Resin-grouted bolts serve as a crucial means of stabilizing yielded coal ribs in underground coal mines. A comprehensive investigation into their efficacy was undertaken by a collaborative effort between the National Institute for Occupational Safety and Health (NIOSH) and Missouri University of Science and Technology (MST). This study involved pull-out tests in multiple locations, including six coal mines and the NIOSH research mine. A total of seventy-three (73) tests were conducted, of wide range of anchorage lengths, ranging from a short encapsulation length of 0.305 m to a fully grouted encapsulation length of 1.524 m. The test findings indicate that when short encapsulation bolts are installed at high rotation speeds, their anchorage capacity is significantly reduced, leading to failure at the bolt-grout interface. Conversely, when these bolts are installed at lower rotation speeds, they exhibit greater capacity, with failure occurring at a higher anchorage load equivalent to the yield load of steel rebar. No matter what the rotational speeds were used during bolt installation in this study, the fully grouted bolts consistently experienced failure at the ultimate load of the steel rebar. Most of the tests carried out on partially grouted bolts with anchorage lengths of 0.610 - 0.914 m have shown behavior patterns like the fully grouted bolts, although exhibiting reduced stiffness. The outcomes of this research offer a profound insight into the ways in which resin-grouted bolts enhance the stability of coal mine ribs.

The synthetic rock mass (SRM) approach and discrete fracture network (DFN) model, generated from the field surveys, are novel methodologies to study fracture rock mass behavior. In this paper, SRM and DFN methodologies were used to understand the influence of natural fractures on the mechanical response and failure mechanisms of stone mine pillars. First, a multi-stage up-scaling procedure with the homogenization process is established. Later, a stochastic sampling process on the SRM model to have further insight on the stone mine pillar mechanics is performed. Two-dimensional Universal Distinct Element Code (UDEC) is utilized to represent intact rock with the Voronoi-Trigon discretized blocks and to capture fractured rock mass behavior. The laboratory-size limestone mine rock specimens are systematically up-scaled to the average width of the field-size stone mine pillars. Then, the DFN model, generated from field data, is sampled to construct the SRM of the field-size pillars. By doing so, the effect of discrete discontinuities on pillar stability is studied by assessing the pillar strengths and failure mechanisms in various width-to-height ratios.

ABSTRACT It has been suggested that effective disaster prevention and response in mining requires that elements of emergency management be incorporated into everyday operations, planning, and decision-making, much in the same way that the industry approaches routine risk management. Since Congress passed the MINER Act in 2006, the newly mandated structural and technological advancements in mine emergency response preparation have been made and are apparent, but measureable improvement in mineworker emergency preparedness is difficult to ascertain. To begin exploring whether measurable indicators of mine workforce preparedness can or should be addressed within a risk management framework, NIOSH researchers examined the datasets from two separate research efforts—one which measured the organizational safety climate and self-reported safety performance, and the other which measured individuals’ perceptions of their own self-escape competency. INTRODUCTION In response to the mine disasters in 2006, the United States Congress passed the Mine Improvement and New Emergency Response Act (MINER Act), which strengthened existing regulations [30 CFR, Part 48] and introduced new provisions to improve mine emergency response. Since that time, new technological and structural features, such as mandated redundant communication systems, personnel tracking systems, and lifelines in primary and secondary escapeways, have been installed in underground coal mines across the country. Additionally, mine operators are required to develop emergency response plans, provide adequate supplies of breathable air throughout the mine, provide more frequent and realistic expectations training, and are required to immediately notify the Mine Safety and Health Administration (MSHA) in the event of an accident. While the structural and technological advancements in emergency preparedness are readily observable and easily measured, levels of individual mineworker preparedness remain difficult to assess. There is a general consensus that deficiencies in competence in the non-routine tasks required for individual miners to effectively respond to a mine emergency likely remain (e.g., National Research Council [NRC], 2013), and NIOSH research continues address this concern (Haas, et al., 2015; Hoebbel et al., 2018). To maximize an operation’s capacity to effectively prevent disasters and/or respond to emergency events, it has been suggested that individual and emergency management efforts be incorporated into everyday operations, planning, and decision-making in much the same way organizations manage routine risks to health and safety (Haddow et al., 2017). To the authors’ knowledge, the mindful intersection of routine risk management policies and practices and emergency prevention and response readiness have yet to be empirically studied within the mining industry.

