing facilities productivity, but they have been applied to mining as pressure for productivity improvement has increased. These techniques can perform four useful functions for mining operations by improving methods, equipment selection procedures, energy use through scheduling, and managing energy costs rather than being managed by them. This article discusses some of the standard techniques for productivity improvement, and how they can be adapted to perform these four functions. Pre-Production The best time to seek high productivity is when a mine is being planned. During pre-production, when most options are still open, an optimum mine design is achievable. At this time, major equipment selections are made, the layout, material handling systems, and processes are designed, and critical energy source decisions are made. By including explicit energy cost projections, decisions can be made that will save significant amounts of money per ton of ore over the life of the mine. Attention can also be given to often overlooked details such as lighting. Some of the same tools used in new mine planning are applicable to productivity improvements at an existing plant. These tools are the charting and modeling methods that have been developed by industrial engineers over the past 75 years. Energy Consumption Before any analysis of energy consumption can be performed, preliminary work is necessary to find the kind of energy used, how much it costs the plant, and when electrical peaks occur. Past bills should be examined to determine the monthly consumption rate and the cost of each energy type. These data should be adjusted to remove effects caused by production fluctuations or weather. Knowing when electrical peak demands occur is necessary because these determine either the electrical capacity needed or the amount that must be bought. At $500-$800 per kW for construction or $5-$15 per kW per month for purchase, they represent a significant cost. By the time these data are collected, planners should be aware of the importance of energy in the total cost of mine operation, and should have the most crucial time data for electrical peak demand. Flow Process Chart Reducing costs can be accomplished by a thorough look at mine systems and how altering one system causes changes in others. By developing and exercising models of production processes outward to the transportation of the final product, otherwise obscured weaknesses may become apparent. There are tools available for analyzing the mine and its associated systems; one of the most useful is the flow process chart. The flow process chart gives a system analysis of a production procedure by classifying all actions into one of five types: operations, inspections, transportation, delays without paperwork, and delays with paperwork. Operation: In an operation, the nature of something is changed. Examples are blasting, washing coal, and processing matter. Denoted by 0. Inspection: Examining an object for identification or to measure some attribute. Examples are determining whether a drill hole is deep enough, deciding how far to cut under an unsupported roof when operating a continuous miner, or checking the lubricant level on a truck. Denoted bye. Transportation: Moving an object from one place to another. Examples are transporting overburden, moving coal on a conveyor belt, and pumping water. Denoted by =>. Delay without paperwork: A delay that is an inherent part of the system. Examples include idle time of a continuous miner while waiting for shuttle cars or for roof bolting, crushed ore in a storage area waiting for grinding and flotation, and people from one craft waiting for another craft to finish. Denoted by D. Delay with paperwork: A delay that can be terminated only by some written authorization. Examples are coal that cannot be removed until authorized by a bill of sale and transportation orders, personnel records, maintenance, or spare parts. Denoted by V. In practice, the flow process chart is completed after data collection and a detailed examination of the process. Data collection is needed to answer questions on distance, quantity, and time for each of the operations. As the analysis is being performed, each step of the process is subjected to these questions: • Why is the step necessary? • What is being moved? • When does the step take place, and why?

"The Detour Lake mine is located 200 km northeast of Timmins, Ontario, Canada.The mill processes a free milling gold ore using both a gravity circuit and leaching with a state-of-the-art carbon-in-pulp circuit. The grinding circuit is a conventional SABC circuit with a tonnage of 55,000 tpd at 95 microns with the addition of pre-crushing upstream of the SAG mill.The Detour Lake mine has one of the largest gyratory crusher in the world (60"" x 113"") and over the years, many improvements have been made to this crusher with the result of improving plant throughput. This paper will present how choke feeding the gyratory crusher has been found to be one of the main influences on crusher product particle size and plant throughput. It will explain how the effect of choke feeding the gyratory was measured and from that, the various changes made to improve choke feeding of the gyratory crusher.IntroductionOre is hauled from the open pit mine and dumped directly into a single 60"" x 113"" primary gyratory crusher. The live capacity of both the feed and discharge hoppers of the gyratory crusher are designed for two (2) truck loads each, assuming a nominal payload of 300 tonnes per truck. The surge bin under the primary crusher feeds an apron feeder that controls the ore feed rate at approximately 4,200 tph onto a 227 metres long x 1.52 metres wide belt conveyor with a nominal capacity of 4,200 tph.The primary crusher is operated at an open side setting of 172 millimetres to produce a F80 feed to the secondary crushing circuit of 135 millimetres."

