A sewer tunnel being constructed through an urban commercial area was to pass the site of a former gasoline service station. The site was known to have contained leaking underground storage tanks. An environmental assessment of soils on the alignment was performed, and measures were implemented to handle expected contaminants. Despite these precautions, an explosion occurred in the heading when liquid gasoline was encountered. Four workers were injured. This paper presents the findings of an investigation into the cause of the explosion, and examines how concerted efforts by the parties involved failed to prevent it. The level of guidance provided by OSHA Code provisions for tunneling in gassy conditions is discussed.

INTRODUCTION A rail rapid transit system called the "Metro" system is presently under construction in the Nation's Capital area. The agency responsible for the design and construction of the Metro system is the Washington Metropolitan Area Transit Authority, an independent tristate organization set up under an Act of Congress and financed by federal, state, and local public funds supplemented by revenue bonds. When completed, the Metro system will comprise nearly 100 route miles. About fifteen percent of this mileage is now under construction, while designs for the-remainder are in progress. The system is scheduled for completion in about eight years at a cost of approximately 3 billion dollars. The Metro system will involve nearly all types of construction generally used for rapid rail transit projects. About half of the mileage will be underground, comprising approximately 16 miles of rock tunnels, 10 miles of earth tunnels, and nearly 21 miles of cut-and-cover construction in earth (see Figure 1.). The remaining 51 miles of the system is planned to be constructed on the surface as at-grade or elevated structures. The system includes 86 passenger stations, 53 of which will be underground structures. Twelve of the underground stations will be large size chambers in rock while other underground stations will be constructed in earth as cut-and-cover structures. Thus, about one-fifth of the system will be designed as underground structures, namely tunnels and stations, in rock. In recent years a great number of papers have been presented at symposiums and have appeared in the literature relative to geologic prediction for rapid excavation and tunneling. Many of these papers describe research projects now underway to investigate tunnel site selection by remote sensing techniques, seismic holography, electro¬magnetic pulse sounding, optical scanning for structural features, and many other exotic geophysical techniques. While it is possible that this research will produce techniques that may prove to be very useful, it appears for the present at least, and perhaps for another decade, that there still will be no substitute for conventional exploration methods such as coring boring, geologic mapping, studies of existing wells, and proven geophysical techniques. Nevertheless, there is still room for tremendous improvement in the conventional techniques for geologic prediction in tunneling. These include: more rapid and accurately aligned coring borings drilled parallel to the tunnel alignment; improved geophysical surveys both prior and during construction; improved methods for predicting ground¬water inflows and water pressures, both of which can greatly influence the cost of driving tunnels. Geophysical techniques can also be ap¬plied to locate deeply incised glacial troughs or ancient buried stream channels that might cross the tunnel alignment. In the meantime, while some researchers continue their efforts to¬ward a breakthrough in new techniques for geologic prediction, it is recommended that other researchers direct their efforts toward sol¬ving specific problems such as accurately predicting ground-water conditions, determining locations of high geostatic stresses, and locations of major fault and shear zones.

The need for ground support research in underground excavation has been well identified in recent years by many studies in the subject area (1, 2, 3, 4, 5). A typical conclusion from these reports (1, 2) is that the drill-and-blast (D & B) mining or the mechanical tunnel boring machine (TBM) methods are expected to be continued in use for the needs of tunneling in the immediate future. The Organization for Economic Co-operation and Development (OECD) sponsored in June, 1970 advisory conferences on Hardrock and Soft Ground Tunneling (3,4). These two reports were based on the combined response received from approximately six hundred inquiries from 17 nations in the summer of 1969 and which concerned all aspects of present-day inadequacies of, and the needed improvements in the technology of tunneling. Some specific problem areas in ground support and tunnel linings were: 1) the ability to successfully predict rock loads that must be carried by the support system; and 2) inadequate methods for determining the load-carrying capacity of the various support systems. Therefore, the problem area is clearly defined by these two objectives: 1) To find means of predicting rock loads; and 2) To develop methodology for determining safe, efficient, and economical ground support design practices. Noting these research needs from a great number of sources, the United States Bureau of Mines, Spokane Mining Research Center (SMRC), acting as agent for the Advanced Research Projects Agency (ARPA), undertook contract research effort in these problem areas. Work by Jacobs Associates has studied a methodology for specification of temporary artificial support based on the dependent sequence of mining operations and prediction of various rock conditions in the pre-excavation stage. All underground operations have limiting factors that influence overall profitability. Hoisting, hauling, available headings, labor, milling capacity and other factors limit how much material can be removed from an underground mine. Consequently, when mines use limited labor to handle the additional rock from overbreak and to perform extra ground support tasks, productive activity is replaced by non-productive work. Mining economics are further penalized when overbreak causes dilution of ore grades. This paper reviews organizational systems that mines have used to create environments which foster the creation of successful overbreak control programs. Actual case histories document the various quality improvement techniques, worker incentives, and practical project management systems used by these mines. A brief review of controlled blasting technology sources and overbreak measurement tools is also included.

