THIS method of briquetting flue-dust, or flue-dust mixed with fine ores, or, in a few exceptional cases, coke-dust, has come into large commercial use in Europe, and a small plant is already in operation in the United States. It promises to become of such great importance to the iron industry of the United States that the following description will certainly interest the majority of our pig-iron producers. Discovered in 1908, the process rests upon the observation of Dr. Schumacher that blast-furnace flue-dust, while possessing of itself no binding or cementing properties, acquires strong cementing properties if mixed with a very small amount of certain salt solutions, the amount of which is so small that the resulting cementation appears to be rather by catalytic action of the salts added than by the mass-action of such chemicals. To be more specific, fresh blast-furnace flue-dust mixed with from 5 to 10 per cent. of its weight of magnesium chloride or calcium chloride solution-that is to say, with from 0.25 to 2 per cent. of its weight of magnesium chloride or calcium chloride-acquires the property of setting within a short time, from 15 to 60 min., and forming a hard cemented briquette. Alkaline solutions appear to have no action such as described, and they even interfere, if present, with the action of the magnesium and calcium solutions. When treated in this way the flue-dust briquette sets very hard, apparently from pure excess of cementing- and setting-power. It is, therefore, possible to . mix with the flue-dust a considerable proportion of inert ore or like material, which has no setting-power by this process, and thus to make a compound briquette containing large quantities of ore mixed with flue-dust, but in which the flue-dust may be regarded as the cement or binding-material. Rehabilitation of the Henry Hudson Bridge is nearing completion, connecting Spuyten Duyvil in the Bronx to Inwood Hill in Manhattan, New York. Constructed in 1936, the Henry Hudson Bridge spans 840 feet over the Hudson River. Challenging geotechnical aspects of this Project required a team of innovators to overcome. Rehabilitation of the existing bridge involved the replacement of sixteen bent pedestal substructures, two abutment pilasters and installation of new deep foundations at the skewbacks. However, the Bridge, roadways, adjacent Metro-North Railroad (MNR) structures and New York City (NYC) Parks land were to remain open to the public for use, with minimal disturbance. In addition to physical constraints, geologic site conditions varied widely across both sides of the River, with varying bedrock quality and depth, greatly impacting the proposed design and construction. Extensive analysis of bedrock conditions was conducted to locate areas of potential wedge failure. The implementation of anchors was used to reinforce rock stability where pedestals and pilasters were replaced. This paper describes several challenging technical and natural aspects overcome in the rehabilitation of the Henry Hudson bridge foundations, including varying geologic site conditions, access restrictions, construction limitations, project specifications and design requirements.

"In port and harbor facilities, jetties and cargo berth terminals are usually installed on piles that remain partially embedded in soil, while a part of it behaves as columns supporting top deck. Technology in vogue adapts a pseudo static analysis in computer where the top deck including piles are analyzed as an integrated system based on finite element method when the piles are modeled as beam elements, while the soil in which it is embedded is modeled as linear or nonlinear springs. Typical berthing force of the ship is considered as a pseudo static force where the dynamic characteristics of the force is taken into consideration by an equivalent impact factor and little study exists on the actual behavior of these type of piles under the transient shock due to berthing impact. Present paper proposes an analytical solution based on which one can compute the dynamic response of such jetty piles considering shear modulus of soil constant with depth. Considering the solution is analytical in nature no special software is needed and can well be adapted fordesign office practice using generic utility software like MATHCAD, MATLAB etc. that makes theproposed model computationally very efficient.INTRODUCTIONResponse of piles under dynamic load has been an important study in the realms of civil engineering, where significant research has been carried out on its dynamic behavior under harmonic loads like one experienced in a machine foundation or under seismic force.A number of researchers have given solution to this problem like Parmelee et al (1964), Tajimi (1966), Penzien (1970), Novak et al (1974, 1983), Banerjee and Sen (1987), Dobry and Gazetas (1988),Chowdhury and Dasgupta (2008).However, most of these theories are based on piles loaded under harmonic load and valid for long piles where structural failure precedes the soil failure considering the piles are long and flexible."