In sampling a process stream, conventional sampling theory deals only with the variation of the analyte content and neglects the impact of the variation in the mass flow on the sampling variance, whether the sampling regime is time-based or mass-based. This is an obvious simplification of the real state of affairs and it is worthwhile to study this impact via simulation.With a facility to simulate large (long) random functions having some thousands of points in the function and the ability to impose covariance functions on the mass flow and analyte variations, such a study is possible. It is also possible to evaluate the impact of correlation between analyte content and mass flows.This paper describes a limited study of the variance increase due to mass flow fluctuations over the usual sampling variance due to analyte variation during the sampling period. The bias in the sampling result is determined for specific modes of correlation. Simulation methods are used as they are the only means by which a sampling result can be compared to the true assay for the lot; in the physical circumstances of mineral process analysis, it is never possible to determine the true process stream assay.CITATION:Lyman, G J, 2017. Roadheaders are very versatile excavation machines used in tunnelling, mine development, and mine production for soft to medium strength rock formations. Performance prediction is an important factor for successful roadheader application and generally deals with machine selection, production rate and bit consumption. Among many different parameters, brittleness is also one of the material properties related to breakage characteristics and can be used as a cuttability parameter from a mechanical excavation point of view. The main objective of the research study is to contribute the brittleness of rock excavated to construct a new empirical equation for predicting the performance of roadheaders in different material and operational conditions. In this regard, a new performance prediction model for medium duty roadheaders based on a brittleness index (BI) is presented. In this study, rock mass brittleness index (RMBI) is defined in order to investigate the influence of BI on roadheader performance. RMBI is an index which can be used to relate the intact and rock mass characteristics to machine performance. Results demonstrated that RMBI is highly correlated to instantaneous cutting rate (ICR) (R²=0.94). Moreover, through the further analysis and normalization, the pick consumption index (PCI) was introduced as a parameter having a good relation with pick or bit consumption rates (PCR) (R²=0.79). Finally, the new predictive models for ICR and PCR showed very good correlations with the actual measured values. The microhardness of single crystals of tungsten disilicide has been investigated by the Knoop method. The average random room-temperature hardness of the WSi, matrix was 1350 kg per sq mm. Hardness crnisotropy was noted with respect to plane and indenter orientation as determined by single-crq.stal X-rny studies. Annealing at 1600" and 1800°C decreased the average hardness to 1310 and 1230 kg per sq tnm, respectively, and produced a second phase identified by X-ray diffraction and electron-microprobe analysis to be wSio.7. Ball-impact experiwzents produced rosettes at 850°C. Optical and electron microscopy showed evidence of slip and cross slip and twinning produced by microhardness indentations. Prismatic (100), [001] slip was found and cor~elated with hardness data. THE present study was undertaken to investigate the hardness anisotropy of as-grown and annealed single crystals of tungsten disilicide. The existence of the silicide WSiz in the W-Si system has been well-established and its structure thoroughly investigated zachariasen2 found WSi, to have a tetragonal C type of structure, similar to that of MoSi, with lattice parameters a = 3.212A, Kieffer et al. studied the W-Si system and measured the density and microhardness (at a 100-g load) of both polycrystalline WSi, and WSi,.,. The values found were 9.25 g per cu cm and 1090 kg per sq mm for WSi,, and 12.21 per cu cm and 770 kg per sq mm for WSi0.7, respectively. According to Samsonov et a1.5 the microhardness of polycrystalline WSi2 is 1430 kg per sq mm (at a 120-g load). EXPERIMENTAL The WSi, single-crystal boules investigated in this paper were grown by a Verneuil-type process using an electric arc by the Linde Division of the Union Carbide Corp.6 The largest specimens were 8 mm in diameter by 16 mm long. The crystals had an average density of 9.01 g per cu cm with a tungsten • silicon content of 99.9 wt pct. The major impurities were: 87 ppm O, 41 ppm N. 54 pprn C, 500 ppm Zr, 50 ppm Na, and 50 ppm Mn. The crystals were silicon-poor, the average