Publication of this information circular results from an invitation extended to the Bureau of Mines by the Utex Exploration Co. to visit the company mine, on the Mi Vida claim about 39 miles southeast of Moab, in the Big Indian mining district, San Juan County, Utah. The Bureau is pleased to present this record of the Utex mine -- a property that in the relatively short period of 4 months has become one of the major uranium-ore producers on the Colorado Plateau. The ore deposit was discovered in July 1952 by Charles A. Steen, a geologist-prospector, now chief geologist of the Utex Exploration Co., in tae course of drilling the first diamond-core borehole in the Big Indian district. High-grade ore, 14 feet thick, was penetrated by the borehole. Sinking of a shaft, from which to develop the ore body, was begun in October, and the first shipment of ore left the property December 6, 1952. Ore production, as recorded on ore-settlement sheets received by the company up to July 15, 1953, totaled 16,000 tons averaging, 0.51 percent U305 and 1.25 percent V205. Discovery of the ore body and the large production of ore were achieved without any financial assistance from the Government.

THE John Fritz Medal Board of Award, at its annual meeting on Jan. 21, 1921, awarded its gold medal and diploma to Sir Robert Hadfield for the invention of manganese steel. On June 1, announcement was made that the Board of Award had awarded its gold medal and diploma to Charles Prosper Eughe Schneider for "achievement in metallurgy of iron and steel; for development of ordnance, especially the 75-mm. gun, and for notable patriotic contribution ' to the winning of the war." The award to Sir Robert Hadfield is for the year 1921; the award to Mr. Schneider is for 1922. The latter was made at this time in order that the medal might be presented by the group of American engineers now visiting England and France.*

This report presents a literature review conducted by the U.S. Bureau of Mines. The review summarizes what has been learned from the large volume of empirical studies on the effectiveness of eight types of interventions intended to increase organizational effectiveness via in the management of human resources. The interventions concern the following: recruitment, selection, training, objective feedback, goal setting, reward systems, participative management practices and organization development. Although many of these interventions appear to have the potential for improving the safety and efficiency of mining operations, it does not appear that they are being widely used. Examples are given of the successful use of a few of these interventions at mining operations. An analysis of the relationship between coal mine productivity and safety is also presented. This analysis generally supports the view that good safety performance and high productivity are NOT incompatible goals. In fact, safe mines also tend to be more productive and vice versa.

This paper describes the mineral deposits and discusses mining and milling methods and related operating conditions in an important gold , silver , lead , and zinc producing area in Ouray , San Juan , and San Miguel Counties , Colo . , embracing 250 square miles . The area is extremely rigged , ranging in altitude from 8,000 to 14,000 feet . White men first penetrated the area in 1860 , when a group known as "Baker's party " reached the Animas River at the present site of Silverton . Difficulty with the Ute Indians and severe winter conditions forced the group to leave and discouraged prospecting until 1870 when a second party reached the area and succeeded in producing 27 tons of ore yielding $ 150 a ton in gold . In 1874 , a treaty with the Indians opened the way for settlement , and mining was established . The total production of recovered gold , silver , copper , lead , and zinc from 1873 to 1947 in the three counties is valued at $347,057,047 . The greatest amount of the total production of metals has come from quartz veins in a series of volcanic flows and tuffs covering the area and comprising a blanket 4,000 to 6,000 feet thick . The igneous rocks were laid down upon an old , erosion surface and are underlain by Paleozoic and Mesozoic sedimentary rocks and pre - Cambrian metamorphic rocks . The entire series has been intruded in late Tertiary time by volcanic dikes and larger intrusive masses . Because of the great differences in relief generally , mining is con-" ducted through long adits to a depth of nearly 3,000 feet below surface in places . Walls of the veins generally stand well , and shrinkage stoping is the most common method of mining . As virtually all of the ore is sulfide below the zone of oxidation , flotation is the common method of milling . Generally , where gold is important , provision is made for its removal by jigs . Bulk concentration of the sulfides is practiced at smaller mills , but the larger operations make a selective lead and zinc concentrate or a lead , zinc , and copper . concentrate . ·

Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are improving but more slowly and yet, serious accidents and fatality rates are not: in fact, these rates of more serious incidents have plateaued or deteriorating. In more dramatic cases, organizations that had ?exemplary? safety statistics, suddenly had a catastrophic or multi-fatality event occurring. Popular books such as In Search of Excellence of Peters and Waterman or Good to Great of Jim Collins created many delusional ?secrets? or recipes, to achieve great performance and yet, most of those ?great? or ?excellent? companies have now declined in their performance, some dramatically and some have fallen from grace, like Fanny Mae, a company Jim Collins identified as a ?great? one, that had to be bailed out in the latest financial crisis. These organizations share several ?delusions? about managing a business, and often, these delusions are created by academics, consultants and authors of popular books about (safety) management. More damaging however is that safety management is defined by a set of key practices or axioms: a focus on human error, compliance to rules and regulations, creating consistency of systems and actions, reacting to accidents and incidents and on prevention of those, measuring safety with performance metrics and rewarding employees on those measurements. All of these axioms contain the seeds of destruction, and they create conditions and dynamics that precede the disaster - and ironically, they are all 'good things' we do in safety management. For example, a mere focus on safety in the business, and the measurement of it, create a culture of repression of mistakes and incidents, leading to a situation where the organization knows incrementally less and less of risks and risktaking behaviour - until the day it all coincides destructively! This paper discusses seven such safety delusions and how they impact on the views and safety management approaches. They are: ??The delusion of (risk) control, that risks can actually be identified, analyzed and mitigated. ??The delusion of compliance, that mere compliance to rules will safeguard the business ??The delusion of consistency, that standard operating procedures will ensure safe operations ??The delusion of predictability, that accidents and trends tell us everything about the risks of the future ??The delusion of human error, that people are the weakest link and that it can actually be dealt with through discipline and enforcement. ??The delusion of statistical trends, that safety is measurable with statistics ??The delusion of invulnerability, that we have achieved high levels integrity and capability to deal wit risks. Organizations that suffered catastrophes all share some or all of these delusions and this paper traces how these played a role in many disasters, ranging from the Titanic, Challenger, Piper Alpha, Bhopal, BP Deepwater Horizon and many more.