Expectations for coal in the early to mid-1970's prompted the government and industry to pursue research and development programs to increase underground and surface mining coal productivity and net coal recovery. Several programs concerned advanced surface mining methods for thin seam coal recovery. Three such programs, along with related areas of interest, are presented in this paper, namely: (1) extended depth augering for trench-strip applications; (2) high recovery auger heads for enhanced recovery; and (3) variable wall mining in low cover longwalls. Conceptual development is examined, prototype testing is discussed, and potential operations are considered.

Geotechnical theory is reasonably well developed to evaluate the stability of rock slopes for surface mining projects. It is recognized widely that structural geology, groundwater pressures and seepage, and blasting are key factors that influence the stability of such slopes. However, our capabilities to model the structural geology, to determine rock strength along discontinuities and to model groundwater pressures and profiles are still very approximate. Plus, practical experience of the rock mechanics engineer is one of the most important inputs to the design of open pit mine slopes. To emphasize the use of practical experience, a number of case examples are reviewed here to illustrate key factors that must be evaluated. These case examples comprise a continuation of other examples presented in the Jackling lecture to the SME in 1986 by the author (Brawner, 1986). Example no. 1 In situations where the orientation of geologic discontinuities will not influence rock slope stability, extremely steep slopes can be developed, providing blasting and groundwater are controlled adequately. An excellent example of this condition is the Republic Pit of Cleveland Cliffs in Michigan. The rock slope in Fig. 1 isover400fthigh with a face angle of approximately 80". The rock is hard and massive, and blasting was carefully controlled. The original design of this slope was at45", a standard design angle at that time. The competency of the rock and the lack of critical geologic structure subsequently was recognized, and the much steeper slope angle was developed successfully. The rock proved to be so competent and the slope sufficiently stable that an underground crusher station was developed with entrance and exit tunnels excavated at the toe of the steep wall.

INTRODUCTION This paper presents (1) modified slipform system which was employed for simultaneous concrete placement for permanent lining and Instruction of a partition wall in the execution of a ventilation shaft 620m deep, and (2) current shaft sinking techniques which have been applicable and efficient for the subsurface conditions in Japan. MODIFIED SLIPFORM SYSTEM FOR PERMANENT LINING OF VERTICAL SHAFT General Information This paragraph deals with rapid construction by using modified slipform system for the permanent concrete lining and a partition wall of ventilation shaft, which has inside diameter of 620m and depth of 620m, of Enasan Highway Tunnel. The general feature of the shaft is illustrated in Fig. 1 and standard section of the shaft is illustrated in Fig. 2. The geological condition surrounding the shaft is mainly of ryolite and surface is of talus, but as a whole, they are rather weak and weathered rocks with faults, fissures and clay seams. 220mm borehole for the geological investigation was used as a drainage hole, predicting the amount of water inflow into the shaft to be 10t/min. The shaft was executed mainly by means of the conventional short step sinking method of 1.2m lift, employing temporary headframe and hoists. Exploration requires processing highly varying kinds and amounts of geologic information. Successful discovery dictates that this be done quickly with maximum confidence. Most phases of exploration are more or less applicable to computer reduction on a routine basis. The use of digital computers to reduce raw data into interpretable form, lets the exploration geophysicist concentrate on interpretation and presentation, and to consistently apply more sophisticated and comprehensive analytical techniques, not practical before. In applying these concepts for the past several years, various efficient programs have evolved which provide more complete evaluation of all data including the pro- vision for more unique solutions. This has resulted in greater refinement and ability to select the most advantageous drill sites in a minimum time at a minimum cost or, in rejecting as soon as possible a project on which additional effort is no longer justified. Practical examples of computer applications in magnetics (air and ground), induced polarization and resistivity are discussed.