silicon content being 22.20 pct (stoichiometric value is 23.40 pct), and tungsten-rich, the average tungsten content being 77.70 pct (stoichiometric value is 76.60 pct). As-received single crystals were ground and analyzed by powder X-ray diffraction technique using Cu Ka radiation. Laue and layer line rotation patterns were obtained on cleaved sections of WSi, single crystals. Electron-microprobe traverses of representative crystals were done using a Phillips-AMR electron microanalyzer. Carbon replicas were used to prepare electron micrographs. This work was done with a JEM-6A electron microscope. Prior to the metallographic examination, the specimens were mounted in Lucite and then polished for short times on polishing wheels using 9-, 3-, and 1-p diamond-grade pastes. Finally they were fine-polished with Linde A powder for 24 hr on a Syntron vibratory polisher. The samples were etched with 4H 2 O:1HF:2HNO3, which is a medium fast-acting etchant. The combination 1HF:2HNO3:5 lactic acid is also a satisfactory etchant. Annealing runs for selected specimens were made at 1600" and 1800°C for 3 hr at 1.0 to 3.0 x 10-5 mm Hg. A Brew tantalum resistance furnace with WSi2 powder for setters was used. The WSi2 powder was the same as that used for the crystal growth. Temperatures were measured with a calibrated W, W-26 pct Re thermocouple and a microoptical pyrometer. Powder X-ray diffraction, emission spectrographic, and electron-microprobe analyses were done after the annealing runs. For microhardness measurements a Tukon Microhardness Tester Type FB with a Knoop indenter was used. Although measurements were taken at loads ranging from 25 to 1000 g, the 100-g load was chosen as the standard load. All measurements were taken at room temperature. Only indentations of cracking classes 1 and 2 were considered.' DISCUSSION OF RESULTS Powder X-ray diffraction analysis showed the as-received crystals to be single-phase WSi2. Laue and layer line rotation patterns obtained on cleaved sections of WSi2 single crystals proved them to be tetragonal WS 2 2 The results also indicated that the c axis of the crystal was oriented parallel to the boule or growth axis. Electron-microprobe traverses across the matrix of the as-grown crystals showed them to be homogeneous WSi,. Optical and electron microscopy of etched crystals, however, revealed that they contained minute amounts of the "golden" and the "blue" second phases as opposed to the "white" or WSi2 phase. The Grasberg block cave rail haulage system will be located in the Grasberg mining district on the island of Papua, Indonesia at an elevation of almost 3000 m above sea level. It will transport 160 000 t of ore per day from multiple chute galleries to the underground crushing systems. The design entails approximately 23 km of rail (ballasted and embedded track), over 120 ore chutes to deliver ore to the muck cars and three unloading stations to deliver ore to the crushers. The rail haulage system is a closed-loop system entirely within the underground mining complex.The GBC rail haulage project will be a substantial operation. Trains consist of 11–12 bottom-dump 20 m3 muck cars driven by a 40 t electric locomotive, about 400 t per trainload. Approximately 12 trains and one spare are required to maintain full production at 160 000 t/d. All rail installations will be 1435 mm (standard) gauge with AS60 steel.The rail system is designed to operate in full automated mode, including chute loading, hauling and dumping. The operation will be controlled from a surface control room.The rail haulage project has been divided into five stages, which will be brought online over the course of ten years. Current development and construction are focused on Stage 1 for initial production that will occur beginning in late 2017. This stage includes 24 chutes in three chute galleries, one unloading station, workshop areas and almost 7 km of track (3 km of embedded and 4 km of ballast).This paper describes the key components of the rail haulage system and reports on the current status of development and construction of the system. Predictive maintenance technologies including vibration analysis, tribology, infrared thermography, ultrasonic detection, and electrical measurements have been applied to large electric mining shovels. The predictive program was initiated on the P&H Model 2300 and 4100 electric shovels. The utilization of these technologies has shown the potential for improving electric mining shovel availability and minimizing maintenance costs. Predictive technologies provide the tools to assess machinery condition. Availability is enhanced by the condition based planning of component replacement. Maintenance costs are minimized by focusing resources through condition based prioritization.