Inactive underground mines now provide essential habitat for more than half of North America's 44 bat species, including some of the largest remaining populations. Thousands of abandoned mines have already been closed or are slated for safety closures, and many are destroyed during renewed mining in historic districts. The available evidence suggests that millions of bats have already been lost due to these closures. Bats are primary predators of night-flying insects that cost American farmers and foresters billions of dollars annually, therefore, threats to bat survival are cause for serious concern. Fortunately, mine closure methods exist that protect both bats and humans. Bat Conservation International (BCI) and the USDI-Bureau of Land Management founded the North American Bats and Mines Project to provide national leadership and coordination to minimize the loss of mine-roosting bats. This partnership has involved federal and state mine-land and wildlife managers and the mining industry. BCI has trained hundreds of mine-land and wildlife managers nationwide in mine assessment techniques for bats and bat-compatible closure methods, published technical information on bats and mine-land management, presented papers on bats and mines at national mining and wildlife conferences, and collaborated with numerous federal, state, and private partners to protect some of the most important mine-roosting bat populations. Our new mining industry initiative, Mining for Habitat, is designed to develop bat habitat conservation and enhancement plans for active mining operations. It includes the creation of cost-effective artificial underground bat roosts using surplus mining materials such as old mine-truck tires and culverts buried beneath waste rock.

A hydraulic rock breaker was developed in Germany by Helmut Darda and uses a wedge-plug and feathers mechanism powered by a 10,000 p.s.i. power pack. During the past four years these tools have been increasingly used all over North America for secondary breaking of boulders and rock. More recently, techniques have been developed to break rock off the solid in trenches and rock emplacements where blasting is pro¬hibited or very limited. Underground operations are commencing to adopt the splitter for scaling plus including systems which can be efficiently applied to chambers and mines. Mr. Neil Duncan, who was originally scheduled to give the main part of this presentation has, during this past month accepted a position with the Alberta Energy Resources Conservation Board in Calgary. The board encourages its members to take part in seminars such as this but, unfortunately, due to the heavy pressure of work, it was impossible for him to attend this conference. Mr. Duncan has, however, forwarded me his notes and suggest¬ions, and using these, combined with a slide presentation, we feel that we can quickly and clearly define the operations and application of the hydraulic splitting technique as it applies to your profession and tunnelling work. The first question that most of you are asking is, "Why use a hydraulic splitter if you can blast?" Shaft 26B of New York City’s Third Water Tunnel was constructed by Frontier-Kemper Constructors, Inc. of Evansville, Indiana. The 176.8 m (580 ft) deep shaft will serve as the construction shaft for the Manhattan spur of Water Tunnel No. 3. Twelve m (40 ft) of over-burden was supported by a rectangular cofferdam. This shaft was excavated in 13 months, through 164.6 m (540 ft) of rock, using 24,040 kg (26.5 tons) of explosives. Concrete lining cycles included the installation of stainless steel pipes embedded in the lining as the shaft advanced. A total of 635 m (2,084 linear feet) of stainless steel pipe was installed.

