Powered haulage continues to be a large safety concern for the mining industry, accounting for approximately 50%of the mining fatal accidents every year. Among these fatal accidents, haul-truck-related accidents are the most common, with 6 of 28 and 6 of 27 fatal accidents occurring in 2017 and 2018, respectively. To better understand why these accidents continue to occur and what can be done to prevent them, researchers reviewed the 91 haul-truck-related fatal accidents that occurred in the USA from 2005 to 2018 and performed bow-tie analyses using the final reports published by the Mine Safety and Health Administration. The analyses explore the context of the accidents with a focus on the initiating event, event outcome, hazards present, and possible preventative and mitigative controls. Overall, the vast majority of the accidents resulted in a haul truck colliding with the environment, and the majority of these events were initiated by loss of situational awareness or loss of control. The majority of the hazards were related to design and organizational controls. The results of this study suggest a need to investigate operator decision-making and organizational controls and to focus on improving design and operation controls such as mine design and operational procedures.

The San Juan Coal Mine, located near Farmington, New Mexico, supplies the San Juan Generating Station with more than 6 million tons of coal annually. To replace dwindling surface mine production, San Juan installed a longwall in 2002. The roof rock in much of the underground reserve consists of interbedded coals, carbonaceous shales, and mudstones. Roof support consists of fully-grouted roof bolts, both tensioned and non-tensioned, supplemented by modified cable trusses. Short-encapsulation pull tests conducted by San Juan’s geotechnical engineering department showed that some of the immediate roof layers provided variable bolt anchorage. To ensure that the roof bolts were obtaining the necessary anchorage, additional tests were conducted in a variety of locations. The results have been used to determine the optimum support design for different areas of the mine. Another series of tests evaluated the performance of several bolt and hole diameter combinations. The experience at San Juan Mine shows that short-encapsulation pull tests can provide the information necessary to improve ground control in variable roof conditions. It also illustrates how a pro-active geotechnical engineering program can be an essential component of a state of the art underground coal mine.

Seventy-four human factors research and development projects were identified for possible Bureau funding. The projects were based on (1) literature surveys, (2) reviews of accident statistics and reports, (3) interviews with equipment manufacturers, MSHA, and Bureau personnel, and (4) site visits to 26 mines. The projects are divided into the following categories: those related to (1) mobile equipment, (2) maintenance shops and areas, (3) plants and mills, (4) organizational factors, (5) safety programs/data, and (6) training. The projects were rated by industry and MSHA people in terms of benefit to the industry. Projects were placed into three categories of priority. The highest priority contained 25 projects with 26 in Category 2, and 23 projects in the lowest category.

The Triton lode is a schistose feldspar biotite chlorite rock with quartz, carbonate, pyrite and some pyrrhotite. Its more chloritic sections strongly resemble the enclosing greenstones, its more feldspathic portions are indistinguishable from the flne-grained phases of the porphyry dyke into which the southern extremity of the ore body merges. The ore body' has a length of 880 ft. and an average width of I2t ft.; in places this widens to as much as 30 ft.The average dip is 85° west and pitch of the ore shoot 75° south.METHOD OF SAMPLING.With the exception of a limited amount of channel sampling (a reference to which is made elsewhere in this paper) all the sampling is by chip sample.The samplers use a hammer weighing approximately 11 lb. and chip directly into the sample bags. Sampling is generally carried out over 3-ft. sections and portions thereof to the nearest half foot-thus a face 8t ft. wide is divided up into two sections of 3 ft. each and one section of 2 ft.-no measurements of less than 4 ft. are used and calculations are all in feet and dwt. per long ton.In the oxidised stopes wp.ere the lode is much narrower, .i.e., 6 ft., sampling is over 2-ft. sections. Award of the Design/Build Agreement for construction of Ontario Power Generation’s (OPG’s) Niagara Tunnel Project was made after an intensive 8-month international procurement process. The 14.4-m diameter × 10.4-km long rock tunnel will divert 500 m3/s of water from the Niagara River to the Sir Adam Beck hydroelectric generating complex. The procurement process and Design/Build Agreement included features to address the owner’s requirements regarding risk allocation and cost and schedule certainty, while encouraging innovative ideas from the industry, both technically and commercially. The process included an international expression of interest and prequalification, followed by an invitation for design/build proposals to qualified proponents, and comprehensive proposal evaluations and negotiations with several respondent proponents. An honorarium was provided to the unsuccessful participants in the proposal stage. The resultant negotiated Design/Build Agreement includes Owner’s Mandatory Requirements, negotiated contract terms and conditions, performanceincentive bonuses and liquidated damages for schedule and water delivery performance, a multistaged Geotechnical Baseline Report, joint risk management, references to preexisting community impact agreements and measures to reduce impact on tourism. It also addresses regulatory approvals and permits, an owner controlled insurance policy that required reference to the International Tunneling Insurance Group (ITIG) Code of Practice for Risk Management as a condition of coverage, site specific safety, security and environmental plans, formal team building, a dispute review board, and escrow documentation.

