Incident investigation is of utmost importance in most high-risk industries and is often used to improve organisational safety. This study was undertaken to examine the content of past incident investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of five elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of three Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed.

Factors governing the dissolution of nickel and iron in pentlandite and pyrrhotite concentrates from Mt. Windarra were studied for aqueous solutions at normal temperatures and pressures. The effects of dissolved dioxygen, carbon dioxide, hydrogen ions, and chloride ions are described. The direct reaction of aerated water wiili pyrrhotite is much faster than with pentlandite, and reaction rates are proportional to the dissolved dioxygen concentration. In both cases divalent metal sulphates are produced initially. The subsequent oxidation of iron(II) to iron(III) and its oxolation produces acid solutions which rapidly attack both minerals. The presence of carbon dioxide or chloride ion has little effect on these rates.

The Thirlmere aqueduct was constructed over 110 years ago to provide water from the Lake District (in the north-west of England) to the city of Manchester some 60 miles (100 km) away. The aqueduct remains, to this day, a key piece of infrastructure in the distribution of raw water in the north-west of England. A short section of the aqueduct was identified as being in poor condition during routine inspections by the owner. Subsequent investigations attributed the conduit damage to the movement of unstable ground over the conduit. Bachy Soletanche Ltd was subsequently appointed to carry out the design and construction of the slope stabilisation works and to undertake repairs inside the damaged conduit. The works were undertaken on an existing slope which, in places stands at a gradient of up to 40º to the horizontal. Strict limitations on plant and equipment were imposed by the client (United Utilities) to ensure that slope and conduit loading and associated vibrations were kept to an absolute minimum during construction. An intensive instrumentation system was installed to monitor surface movements, ground movements at depth, conduit movement and strains across existing planes of weaknesses within the conduit. The solution comprised the installation of spaced piles up-slope and down-slope of the conduit. Restrictions were imposed on the minimum stand-off distance between the outside face of the conduit and the piling works so targeted permeation grouting was undertaken to improve the ?connection? between the conduit and the down-slope piles. The presence of existing sub conduit groundwater flows was suspected and the permeation grouting was focused in areas to minimize disruption to the groundwater regime. The paper presents the basis of the design and highlights the combined construction and instrumentation methods adopted to minimise damage to this valuable asset.

Handling of heavy vehicles such as tractors, trucks and buses require a large roll stiffness which causes large high accelerations at the seat level during impacts. To provide comfort and minimize the energy transfer from the chassis and the seat a pneumatic active seat suspension is proposed. An active seat suspension design and control algorithm under development at the University of Illinois at Chicago, UIC, is being developed and tested. Preliminary results are presented in this paper. The design of a passive suspension typically consists of optimizing the value of two parameters: the stiffness and the damping of the suspension. The general dynamic performance of the suspension is limited to the conditions under which these parameters were obtained. A change in the input conditions might lead to poor suspension and an amplification of the vibration transmitted to the body. The focus of this paper is a robust, semi-active suspension system with a variable controlled damping and using the body response an index measure to minimize the acceleration at the interface of the seat and operator. A summary of existing suspensions, such as MR and ER fluids, and spring loaded and dual valve shock absorbent will be discussed to highlight the need of a semi-active pneumatic suspension system design. Methods A model of the proposed suspension was developed in MATLB (Simulink) and different control strategies for the valve position in relation to the cylinder pressure tested. The effects of stiffening and softening resulting from pressure changes in the cylinder were examined. The vertical accelerations of the seat was computed for different control strategies and configurations of the suspension and compared to the response of a passive seat suspension. A lump -mass model was created to represent the human body including the head, the upper, middle and lower torso as well as the legs. The connective forces between body segments were modeled through modal analysis techniques from previous experiments at the Vehicle Technology Laboratory. ISO standards and absorbed power were used to evaluate the different configuration of the seat suspension system in relation to the dynamic response of the operator. Results Initial results of the semi-active suspension system show a significant reduction in the RMS value of the acceleration of the seat. A reduction of the total absorbed power by the operator is expected to provide an insight into the control strategies adapted in the active suspension.

The Detroit salt mine is operated by the Detroit Salt Co. It is one of the oldest underground room-and-pillar mines in the United States still in production. Mining at the Detroit salt mine began in 1906 with the sinking of the No. 1 shaft. Mining continued through the years, becoming progressively more modern in its equipment and techniques, until it was closed in 1983. In 1997, after a period of care and maintenance, the newly formed Detroit Salt Co. decided to reopen the mine. Upon completion of an extensive rehabilitation program, salt production resumed in 1998. Production currently continues at a rate of 910kt/a (1 million stpy). To date, the Detroit salt mine has mined more than 45 Mt (50 million st).This mine is unique because it lies under a heavily urbanized area within metropolitan Detroit. The mine and its history make a good case study for the continuing coexistence between the mining industry and its environs.