Computer-based mine management systems are being used in many surface mines to assign haul trucks, track equipment and manage information. Advances in digital communications allow these systems to be used in underground operations. Underground mine management systems help maximize production by providing gains in equipment and manpower use, drawpoint equalization in block caving operations, preventive maintenance and safety. Current systems use allocated or "locked" train and load-haul-dump (LHD) dispatching. Allocated dispatching rarely optimizes open pit product' on. But it offers an efficient mode of operation underground and where options are more limited. The primary objectives of underground mine management systems are to maximize production and mini¬mize contention time through allocated train, truck or LHD dispatching, traffic control, equipment monitoring and comprehensive reporting. The main components of an underground mine management system include: ? A central computer (personal computer or worksta¬tion) in a central control center. It sends instructions to train, truck or LHD drivers and allows dispatcher interaction. ? A field computer system (FCS). This consists of a touch-screen graphics console (GC) and mobile hub - on each train, truck or LHD to receive and display instructions from the central computer and accept driver input. ? A data radio network based on leaky feeder (data, voice, video) or microcell radios (data only) and advanced token-ring technology. This passes information between the central computer and the FCSs. ? Radio frequency (RF) tags mounted at boxes, weigh bridges, tips and rim tunnel main haulage ways to locate trains or LHDs (Luke, 1993). Such a management system must be in place before implementing computer-based dispatching (Vagenas, 1991) for production optimization.

In the Kansas City metropolitan area, mineable Bethany Falls Limestone occurs 80 ft below river level under the 700 acre Park College Campus on the north bank of the Missouri River at Parkville. For 20 years prior to this project various firms passed over this opportunity to mine this property for construction aggregate because of uncertain costs, financial risk, and the difficulty of constructing an entry tunnel. This paper describes the drill and blast construction, rock bolting, and shotcrete lining of a 750 ft. long, 15% declining grade, truck tunnel through several Pennsylvanian limestones and shales.

INTRODUCTION Lesotho is a mountainous area of southern Africa from which water is to be exported in an extensive tunnel system, to industrial regions inland. The related tunnelling project has involved a num- ber of drives using tunnel boring machines [TBMs] to excavate about 100 km of 5 m diameter water tunnels [von Glehn and Bluhm, 1995). This paper describes the ventilation and cooling of some of the tunnel drives from both the operational and design points-of-view with a particular focus on heat generation. There were many common features in all of the drives but this paper is focused mainly on the Hlotse drive which was 18,4 km long. The drives were ventilated using forced ventilation systems to provide appropriate air flow throughout the tunnels and face zones. In addition, the Hlotse drive required refrigeration equip- ment which provided chilled water to the tunnel. While the sec- ondary ventilation systems play an important role in gas and dust handling, the paper concentrates on the primary ventilation and cooling issues. The ventilation of these tunnels was an exacting exercise be- cause: • Rock temperatures and geothermal heat flow were high. • TBMs with relatively high power ratings were used. • Diesel locomotives were used. • Drives were relatively long. • High altitude meant a low air density. An important feature was the simulation and monitoring of the ventilation and heat flow components and the project was characterised by analysis, monitoring and ongoing tactical decision-making throughout the progress. The thermodynamics of the systems were complex because there were many interactive effects and analyses were carried out using special computer pro- grams. The monitoring confirmed the accuracy of the models, and in this manner it was possible to confidently ensure healthy and safe working conditions and still minimise costs. Local ambient climate conditions range from temperatures higher than 35 "C in summer to below -10 OC in winter. Based on available statistical data and the thermal storage/damping effects in the system, design summer ambient conditions were taken as 15 OC/25 "C wet-bulb/dry-bulb. The barometric pressure was 80 kPa and due to the altitude, the ambient air density was only 0,9 kg/m3. The local Authority specified a maximum in-tunnel wet- bulb temperature [at any point] of 32,O OC and a mean wet-bulb temperature [from all locations] of 27,5 OC maximum. The maxi- mum height of ground cover above the tunnel was 1 200 m and the maximum virgin rock temperature was 41 OC; see Figure 1. Diesel dilution criteria specified by the local Authority was a minimum of 0.1 m3/s per rated kW of diesel engine. Other requirements related to gases such as CO, CO2, NOx and CH4 [and the need for intrinsically safe equipment] but these are not of direct relevance to this paper. The actual average face advance was about 30 m/d with good days achieving 60 m/d and good months achieving 1 000 m [23 working days]. The original design tunnelling rate was 50 m/d. DESCRIPTION OF HLOTSE DRIVE VENTILATION AND COOLING SYSTEM The ventilation requirements in the tunnels were dictated by heat and diesel dilution needs. The best ventilation and cooling policy is generally a balance between using increased quantities of fresh air or refrigeration [or both]. In this particular scenario it turned out that, since the diesel emission criteria required large quantities of air, the refrigeration needs were modest. The drive was ventilated using a ducted, forced ventilation system from fans located at the portal. The maximum ventilation requirement was 51 m3/s when the drive was at 18.4 km. From a heat flow point of view, the worst scenario was a heat load of 3.5 MW when the drive was at 7 km. This was cooled by the ventilation air and a supply of chilled water to the tunnel. Refrigeration and chilled water system In the design phase, a detailed comparison was carried out between two general alternatives for providing refrigeration. First, was a system in which refrigeration sets and air coolers are installed on the TBM train; the refrigeration sets are cooled by condenser water piped to and from cooling towers at the portal. Second, was a system in which refrigeration water chillers are in- stalled at the portal and chilled water is piped into the tunnel. The detailed comparison indicated that the capital and running costs of the second system were at least 60 % lower than the in-tunnel plant. There were also many obvious practical benefits for favouring the portal system. The refrigeration plant supplied 23 11s of cold water at a temperature of 10 OC. After providing the cooling effect in the drive, the water returned to the portal where it was initially cooled in open-circuit evaporative pre-cooling towers, chilled in the refrigeration plant and then returned to the tunnel. The cold water flowed into the tunnel in an insulated supply pipe and returned in an uninsulated pipe; the water was simply circulated to the end of the pipe and returned. The cooling effect in the tunnel was achieved entirely through heat transfer from the pipe [long linear heat exchanger] and no air coils or other heat exchangers were required. The cooling requirements were satisfied by the heat transfer to the returns chilled water steel pipe [200 mm]. The pipes were eventually installed to a maximum distance of 10,8 km in what was considered a very practical and cost effective solution.