The Cuyuna iron range in Crow Wing County, Minn., is the principal producer of manganiferous iron ore in the United States. The ore is mined from a manganiferous iron formation in the bedrock of the Cuyuna district. This iron formation is divided into two general lithologic types-thin-bedded and thick-bedded. The unoxidized, thin-bedded iron formation is composed of iron silicate with associated fine-grained carbonates of iron and manganese. The thick-bedded iron formation carries iron and manganese oxides with intimately associated silica. Oxidation and leaching of the original iron formation have been the most influential factors in producing manganiferous iron-ore concentrations in quantities large enough to exploit on a commercial basis. The unoxidized and the partly oxidized and enriched iron-formation materials are suitable as a source of ferrograde manganese ore. These materials have been classified according to their amenability to mineral dressing and degree of oxidation and leaching.

Introduction If one had been called upon to describe petroleum oil refining seventy-five years ago, he could have acceded to the request quite satisfactorily in about fifteen minutes. Fifteen years ago, an hour and a half would have been sufficient to give a general idea of oil refining, and of the equipment used for this purpose. Today, so extensive has been the research into all phases of the refining and manufacturing of petroleum products, and so rapid the strides in improvement in technique and equipment, that I doubt if a week would suffice to give an adequate outline of ail the ramifications of present-day oil refining. I propose to only 'hit the high spots', dealing particularly with the equipment and methods employed at the Imperoyal refinery, which you are to visit. Crude oil, the refiner's raw material, contains essentially only two elements, hydrogen and carbon. These are not present in the free state but are combined in various proportions as 'hydrocarbons'. A given crude oil is not made up of just one hydrocarbon, but of myriads; some solid, others liquid, and still others gaseous at atmospheric temperature and pressure, ail dissolved one in the other. Moreover, the nature and the relative amounts of the several hydrocarbons vary very considerably in crude oils from different sources. That is, a 'crude' from Oklahoma will contain hydrocarbons not present in crude from Mexico, and is different again from the Venezuela crude. Not only this but oil in a certain well in, say, Peru may be differently constituted from that in a well half a mile distant from it.

INTRODUCTION NUMEROUS examples of troubled zones, interfering with the normal development of coal seams, have been cited in the literature. From a geological point of view they can be subdivided into structural and sedimentary troubles. Of the latter type, wash-outs are usually the most bothersome. Some years ago, A. A. Thiadens and the author arrived at the conclusion that they represent fossil river-beds. A method of predicting the course of these rivers was developed, which has been successfully applied in the Netherlands coal measures, and more recently by P. Stassen in some of the Belgian coalfields and by the author in the Sydney coalfield. The Sydney coalfield has long .drawn the attention of geologists, and three main periods of systematic geological research may be distinguished. The first period, 1872-1900, is covered by the studies of Ch. Robb and H. Fletcher (32, 10-13) (1). Their detailed sections of the cliffs have lost nothing in value, and are frequently consulted today.

Tunnel excavation using tunnel boring machines (TBMs), has become increasingly common in recent years, despite the fact that precise evaluation of certain risks have not kept pace with the use of these machines. One of the risks easily overlooked by Engineer and Contractor alike are the effects of abrasive ground on the costs and schedule of a given project. The impacts of worn and damaged TBM cutter heads have been observed on hundreds of tunnel projects around the world. It would appear that a reliable prognosis of the abrasiveness of soils on a project would be of great importance for designers, clients and contractors alike. Several well acknowledged test and prognosis methods already exist for rock, however there is only very limited knowledge available to describe the abrasiveness of soil and its impact on soft ground TBMs. This paper will examine approaches to this problem and suggest a new approach based on a current project undergoing design.