With the advent of faster micro-processors, the use of numerical methods to simulate complex fluid dynamic phenomena in three dimensions for use in design has become prevalent in the automotive, and turbo-machinery industries. The Computational Fluid Dynamics (CFD) method divides the region of interest into small control volumes which form the mesh representing the physical characteristics of the problem, and uses the finite volume method to intergrate the equations for the conservation of mass, momentum, energy and species over each control volume. Recent developments in CFD software expedite mesh generation, and enable the use of unstructured grids, comprised of tetrahedral volumes in three dimensions and triangular areas in two. CFD more accurately represents complex geometries and allows for relative movement of meshes enabling simulation of multiple moving bodies. 'ibis paper presents two examples of how CFD simulation can be used to assess mine and tunnel ventilation problems formerly addressed by application of analytical solutions which were developed assuming ideal incompressible conditions. CFD simulation is used to evaluate the impact of varying the airflow in a descentionally ventilated airway on the layering along the roof of smoke and hot gases resulting from a vehicle fire. Control of the smoke layer is required to enable safe egress from the vehicle, particularly if the vehicle is for personnel transport, and to ensure control of the fire contaminants throughout the ventilation system. The airflow required to prevent layering against the ventilation direction, calculated from the Bakke and Leach relations (Bakke and Leach, 1962), is compared with the CFD simulation results. An evaluation of the pressures, generated as a vehicle enters a tunnel portal, using CFD simulation, is also presented for unflared and flared portal configurations. These simulation results are compared with predictions derived using an analytical method which assumes one-dimensional and incompressible flow. Results of the CFD simulation are presented in an animated video format. SIMULATION OF BACKLAYERING In designing a ventilation system for a transit tunnel, the ability of the ventilating air to control and prevent backlayering of smoke and hot gases resulting from a vehicle fire is of prime concern. The buoyant nature of hot smoke causes it to rise relative to the colder, fresh air provided by the ventilation system. If the vehicle fire occurs in a descentionally ventilated tunnel, the smoke may tend to move upgrade in a layer above the incoming ventilation airflow. The layer may become thick enough to engulf a substatntial part of the tunnel cross-section upgrade of the incident that comprises the evacuation route. This effect is termed "backlayering' and it is similar to the development of methane layers in mines for which most studies related to backlayering have been done. Prediction Techniques Analytical A number of studies have been conducted (Bakke and Leach, 1962) to define the characteristics of this phenomena and as a result have produced relations which are used both in the mine and transit ventilation fields to define the air velocities required to control layering. In the transit industry the air velocity required to prevent the backlayering phenomena from occuring during a vehicle fire is called the "critical velocity" (Associated Engineers, 1975) and is dependent upon a number of factors: tunnel height, cross-sectional area and grade; ambient air temperature and density; and the heat release rate of the fire. Common practice in transit ventilation design is to provide an airflow which meets or exceeds the critical velocity. In order to determine whether or not the critical velocity can be achieved with a particular ventilation system, a one-dimensional simulation of the tunnel network is typically performed using programs such as the Subway Environment Simulation program (SES) originally developed in the late 1970's (Associated Engineers, 1980). The results obtained from SES are compared to the critical velocity to determine the adequacy of the ventilation system. Computational Fluid Dynamics For the backlayering simulations, a commercial CFD code which has been used successfully in a wide variety of engineering applications, was used. It provides numerous options for modeling laminar and turbulent flows, multiple turbulence models, definition of multiple species and chemical reactions between them, a variety of boundary conditions (including constant pressure and constant velocity inlets) and the ability to apply user-defined FORTRAN subroutines. It includes the ability to model conductive, convective, and radiative heat transfer. FLUENT also permits the use of "body-fitted coordinates" to match the computational mesh or grid to complex real-world geometries. Computational Fluid Dynamics Model The model developed to simulate the backlayering phenomena is comprised of an airway of rectangular cross-section, 4 meters wide, 4.5 meters high, and 200 meters long. One of New York City?s largest subway projects is under way 43 m (140 ft) beneath the surface of downtown Manhattan. The East Side Access project, more than 30 years in the making, was halted after a partial excavation in the 1970s due to a lack of funding. The project, once dubbed the ?Tunnel to Nowhere,? has been revitalized into a groundbreaking subway line that includes unique tunnel boring machines (TBM), construction methods and muck removal systems. Project overview The East Side Access project involves construction of a new 6.7-km (4.2-mile) subway line needed to relieve heavy traffic congestion between the boroughs of Queens and Manhattan. The line will serve approximately 160,000 commuters daily between Grand Central and Sunnyside rail stations. Manhattan and Queens will be connected under the East River by the 63rd Street tunnel, a submersed double-decker tube. The submersed tube consists of reinforced concrete sections barged in place on the riverbed. The tube contains operational subway lines on the top deck, while the bottom deck (the ?Tunnel to Nowhere?) will become operational when the East Side Access project is completed in 2013.

