The main objective of occupational health and safety research is to minimize or eliminate the events that may cause fatal or non-fatal injuries to human workers. A commonly used technique is to devise an “incident prevention plan”, which is more often the product of thorough investigations of past reported incident events. Incident documentation and methodical reporting and systematic and thorough data collection, often help identify the root cause of the incident – in other words, the etiology of the incident. These retrospective analyses of the incidents can efficiently identify the future steps to take to achieve the objective of minimizing occurrence of “events of interest”. This paper introduces a novel technique – “Taxonomic Analysis”, which identifies the root causes of an event (an incident in this case) and can provide future direction for corrective measures to reduce the probability of occurrence of the event. A taxonomic analysis involves systematic and organization of data based on observation, description, and classification. This paper utilizes the rock fall related incident narratives, available with the Mine Safety and Health Administration (MSHA) database (1984-1999), for a taxonomic analysis. The study found that 67.6% of groundfall incidents occurred in supported areas with the remainder (32.4%) in unsupported areas. Some form of rock mass failure appears to cause about 50% of groundfall incidents, although in 39.7% of the overall groundfall incidents or two-thirds of those involving rock mass failure, the exact nature of the rock mass failure could not be determined from the narratives. Human activities factors such as scaling or barring down, drilling or bolting, and setting timbers or cribbing accounted for another 34.5% of the groundfall incidents. Finally, support system failures made up the remaining 15.5%. Results from this taxonomic analysis may suggest focus areas toward which preventive measures and future research activities could concentrate.

Researchers at the National Institute for Occupational Safety and Health partnered with the water well industry to develop engi­neering control technologies and training products to address noise exposure at the job site. Noise­induced hearing loss continues to be a problem in the well drilling industry. Research con­ducted by NIOSH found that air rotary drill rigs produce sound levels more than 90 dB(A) over the course of an eight­hour workday. Sounds levels above 90 dB(A), identified during hammer drilling and the hammering of casing, exposes workers to levels that can damage hearing. A typical air rotary drill rig provides no barrier, for noise and dust pro­tection, between the drill’s control panel and the drill stem. The development of a partial cab is the clearest example of NIOSH’s research partnership with the water well industry. NIOSH researchers determined that an engineering noise control to protect the oper­ator at the rig’s control panel would be effective, based on typical work site conditions. Many surface rigs do not have full cabs as a result of the original equipment manufacturer design or the high cost of aftermarket products. Developed as a partial cab, the NIOSH engineer­ing noise control design addressed stakeholders’ needs associated with cost, transport, and worker usage (Figure 1). Field tests found the partial cab effective in reducing sound levels at the rig’s control panel, a typical location for a drill rig operator. Sound levels at the control panel were reduced from 104 to 96 dB(A) (Figure 2). Coupled with continued use of personal protective equipment, such as hear­ing protection devices, the partial cab can help to reduce noise exposure. NIOSH researchers created a CD with informa­tion about noise exposure and the partial cab. Most important to well drilling companies is information on how to construct and attach the partial cab to drill rigs. The CD can be viewed on a standard computer and includes a video of the partial cab in use on a work site. Computer­aided design drawings are included on the CD and provide detail of the cab design as well as information on construction mate­rials and design details. Additionally, the CD provides background infor­mation on noise­induced hearing loss and hearing protection devices to increase knowledge about this occupational health concern. This information can be viewed by all company employees to help promote the partial cab’s importance to workers using the cab. More information on the partial cab noise control technology can be downloaded from www.cdc.gov/ niosh/mining NIOSH researchers also examined the health and safety training needs of workers in the ground water industry. It was quickly realized that most workers learn health and safety on the job and do not attend any sort of formal training. In addition, there is not a lot of time for daily “tailgate talks” or “shop talks” on health and safety issues. As a small business industry, the owner is often operating a drill rig along with his other employees. In general, new employees learn the job in an apprentice style sys­tem. As a result, workers and owners have few opportunities to access uniform health and safety information based upon the job — an outdoor work environment with daily change in work sites. With this information, NIOSH researchers devel­oped two products that deliver health and safety information to fit into the daily work of well drillers. The aerospace and automotive industries are facing a significant risk for cumulative trauma disorders from high-repetition, long-duration tasks. Additional risk factors such as shocks, vibrations and sustained uncomfortable postures oftentimes contribute to musculoskeletal, neurological injuries associated with the hand-arm system. The power tools used for the operation reduce the comfort and working efficiency of the operators, thus lowering their health and safety and the quality of operation. This paper investigates the fastening operation on the assembly line of a major aerospace company for quantifying hand-arm vibrations with the objective of developing a dynamic model of the hand-arm system. The model will be used to ascertain the effect of the various risk factors on the quality of the operation. Experiment Setup A system of three tri-axial accelerometers is used to collect vibrations entering the hand-arm system and their positioning follows the ISO-5349 standard. Reaction force during the operation is measured using a force sensor. The posture of the hand-arm system is measured using two goniometers and one torsiometer. The pilot study conducted has two subjects and one pistol-grip power tool. The experiment setup and positions of various sensors is depicted in Figure 1. This report summarizes the results of a review of foreign coal mining literature conducted under the Bureau's Advancing Coal Mining Technology program. In addition to a discussion on the historical development of mechanized longwall systems, and the current state-of-the-art method in Europe, this report summarizes the previous and ongoing Bureau efforts to adapt longwall technology to U.S. conditions. The bibliographic section of this report, divided into major categories such as mine design, face management, roadway development, strata control, powered roof supports, coal winning, conveying, and automation, contains over a thousand individual entries. The purpose of this report is to provide U.S. mining engineers with background information on longwall methodology and equipment developed in foreign countries (principally Europe). The Bureau of Mines, as part of its Advancing Coal Mining Technology program, is funding R&D projects to demonstrate that the longwall mining method, successfully used in the deep mines of Europe, can boost productivity and improve safety in certain U.S. mines.

