NIOSH has developed an active proximity warning system called HASARD (Hazardous Area Signaling and Ranging Device) for warning workers as they approach known dangerous areas around heavy mining equipment and other dangerous work zones. HASARD is composed of a transmitter and a receiver. The transmitter generates a 60-kHz magnetic field using one or more wire loop antennas. Each antenna is adjusted to establish a magnetic field pattern for each hazardous area. The receiver, worn by the worker, is a magnetic field strength meter. The received signal is compared against preset levels which are calibrated to identify levels of danger. The receiver outputs can include visual, audible, and vibratory indicators and it can also be made to disable machine functions. HASARD was field tested on a Joy 12 continuous miner, a Komatsu 210 M Haulpak, and on a highwall launch vehicle. Slight modifications were made for each application. HASARD provided warnings as designed and proved to be rugged enough to withstand the harshest of production environments.

Svedala started a development program in 1997 to develop a process to convert waste tires into valuable commodities. A review of numerous approaches revealed short-comings in the various approaches either process, mechanical, combination of factors, or trying to emphasis the production of low value products. A market study revealed that a process that could produce a good grade of carbon black would be economically feasible. This paper discusses the laboratory and pilot plant development and commercial plant design.

A cover with capillary barrier effects (CCBE) is being constructed on the AA Leach Pad at the Barrick Goldstrike Mine site near Carlin, Nevada. The purpose of the cover is to eliminate/minimize infiltration of meteoric waters into the leach pad. Laboratory and numerical analyses indicate that Carlin Formation siltstone (TCS), a locally available material, has the necessary hydrologic characteristics when placed on the leach pad to act as a CCBE. A pilot study has been conducted on a small-scale test cover placed on the AA Leach Pad to examine the cover performance under irrigated conditions. The preliminary test results demonstrate that the designed cover is capable of preventing meteoric water infiltration.

Industrial mineral production in Alberta, worth $468 million in 1997, comes from a dozen types of industrial minerals, mined by about 400 producers. Cement and lime from Paleozoic limestone formations exposed along the Rocky Mountains account for the greatest production value. Sand andgravel is the primary source ofmineral aggregate. Almost two-thirds of this aggregate is consumed in the Edmonton and Calgary regions. Sulphur produced at 48 sour gas processing plants and two oil sands plants was valued at $84.8 million in 1997 and accounted for 80% ofCanadian sulphur production. Four companies solution mine the Elk Point salt beds to produce chlor-alkali chemicals, industrial salt and sodium chlorate. Natural brine is also produced as a dust control product. Al $22 million, peat moss is a major industrial mineral product in Alberta. Other minerals with noteworthy production include shale, stone, silica and ammolite. The commodities with the greatest development potential are diamond, silica and oil sands tailings by-product minerals.

La segunda fase, integra todos los sistemas de supervisión y control de planta con los sistemas de seguimiento “online” de Mantenimiento (fase 1), desarrollando un solo sistema de tratamiento y análisis de información de planta; y la tercera fase comprende la instalación y el desarrollo de aplicaciones basadas en tecnologías Cliente/Servidor con base SQL, que finalmente proporcionan la información y los medios para realizar consultas que optimizan el seguimiento y toma de decisiones relacionado al mantenimiento preventivo/predictivo.

This report summarizes ongoing research at the National Institute for Occupational Safety and Health, Spokane Research Laboratory, in which collision warning systems for surface mining dump trucks are being evaluated. Common accidents involve these large trucks running over smaller vehicles or pedestrian workers. Collision warning systems currently use one of several methods, including radar, radio-frequency-signal detection, or ultrasonic signals, to detect and warn of the presence of an object or person in the blind spots of the mining equipment. Most available systems have not been tested on large off-highway dump trucks. This report evaluates several systems on two sizes of trucks, a 50-ton-capacity truck commonly used in quarries and construction and a 240-ton-capacity truck commonly used in open-pit mines. Tests were conducted to determine false alarm rates, alarm effectiveness, and reliable detection zones for a person and a pickup truck. The results indicate that radar and radio-frequency identification systems show promise for this application and that several of the improved systems are ready for extensive field tests. However, challenges still exist in applying these technologies to large trucks.

