The main objective of occupational health and safety research is to minimize or eliminate the events that may cause fatal or non-fatal injuries to human workers. A commonly used technique is to devise an “incident prevention plan”, which is more often the product of thorough investigations of past reported incident events. Incident documentation and methodical reporting and systematic and thorough data collection, often help identify the root cause of the incident – in other words, the etiology of the incident. These retrospective analyses of the incidents can efficiently identify the future steps to take to achieve the objective of minimizing occurrence of “events of interest”. This paper introduces a novel technique – “Taxonomic Analysis”, which identifies the root causes of an event (an incident in this case) and can provide future direction for corrective measures to reduce the probability of occurrence of the event. A taxonomic analysis involves systematic and organization of data based on observation, description, and classification. This paper utilizes the rock fall related incident narratives, available with the Mine Safety and Health Administration (MSHA) database (1984-1999), for a taxonomic analysis. The study found that 67.6% of groundfall incidents occurred in supported areas with the remainder (32.4%) in unsupported areas. Some form of rock mass failure appears to cause about 50% of groundfall incidents, although in 39.7% of the overall groundfall incidents or two-thirds of those involving rock mass failure, the exact nature of the rock mass failure could not be determined from the narratives. Human activities factors such as scaling or barring down, drilling or bolting, and setting timbers or cribbing accounted for another 34.5% of the groundfall incidents. Finally, support system failures made up the remaining 15.5%. Results from this taxonomic analysis may suggest focus areas toward which preventive measures and future research activities could concentrate.

AMERICANS like engines, but more than anything they like powerful engines, and next to that they want them quiet, silent, smooth; perhaps a slight purr might be permitted, but they must not knock. To provide a gasoline to fit all these requirements, American petroleum technologists have experimented for years, and mounting octane numbers of the gasolines the refiners have produced both before and during the war bear witness to their success. The question on everyone's mind now is what may we expect in the way of engine fuel in the postwar period? The gasoline drouth during the war rationing, the inconvenience of doing without a car were major annoyances to all motorists, but they put up with them pretty cheerfully for the most part. However, now that the war is over, and the jalopies are all out and the better cars have been taken off the jacks and brought out on the road, the call is for "Some of that good gas-fill her up, if you please."

The literature dealing with the electrical conductivity of solid dielectrics, including all its aspects and its relationships to other physical properties of these materials, is extensive and scattered in many books, phamphlets, and periodicals. It is believed that most of the important contributions to this field up to October 1943 have been listed and abstracted in this circular. Also included are some general references on static electricity. Methods of removing static charges are described; methods of and conditions for measurements of resistivity are discussed; equations for the calculation of resistivity are given; and methods of resistance measurements in Situ are reviewed. Data on the variations of resistivity with changing conditions as well as explanations for them have been collected, and three tables showing the resistivity of various dielectrics at different humidities are included. The appendix contains a short development of the mathematical formulation of the charging and discharging currents due to interfacial polarisation. It is a precaution readily applied to stationary machines anti may often be utilized to good advantage to rid persons and portable equipment of an acquired charge. There are many situations, however, in which direct low-resistance grounding cannot be used, either because the charged bodies are insulators or are inaccessible, or because contact to a ground lead might give rise to a hazardous spark. If the charged body is a conductor, contact made to a neighboring conductor, even though the latter be insulated from ground and of relatively small capacitance, may also result in the occurrence of an incendive spark. In such instances, static electricity can sometimes be removed most satisfactorily by means of strong atmospheric ionization produced by radioactive material, X-rays, ultraviolet light, or A. ALABAMA'S mineral industry continued to expand. This was a record year for many commodities, with expansion in almost all industries. New records were established for the production of native asphalt, fixe clay, miscellaneous clay, kaolin, bentonite, lime, scrap mica, salt, sand and gravel, crushed limestone, crushed marble, and tale. Among the States, Alabama ranked second in the production of bauxite, third in the production of scrap mica and native asphalt, and fifth in iron ore. The mineral industry of Alabama was dominated by the mining and processing of coal and stone, the manufacture of cement, and the production of crude petroleum, which together comprised 87 percent of the total value of production, compared with 86 percent in 1963. The iron ore industry continued to decline. This was the first year in which consumption of imported iron ore exceeded the consumption of domestic iron ore. The coal industry recovered from the slump caused by the abandonment of red iron ore mining by United States Steel Corp., and production expanded 17 percent, for the best year in coal mining since 1948. During the year, the one-billionth ton of coal was mined within the State.