THE Spanish mine is in Nevada County, California, 21 miles northeast of Nevada City by road, and 3 miles north of the town of Washington. The mill and surface buildings are on Poorman's Creek at an ele¬vation of 3,000 ft. About 35 men are employed at the property, which is producing 50 tons of ore per day. Electric power, purchased from the Pacific Gas and Electric Co., is brought in from Alleghany, seven miles away. In the early days the mine was operated by F. W. and P. R. Bradley. Between 1883 and 1899 the production was approximately $900,0)0 in gold. The ore treated during; that time yielded an average of 60 cents per ton and was mined and milled at a profit. The surface, oxidized, ore deposit was 30 to 100 ft. wide and was mined by the glory-hole method. The milling-plant at first consisted of a Blake crusher an-1 a small Huntington mill; later three large Huntington mills were added to the plant. In 1893 a new mill was built at a lower adit- It consisted of 10 stamps and four Huntington mills. The gold was secured by amalgamation, the recovery being about 50 per rent.

Titanium can exist in three different oxidation states. Its oxides are numerous and in presence of iron and oxygen, iron-titanium oxide compounds can also form. Some of the oxides are non-stoichiometric, making the determination of the thermodynamic properties of titanium and its oxides in molten steel and in slag difficult. Therefore, for the assessment of titanium activity in molten steel using the electrochemical measurements of oxygen partial pressure, the identification of oxides in equilibrium with titanium is important. This paper reports on an investigation of solid titanium oxide inclusions, which have been identified by combining different methods of analysis on quenched samples. These samples were taken from a Fe-Ti-melt at 1600°C, quenched in cold water then mounted and polished for optical microscopy. The oxide inclusions were analysed using transmission electron microscopy (TEM) with energy dispersive spectroscopy (EDS). Both single crystal and polycrystalline diffraction patterns were used to identify the oxides by comparing the d spacings with those in the ICDD X-Ray Diffraction (XRD) database. XRD was used to analyse the oxide particles on the top of slow cooled samples. It was found in the TEM study that the similarity of the diffraction patterns of the various phases meant that unique identification was difficult to achieve in some of the diffraction patterns. However, TiO, Ti2O3, Ti3O5, Ti4O7were identified in quenched samples, while TiO2, FeO.TiO2and iron oxides were seen in slow cooled samples. Hydrogen and carbon monoxide reduction data were obtained on pelletized materials of the type currently being prepared commercially in Australia.Pellets, approximately 1 cm diameter, made from Deepdale, Iron Prince, and Mt. Newman iron ore were reduced by hydrogen over the temperature range 500°-900°C and the reduced pellets were examined by thermogravimetric, microscopic, electron probe, and x-ray techniques. After an initial time delay it was found that up to 95 per cent reduction the data could be described by a porous model where gaseous diffusion through the solid is rate controlling. Independent measurements of effective diffusivity supported the values obtained from the reduction data. Above 95 per cent reduction the rate decreased markedly due to the formation of retained wustite or fayalite.Comparison with reduction by carbon monoxide over the temperature range 750°-900°C showed that the rate of reduction was approximately five times slower than for reduction by hydrogen.The rate of reduction was found to be a function of the initial properties of the pellets which may account for the widely conflicting results that have been obtained with previous investigations on porous pellets.

Keywords: Pyrometallurgy, furnace, copper coolers, freeze lining, monitoring, Saveway The use of water-cooled copper components as sidewall elements of submerged arc furnaces and other pyrometallurgical applications has become increasingly common. The high cooling efficiency of copper has made it possible to increase the service life of furnace refractories when compared to conventional refractory linings. However even the self-healing effect of ?freezing? worn areas of the lining does not necessarily protect the copper coolers against wear or corrosion completely. In order to avoid furnace explosions, various methods have been used to monitor the wear of coolers. Unfortunately, these have not been able to consistently identify localized wear. Thos Begbie (South African) and SAVEWAY (German) jointly developed technology to embed a line sensor into the copper cooler during the casting process. This system allows an exhaustive monitoring of the copper cooler. A SAVELINE sensor is placed inside the copper-cooling panel between the water-cooling passage and the hot face of the panel. The sensor follows the meandering shape of the cooling water channels. In the first instance, the sensor provides analogue information as the thickness of the copper reduces. Secondly, this wear signal will be confirmed by a signal of interruption if the sensor is washed away. Because of the structure of the sensor, a self-diagnosis is possible and indications can be confirmed (double proofed). Subsequent to the manufacture of some test pieces, coolers for a large submerged arc furnace were manufactured. The sensorized coolers have been installed at the intermittent surface (slag line) where the largest amount of corrosion has been encountered. The system has been operating since December 2005 in what is considered one of the largest submerged arc furnaces in South Africa.