The recent spate of catastrophic events at U.S. mines raises the question as to whether or not the mine safety and health system in the U.S. is broken. The prescriptive regulatory regime in the U.S. is not a cure-all for a broken system. Without totally dismissing the contribution of a prescriptive regulatory approach, this paper argues that safety in the U.S. mining industry would be enhanced if the nation adopted a scheme that provides a balance between specification standards and two alternatives: performance and systems-based standards. This would allow effective and flexible means to continuous health and safety performance improvement. Mine operators, regulators and politicians could then go beyond thinking that compliance alone equals strong safety by creating a proactive system that emphasizes continuous improvement. By adopting performance and systems-based standards, exemplary safety and health performers can be recognized, allowing regulators to focus their efforts on those without sound occupational safety systems and who have substandard safety performance. The widely accepted kinetic model for the gaseous reduction of hematite is critically examined. The formation of the wustite layer during internal reduction cannot be explained in terms of solid-state diffusion. Present evidence indicates the existence of a micropore system in the wustite adjacent to the magnetite with the gas/solid type of reaction occurring not only at the iron/wustite interface but also at the wustite/magnetite interface. During the partial reduction of dense hematite pellets, in a controlled atmosphere, porous magnetite and wustite are formed as products. The mean pore size, as measured by mercury injection, increases with time at constant temperature and with temperature increase over the range 6500C to 950°C. The sizes of the pores indicate that Knudsen diffusion is the principal gaseous transport mechanism across thick layers of magnetite or wustite.The kinetics of the gaseous reduction to iron are consistent with a mixed control mechanism involving the interaction of gaseous diffusion effects with a first order reversible chemical reaction at the iron/wustite interface. Because of the larger pore sizes in the iron layers, ordinary molecular gaseous diffusion must be the principal transport mechanism. The usual experimental methods used for establishing the validity of chemical rate ~ontrol are incapable of differentiating between the mixed control- mechanism and exclusive chemical control.Some implications of mixed control kinetics are developed, including a predicted independence of rate on reducing gas pressure at higher pressures.INTRODUCTIONAn extensive literature review of the reduction of iron oxides has been compiled recently by Themelis and Gauvin (1962). From the large number of papers published and the widely divergent opinions of workers in this field, it appears that a simple unified quantitative analysis of reduction kinetics is not possible. Introduction During the last decade, the metallic minerals segment of the industry could be categorized as a "feast-or-famine" business. It is no secret that metallic minerals are cyclical. Employment opportunities greatly depend on the supply and demand for its mineral products. It is also not unusual for there to be a lag between the industry's manpower requirements and academia's ability to produce new graduates. Due to this lag, there will be years with manpower shortages, as in the mid-1970s. At other times, supply will exceed demand. The question is where will the jobs be in the years to come? This presentation will review reasons why employment opportunities have declined within the metallic minerals industry. It will also predict future demand for mineral engineering graduates. And it will outline strategies for obtaining an employment offer. Historical Review - Employment From 1977 through 1983, about 75% of all metal mining employees were directly engaged in production activities, down from 83% in 1965. This decline reflects increased use of labor saving equipment, and more government regulation and foreign competition. Metal mining's employment decreased from 74,000 jobs in 1982 to fewer than 68,000 in 1983 - an 8% decline. The overall industry is having these problems because of the general downturn in the economy. For example, at the beginning of this decade, US automobile and steel industries were in trouble. Interest rates were too high and volatile. This adversely affected construction, housing starts, and manufacturing. All these industries, being large users of base metals, drastically affected the metals industry. US copper companies are still having problems. Reports indicate that many copper employers have cut production, laid-off workers, and permanently closed mines, refineries, and smelters. A decline in the demand for US produced metal products, such as copper, obviously affects some mineral engineering employment opportunities. Other factors to consider when looking at employment opportunities and why it has become difficult for US metal producers to compete in the world market include: • The grades of ore bodies of many US minerals are considerably lower than the grades of ores being mined elsewhere in the world. • The grades of ore bodies currently being mined in the US are steadily declining. • Foreign producers are largely government controlled and are not necessarily subject to the realities of supply and demand. • US environmental regulations have considerably added to production costs. Foreign producers in most countries do not have to worry about these additional costs. • US mines are getting deeper. This adds to the cost of production because of the increasing distance the ore has to be transported. • Plants and equipment are outdated. Many companies find themselves operating with equipment and plants that need modernizing or replacing. This obviously hinders competitive productivity. • Labor and management have had problems. Work stoppages in certain mining businesses and high wage and benefit costs in relation to foreign producers place US producers at a disadvantage. • Much land is unavailable. The US government owns most of the land considered to have the best potential for minerals development. Most of this land is unavailable for mining or exploration. Therefore, it appears that finding employment with a company whose primary business is mining and processing most metals, such as copper, in the US will be quite restricted and competitive. Future Demand It is difficult to anticipate what key employment opportunities within the next few years will be available to mineral engineering graduates. The Engineering Manpower Commission estimated that US engineering schools awarded 72,471 undergraduate degrees in 1983 vs. 66,990 in 1982. This was the eighth consecutive year in which the number of baccalaureates awarded has increased. On a brighter note, the US government's Occupational Outlook Quarterly stated that the growth rate through 1995 for all engineers is an encouraging 49% over present levels. South Africa is one of the leading producers and exporters of coal globally. A significant amount of the country’s production is obtained from previously mined underground bord and pillar workings. This coal is in the form of pillars and remnants on the roof and floor of the old workings. The good quality coal pillars were left behind as primary support during underground bord and pillar mining operations. Due to the depletion of virgin coal reserves, the pillars and remnant coal are now removed using opencast mining rather than underground methods. However, the secondary extraction of pillars and remnant coal from the old workings using opencast methods entails some serious challenges that have a negative impact on the safety and productivity of the operations, affecting both personnel and machinery. If these risk factors and challenges are managed properly, then the opencast mining operations could remove the pillars safely at recoveries competitive with those of virgin coal operations. In this study we review the recurring challenges affecting opencast pillar mining by means of field investigations and consultations with experts at five opencast pillar mining operations, and evaluate the best practices used to combat these challenges. It was found that each mine has its own unique conditions and challenges.