In underground coal mining, particularly room-and-pillar methods, the coal pillars play a significant role in roof stability. If the pillar dimensions are increased, the pillar bearing capacity also increases, but more coal remains in the pillar and therefore the recovery of mining operation decreases. Therefore, determining the optimum pillar dimension based on technical, economical and performance parameters is important. In this paper, the coal pillar is designed by using the concept of ground reaction curve (GRC). For this purpose, the GRC for coal pillars is drawn and then the elastic and plastic zone of coal pillar behavior is determined. Based on elastic and plastic zone, different equations are presented to draw a pillar reaction curve and the process of coal pillar design based on GRC is explained. As a case study, this new technique is applied to study the coal pillar state in Tabas coal mine of Iran. The results show that the presented new approach is successfully used in the design of the optimum coal pillar dimensions, and also determines the elastic and plastic behavior limit, which is important especially for the logical design of yield pillars.

The treatment of hypersaline waters originating from numerous gold projects within the Western Australian Goldfields region via membrane technology has been assessed. A combination of Ultra-filtration (UF) and Nano-filtration (NF) using Ecotechnol’s modified elements effectively removes the polyvalent ions responsible for the high lime and cyanide consumption as well as the prevalence of scale formation characteristic of gold operations in this region. Through removing up to 98% magnesium, 94% calcium and 99.7% sulfates from hypersaline water with total dissolved solids from 40,000mg/L to 250,000mg/L, Ecotechnol and subsequent independent tests have demonstrated up to an 87% reduction in lime consumption and through the ability to increase the operating leach pH, reduce cyanide consumption by up to 28%.CITATION:Tapley, B, Stoitis, N and Lien, L, 2015. This circular describes four modern change houses of the Lake Superior iron - ore district , all conceived and constructed since 1937. The oldest change house described herein was constructed and occupied in 1938 , yet it embodies the modern improvement of air - conditioning . The purpose of this report is to describe and illustrate improvements with regard to health and safety features combined in the construction , arrangement , ventilation , heating ,lighting , and other features , with the aid of photographs , sketches , and diagrams . The modern change house is a fire - resistant structure in which intermingling of men in mine clothes and men in street clothes , hazardous dust , heat and humidity , crowding , poor lighting , slippery and muddy floors , and inadequate facilities for clothes and for bathing have been largely eliminated . The miner coming off shift at a modern mine no longer faces the elements when he gets off the cage . He travels through a tunnel to the change house , where he is cooled gradually by room temperatures as he passes from the mineclothes section and shower room to the street - clothes room . He is assured of dry clothes when he is ready for them . No longer is he sent out of the change house into a muddy yard to drive away as best he can . The modern change house has an improved parking lot and graveled or concreted walkways from it to the change house entrances .

Optimum Colliery is presently experiencing a shortfall of stripping capacity in order to meet its production requirements. Due to the long dragline procurement times, several alternative stripping methods were evaluated in order to alleviate this shortfall in the interim period. Based on the experience gained at the mine. The writer will discuss some of the important factors which influence dragline performance and show how these factors may by used in a theoretical approach to analyse various dragline applications.

Several major impediments threaten the successful delivery of tunnel and under-ground construction projects. In addition to technical design and construction risks associated with tunnel projects, many of which are well understood, budgetary pressures, funding constraints and the complexity of procuring, administering and executing multiple concur-rent projects in a large program could also have a catastrophic impact on the program?s objectives. Contributing factors often include poorly managed design, procurement, construction, managerial, organizational, market and stake-holder risks, as well as nonconforming inputs or outputs to the costs and schedule, or any other potential inabilities to deliver the required or desired results. A strategy to prevent ?failure? of a project requires a proactive and holistic approach to risk management. Through planning, identification, analysis, management, monitoring and control of the project?s risks (both threats and opportunities), attention to management of risk will in-crease the probability and impact of positive events and decrease the probability and impact of events adverse to a project?s objectives.