This Bureau of Mines report presents three methods for designing bulkheads to impound water underground: (1) thin and thick plate design; (2) South African plug design; and (3) single and double bulkhead seal design. Related areas critical to the long-term effectiveness of underground water impoundments are also addressed. These include bulk-head anchorage, concrete specifications and placement, the grouting of permeable strata, and the sizing of barrier pillars. A case study involving hydrostatic tests conducted on a single bulkhead seal constructed in the Safety Research Coal Mine of the Bureau's Pittsburgh Research Center is presented in an appendix.

The mining industry of Cameroon continued to be dominated by crude petroleum. Crude petroleum production has become the most important facet of Cameroon's economic development. Oil production was estimated to be 6.3 million tons in 1983 compared with 5.2 million tons in 1982 and 4.3 million tons in 1981. Over two-thirds of its export income was attributed to crude oil. The country's only refinery, dedicated in 1981, has a production capacity of 2 million tons. Cameroon has become self- sufficient in petroleum products since commissioning the refinery. The oil sector contributed approximately 13.5% to Cameroon's gross domestic product in 1982-83. Oil production was expected to rise from 125,000 barrels per day in 1983 to 156,000 barrels per day in 1985 when output peaks. Cameroon's ultimate recoverable reserves have been calculated at 100 million tons, or 750 million barrels of oil.

As longwall shields are lowered in preparation for advance, dust from the canopy falls directly into the air stream with the potential of becoming entrained. Historical dust sampling data from the early 1980s shows that shield advance contributed an average of 12 percent to the airborne respirable dust levels found on longwalls in the United States. Data from the 1990s indicates that shield dust liberation had increased to an average of 23 percent. Increased longwall production levels over this time resulted in the need to move a larger number of shields during each shift, while an increase in air velocities on the face was also occurring. These factors combine to suggest that greater quantities of shield dust are being entrained into the face air. A series of controlled laboratory tests were conducted in a test stand simulating shield dust entrainment to evaluate changes in airborne respirable dust levels at different air velocities. Significant differences in respirable dust entrainment were observed. The test facility, test methodology, and dust sampling results will be reviewed and discussed