The research project reported on represents the first comprehensive attempt to measure the vibration levels of a variety of tools and equipment in the South African mining industry to determine the effect thereof on the health of workers and operators. Although human vibration has been studied extensively internationally only one reference pertaining to the South African mining industry could be found. From studies conducted in Canada and elsewhere it was evident that rock drills do have potentially hazardous levels of vibration. The scope of the research included measurements on a broad spectrum of tools, machines and vehicles used in the South African mining industry. Both hand-arm vibration, where the majority of the vibration enter the body through the hands, and whole body vibration, where the whole body is exposed to vibration through contact by the buttocks or feet, were investigated and measured. More than 700 sets of vibration data measured on 70 machines at 15 mines and workshops were obtained. Data were obtained for at least 24 different types of equipment and machines that were previously judged to have medium to high levels of vibration. All measurements were conducted in accordance with the international standards: ISO 5349, on hand-arm vibration, and ISO 2631-1, for whole body vibration. The equipment with the highest vibration levels in the hand-arm category was: rock drills, both pneumatic and hydraulic; pavement breakers and jackhammers; hand-held compactors; pneumatic wrenches and electrical hammer drills. Of these the rock drills had weighted vibration levels in excess of 20 m/s2. As far as whole body vibration was concerned the highest values were measured on earth-moving equipment. The major conclusion from this study is that the measured vibration levels are sufficiently high to create an enhanced level of risk of vibration-induced disorders in a significant proportion of the operators of hand-held equipment. The most notable examples are hand-held rock drills, pavement breakers, jackhammers and certain selected workshop tools. The results of the study indicate that a comprehensive epidemiological study should be undertaken on present and past workers to determine the prevalence of vibration-induced disorders.

The relationship between neural network (NN) performance and the parameters that form the architecture of the neural network is complicated. This paper studies an issue that is still controversial—the effect of the number of neurons and number of hidden layers on NN performance. This paper tests if the above parameters affect network performance when the NN is trained according to the quick stop strategy. It was determined that the number of neurons did not have any effect on the NN performance for a single layer or a double layer network. It was also found that the training error and calibration error could be poor indicators of the prediction correlation coefficient.

At that time the financial markets were on the verge of collapse and my presentation described the issues. Unfortunately I was correct, and global markets went into a steep nosedive, creating the largest economic downturn since the Great Depression.In hindsight, there were plenty of signs that this was coming. The Supercycle had created massive new wealth. This wealth was largely invested in the United States due to the country’s political stability and credit worthiness. In 2004, the large investment banks convinced the U.S. Securities Exchange Commission (SEC) to allow them to triple their borrowing leverage ratios. This made the banks more vulnerable to market volatility. The banks then lent money to NINJAS - people with no income, no job and no assets. Even worse was the fact that these unqualified home owners used adjusted-rate mortgages (ARMs) which they, of course, intended to refinance prior to interest rates rising. These mortgages were then bundled up by the banks and sold to investors as “safe” investment products. The money flowed into these products because the rating agencies gave these instruments high marks. These same agencies earned their fees from the very same banks seeking high ratings. (No conflict there). Private debt exploded and peaked out in 2008.These mortgage products were sold in an unregulated shadow banking system and the ARMs came due. Toxic mortgage default rates skyrocketed and the mortgage-backed securities collapsed in value. This house of cards had a negative feedback loop that accelerated the market crash and resulted in more than $1.5 trillion being lost in the securitization sector, a 47 percent decline in the Dow Jones Industrial Average, foreclosure rates skyrocketing and housing prices collapsing. The financial markets were in a nosedive."

Longwall face-to-face equipment transfers or moves are, for many companies, the largest single source of non-productive time in a longwall mining system. Detailed planning and resourceful thinking are paramount for economical, safe, and timely longwall moves. New equipment and innovative techniques are frequently tested in order to reduce overall move time. Determining the cost effectiveness of these ideas is difficult since the decision is influenced by many subjective factors. A project scheduling simulation model of the longwall move process was developed in order to analyze and assess the economic viability of innovative transfer methods and equipment. The model provides a project duration frequency distribution and lists of critically indexed activities and paths. It compares the monetary savings from reduced transfer time and capital costs for several innovative ideas.