Hardman Construction was faced with the challenge of completing a large earth retention project on a constricted downtown Detroit site, while maintaining a safe and productive worksite. To do so, new technologies using robotic equipment was instituted. HISTORY The historic J.L. Hudson Building (Hudson’s) in Detroit, Michigan was the premier retailer in the city’s downtown district from the 1890s through the 1960s. The massive flagship store sat at 1206 Woodward Avenue, one of the busiest streets in the Detroit area. Hudson’s broke ground in 1891 and would go through twelve renovations and expansions. The last renovations took place in 1946, which gave the building the title of being the largest department store in the world at the time. It spanned an entire city block and boasted 2.2 million square feet of retail space. By the 1970s, Detroit was in an economic downturn. The Hudson’s department store finally closed its doors in 1983. Besides a few working offices, the building sat vacant. The building was unique in that it had four levels underground, offices, mechanical areas, and equipment storage. Following years of neglect, the Hudson’s building was seized by the state and transferred into the hands of the city. The city had it demolished in 1998, (shown in Fig. 1) to construct a new, four- level underground parking garage as well as a potential future above-grade development (Sewlck, 2017). The building was imploded in one of the largest implosions of that time. The building rubble filled the underground rooms and helped to stabilize the exterior concrete walls and keep them from collapsing.

O trabalho focou na síntese de materiais com forma e tamanho controlados por meio de metodologias simples e ambientalmente amigáveis e na preparação de estruturas híbridas com dispersão uniforme entre componentes. Foram obtidos hidróxidos, óxidos e híbridos, cujos parâmetros de síntese (como temperatura, tempo de reação, entre outros) foram estudados. Uma vez caracterizadas, empregaremos as estruturas do tipo M/NiO (M = Au, Pd) como catalisadores nas reações modelo de oxidação do álcool benzílico e de redução do 4-nitrofenol, com o intento de estudar a natureza da interação metal-suporte.

In 2007, Atlas Copco introduced an integrated tunnel scanner/profiler. The tunnel scanner, which is integrated into the control system, is mounted on the drill rigs, and replaces the previous variants of non-integrated scanner equipment. The Atlas Copco Tunnel Profiler, which is the name of this system, is currently being used by several contractors and mines on various projects in Scandinavia, Finland and Australia. Atlas Copco Tunnel Profiler can scan a tunnel of 65 m2 in just five minutes and is integrated with the computerised control system for the drill rig. This integration means that the results of scanning are immediately available for analysis on the computer screen both in the drill rig and in Tunnel Manager; a software for tunnel and drift planning.In 2008, a method was introduced for locating the drill rig with a total station navigation (TSN) system, integrated into the control system. TSN systems are used by a number of contractors in Scandinavia and Finland, as well as elsewhere in the world. Using a TSN for navigation is in itself not new, but when this is integrated with the control system it provides the opportunity for individual drill rig operators to use this method, without the direct assistance of a surveyor. TSN gives a very precise indication of the drill rig’s position in relation to the tunnel design. An accuracy of ±1 cm compared with the ±10 cm which is normally obtained with the traditional method using tunnel laser and sight plates on the drill rig’s feeder. The closer a tunnel is blasted to its planned profile, potentially the more is saved in terms of less shotcreting, extra rock bolting and other corrective work. A better quality tunnel is the key to better overall economy and today’s technology makes it possible to achieve that goal in every type of tunnelling and mining project.In addition to total station’s ­navigation and Tunnel Profiler, the concept of ‘high precision tunnelling’ also includes:•Software tunnel planning: Tunnel Manager, Tunnel Manager Pro and Tunnel Manager Measure while drilling.•Measurement While Drilling (MWD)collects the various drilling data and the interpretation of these for the early identification of variations in the rock.•Rig Remote Access (RRA) is a support for wireless or wired communication between drill rig and planning office. The result is being able to send planning data to the drill rig and to simultaneously have the facility for logged files to be sent from the rig to the office. There is an option of an external computer logging in to the drill rig’s control system to view menu screens and settings, and remotely troubleshoot and plan appropriate action.

Tracking plant and personnel in underground environments provides multiple potential benefits for a mine including increasing safety, better monitoring of production, traffic control, and in the future monitoring and control of autonomous vehicles. The main techniques for wireless tracking are to use radio frequency identification (RFID), signal strength based techniques, and time of arrival (TOA) based techniques. RFID does not provide continuous tracking and signal strength techniques have poor accuracy in the complex radio propagation environment present in mines. TOA based techniques are potentially the most accurate, but most current equipment does not realise the potential of the technique. CSIRO has been developing the wireless ad-hoc system for positioning (WASP) system for wireless tracking in a range of challenging environments including underground mines. WASP has been licensed for sporting applications and has delivered encouraging results in early trials for a surface and an underground mining application. The system utilises a number of ‘anchor nodes’ that are placed at surveyed fixed locations within the mine, and tags attached to plant or personnel to be tracked. The system provides an ad hoc data communication network, thus wired data connections are not required to the anchor nodes. WASP uses transmissions in the 5.8 GHz ISM (instrumentation, scientific and medical) frequency spectrum to perform highly accurate ranging between nodes, hence tracking of tags. The localisation performance of the system depends upon the radio propagation environment, ranging from 0.1 m in low multipath conditions. In this paper we describe the WASP system and provide results of initial testing in an underground mine that shows that the system achieves high ranging accuracy, hence localisation accuracy.