Nowadays mining projects are seeking new versions of evaluation that are based on the flexibility in the project. Real option valuation (ROV) is one of the modern evaluation methods that provides a tool to adapt and revise mining projects under uncertainty and future variable movements. Most evaluation approaches simply assume that some variables are fixed, such as production rate, variable cost, fixed cost and lifetime of project. The first section of this paper reviews a comprehensive study of ROV in mining projects. The paper then introduces a new model that solves problems where previous methods lacked. The new method endeavours to find maximum mining project value by adding total cost as a function of production rate into ROV. The second section has been applied to a new model on the Century zinc mine in north-west Queensland, thus illustrating the future overview of that mine. The new version of ROV gave a significant positive value for the Century Mine, when the closure and reopening options where available throughout the life of the mine. Consequently, the new method evaluates real options to add value to mining projects, maximising projectÆs value, estimating cost function, optimising production rates and offers opportunities in projects to amplify gains or to mitigate losses.

Ticor South Africa is currently involved in the mining and processing of a heavy mineral oredeposit at Hillendale Mine on the KwaZulu-Natal north coast. The orebody is characterized by a high ultra fine fraction (-45µm) that has a variable rheological behaviour when subjected to shear and hydration. Experience at Hillendale has shown that the ultra fine mineral tailings are sensitive to shear history in terms of shear intensity and shear duration as well as hydration history. This phenomenon expresses itself right through the primary beneficiation process influencing the mineral separation, but is especially prevalent on the residue disposal i.e. slurry transport and deposition on the tailings impoundment. Tube viscometer test work was conducted on the flocculated fine tailings that indicated that there was a change in rheology as a result of shear. This paper presents the results of the on-site test work conducted at the mine to measure the rheology at various stages in the process.

Rare earth elements (REEs) and their compounds are essential in a multitude of mundane and sophisticated applications such as renewable energy technologies, catalysts, medicine, and defense applications. The global supply deficit could be attributed to the destabilization of REE market that has, as a result, incentivized indigenous production of REEs. Cost-effective and environmentally benign opportunities are being explored for the production of REEs from non-conventional mineral sources including coal and coal by-products to stabilize the supply chain. Coal from Healy coal mine and Wishbone Hill deposits in Alaska have been quantified for REEs and were found to possess elevated total REE concentrations as compared to world average levels (60– 70 ppm). The objective of the study was to concentrate the REEs in the froth phase by froth-flotation of the fine size fraction (< 75 μm). A Box-Behnken design of experiment was employed to ascertain the main and interaction effects of operational parameters on response variable by analyzing 3D response surfaces. Experiments were carried out to maximize REE recovery and concentration by modeling and optimization of three operating variables (frother dosage, pulp density (% solids), and collector dosage). Based on the results, the optimum flotation conditions for maximum recovery at an elevated concentration of REEs in the froth fraction was obtained at 4.2% solids and 32.7 ppm of frother dosage for Healy sample and 10% solids and 37.9 ppm of frother dosage for Wishbone Hill coal sample. The quadratic relationship between the input and response variable was found to be independent of collector dosage suggesting good surface hydrophobicity and floatability of the two coal samples.

In the paper the method for estimation of settlement of vertically loaded piles in a group is presented. The method is generally based on the hybrid procedure with non-linear solution for single piles and with interaction between them for the low-strain shear modulus. Some attention is paid to the influence of deformation and strength parameters used in calculations. The applied model is verified in the framework of the research programme carried out recently in the Geotechnical Department of Gdansk Technical University. It concerns monitoring and analysing the settlements of the existing structures on piles. A comparison of calculation and measurements results for two tested buildings is presented as an example.

Mixed cobalt-nickel sulphides precipitated from ammoniacal process liquors derived from nickel sulphide concentrates, mattes, and reduced nickeliferous laterites have been characterized.The mixed sulphides are prone to oxidation on standing and are rapidly leached under relatively mild conditions. The kinetics of the leaching reactions are controlled principally by temperature and oxygen partial pressure. Although the samples have different CO:Ni ratios and have undergone varying degrees of oxidation, there is little difference in their leaching behaviour. The results suggest that a single plant would be able to process mixed sulphides of varying composition. Any copper and zinc present in the mixed sulphides as impurities also go into solution and must be removed prior to recovery of the cobalt and nickel in metallic form.