Bergbau AG Niederrhein, a subsidiary of Ruhrkohle AG; Essen, Germany, is working coal mines on both sides of the Rhine River. The Prosper Mine in the City of Bottrop has a production of about 13,000 tons per day and has been in operation since the turn of the century. Mining has moved to the north as coal deposits south of Bottrop are exhausted. To facilitate ventilation, material handling, and transportation new shafts are being constructed in the northern coal fields. The Prosper 10 shaft is being sunk to develop the Prosper - Haniel coal field which has reserves of 280 million tons. In October, 1976 Bergbau AG Niederrhein awarded the shaft sinking contract for the 1047 m deep, 8 m inside diameter Prosper 10 shaft to Gewerkschaft Walter; Essen, Germany. Construction costs will amount to about 40 million DM with construction completion scheduled for 1981. GEOLOGY The shaft, located near the town of Kirchhellen north Bottrop, is being sunk in the Grafenwalder Horst between two faults on the south west flank of the Kirchheller Anticline. 270 m of upper cretaceus sands and marls overlay sandstone bedrock at the shaft location. The upper 124 m of this section contain highly saturated sands interspersed with marl. In order to successfully sink the shaft through this section, ground stabilization by freezing was used.

The East Bay Municipal Utility District’s Claremont Tunnel is an 18,000-foot long water tunnel that crosses the Hayward Fault zone near Oakland, California. It was originally completed in 1929. This project involved the construction of a bypass tunnel through the fault zone, including an enlarged vault section designed to accommodate up to 8.5 feet of horizontal offset. Ground conditions encountered included sheared and crushed serpentinite and clayey fault gouge, and presented specific challenges for construction. A special excavation and support sequence used concrete-backfilled side drifts as foundations for the vault steel sets. It also allowed for a 20-foot-wide top heading and bench excavation in soft and squeezing ground conditions.

This paper presents a successful case history of the ground improvement efforts and excavation of twenty-three cross passages between twin bore tunnels driven by EPB tunneling machines in Los Angeles, California on Section 1 of the Westside Subway Extension. Unique and complicated ground conditions local to the tunnel alignment include San Pedro sands, siltstone/claystone Fernando Formation, and zones of “asphalt impacted soils.” To provide water cut-off and soil stabilization, jet grouting was performed prior to TBM excavation at the location of many of the cross passages. However, upon investigating the cross passages prior to excavation, the subsurface conditions required additional measures and SEM tools to ensure successful completion of the project.