Outokumpu Harjavalta Metals Oy has been producing nickel since 1962, based on conventional flash smelting technology followed by electrowinning of nickel. In the 1980s, a research project was initiated to develop a converterless direct smelting process to produce matte for further refining in the then existing refinery. The process was tested on a pilot plant scale at Outokumpu's research centre in Pori and was found to be viable. The process was named the Direct Outokumpu Nickel Smelting Process or the DON Process. At the turn of the 1990s, Outokumpu studied various options to develop its nickel business and the Harjavalta operations. As a result of this work, the DON Process was further developed to produce two mattes: low iron matte in the flash furnace to be leached in the existing leaching circuit and high iron matte in the electric furnace to be leached in the new leaching circuit. In Harjavalta's case, it was essential and important that the process could use the existing flash furnace and electric furnace without converters.

"A drilled pier foundation was originally designed to support the proposed G. Wayne Clough Undergraduate Learning Commons on the campus of Georgia Tech in Atlanta, GA. Reflective of the site geology, the original drilled pier foundation was divided into a Northern and Southern portion with an allowable end bearing capacity of 60 ksf and 100 ksf, respectively. After initial pricing on the original design, several value engineering options were submitted that featured changes to the original design capacity and construction method. The project team selected the value engineering option that involved conducting an Osterberg Load Cell test to assess end bearing of the site bedrock and skin friction. Results of the Osterberg Load Cell test justified increasing the allowable end bearing capacity of the site bedrock to 100 ksf in the Northern portion and 125 ksf in the Southern portion. Considering the final foundation design and Osterberg Load Cell test program, a net savings of approximately $259,000, or approximately 34%, was obtained over the original design.Project BackgroundThe G. Wayne Clough Undergraduate Learning Commons (CULC) is a 5-story academic building on the Georgia Tech campus in Atlanta, Georgia. The CULC was completed in 2011 and consists of a concrete frame structure supported on a drilled pier foundation.After pricing of the original drilled pier foundation design, several value engineering (VE) analyses were performed by the foundation contractor in an effort to demonstrate the potential for a reduced foundation cost. The VE analyses focused on variations in design assumptions, as well as construction techniques. This paper presents a case study that demonstrates how the value engineering process can be used to reduce foundation costs.Site Investigation and Subsurface ConditionsThe CULC building site is located within the Southern Piedmont Physiographic Province, an area underlain by metamorphic and igneous rocks. McConnell and Abrams (1984) indicate that the site is located at the boundary between two distinct geologic formations. The Northern portion of the site is underlain by the Wahoo Creek Formation while the Southern portion of the site is underlain by the Stonewall Formation. The following paragraphs briefly describe the geotechnical investigations performed by the project geotechnical engineer (GE) for the CULC."