The 1983 Energy Plan calls for an increase in North Island coal production from both underground and opencast sources from the 1982/1983 production of 843 000 tpa to approximately 3.4 Mtpa within the ten year period to March 1993. This increased production is scheduled to come from the following sources: Huntly West No 1 underground mine reaching full production of 1.26 Mtpa. Huntly East underground mine increasing production to 750 000 tpa. The proposed new Huntly West No 2 mine producing 100 000 tpa in its first year of operation û 1992. The proposed Ohinewai opencast producing 920 000 tpa for supply to the steelmaking plant at Glenbrook, replacing Weavers opencast which will be worked out in 1990. Rotowaro opencast producing at 220 000 tpa (currently being achieved). Two additional sources of supply which are not scheduled to begin production for at least ten years, and both of which have been designated for coal fired thermal power generation, are the Maramarua and Mokau coalfields. It was originally envisaged that development of both coalfields would be a combination of underground and opencast mines. The form of development is yet to be confirmed by detailed feasibility studies. The location of the Maramarua, Ohinewai (Waikare Coalfield) and Mokau deposits is shown on Figure 1. Worley Consultants Limited, in conjunction with their partners Rheinbraun Consulting GmbH (Germany) and Longworth McKenzie Pty Limited (Australia) have been involved with investigations and mining assessments for the Maramarua, Mokau and Ohinewai prospects. The Maramarua and Mokau studies form part of the New Zealand Coal Resources Survey programme of updating information to the Pre-Feasibility Mining Assessment level before proceeding to detailed feasibility studies. The Ohinewai study was commissioned directly by Mines Division to investigate the feasibility of mining coal by 1989 before reserves at Weavers opencast are depleted. This paper describes typical studies which are now being commissioned by Mines Division to investigate large-scale mining development and the effect geotechnical-conditions may have on coal recovery. The magnitude of these projects is reflected in the proposed outputs û up to2 Mtpa from individual mines û and the introduction of new continuous mining systems for both underground and opencast mines on a scale which has not previously been contemplated.

Barite Worldwide barite production for 1985 decreased 1.3%, while US barite production increased 1.4%, according to US Bureau of Mines statistics. The increase in US production is attributed to heavy demand for oil and gas drilling during the first quarter and an unrealized heavy expected demand in oil and gas markets for the last quarter. Nevada accounted for 82.1% of the 807 kt (890,000 st) of barite produced in the US last year, according to the Bureau. Other producing states were California, Georgia, Illinois, Missouri, Montana, Tennessee, and Washington. In 1985, the US imported 1.8 Mt (2 million st) of crude barite and 64 kt (71,000 st) of ground barite. China led the way with 43.3% of the total. Imports from India accounted for 21.6%; Morocco, 15.5%; and Thailand, 5.6%. The 1.8 Mt (2 million st) of crude barite imported is an 18.8% increase over 1984. Apparent consumption of barite for 1985 was about 2.6 Mt (2.9 million st), a 15.7% increase from 1984. Due to very low drilling rig activity during the last quarter of 1985, actual consumption is probably less as crude barite inventories at mines and plants increased. According to the Bureau, 90% of barite consumed in 1985 was for the oil and gas market. Paints, rubber, glass, and other chemical uses consumed the other 10%. With the price of oil falling dramatically early in 1986, the outlook for oil and gas drilling looks to be significantly lower than 1985. This lower demand for barite, coupled with the high year-end inventories, should lower both imports and domestic production. Significant lower prices on imported barite are expected, especially from China, Morocco, and India. This also could further lower US production of barite this year.