According to industry representatives, U.S. producers were listing unalloyed 95.5 percent aluminum ingot in October at 53-57.5 cents per pound, but reportedly much is being priced at 53 cents. New York dealers raised their October positions by about 3 cents to 56-56.5 cents, mainly to cover spot supply shortages. London Metal Exchange 3-month forward contracts traded up in October from 52.3 to 57.3 cents per pound and closed at 56.4 cents. ASBESTOS The Jaquays Asbestos Corp. has announced the imminent closure of its mine and mill in Globe, Arizona. This is the last asbestos producer in the State having operated continuously, except during the early thirties, since 1914. Cost of compliance with environmental regulations was given as the reason for closure.

Since human daily demands for fresh water have been increased significantly, the supply of fresh water for the mining industry, especially froth flotation, should be considerably reduced. In addition, stricter environmental standards have been implemented in many countries to reduce the risks due to the disposal of wastewater, many flotation operators have to seek more efficient ways to utilize the limited fresh water sources. Recycling water with high concentrations of impurities is a normal strategy for most of the mineral processing plants; however, water quality should be carefully considered to guarantee a smooth flotation process. Recently, the application of seawater in flotation has attracted much attention due to its availability and abundance rather than fresh water. However, the influencing mechanisms due to the application of seawater in flotation have not been fully understood especially the reactions between mineral particles and bubbles in the presence of high concentrations of cations and anions. This would impede the application of alternatives to fresh water in the flotation. Therefore, this paper reviews the current water sources and the technologies for water recycling in the flotation process. Specifically, the life cycle of water in flotation was proposed. In addition, the strategies including high pulp density flotation as well as the most promising alternatives to fresh water have been discussed in details, with the related mechanisms being demonstrated.

The mining industry has been relatively slow in adopting these technologies compared to the other industries, but this scenario is changing fast.Challenges. Mining companies globally are going through a series of challenges such as declining ore grades with complex metallurgy, increasing depth of ore bodies, high capital and operating costs, lower productivity, shortage of technical resources, significant pressure from stake-holders to deliver, along with growing environmental and corporate social responsibility issues. Improvement in profitability would need significant improvements in operational performance.Consequently, over the last few years, there has been a major focus among some mining companies to imbibe operational excellence. This has been only possible because of the replacement of a silos-based approach by an integrated view of the organization. Such an integrated view is now possible via the evolving digital technologies. Even so, such a transformation calls for a change in culture, which, for most organizations is not trivial. Attempts in the past to integrate functions, such as through mine-to-mill initiatives in the mining industry, have demonstrated significant benefits to many operations (McKee, 2013). Still, sustaining these benefits has clearly been a challenge due to manpower turnover and lack of proper systems and structures that integrate information between business functions within an operation and between operations and the corporate level.Opportunities. "The management of outlier grades in positively skewed gold distributions is a contentious issue. Incorporating outliers in standard ordinary kriging (OK) estimation procedures in a way that honours the data without smearing extreme grades into surrounding areas has been problematic. Cutting or capping of outliers to mitigate their influence in estimation techniques is common practice, while methods that manipulate the OK system of equations fail to honour the data. We propose a method of post-processing of kriging weights that provides realistic OK estimates and mitigates smearing without manipulating kriging equations or changing the original grades. The method requires that the data is not clustered, is approximately equally spaced, and is of the same support. Positively skewed data is ordered on attribute grade and nonlinearly transformed to a Gaussian histogram of categorical bins whose frequency is based on their likelihood of occurrence and location in the sample distributions. Factors that restrict kriging weights are calculated by dividing the percentage frequency of data in each bin by the percentage frequency of data in the bin with the highest frequency. Restriction factors applied to the kriging weights in the OK estimation restrict the range of influence in proportion to their probability of occurrence in the distribution. Smear reduction post-processing is easy to implement and addresses issues arising from negative kriging weights while considering the spatial location of samples, the sample grades, and their probability of occurrence. The method mitigates both smoothing and conditional bias.IntroductionOutliers are sample observations that deviate considerably from the standard or expected (Hawkins, 1980). These deviations can be high or low in grade depending on the distribution. The presence of outliers in a mine sampling campaign could provoke mixed reactions. Mining engineers might view high-grade outliers as a promise of achieving higher grades, while the geostatisticians view outliers as affecting the accuracy and precision of the overall estimate, among other issues. Having identified outliers, the geostatistician must decide how to account for their presence and treat them so that true underlying estimates are not distorted. Advanced knowledge of the deposit allows the geostatistician to identify and treat outliers acceptably, especially where samples are limited in number and the true grade distribution is uncertain. If ignored, or treated incorrectly, outliers are likely to lead to smearing of extreme grades into the surrounding estimates, thereby steering regions towards potential under- or over-estimation."