With the recent technological advances; Tunnel Boring Machines (TBM) have become a viable economic alternative to drill and blast in hard rock excavation. These machines are finding increasing use in underground mine development due to the potential benefits they offer. The successful application of TBM technology to mining depends on the selection of the most suitable equipment and cutting tools for the rock and ground conditions to be encountered. In addition to required testing and geotechnical investigations, cutterhead design optimization is an integral part of the machine selection to assure a successful application of the machines in a specific project. This paper presents and discusses the back-ground of TBM application in a mining environment and the lessons learned, the process of conducting site investigation and the necessary laboratory testing together with machine selection and performance estimation for TBM use III mine development.

DESCRIPTION OF PROJECT The Straight Creek Tunnel site is located approximately sixty (60) miles west of Denver, Colorado, on Interstate Route 70 in the vicinity of Loveland Pass, in the Rocky Mountains. The ultimate project will consist of dual two-lane vehicular tunnels with a building at each portal to house mechanical ventilation equipment and other installations. The tunnels run virtually east-west on an ascending 1.64 percent grade for a distance of approximately 8,900 feet, and will pass beneath the Continental Divide at an elevation of about 11,000 feet above sea level. The maximum cover is 1,450 feet and each tunnel is approximately 48 feet wide by 50 feet high as excavated. The work now under construction consists of the North Tunnel, which initially will have two-way traffic. Ultimately it will be the west bound lanes for Interstate 70. The completion of the North Tunnel will be the fulfillment of a long felt need for better communication through the high Rocky Mountains of central Colorado and is a vital link in Interstate Route 70. "On January 1st 1996 Inco Limited, Tamrock OY, Dyno Explosives Group and CANMET formed a consortium for the development of robotic mining equipment. This project is a five year research and development agreement that combines the talents of the above organizations to facilitate the creation of mining equipment and systems for robotic mining.The projects are based on the communication system developed by Inco Mines Research and currently sold and marketed by Automated Mining Systems Limited. Main projects include the development and implementation of a positioning and navigation module (including a software mapping system that will be inherent to each piece of equipment), robotic systems for development and mining, and software systems to support the use of this equipment. Included in the program is the establishment of a test mine, at Inco’s 175 ore- body at Copper Cliff. This facility will allow collaboration of companies and research organizations for the design and testing of techniques and equipment for Robotic mining.This paper discusses the creation of this new research alliance, the Mining Automation Program or MAP."