"This paper presents the main features of the Wolf Creek Dam Foundation Remediation Project, highlighting the innovative construction approach and stressing on how the collaboration between the contractor and the owner became a key element for the success of this project.The main rehabilitation contract, $341 M, was awarded in 2008 for the construction of a 980,000 sqft concrete barrier. Most of this 2ft-wide concrete barrier wall is being built to depths reaching 277 ft using innovative construction techniques proposed by the contractor and under quality control measures that exceed by far the industry standards.The construction of the barrier wall entails the use of nine different foundation techniques which run concurrently following very stringent requirements in terms of verticality, distance between open elements, strength, permeability and dimensions that control the installation sequence of each barrier wall element.The innovative construction approach by the contractor and the collaborative efforts by the contractor and the USACE (“US Army Corps of Engineers”) have brought this unique and very difficult project to a successful completion, setting new standards in the dam foundation industry and nine months ahead of scheduleINTRODUCTIONWolf Creek Dam is on the Cumberland River in south central Kentucky. It provides hydropower, flood control, water supply, and water quality benefits for the Cumberland River system and surrounding region. The lake is a source of recreation that has attracted more visitors (4.89 million) than Yellowstone National Park (2.87 million).The dam and its adjacent reservoir reside upon a heavily Karst bedrock foundation. In late January 2007, the USACE placed the dam under a 'high risk' for failure designation. As a temporary measure, the pool elevation was lowered to 680 ft Above Main Sea Level (“AMSL”), 80 ft below its maximum capacity, with huge impacts to the local economy while the major, and ambitious, $ 594 million remediation program to bring the dam back to its full operating condition was carried out.The main rehabilitation contract of $341 million was awarded in 2008 for the construction of a 980,000 sqft concrete cut-off barrier. "The Bolivar Dam, located near the Village of Bolivar, Ohio is one of a system of dams designed to provide flood control and water conservation in the Muskingum Watershed. The Bolivar Dam is a “dry dam” that only impounds water during flood events. In order to address problematic seepage, which negatively affects the structural stability of the dam, the U.S. Army Corps of Engineers (USACE) designed a partial-depth and partial-length Seepage Barrier Wall (SBW) through the upstream slope of the dam. For this project, TREVIICOS constructed a 36 in (914 mm) positive SBW by plastic concrete panel wall method and excavated using hydromill and clamshell equipment. The plastic concrete was manufactured on-site, which allowed for enhanced quality control and prompt service. This required the installation, commissioning and operation of a concrete batch plant on-site. This paper presents the challenging aspects of the design, manufacture and placement of the plastic concrete for the SBW in order to meet contract strength and permeability requirements as well as workability requirements necessary for tremie concrete placement.Project General InformationBolivar Dam is located in Northeast Ohio (Fig. 1). It is one of a system of sixteen dam projects designed to provide flood control and water conservation in the Muskingum Basin, which covers 20 percent of the State of Ohio (Fig. 2). Bolivar Dam is on Sandy Creek, a tributary of the Tuscarawas River.The Dam has been constructed, operated and maintained by the USACE, for the Muskingum Watershed Conservancy District. Its construction was completed in 1937 and it has been in operation for flood control since 1939.Bolivar Dam is a rolled earth fill dam, with a typical embankment section made of a central impervious core flanked upstream and downstream by a pervious outer shell with downstream rock toe. The embankment has a maximum height of 87 ft (26.5 m) and a crest length of 6,300 ft (1,920 m), and is comprised of the main embankment starting on the left abutment of about 1,500 ft (457 m) heading to the Northeast, followed by the terrace embankment of 4,800 ft (1,463 m) heading to the North. An upstream impervious blanket extends up to 500 ft (152 m) upstream of the dam along the majority of its length. The outlet works are to the left of the embankment, and consist of an intake structure with six gates, followed by two outlet conduits that finish in a stilling basin. To the left of the outlet works is the uncontrolled emergency spillway, which was doubled in width to 540 ft (165 m) in 1989, and excavated material utilized for construction of a stability berm against the upstream face of the main embankment. A concrete parapet wall was also installed along the upstream side of the crest during this work. In 1982, a toe drain, downstream filter berm and line of 35 relief wells were installed. The filter berm was recently extended an additional 200 ft (61.0 m) downstream and granular filters were placed over an approximate 2000 linear ft (610 m) reach of the lower terrace slope. Rigid inclusions were used to support roadway fills and mechanically stabilized earth (MSE) walls for the South Capitol Street Corridor project. The first phase of this two-phase, nearly $1 billion project will include the replacement of the Frederick Douglass Memorial Bridge, including two approach ovals east and west of the Anacostia River, the replacement of the I-295/Suitland Parkway Interchange, and the construction of several traffic ramps. The bridge itself carries over 70,000 drivers daily and is expected to open in 2021. Ground improvement is required over a portion of the project footprint. Within the ground improvement area, the construction includes MSE walls up to 35 feet high and embankment fills up to 28 feet for which CMC rigid inclusions (RIs) were used to mitigate settlement and stability. The proposed construction runs over multiple old, in-service utilities that the project requirements indicate could not tolerate any stress or settlement due to the construction. As such, solutions that spanned the utilities were required. A concrete slab of varying thickness was used to span a fragile 108-inch-diameter sewer pipeline and 48- inch-square-precast-concrete box beams were used to span twin utilities that were buried just a few feet below current working grades on site. Unreinforced RIs were used to support these structures. The center to center spacing of the RIs perpendicular to the pipe run was 26.5 ft for the concrete slab and 62 feet for the precast--concrete box beams. The axial forces on the RIs were up to 267 kips. The RIs were not structurally connected to the slab or box beams. A single-element load test was performed to confirm the load-carrying capacity of the RIs. This paper will focus on the utility protection slab over the twin utilities and provide an overview of the design of the structural and geotechnical solutions and will present the results of the load test performed. INTRODUCTION The new Frederick Douglass Memorial Bridge (FDM Bridge) and reconstruction of the Suitland Parkway/ I-295 interchange (South Capitol Project) is a part of the Anacostia Waterfront Initiative (AWI) Program, whose goal is to transform the shores of the Anacostia River. The existing FDM Bridge is approximately 70-years old and has been classified as “functionally obsolete”. The completed South Capitol Project will improve the multi-modal transportation options, increase pedestrian and vehicular safety and provide additional green space. The proposed FDM Bridge, a rendering of which is shown in Fig. 1, is designed to become a landmark bridge in the Washington, D.C. area.