In common with general experience in the iron-making industry overseas, Australia has in recent years suffered a deterioration in the physical quality of her iron ores in their suitability as a direct blast furnace feed. The increasing amounts of fines and the more friable nature of lump ore, especially in the ore from the Cockatoo Island deposits in Yampi Sound, Western Australia, have focused attention on problems of ore preparation and agglomeration, particularly sintering.Although very small pilot scale Dwight-Lloyd type sinter machines were operated at the steel-making centres at Newcastle and Port Kembla during World War II, no sizeable capacity was installed, so that ore was charged to the blast furnaces almost entirely unscreened, and the resultant flue dust was stockpiled.Towards the close of the war, the pilot plant at Newcastle was replaced by a small machine, adjacent to the flue dust stockpile, with a rated capacity of some 20 tons per hour. After approximately five months of part-time operation, this plant was closed down in January, 1945. Production was not resumed until December, 1950, when, due to a temporary ore shortage, it was decided to supplement available ore with sinter produced from the large flue dust stocks of about 300,000 tons. In spite of mechanical and process difficulties, inherent to the design of the plant, operation was continued on a two-shift basis, with several shutdowns for extensive overhaul and plant alterations, until September, 1954, when production was put on a three-shift basis. Addition of Yampi ore fines to the sinter machine burden was not commenced...

Environmental incidents involving mine tailings and waste rock inevitably become highly publicised and serve to damage the credibility of the entire industry. The recent serious incident involving a spill from a Romanian mine, was blamed for triggering æone of the worst environmental catastrophes in Eastern EuropeÆ. It is important to clearly demonstrate to the general public that these incidents are not representative of the industry as a whole. Where such incidents have occurred, the causes are generally related to poor design and/or construction, inadequate supervision and/or monitoring and maintenance, lack of independent expertise, inadequate risk identification and treatment and no provision to ensure that rehabilitation and long term management of the site occurs. Many people are unaware that these issues have been considered and provided for at the Martha Mine. Despite the fact that the first tailings storage facility has performed very well over fifteen years of operation, the long term safety and security of the tailings storage facilities continues to be questioned and debated by the public. The best way to address this is to provide quality information to enable a better understanding amongst the general public.

On December 27, 1961, a fire and explosion destroyed the blasting agent mixing plant and an attached storage barn of the Whitaker-Atlas Supply Co., near Norton, Wise County, Va. Five residences, an office building, four trucks, and three automobiles were completely destroyed; a house trailer and several other trucks and automobiles were severely damaged; and other property damage, including much window breakage, occurred up to 3 miles or more airline distance away. Through the quick action of the fire marshal of the Norton Fire Department, Norton, Va., there were no fatalities and only five persons required hospitalization. This incident is of particular interest because of the widespread practice of mixing fuel oil (FO) with ammonium nitrate (AN) to prepare low-cost blasting agents, in many cases under conditions and circumstances not very different from those presented here. A representative of the Bureau of Mines Explosives Research Laboratory, Pittsburgh, Pa., and field representatives of the Bureau's Health and Safety Activities conducted the investigation. Information in this report was obtained from representatives of the Whitaker-Atlas Supply Co., the Norton fire marshal, and others who were on the scene either shortly before or after the incident occurred. Representatives of the Manufacturing Chemists' Association, the Institute of Makers of Explosives, and individual ammonium nitrate producers participated in the inquiries and investigations.

The objective of the ? 200 million ($280 million) project Parma Railways Station-Urban Transformation was the reconnection of the North and South parts of the town by constructing an 80 m (250 ft) wide sunken artery passing under the four double railway tracks of the station without any impact on train traffic. The site of the project, adjacent to the Parma River, presented water seepage problems at any excavation depth. The use of conventional large diameter bored pile machinery was not allowed by the Railway Authority because equipment dimension and construction phases do not comply with train traffic safety regulations. Foundations for the bypass were thus constructed by minipiles nested into jet-grouted soil using machinery operating within the existing head room and at a safe distance from the tracks. Jet-grouting treatment reached depths of 25 to 28 m (80 to 90 ft) from tracks? grade and embedded minipile-heads were terminated at 2 to 3 m (6.6 to 10 ft) below grade. Load tests on minipiles were pushed to failure. Failure occurred at a 240 t (265 t) load without breakage of the bond between the jet grouted soil and the minipiles. A 0.93 MPa (135 psi) shear resistance ??? between minipile steel casing and the jet grouted soil was measured prior to the failure of the steel casings. Overall measured foundation settlements were well within expected values and were considered insignificant.