The Gatineau shaft described in this paper was an excavation of about 38 m diameter and 21 m depth constructed in Gatineau, Quebec, across the Ottawa River just north of Ottawa, Ontario. A large sewage lift pumping station-was to be constructed in this excavation. Subcontractor for the shaft excavation and initial support was Beaver Underground Structures Ltd., of Dorval, Quebec. Mr. William C. Cox of Beaver conceived the idea for the support system used in this excavation, and was responsible for the construction activities. Initial analysis and design concepts for the lining system were developed by Mr. Ronald E. Heuer. Detailed lining design and construction monitoring were the responsibility of Mr. Jean-Michel Loignon. Geologic conditions indicated by two borings within the shaft area are illustrated in Fig. 1. The shaft excavation would be in the sensitive Leda silty clay. The clay is of stiff to very stiff consistency, with an insitu vane shear strength averaging about 115 KPa (2400 psf) and a reported low value of about 87 KPa (1820 psf). Low strength values were from the middle of the clay layer, with higher strengths at the top and bottom. Average shear strength at the level of the base of the excavation was about 137 KPa (2860 psf). Remolded vane shear strength averaged about 20 KPa (420 psf), with a reported low of 13 KPa (270 psf). Based upon average strength values, the clay sensitivity is 5.8. The clay has an average water content of 53% (range 43 to 66%), which is near its liquid limit (average 56%). The clay unit weight is about 1760 kg/m3 (110 pcf). Underlying the shaft site at shallow depth is limestone bedrock within which the piezometric head is approximately equal to the original ground surface elevation. For shaft support, a -cast-in-place concrete wafer system was considered, but was discarded because of time restrictions imposed by concrete placement and curing. The United States Bureau of Reclamation is constructing the $200 million Navajo Indian Irrigation Project to irrigate 110,000 acres of land on the Navajo Indian Reservation in northwestern New Mexico. Water from the San Juan River is impounded by Navajo Dam and will be conducted for 26 miles along a series of tunnels and canals to the area to be irrigated. Tunnel No. 3 is the third in a series of six tunnels along the route. Bids for construction of Tunnel No. 3 were received by the Bureau of Reclamation in September, 1970. Fluor Utah was low bidder at $8.7 million. Ten of the 12 bids were based on excavation by tunnel boring machine. The consulting firm of A. A. Mathews, Inc., was retained by the contractor to advise on geologic and rock mechanics conditions after support problems developed in the tunnel. Tunnel No. 3 is 15,264 feet long, has an 18 foot lined diameter, rises approximately one-half inch per 100 feet in the direction being excavated, and is 6,000 feet above sea level. There is one short curve in the alignment. Minimum concrete thickness is 12 inches with eight inches from 'A' to 'B' line making a pay diameter of 21 feet, 4 inches. The tunnel is supported with rock bolts and steel pans. Mole excavation began on May 13, 1971 from a blasted starting chamber. After 46 feet were excavated the mole was shut down and the trailing muck handling equipment was pulled into place. After advancing 1,700 feet in 34 days, unanticipated ground water conditions and ground support problems were encountered slowing moling so that the next 1,000 feet of advance consumed 97 days until December

Several years prior to 1962, it became obvious that in order to develop ore reserves below the Storke Level, it would be necessary to devise a method to facilitate the ore being lifted three hundred feet to the surface or six hundred feet to the mill. The final plans called for an underground crusher complex with two stages of crushing and a conveyor belt gallery for transport of the ore six hundred feet above, to the third crushing stage. Access into the area was gained in 1962, by sinking Five Shaft approximately one quarter mile southwest of the proposed complex site, and drifting into the area. Collar elevation of Five Shaft is three hundred feet above the 600 Level Crusher Complex. All muck was loaded into ten-ton granby cars, hoisted to the surface and dumped. (See Figure 1 for the relationship between Five Shaft and the Crusher Area) Drifting into the Crusher Area contunued until 1965 at which time the Crusher Room Cutout was started. The Crusher Access Drift (CAD on Figure 2) war driven and mining pm- ceeded according to the description on drawings marked Figure 3a and 3b. After the arch portion of the Cutout was excavated and concreted, removal of the center "pillar" commenced. This muck was removed with an Eimco 123C crawler-mounted front-end loader. (See Figure 4) A total of 576,000 cubic feet of rock was removed from the Crusher Cutout. All sup- port was afforded by grouted roofbolts on a specified pattern, as shown in Figure 3c. Crane support columns were put into place and held by braces welded to roofbolt heads. Sidewalls and endwalls were formed and concreted. (See Figure 5 for the finished product)