In recent years, west-central Arizona and adjacent California have become the focus of geologic and exploration attention because of its excellent exposure of detachment-fault terrane. Detachment faults, normal faults with nearly horizontal fault planes, are the result of regional extension. Rocks on the lower plate consist of mylonitized intrusive, metasedimentary and crystalline rocks highly altered to chlorite; upper plate rocks comprise sedimentary, volcanic, metasedimentary, and metavolcanic rocks. The movement of the detachment fault causes the rocks on the upper plate to be severely fractured and brecciated thus making them Ideal conduits for mineralizing fluids. Mineralization favored the more reactive sedimentary and volcanic rocks of the upper plate. Three gold deposits, located in upper plate rocks, are currently being mined along the Colorado River corridor: the Picacho Mine near Yuma, Arizona, 70 mi south of the study area; the American Girl Mine In southeastern California, 60 mi southwest; and the Copperstone Mine near the southern end of this study area. Samples from the above three mines were used to create a geochemical model of detachment-related gold deposits. Antimony, arsenic, barium, copper, mercury, molybdenum, manganese, tungsten, and zinc all occur In the mine samples in concentrations at least three times greater than found elsewhere in similar rock types. Samples taken by the Bureau from the wilderness study areas contain similar anomalous concentrations of the same elements as well as elevated concentrations of several other elements Including: lead, fluorine, cerium, uranium, and samarium.

The Trail Lake RARE II area comprises 1,300 acres (526 ha) in Lassen and Plumas Counties, California, adjacent to the Caribou wilderness. No mining claims, prospects, or quarries are reported within the study areas, and no evidence of potential mineral deposits was found during a field reconnaissance. Quaternary basaltic lava flows uniformly underlie the area. INTRODUCTION The Trail Lake (No. B5-095) RARE II further planning area occupies 1,300 acres (526 ha) adjacent to the Caribou Wilderness in Lassen and Plumas Counties, California. Susanville is about 30 mi (50 km) east and Chester is about 17 mi (27 km) south of the study area. Access from State Highways 44 and 36 is provided by well-maintained gravel roads.

Previous studies conducted by the National Institute for Occupational Safety and Health (NIOSH) have shown that approximately 90% of coal miners and 49% of metal/nonmetal miners had a hearing impairment by age 501. Mine workers are exposed to additional noise levels underground due to the reflection of machine generated noise that would otherwise dissipate in an above ground setting. Determining the sound absorption coefficients is essential in modeling the sound field in an underground mining environment. Sound absorption is a measure of the amount of acoustic energy that strikes a surface and is absorbed, rather than reflected. The acoustic environment that mining machines operate in is a critical factor affecting the sound pressure level exposure for mining machine operators. Impedance tube testing is an inappropriate method for determining underground absorption coefficients due to the brittle composition of some of the materials such as coal and slate. Classic absorption coefficient estimation using T60 measurements will not work well in an underground environment because the theory assumes: a finite room, a diffuse field, and relatively uniform absorption. None of these assumptions are true in an open-ended mine entry. This paper will present a method using a ray-tracing technique to determine absorption coefficients for underground mines. Absorption coefficients are determined and presented for octave bands from 63 Hz to 8 kHz. The absorption coefficients will be essential for determining and predicting potential overexposure to machine operators in different mine environments.

ABSTRACT: A method (patent pending) has been developed to determine the quality of environmental cab filtration systems. This method utilizes specially configured filter cartridges to remove carbon dioxide from the ambient air with the environmental cab’s air filtration system. A real-time gas monitor is placed inside an unoccupied cab with the filtration system running to time decay the carbon dioxide to its lowest concentration or equilibrium level. The outside carbon dioxide concentration level can be measured concurrently with another gas monitor or with the same inside cab gas monitor during a representative period before and after the inside sampling is conducted. Cab filtration system leakage is comparatively deduced from the difference between the special cartridge filter efficiency and overall cab efficiency. Laboratory trials have illustrated that desirable high efficiency test filter configurations (greater than 99 % efficiency) can be devised, but have air quantity and test time limitations. These trials also demonstrated that CO2 penetration was reflective of controlled air leaks around a high efficiency filter under equilibrium conditions. This report examines the elements of this patent pending method.