Purchasing a new set of longwall shields requires a substantial investment. A poor shield design can lead to economic hardships, safety concerns for the mine workers, and closure of the mine. This paper addresses several key points that should be considered in the procurement process: (1) understanding your goals and the logic in selecting a higher capacity shield, (2) the importance of completing performance testing before production shield fabrication begins, (3) making sure performance testing is properly done, (4) measuring load(stress) development during performance testing, (5) testing a shield to failure, and (6) the value of buying extra shields. In addition, several challenges are proposed for consideration in future shield designs. These include (1) 100,000 life-cycle expectancy, (2) improved hydraulic diagnostic capability, (3) smart load control by optimizing setting pressures, (4) lubricated link joint design concepts, (5) composite material applications to reduce shield weight, (6) incorporating periodic weighting predictive algorithms into the data-processing software, (7) advanced component load measurement on two to three specially instrumented shields to detect loading in the structural components, and (8) constant set leg cylinder design.

Maintaining ground stability in the gate roads, particularly the tailgate, has always been critical to the success of longwall mining, both in terms of safety and productivity. Several new support technologies have been developed in recent years to replace conventional wood and concrete cribbing for secondary roof support. Since their performance characteristics are unique, the best practices that have been developed with conventional wood cribbing may not be applicable for these alternative support technologies. Therefore, with so many options to consider and the importance of achieving adequate ground control at minimal cost, the trial-and-error approach to longwall gate road support is no longer prudent. This paper discusses a design methodology for standing secondary tailgate supports. This design technique requires in-mine measurements of tailgate support loading and convergence to establish a tailgate ground reaction behavior based on support and strata interaction. The methodology uses the performance characteristics generated in the National Institute of Occupational Safety and Health's (NIOSH) Mine Roof Simulator (MRS) to match the stiffness and load characteristics of various supports to the measured ground reaction behavior. It can be used to determine the appropriate application of alternative roof support systems or to design in-mine trials so that a fair and equitable comparison of different support systems can be made. A case study of the methodology at a western Pennsylvania mine site is presented in the paper, including a comparison of four alternative support technologies to the conventional wood and concrete cribbing historically used at this particular mine.

This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. Presplit blasting in strip mines is aimed at creating a safer wall to work under during the coaling operations. Good quality presplits are also necessary for effective throw blasting and drainage of the blast block. A less obvious but important function is full coal recovery. Optimum Colliery has been experiencing rock freezing against the presplit highwalls. The rock formed in patches along the highwall and trapped coal beneath it. The material that was &ozen comprised the rock between the back row of holes and the presplit and was often up to 3 m thick (the stand off distance between the back line and the presplit). The cozen rock formed predominantly along the bottom halfor bottom two thirds of the highwall. Along the top portion of the highwall, the presplit crack was usually well formed with visible barrels. This problem, or similar variations of it, is common on many South African strip-mine operations. Investigative work was carried out at Optimum Colliery to correct the problem. This paper presents the information and measurements that were made and discusses the solutions that have been applied to solve the problem. It was found that there were two independent causes for the tiozen material, with each contributing more or less to the frozen rock along the highwall. It was found that the presplit was not forming properly in places, especially closer to the bottom of the highwalls. The reasons and the solutions are presented. The second cause was found to be associated with reduced detonation velocity in the back line holes coupled with low confinement from long delay periods. Emulsion explosive that is sensitised with air bubbles was found to be prone to this phenomenon near the back of high-energy blasts. A change in the explosive formulation in the back-line holes, together with reduced delays in the back line, has resulted in significant improvements. The details are presented in this paper.