This paper is one of a series being prepared by the United States Bureau of Mines describing quarrying methods and costs at stone quarries throughout the United States. These papers are designed to disseminate technical information regarding the methods used. The cost tabulations represent operating expenditures only and not total production costs. It is recognized that publication of total production costs might in many instances cause embarrassment to individual producers as well as to the industry as a whole. On the other hand, operating costs are essential to the technical discussion and study of the methods employed. The attention of the reader is specifically called to this differentiation in order that no misunderstanding of the scrope of the cost tabulations shall ensue. "It is known that copper loss to the slag phase can be in the forms of chemical dissolution and mechanical entrainment. The contribution of each mechanism to the total Cu-loss depends on different parameters and may vary significantly. To reduce emissions to the atmosphere and to shorten the converting cycle, most industrial processes are aiming at production of high matte grades. Increased oxygen-to-sulfur potential, required by these processes, results in a highly oxidized slag, which in turn leads to an increased concentration of the chemically dissolved copper in the slag phase. Hence, slag cleaning processes are becoming increasingly important for economic viability of the smelting operations. Hydrometallurgical, pyrometallurgical, and mineral processing routes have been used for copper recovery from slag. The current article reviews a trade-off study evaluating two of the commonly used methods (namely flotation and electric slag cleaning furnace) by comparing the technical pros and cons, operability/maintenance requirements, and capital/operating costs of the operations.INTRODUCTIONLarge volumes of smelter and converter slags are produced by copper smelters around the world. It is estimated that for production of each ton of metal 2.2 tons of slag is produced (Gorai, Jana, & Premchand, 2003). Considering the current pressure on the mining and metals industry for “zero-waste, sustainable and environmentally friendly” extraction of metals, it is absolutely essential for metal producers to improve their efficiencies in order to remain competitive. To meet these objectives, almost all metal producers, from steelmaking plants, ferroalloys, base and precious metals producers, are trying to reduce their metal losses to the slag phase. In particular, copper producers are aiming to produce higher matte grades in their smelting furnaces to reduce the converting time and improve their overall throughput. Copper smelters need to minimize their copper/precious metal losses to the slag phase before final landfilling/use of slag. The current article, therefore, briefly reviews the copper loss to the slag phase, methods of metal recovery from the slag and then presents a trade-off study conducted for a green field flash smelter of the sulfide concentrates. A comparison was made between two of the major process routes for metal recovery from slag, namely slag cleaning furnace and flotation."

TICOR South Africa is a new titanium feed stock producer. In the Ticor South Africa plant the ilmenite feed to the smelter plant is roasted to enhance its magnetic properties. This allows for the magnetic separation of chromite from the ilmenite. The magnetization reaction relies on partial oxidation with both under- and over-oxidation causing insufficient magnetization. To subject the roaster plant feed material to the required physical conditions a circulating fluid bed (CFB) roaster is used. This paper describes the development of a laboratory model that will aid in the control of the CFB roaster.

The Price Road Drainage Tunnel was constructed for the Arizona Department of Transportation (ADOT) in 1988 and 1989 to provide surface water drainage for the proposed outer Loop Highway (SR 117). The 5.4 km (3 mile) long, 6.6 m (22 ft) excavated diameter tunnel was driven through sand, gravel and cobbles utilizing an open face digger shield. Precast concrete segments were erected for initial ground support. A second-pass reinforced concrete final lining was placed, which was followed by contact grouting. This paper will consider the owner's representatives' view of the effectiveness of using precast concrete segments for this project. Topics discussed include segment design and erection, ground support, geotechnical instrumentation, segment erection verses contact grout interactions and tunnel production.

The objective of this contract is to design, build, surface test, and evaluate a breadboard prototype of the Variable Wall Mining System so that data are available for the design of a production machine. The Variable Wall Mining System (VWMS) consists of a series of side cutting augers, connected end to end, and distributed across a panel. Supplying rotary power at the end of the string and anchorage from conventional longwall roof supports, the augers are sequentially thrust into the face and then lifted to the roof to extract the coal in a continuous manner. The augers not only cut but also transport the coal. The effort described in this report summarizes the operation characteristics of the VWMS, the design methodology used to develop various working models, the surface testing of a ½ -scale version of the VWMS, prediction of power requirements and performance potential of a full-scale system, and a critique of the VWMS concept, conclusions, and recommendations.

Addition of a 500 MW unit to the existing units at Ramagundum Thermal Power Station requires blasting of large volumes of rock very close to sensitive installations like Turbo generator, Generator transformer and Power cycle equipments. Levelling of the areas and excavations for footings are, therefore, to be carried out cautiously. The existing sensitive installations should be protected against ground vibrations and flyrock. The paper describes the details of a few of the many blasts carried out successfully wherein the ground vibrations were restricted and flyrock was avoided.