Longwall mines typically use some form of standing support for secondary roof support in longwall tailgate entries. Although there have been several new support products developed for this application, there remains no universal design criteria to optimize the application of these support technologies. The requirement for optimization and proper support selection is to understand the degree of control that the support has on the ground behavior. The ground reaction curve and numerical modeling was used to evaluate the impact of standing support on ground behavior. LaModel was used to evaluate the impact of standing support on main roof and floor behavior and pillar yielding. The conclusion drawn from this study was that standing supports do not have sufficient capacity to control main roof or floor loading or prevent the resulting convergence of the tailgate entry. However, it is imperative that this “uncontrollable convergence” be considered in the support design to prevent premature failure of the support. A FLAC model was used to evaluate the near-seam roof and floor behavior in conjunction with the global vertical and horizontal stresses. The model suggests that standing roof supports can have some impact on the ground behavior as the elastic response of the rock is exceeded and rock structure deteriorates from the stress concentrations that develop around the tailgate opening. During this phase, the capacity and stiffness of the standing support can be critical to the stability of the opening, as eventually the rock mass will be transformed into a partially detached structure whose weight must be supported by the standing support. This work is part of a ground control program of research at the National Institute for Occupational Safety and Health (NIOSH) aimed at improving mine safety by reducing roof fall injuries and fatalities.

A general description of the activities and results of bauxite investigation by the Bureau of Mines at its bauxite project in Saline and Pulaski Counties, Ark., has been given in Report of Investigations No. 4251. The factual data obtained during the project are reported in a series of Reports of Investigation beginning with No. 4251, of which this is the fifth. The data are accompanied by maps showing the location of holes drilled and deposits developed. Each area upon which work was done is reported by half townships. Each bauxite deposit investigated was assigned a name and a number for ready reference and identification, and its location and name are shown on the appropriate half-township map. The description of each half township is followed by a description of each deposit developed in that half township. The 14 half-township maps and 77 detail maps of the bauxite deposits in this series of reports give the locations and numbers of all the holes drilled on the project except the deep holes drilled to test the geophysical anomalies. The locations of those holes show on figure 13 in Volume I. The descriptions of the deposits developed are followed by complete detailed logs of the drill holes. Many of the widely spaced exploratory holes did not discover bauxite of minable thickness and grade. The pertinent information obtained from the widely spaced exploratory drill holes has been summarized and tabulated, and a table for each half township is given with the discussion of that area.

This Bureau of Mines study investigated molten salt electrorefining for beneficiating low-grade, high-carbon ferrochromium products which can be readily produced from domestic chromite concentrates. A second objective was to refine commercial ferrochromium into low-iron chromium products. Electrorefining of both types of ferrochromium in molten CaC12-NaCl-CrC12, CaCl2-MgC12-CrCl2, or KC1-LiCl-CrCl2 electrolytes produced chromium-iron products with virtual elimination of other impurity elements and some lowering of the iron content, Over 90 percent of the chromium present in the feed materials was recovered. The electrodeposited products were upgraded further by magnetic separation techniques into chromium containing less than 5 percent iron. The technical feasibility of preparing a premium chromium product from a low-grade, high-carbon ferrochromium made from domestic chromite was demonstrated. Chromium virtually free of carbon, oxygen, and silicon from high-carbon ferrochromiums was produced. In addition, the amenability of three commercial grades of ferrochromium to electrorefining and magnetic separation for preparing chromium metal containing less than 5 percent iron was determined.

On-the-job training (OJT) is a very common method of teaching job skills. Much of the training is done by experienced workers. This paper discusses small investments that can help improve the effectiveness of OJT. Effective OJT involves some structure and planning in the transfer of responsibility for task performance from the trainer to the novice. Assisting the OJT trainer could involve helping develop up-to-date job analyses and offering strategies for teaching and evaluating job skills. The scope of this paper addresses (1) organizational considerations supporting structured OJT, (2) general strategies for structuring OJT, (3) a typical approach for conducting OJT, (4) characteristics and duties of OJT trainers, and (5) limitations of OJT as a teaching method. Health, safety, and operational skills can be connected through training. Haul truck operator training is used to illustrate some of the concepts discussed in this paper.