Concentrations of bright green material in the "Golden Mile" of Kalgoorlie, which carry particularly high gold values, are known locally as "green leader".This assemblage consists largely of quartz, sericitic muscovite, carbonates, and pyrite. Some of the sericitic muscovite contains vanadium, giving the rock its green colour. Vanadium oxides, including nolanite, are also present. Tellurides identified in this study are calaverite, altaite, petzite, melonite, and coloradoite. Native gold was found in most of the samples; some of the gold, at least, has been formed from the decomposition of gold tellurides.It is suggested that mineralizing solutions carrying gold and tellurium leached vanadium from the host rocks and subsequently deposited gold tellurides and vanadium-bearing minerals in the "green leader".

To mitigate and control the combined sewer overflows from 17 outfalls in the City of Detroit, three outfalls in Dearborn Heights and eight outfalls in Redford Township, the Detroit Water and Sewerage Department has completed the preliminary design of a storage tunnel in 2005. In 2006, a final design has revised the alignments and the inside diameter (ID) of the tunnel. The revised tunnel system includes an 11.28-km(7-miles) 9.14-m (30-ft) ID tunnel, a 24.4-m (80-ft) ID pump station, two work shafts about 15.2-m (50-ft) ID each, and 14 drop shafts with various IDs. This paper discusses the latest development of this tunnel system.

This U.S. Bureau of Mines report discusses wire ropes used for hoisting in U.S. mines. The in-formation presented was extracted from a large body of data compiled by the U.S. Mine Safety and Health Administration (MSHA) using a data base management system (DMS). Further, these extracted data were then manipulated to provide tables of data that show the types and amount of wire rope used for hoisting. This report describes the different types of wire ropes and how a DMS can be used to manipulate MSHA's wire rope inventory data. The data indicate that more 1-1/4-in-diam ropes, 6x27 construction ropes, and right lang lay ropes are used in U.S. mines than ropes of any other diameter, construction, or lay type.

"Through-the-Earth (TTE) radio has been proposed as a means of emergency communications in mines. The propagation of a TTE radio signal depends chiefly on the subsurface conductivity distribution. Robust forward models of signal propagation can be computed based on a realistic conductivity estimate. These models can be used, for example, to optimize frequency, estimate signal range, identify zones of anomalously strong or weak signals, or to optimize the position of the above-ground TTE radio unit. A method was developed to determine the conductivity distribution of a mine overburden using TTE radio transmission data. At multiple locations in an underground test mine, we recorded a wideband electromagnetic signal, spanning a frequency range of approximately 500 to 8,500 Hz, transmitted from a vertical magnetic dipole at the surface. Using a finite-difference time-domain forward modeling code, a layered Earth conductivity model that accounts for topography was fitted to the observed data, which minimizes the difference between the calculated and observed attenuations for each transmitter-receiver pair, for each frequency of interest. Introduction and objectiveThrough-the-Earth (TTE) radio has been proposed for emergency underground communications. The most common type of TTE radio is based on the transmission and reception of signals by electromagnetic induction. These signals are in the extremely low to very low frequency range — in practice, between 400 and 9,000 Hz. This technology was historically developed for mine safety, although recently other applications have been suggested, such as communications in civilian infrastructure like high-rise buildings or tunnels, in the military, and even for wireless detonation and activation. In a coal mine, which is a setting of particular importance for TTE radio due to the higher risk of accidents, day-to-day communications can be done through a wired link. High-frequency line-ofsight systems, whose signals cannot penetrate rock, are also used for communications from nonfixed points. Another option is a leaky feeder system, which is a permutation of a line-ofsight system and a wired link, where a partially shielded coaxial cable allows the signal to “leak” (Delogne, 1982). In an incident such as a mine collapse or explosion, a TTE radio system can still provide a communication link, as wired systems can be severed, and line-ofsight systems cannot penetrate through collapsed portions of the mine, or to the surface (Forooshani et al., 2013; Yarkan et al., 2009)."