"The most important factor in Canadian mining is not the operating cost of current projects but the availability of capital for future projects. The future of mining at depth will be producing ore from the top of the orebody working downward with very effective, tightly controlled mining methods, i.e. either selective mining or bulk mining with well controlled dilution and fragmentation. The key characteristics of future mines will be smaller access excavations, more effective ground support, very precise blasting techniques and more efficient and reliable materials handling systems. Equipment will be smaller and designed for high efficiency and low maintenance, reducing both the capital and operating cost. This means there will be fewer scoops, more continuous mucking machines and conveyor belts, both lateral and vertical.Orepass failures have become accepted and the resultant redevelopment and retramming has become common. The capital investment in so-called permanent ore passes and crusher stations becomes questionable, particularly when crushers are only necessary for less than 10% of the ore going through the system. The best way to solve the chronic instability of ore passes is, as with all of the stability problems at depth, by reducing the excavation size rather than increasing ground support. Smaller access drifts would reduce the capital cost and increase the rate of development as well as reduce the amount of waste to be handled, reducing the ground support and the ventilation requirements. Large drifts and ore passes are essential only for the equipment needed to handle large pieces of ore and the productivity of scoops is very expensive when the requirements for ventilation and large access drifts are taken into account. The key issue is the fragmentation generated by primary blasting.The future of mining in Canada will depend on simple and robust technology geared to the environment in which it will have to operate. Its survival will depend on some-thing much more fundamental and difficult to accomplish than a technological advance — it will require a dramatic change in the way the mining process is managed, how work is organized, how effort is rewarded and how costs are measured — a complete change in mind-set."

In a field test at Mount Isa Mines Ltd., a system developed by ICI Australia Ltd. for bulk loading "MOLANITE", an ammonium nitrate slurry blasting agent into ring drilled holes of wblcvel open stopes, was shown to be satisfactory. The fragmentation produced by the slurry was a significant improvement over ANFO and the stope production rate was increased by approximately 50%. Indications are that the charging rate and labour costs will not be greatly changed if a slurry system is used.

Cross-belt samplers are designed to sample particulate material in processing plants, usually when there is no space to install a conventional linear falling stream sampler. Most of the available cross-belt samplers are mounted on the top of conveyor belts, and remove each increment from a loaded moving conveyor with a 360° rotation of an open faced cutter, in a plane perpendicular to the material flow. Despite the practicality of this equipment and the fact that it occupies very little space in the plant, cross-belt samplers are well known for collecting biased samples, especially due to the increment extraction error. Sampling consultants around the world tend to condemn this sampling equipment, claiming that it is incapable of collecting the fine material from the bottom part of the load, and, since the sampling probability is not uniform for all fragments and depends on its position in the stream, this kind of sampler generates biased samples and its use should be avoided. Furthermore, the greater the segregation effect between particles, the larger the bias. However, some plants, whose initial design did not plan for the installation of a falling stream sampler, must rely on cross-belt samples for process control and reconciliation. For the past years, sampling equipment manufacturers have been putting effort in modifying the cross-belt sampler design in order to comply with the principles of correct sampling. Changes to the cutter material, shape, position, speed and movement in relation to the material flow have been made, and are continually being tested for different types of ore with various chemical and physical characteristics. This paper presents the improvements made to a cross-belt sampler at a gold plant feed in Brazil and describes the tests performed until it was possible to validate the new sampling equipment for reconciliation purposes.CITATION:Chieregati, A C, Amaral Jr, E A and Souza, J C O, 2017. "To perform a probabilistic risk assessment (PRA), a method of quantifying the risks associated withinfrequent, high consequence events, consequence models that generate representative estimates of human health consequences for a wide variety of representative scenarios are commonly used. Criteria that suitable models must possess include: (1) input requirements necessary to generate large volumes ofprobabilistic simulations; (2) the capability to output injuries with the necessary details and resolution;(3) the ability to capture the variety of different scenarios inherent in an assessment; (4) acceptablecomputational requirements; and (5) sufficient stakeholder buy-in for community acceptance."