Rammed stone columns and similar reinforcement systems are used worldwide more frequently as a cost-effective method for improvement of foundations on soft soils. The effectiveness of the treatment method is evidenced by the increase in bearing capacity and reduction of compressibility of the treated ground. In this study, two job sites with over 500 stone columns were studied. On both jobs sites, shallow soft silty soils were found and stone columns were designed to support the foundations of MSE walls, which serve as abutments for viaducts. In order to verify the soil improvement efficiency CPT tests were performed before and after the construction of stone columns, also plate load tests were performed on both job sites to verify the deformation modulus of the stone columns. From CPT data it was observed an increase up to 30% of sleeve friction and 20% of tip resistance, which demonstrated a higher deformation modulus maintaining the same strength in the soil after stone columns construction. The plate load tests yield load-deformation curves which indicated that the original assumed modulus was conservative. As a result of these findings, optimization of the settlement reduction ratios was conducted using the modified values of deformation modulus for the soil and stone column, obtaining a reduction up to 30% of stone columns for further job sites with similar soil conditions.

Fugitive dust from mineral extraction and processing activities can significantly influence air quality, the environment, and the health and quality of life of workers and surrounding communities. Poor engagement and communication between experts and lay persons, disciplinary silos and polarised viewpoints have made it difficult to develop a holistic understanding of the complex health issues associated with environmental emissions of mine dusts, and consequently to design meaningful and integrated approaches to address such issues. The collaborative Global Challenges Research Fund (GCRF) Mine Dust and Health Network, hosted by Minerals to Metals at the University of Cape Town, serves as a collaborative think-tank to inform research directions within and across disciplines, government policy and regulations, health monitoring programmes at public clinics, industry best practice, community healthcare and impact prevention programmes. It focuses initially on the southern African region, but with the intention to expand to mining-intensive developing countries within the rest of Africa, South America and Asia. This paper outlines the approach and activities of this global transdisciplinary and intersectoral network and presents some of the key outputs and achievements to date. Interest in early gangue rejection has grown substantially in the minerals industry to reduce energy and water consumption arising from excessive comminution. Recent developments in coarse-particle flotation have proven successful by promoting a quiescent hydrodynamic environment to limit particlebubble detachment and by helping to convey more buoyant particles to the overflow. Current approaches also benefit from upstream classification to remove the finer particles ahead of the coarse-particle flotation. In this study, coarse-particle flotation was investigated experimentally using a novel hybrid gravity-flotation system called CoarseAIR™ that exploits the hydrodynamic advantages of the Reflux® Classifier. The system consists of a lower fluidised bed housing, with a system of inclined channels above. The inclined channels enhance the segregation and hence rejection of intermediate through to relatively coarse particles containing little or no surface liberation. This approach overcomes the need for pre-classification, while offering strong transport of coarse particles made buoyant through their attachment to air bubbles. The paper explores the strong relationship between the solids transport to the overflow and the input gas flux, showing firstly the potential for significant mass rejection at the relatively low gas fluxes, and robust performance in terms of recovery across the full range of gas fluxes. Development quarterly mine plans is generally time-consuming and requires consideration for many operational constraints and competing objectives. With lengthy turnaround times, there is often limited opportunity to adjust the quarterly plan details to account for differences between actual state of the mine operation at the time of execution and expected state at the time that the plan was developed. Actual mining task completion times can vary significantly from the expected task completion times, due to many factors. In the absence of revision to account for differences between the planned and actual state, resource allocations and mining sequences, embodied in the quarterly mine plan may no longer be the best approach at the time of execution to achieve an efficient mining operation. Simulation and optimisation tools can be used to support rapid refinements for development and execution of weekly mine plans. When initialized with an operations data feed to establish the actual state of the mine operation, optimisation models can be used to facilitate both automation and revision of the quarterly plan to produce a weekly or daily plan for execution. This approach can remove much of the manual burden of revision if the existing quarterly plan no longer represents the best approach to meeting production objectives. Following initialisation with mining actuals, simulation models can provide a representation of the range of likely mine operations outcomes. The models can be used to accelerate the planning process by providing visibility of future performance and identifying likely operational issues. They can also be used to test the robustness of the plan with respect to performance variability and response to disturbances. Following identification of issues, simulation models can also be used for evaluating mitigation measures, through automated testing of alternative plans. Integration of mine data feeds with simulation and optimisation tools enables automated optimal responses to variations between the quarterly mine plan and actuals to be tested prior to release of and execution plan and implementation. The use of simulation and optimisation in weekly mine planning will be discussed in this paper with reference to surface mining examples.