Underground mining is relatively safe today, but no technique or equipment is completely fool proof and 100% reliable. The only sure way to prevent accidental death or injury to underground workers is to remove them completely from the hazardous environment. This alternate approach to underground work is now feasible. Tunnelling and mining systems can be built that could be remotely controlled by men operating consoles in a surface control room. This concept, called a teleminer, would use recently developed remote control technology to remove man from underground work but still give him the capability to remotely operate mining and excavating equipment as if he were actually in the working area. It is appropriate to present this concept in the session "Environmental Controls" because the rationale for environmental control in underground areas can be significantly modified if man is excluded from the underground operation. The development of teleminers cannot only reduce or eliminate most of the requirements for conventional environmental control, but it can also create new and powerful techniques for rapid tunnelling and excavation. TELEMINER CONCEPT The concept of remotely controlled mining machines is neither new nor untried.(1)(2) Conventional remotely controlled mining equipment employs the standard techniques of extending the controls of the equipment to a remote console. For example, the rotary cutters on a remotely controlled tunnelling machine are started or stopped by a human operator turning a switch on the remote control console.

INTRODUCTION There are currently around fifteen companies in the United States and Europe manufacturing hard rock tunneling machines. In recent years upwards of 200 hard rock tunnels of lengths up to 12 miles and diameters to the order of 30 ft have and are being successfully driven by over 100 different machines. It has been predicted that within the next decade over 2,000 miles of tunnels will be driven for transportation purposes in the United States alone (OECD Advisory Conference on Tunneling, Washington, 1970) and it is expected that eventually most tunnels including mine drivages will be excavated by machines. All tunneling machines have certain common basic functions; thrust is applied to drive or hold the rock cutting tools into the face, and torque is applied to rotate the cutting tools over the face so that they can continuously break out rock. The speed of head rotation, together with the torque requirement, determines the cutter head power. (The power required to push the head forward is usually negligible.) There are many factors which determine how successfully a given tunneling machine will perform in a given rock but one of the most important is effective utilization of cutter head power. This paper is a review of the specifications and performance of hard rock tunneling machines, obtained from manufacturers' catalogs, published literature, talks with manufacturers and visits to operating machines throughout the United States and Europe.

LOCATION AND HISTORY The Lakeshore Copper property is located on the Papago Indian Reservation approximately 30 miles South of Casa Grande, Arizona. This major porphyry type deposit was discovered by El Paso Natural Gas Company in 1966. Following an extensive diamond drilling program, El Paso invited several mining companies, including Hecla, to examine the property with a view towards participation. Hecla's examination, and the attendant feasibility studies, led to agreements, which were finalized in September 1969, whereby Hecla would acquire a 50% interest in the property, and would act as operator. The Lakeshore deposit covers an area approximately 3,000 feet in width by 6,000 feet in length. It consists of an upper zone of disseminated copper oxides, averaging approximately 100' in thickness overlying a zone of disseminated copper sulfides up to 1,000 feet thick. Near the base of the disseminated ore is a blanket like zone of high grade copper sulfides, referred to as the tactite ore body. The mine is being developed to produce approximately 6,500 tons per day of oxide ore and 9,000 tons per day of sulfide ore. This represents a total daily production rate of 15,500 tons per day, which will be achieved by underground mining methods using rubbertired dieselpowered mining equipment. DEVELOPMENT LAYOUT The mine is being developed by two parallel 15° decline tunnels. The declines were collared 5,000 feet southeast of the ore body, and are being driven in a northwestery direction, to pass beneath the tactite ore zone. In-pit crushing and conveying of waste has been utilized in the mining industry for over 30 years. In recent years, innovations in crushing, conveying, and particularly waste and leach stacking technology have sparked a renewed interest in continuous haulage systems for large-scale open pit applications. A review of the advantages/disadvantages and operating techniques of crushers, feeders, conveyors, and stacking systems currently available is made. The paper examines some operations that are currently using large scale continuous haulage systems. Finally, a case study is presented that evaluates the application of a continuous waste haulage system in a large open pit mine in the Western United States, The case study includes a economic comparison with the existing truck haulage system.