"The National Safety Competition of 1930, the seventh annual contest conducted by the United States Bureau of Mines to promote safety in the mines and quarries of the United States, has been concluded, and the five winners of the bronze trophies, The Sentinels of Safety,' which are provided by the Explosives Engineer magazine and awarded annually for the most excellent safety records, have been determined by the committee of award. One trophy was awarded to an anthracite mine in Pennsylvania, one to a bituminous coal mine in the same State, one to an iron-ore mine in Michigan one to a gypsum mine in Ohio, and one to an iron-ore opencut mine in Michigan.These trophies were awarded to the companies for having the smallest loss of time from accidents in proportion to the total number of man-hours worked by all employees at their mine or quarry. Honorable mention was awarded to each mine or quarry that ranked second, third, fourth, or fifth in its own ground and to all plants that were operated without a lost-time accident during the contest year. Eighty-four out of 337 plants that competed for the trophies were operated without an accident that disabled an employee beyond the day on which the accident occurred.Under the contest rules, all mines and quarries were divided into five groups, four of the groups representing different classes of underground mining operations (anthracite, bituminous coal, metallic ore, and nonmetallic mineral), and the fifth group repre-senting the quarrying or opencut mining of minerals whether metallic or nonmetallic. Twenty-seven mines were included in the anthracite group, 68 in the bituminous group, 52 in the metal group, 24 in the nonmetallic-mine group, and 166 in the quarry and opencut-mine group. The total of 337 mines and quarries was 26 more than the number of plants that completed the contest during the previous year.The committee of award which determined the winners of the contest was composed of seven members, all known throughout the mineral industry but not connected with the United States Bureau of Mines. The committee included W. H. Cameron, managing director of the National Safety Council; William Green, president of the American Federation of Labor; James F. Callbreath, Secretary of the American Mining Congress; T. T. Read, Vinton professor of mining of Columbia University and editor of Mining and Metallurgy; C. B. Huntress, executive secretary of the National Coal Association; A. J. Curtis, assistant to the general manager of the Portland Cement Association; A. T. Goldbeck, director of the bureau of engineering of the National Crushed Stone Association."

The present Bureau of Mines investigation measured low-temperature heat capacities of NiBr2 (8 to 302 K) and NiS04 (9 to 70 K) by adiabatic calorimetry. The standard entropies (S°, 298.15 K) of NiBr2 and NiS04 were calculated as 29.26 ± 0.06 cal/deg mole and 24.21 0.04 cal/deg mole, respectively. (1 cal = 4.1840 joules.) Heat-of-solution measurements of anhydrous NiBr2 were used to determine its enthalpy of formation (?Hf°, 298.15 K) as -50.64 ± 0.31 kcal/mole. High-temperature enthalpies of NiBr2 and NiS04 were measured by drop calorimetry to 1,150 K and 1,002 K, respectively. Enthalpies, heat capacities, and related formation functions are tabulated to 1,200 K.

Work on contract H0272007 is summarized for the period of 1 February 977 to 1 February 1978. The initial evaluation of two potential systems for the location of well casings, voids, and tunnels in coal are described. The pulse measurement equipment used for the evaluation of the antennas selected possible use with a pulse time domain system and an FM-CW system is described. A TEM horn antenna design potentially suitable for use with these systems is given. An operational comparative analysis between various antenna types is presented.

This report presents a complete qualitative picture of knowledge of the composition of petroleum as of April 1965. The material is presented in detail on two imaginative maps--one, the Hydrocarbon Hemisphere, depicts the hydrocarbons that have been identified in petroleum; the other, the Heterogeneous Hemisphere, shows the sulfur, nitrogen, and oxygen compounds that have been found. All compounds are placed on the maps according to boiling point.