Basing on five years of experience, this paper discusses modern e-drive systems used in underground transport. This covers Energy distribution and cabling principles, inverters and motors as well as auxiliary aggregates. Also an overview of different regimes for battery charging and the energy management and the method of control of the traction are lined out: This means that throttle and brake are not handled separately, but integrated into “longitudinal control” algorithms which assure that a commanded speed is maintained while synchronizing the multi motor systems and taking into account the constraints imposed by e.g. slope grade and battery parameters.

Based on the mine safety and health administration (MSHA)reports, mining truck accidents are among the significant causes of fatalities in the mining industry. The major causes of haulage accidents can be divided into non-driving and driving factors, such as truck-related sudden failures and operator performance. Numerous parameters affect trucks' safe operation, but situational awareness plays a significant role. Due to the distinct structure of mining trucks, making them efficient for mining purposes, the operator has a very limited field of view (FOV), making them prone to accidents resulting in losing lives. The cabin's position blocks the truck's right and rear sides, creating several blind points around it. Aiming to test/train the operator's awareness of the surrounding, a proximity detection system is developed through a multiplayer virtual reality (VR) application to investigate the human factor in equipment collisions. The operators were examined and monitored when asked to conduct what exactly happened in the Marigold mine truck accident without being aware of the scenario. After receiving sound, vibration, and visual alarm warnings, their reaction and performance were analyzed. The platform is developed using the Unity game engine for HTC VIVE virtual reality goggles. An Omni-direction treadmill and a motion simulator platform are utilized for the player and dump truck operators. Fifteen students were monitored when driving the dump truck receiving/not receiving proximity detection alarms. None of them repeated the accident when receiving proximity alarms. Also, they found the visual alarm the most comfortable while driving the truck.

Buying centres are the first point of contact that determines whether ore will be considered as having sufficient provenance to be considered ethically sourced. The information that needs to be captured on the blockchain platform ranges from reports on in-loco inspections and audits of mining and purchase sites. This will guide passive and active monitoring of mining activities, trade-offs for identification systems, including biometrics for the registration of artisanal sellers, scrutiny on the selection of security measures and companies for buyer centres and stockpiling warehouses, evaluation of active and passive infrastructure security, and remote monitoring applications. Assistance with selection and training in geochemical testing instruments to determine the grade of ore material or concentrates and sorting, on-the-ground investigations, guidance on monitoring methods from stockpile and packing area along the transport route to the final destination are required.

Learning is an iterative process, and we might reasonably expect that after more than a century of mining during an era of sustained technological development, (following a millenium or two of unrelieved hard physical graft), that the more obvious physical hazards could by now be competently managed. It is perhaps a reflection on the perversity of human nature that we have not fully learned our lessons in this exacting environment, and that worse, we have still too often failed to profit by the experience of those lessons. The more obvious underlying factors which impact on this situation are examined briefly, as a prelude to consideration of some outstanding examples of recurrent problems experienced in recent years. Reasons for the continued existence of sub-standard practices are outlined, with a focus on the human element, and finally proposals for appropriate remedial strategies are summarised. This paper therefore seeks to examine and discuss some of our deficiences and to suggest some directions for the way ahead.