Mintek and BacTech have joined forces to provide the metallurgical industry with bacterial-oxidation processes for the treatment of sulphide ores. The processes are aimed at both refractory gold-bearing and base-metal concentrates. Previously, BacTech had commercially implemented the refractory gold process at the Youanmi Gold Mine in Western Australia. More recently, the process has been incorporated in the Sulphide Treatment Plant at the Beaconsfield Gold Mine in Tasmania, Australia. In addition, Mintek and BacTech have recently signed an agreement with the Laizhou Gold Metallurgy Plant in China?s Shandong Province, which will see the third commercial realization of the technology for refractory gold. Development of the base-metal technology is the subject of several papers in the proceedings of this colloquium. The implementation of the refractory gold technology at Beaconsfield is discussed in detail. Some important aspects that are covered include: ?Test work, and the choice of bacterial culture ?Process design choices, and the advantages of the Mintek-BacTech process ?Inoculum build-up ?Plant commissioning ?Plant performance. Besides the Laizhou project, Mintek and BacTech are currently investigating several other commercial prospects for the technology. These include the use of the technology for a gold-copper deposit, where the production of both gold and copper is envisaged.

Mineralisation was identified at Gladstone Hill as far back as the late 1870s-early 1880s, shortly after the initial discovery of mineralisation at what is now the Martha Mine. Indeed, early recovery problems associated with the Martha mineralisation led to the preferred development of ore bodies within what is now the Union Hill Joint Venture tenement; ore bodies such as Silverton, Amaranth, and Union to name a few. Gladstone Hill forms the basis of the Union Hill Joint Venture, a 50:50 joint venture managed by Au-Ag Resources Ltd (a fully owned subsidiary of Otter Gold Mines Ltd), and Welcome Gold Mines Ltd (a fully owned subsidiary of the Normandy Group). The joint venture participants also operate the adjacent Martha Mine. Mineralisation of Gladstone Hill, as with mineralisation at the adjacent Martha Mine, has been interpreted as a low sulphidation epithermal Ag-Au system hosted within hydrothermally altered andesites of the lower Coromandel Group. Drilling in recent years has successfully identified extensive mineralisation associated with a number of vein sets throughout the tenement including the Gladstone Vein set, the Moonlight Lode and more recently along a north-easterly trend towards the historic Favona workings. A wide range of vein styles is apparent, ranging in character from thin stock works to broader lode-style banded chalcedonic to cherty quartz and have been interpreted as representing multiple phases of emplacement. At shallower levels mineralisation is often hosted by vein clasts within vent breccias. Visible gold is rare, with mineralisation generally associated with very fine-grained pyrite, arsenopyrite and minor electrum. Percentage levels of combined sphalerite-galena-(+/- chalcopyrite) have been intersected at depth however these have been gold poor to date. The potential for the Gladstone Hill area remains to be clearly defined as drilling to date has yet to close off mineralisation both along strike and down dip. If further drilling is successful, the joint venture partners believe Gladstone Hill could contribute significantly to the economic future of current operations.

The presence of an extensive soft siltstone layer in the middle of the inter-burden sequence of rocks at Syferfontein Colliery in the Republic of South Africa prompted tests to determine the effect of different mid-column decks of inert material in the siltstone layer. The objective was to achieve significant cost savings in explosives use whilst not degrading dragline productivity. An important factor that had to be considered was that the initiation systems available in South Africa are not accurate enough to ensure simultaneous initiation of explosives decks in a blasthole. A number of inert materials were tested including ash, drill cuttings, river sand and crushed stone. It was found that the ash, drill cuttings and river sand did not effectively attenuate the shock wave and that the risks of sympathetic detonation or partial disruption of one or the other explosives deck in a blasthole was high. This became much worse when the inert material was saturated with water. Crushed rock was the most effective in attenuating the shock wave, but this material was very expensive to use because it had to be imported onto the mine.