INTRODUCTION Development work at Carr Fork Mine of Anaconda Copper Company in Tooele, Utah commenced in August 1974. A decision to install an underground gyratory crusher for primary crushing run-of-mine ore and waste was made in 1976. Higher throughput rates, larger reduction ratios and greater flexibility in terms of meeting immediate and expanded production needs were the main reasons for selecting the gyratory crusher over the jaw crusher. UNDERGROUND MATERIALS HANDLING Underground crushing and conveying system was designed for a capacity of 16,300 mtpd of ore and waste on a 2-shift, 5-day week basis. Engineering and design of crusher complex was performed by Anaconda engineers at Carr Fork. Figure 1 represents the longitudinal section of mine showing the general materials handling system. Mine area is trackless except for materials and production haulage which is run on rail track on both 1200 and 970 levels. Ore from stopes and waste rock from development headings are handled by 3.8 cu m (5 cu yd) Jarco scoops and dumped into 2.4 m (8 ft) diameter ore and waste passes at the 1200 level. The material gravitates down to the 970 level where it is loaded into 12 cu m (420 cu ft) ASEA mine cars using side loading chutes. INTRODUCTION Mount Isa Mines Ltd. operates a copper and silver- lead-zinc property at Mount Isa in northwest Queens- land, Australia. Mount Isa is located in the tropics at latitude 20°43'S and longitude 139°30’E (Fig. 1). It is on the upper watershed of the Leichhardt River and at an elevation of 365 m above sea level. The average rain- fall is 360 mm/y, the greater part of which falls in January to March (the hot summer months). The mean summer temperature for these months is 30°C (dry bulb), with a maximum temperature of 43°C; the mean wet bulb temperature is 25°C with a maximum of 28°C. Mount Isa is located geologically towards the western edge of the Precambrian Shield of northwestern Queensland. This complex forms a north-tilting dissected peneplain which extends in the north-south direction for 640 km, with widths varying from 32 km to 190 km. The combination of differential erosion, regional strike, and steep dips has produced a corrugated topography of low north-south orientated hills and valleys. The field was first discovered in 1923 as a silver- lead-zinc field. Mount Isa Mines Ltd. was formed the following year and subsequently became the sole operating company on the field. Lead and copper ore have been mined and treated separately and simultaneously at Mount Isa since 1953. Intensive exploration commenced in 1954 and an active exploration program provided the justification for a series of production expansion programs in both silver- lead-zinc and copper (see Table 1 ). The most recent and largest production expansion program at Mount Isa commenced when a decision was reached in December 1969 calling for an increase in copper production from 100 000 to 150 000 t/y of blister copper. A major factor which influenced this decision was the substantial additions to the ore reserves in the so-called 1100 ore body which is a massive copper sulfide ore body with a known strike length of approximately 2500 m, a height which is variable up to 300 m, and a width of generally 300 m up to a maximum of 370 m (see Figs. 2 and 3). Host to the copper mineralization is a complex and variable rock generally called "silica dolomite." The ore body strikes 6.02 to 6.1 rad (345° to 350°), dips west between 0.78 and 1.13 rad (45° and 65°), and is truncated at depth by fault implaced basement rocks. It first occurs some 600 m below the surface and bottoms at about 1000 m below the surface. The unconfined compressive strength of the silica dolomite ore varies from 150 to 250 MPa, whereas the basement rocks, carbonaceous mylonite, slaty shale, buck quartz, and greenstone (a chloritic schist) are considerably weaker, varying in strength from 20 to 80 MPa. The distribution of mineralization within the ore body is variable, but can be considered as generally high grade at or just above the basement contact and diminishing towards the upper reaches of the ore body. INTRODUCTION Soft ground tunnels may be above the ground water table or they may be subaqueous. The ground may be clays, silts, sands, gravels, glacial deposits, boulder tills, and any of these in any combination. Because of the relatively low strength and deformability of soils, difficulties often arise in soft ground tunnelling. These soil characteristics and resultant tunnelling difficulties, have had a direct influence on the development of "Lovat" Tunnel Boring Machines. We have been required to recognize and adapt to unusual ground conditions and to follow through with design, fabrication, and field maintenance to ensure dependable, trouble-free operation. The basic "Lovat" machine is a fully shielded, wheel-type with the cuttinghead driven by a monorace type bearing with gear drive incorporated in the bearing. Mechanical innovations have been added to the basic machine which complement the resources of manpower, equipment, and materials available to the contractor for a specific operation. FULL FACE SUPPORT The installation of hydraulically operated closure doors or flood control doors, at the cuttinghead, is one of the early "Lovat" technical innovations.