What do all iron ore mines have in common? Perfect production days! Yes, those harmonious 24-hour periods when the stars align and the daily output number rockets past the long-term average to hit new heights. While perfect days are always welcome, the focusing question iswas that just a ‘flash in the pan’ (due to a special cause) or an indication of the process’s capability (with clues to the common causes) which, if addressed, could make every day perfect? In the context of today’s operating environment, with the need to maximise productivity at minimum cost, it’s an important question.Investigations reveal that the throughput difference between a perfect production day and the average is often in the order of 40 per cent, and that bulk solids handling (BSH) system deficiencies are the largest (&gt;50 per cent) class of lost opportunity. Root causes for underperformance of elements in BSH systems can often be traced back to the detailed design and even study-stage oversights. Essentially baked into the fabricated plant, operations are effectively forced to accept them at the point of practical completion and then deal with the consequences for the life of the mine. Subsequent normalisation of these defects in the planning process reduces visibility and hence the impetus for long-term solutions in favour of lower targets and quick fixes to keep the show on the road. On an industry wide basis the design processes and assumptions responsible for BSH system underperformance become enshrined as common practice and hence repeated across sites and organisations.In addition to making the case for action, practical steps that can be taken to help realise the significant productivity prize and avoid the problems in first place are provided.CITATION:Wellwood, G A, 2017. This paper is a follow-up to the paper presented at the SME meeting in April of 1996. Since then, the El Abra facility has been started up, 9 months ahead of schedule, and has processed about 45 million tons of ore and produced over 700 million pounds of copper. For those unfamiliar with the El Abra materials handling and leaching facilities, a brief overview of the facility is provided. The paper will then deal in detail with such aspects of Conveyor 103, the long regenerative overland conveyor, including: actual running resistance starting and stopping procedures emergency brake test at 10,000 t/h Additionally, we shall take the opportunity to report on a catastrophic culvert collapse in October 1996, which stopped the conveyor abruptly at full speed and under high load condition, causing a belt breakage. We would like to use this opportunity to thank the management of El Abra and Cyprus Amax for providing assistance and allowing us to present this paper. The El Abra team deserves a great compliment for building this one billion dollar facility in record time in a remote area of a high-altitude desert with no existing infrastructure, and for bringing it so quickly up to full operating capacity. The El Abra management had to assemble a completely new operator team. Considering the above circumstances, it took a great effort by all involved to achieve success. The official performance test which was required by the lending banks was successfully concluded in November 1997.

"Column and other non-conventional flotation approaches continue to provide benefits where conventional flotation processes have exhibited limited success. This is particularly evident with respect to flotation of ultrafine and coarse particles. Advancements in air sparging technology have provided the ability to produce finer bubbles than previously possible. The generation of finer bubbles coupled with the froth washing capabilities of a flotation column has demonstrated improvements in ultrafine particle recovery while simultaneously producing a high-grade float product. On the other side of the spectrum, the use of these sparging technologies while applying flotation fundamentals to a fluidized-bed reactor has increased the maximum floatable particle size up to and in excess of 3 mm. Benefits of higher recoveries and reduced reagent consumption have resulted from the use of these technologies. An additional benefit includes a reduction in plant grinding requirements. As such, this paper describes the application of Cavitation-Tube sparger technology and fluidized-bed flotation (HydroFloat). A review of data from industrial and pilot-scale applications is also provided.INTRODUCTIONConventional (mechanical) froth flotation is considered the workhorse of the minerals processing industry. This low-cost process makes it possible to selectively recover minerals by exploiting the difference in surface characteristics (e.g., wettability) of the valuable component and the waste material. Unfortunately, the upper particle size limit for flotation rarely exceeds 0.600 mm, and is typically maintained at less than 0.300 mm for most industrial applications. For many commercial base metal plants, the particle size range for optimum recovery is typically between 0.01 and 0.1 mm (Lynch et al., 1981). This finding is not novel. In fact, the effect of particle size on flotation recovery was studied as early as 1931 by Gaudin et al. who showed that coarse (+0.250 mm) and extremely fine particles (-0.010 mm) are more difficult to recover than particles of an intermediate size. The loss of recovery is due to inherent limitations associated with the flotation process, including bubble-particle adhesion and collision, detachment, buoyancy, and transport."

The program was carried out to develop deep kerfing and core breaking techniques to be incorporated into the design of a field prototype hard rock mining machine. The field machine is designed to mine non-abrasive bedded deposits up to 20,000 psi in uniaxial compressive strength. The mining technique involves cutting approximately 2 ft deep parallel kerfs spaced 2 ft apart in either a horizontal or vertical direction and subsequently breaking out the cores so defined. The prototype machine is intended for demonstration of the technique in a quarry or equivalent field environment. The program was aimed at minimizing the required kerf width, obtaining reliable breaking within that kerf width and determining cutter blade requirements necessary to achieve a mining rate of 20 ft3/min. Full scale breaking experiments and 1/2 scale kerfing experiments were employed to evaluate the system. High strength concrete test specimens were used to simulate the rock being cut and broken. The test results indicate the kerf/break technique can achieve the desired mining rate by utilizing two 7 ft diam blades to cut 2 in. wide by 24 in. deep kerfs. A lateral load applied at the base of the kerf (base shear) was selected at the core breaking technique to be employed by the full-scale prototype machine.
