The purpose of the development described in this report and as stated in the contract work statement is to "design, develop, fabricate, test and demonstrate, and deliver to the government permissible low cost cylindrical stress gauges and associated components capable of measuring change of stress as a function of time in underground coal mines. The cylindrical stress gauges shall be suitable for installation in bore holes". The technique adopted for measuring rock stress changes is to preload a steel cylinder diametrically across the sides of a 1-1/2 in diameter bore hole and detect deformation of the cylinder from stress changes in the rock, by means of a vibrating wire transducer. By calibration, changes in the period (time for one wire oscillation at the natural frequency) of a high tensioned wire stretched diametrically across the cylinder in the direction of load application are related to stress changes in the rock. To completely evaluate the stress field change in the plane of the bore hole, three gauges set at an angle to each other are required.

Spin-fin pipe piles can develop large loads in tension, thus have evolved as a foundation product of great potential. Currently, over 500 spin-fin piles have been used successfully in dolphins, docks, buildings and bulkheads. Spin-fin piles are pipe piles with plates welded near the tip at a batter-in-screw fashion. Thus, as the pile is driven, the pile screws into position. Tests show this pile type to have highly unusual stress/strain relationships, wherein plunging failure is usually never reached either at normal or large deformations. Tension capacity can approach pile capacity in bearing; thus, large reserves are usually available for tension. Tests have been performed on 8-5/8-inch to 24-inch-diameter piles in silt, sand, and gravelly soil, to provide a broad spectrum of data.

The effectiveness of an industry?s safety activities can be gaged best by the accident records of that industry. In an effort to gage conditions in the petroleum industry, and with the objective of helping to reduce the number of accidents in it, the Bureau of Mines from time to time has tabulated, analyzed, and published pertinent statistical information3/. These reports have dealt with distribution, causes, and costs and related phases of accidents for various and collective divisions of the petroleum industry in several sections of the country. This type of study aids the personnel of the Bureau in planning the course of its activities in accident-prevention work and gives useful data to persons engaged or interested in the promotion of safety in the petroleum industry, which will aid them in their efforts to reduce the number and severity of accidents. A majority of petroleum operators and ?service" contractors have acknowledged their responsibility to provide their employees with safe working conditions and to their stockholders for greater economic value of invested capital by establishing safety divisions within their respective organizations. Such divisions have a primary duty, namely, to see that properly designed and guarded machinery and working equipment are furnished the employees and that they are trained in the safest practical methods of performing their work. The Groote Eylandt manganese ore deposits contain mainly pyrolusite and cryptomelane with lesser amounts of the manganese minerals lithiophorite, todorokite, psilomelane, and nsutite. The major non-manganese minerals which occur are quartz, kaolinite, and goethite.Determination of the mineralogy relies mainly on x-ray diffraction analysis. Quantitative methods for the determination of pymlusite and cryptomelane in Groote Eylandt manganese ores have been developed, A large amount of isomorphous substitution may occur in the manganese minerals and a number of minor and trace element associations have been noted. A correlation has been established between the potassium and cryptomelane contents for a wide range of ore types. Anomalous calcium, magnesium, and sodium values occur in samples containing todorokite, and barium is associated with psilomelane. Groote Eylandt lithiophorite appears to contain negligible amounts of lithium (<O· 05 per cent).A relationship between the H20+ and the Al203, K20, and Fe203 contents has been estahlished indicating that much of the combined water content is due to the presence of kaolinite and goethite, and cap be removed by beneficiation of the ore.

Wardrop Engineering and the Placer Dome?s Campbell Mine Design Team utilized leading edge directional raise boring techniques and an innovative, remote controlled application of steel fibre reinforced shotcrete ground support to achieve improved mine ventilation and a superior, safety focused secondary emergency egress. While more standardized methods may have provided a sufficient solution to either the requirement for a ventilation raise or for a traditional escape ladder way, innovative methodology ensured both objectives were met with a single, enhanced solution. Placer Dome committed to provide a solution that minimized energy consumption, optimized air delivery and facilitated an escape way focused on minimizing the negative physiological aspects associated with mine evacuation. The project was successful in providing a new airway, returning 47 m3/s (100,000 cfm) from the 39 Level to 30 Level, minimizing risk to the raise equipping crew and providing a fully equipped escape hoist. The resulting secure escape environment provided an efficient means of evacuating personnel working at depth without necessitating a potentially dangerous exhausting climb to surface. The project has substantiated, that optimized safety focused design can be achieved without jeopardizing project economics.

The development of research activities at Risdon and the implementing of a policy of instrumentation in plant processes have, in recent years, entailed a considerable increase in research staff and created an urgent need for more space and modern laboratory facilities. The existing laboratories have been in use for 25 years and are now inadequate. New buildings have been authorized.The new buildings will comprise, firstly, a laboratory building of three floors giving a totai .floor space of approximately 11,000 sq.ft., and a pilot plant building as an annexe to the laboratory building, of approximately 4,900 sq.ft. The laboratory building will accommodate from 20 to 22 research workers, and additional plant operatives will be used for the control of processes in the pilot plant building. The new buildings are designed to provide facilities for three principal functions; the investigation and improvementof existing processes, the development of new processes up to the pilot plant stage, and the control and maintenance of instruments.LOCALITY AND ORIENTATION OF BUILDINGSThe site of the buildings is elevated ground on the eastern side of the main plant, near the main entrance and general office building. SINCE oil journals commenced to feature the progress of Iraq pipe-line developments and since newspapers undertook to follow the discussions between a certain large oil company and an Asiatic nation, I have repeatedly been asked questions that are only answerable by setting forth the essential geographic and historical facts of concessions in that part of the world. In so doing effort will be made to correct erroneous statements made in various quarters: and mistakes herein are being kept as low in number as possible. Perhaps the best-known concession in the Middle East is the one of the Iraq Petroleum Co., covering the greater part of the portion of the kingdom of that name- lying east of the Tigris River. West of the stream is the concession of the British Oil Development Co., which takes in all presumed petroliferous areas in western Iraq. No other important oil concessions exist in that country, except for the lands of the Khanikin Oil Co., which operates in "Transferred Territories" on the border of Persia. East of and continuous with the Iraq fields are those of Persia where the Anglo-Persian Oil Co. holds the well-known D'Arcy concession comprising three-quarters of the country, omitting the five northern provinces. This is the only valid existing concession in Persia, so far as known, except certain old firmans that permit development at Semnan in central Persia and possibly also certain regional grants to a French banking syndicate in eastern Mazanderan Province.

The objectives of this paper are to introduce the International Code of Practice for Risk Management of Tunnel Works and to illustrate to the participants of the 13th Australian Tunnelling Conference 2008 how the insurance industry would like to see the future of tunnelling insurance. Tunnel construction projects have suffered large losses over the years as can be shown from various loss examples and a list of major losses over the last ten to 15 years. Since the consequences of insufficient risk control had to be borne by insurers, these losses have heightened the need for improved and adequate risk management measures so that insurance cover can also in future be provided for this hazardous industry sector. Observations are made as regards to the prevailing trends in the tunnelling industry, which contribute to the increase in risk and have adverse consequences for the insurance industry. To find a solution to this dilemma various options are examined with the suggestion of a Joint Code of Practice for Tunnel Works being agreed by all stakeholders, which would become essential to secure future insurance protection for such project work. The British Tunnelling Society has, in cooperation with the Association of British Insurers, developed such a code, which the International Tunnelling Insurance Group has now adopted for global application in international markets. The content and concept of this code is explained to establish a framework of understanding and applied risk management principles for tunnel works. This initiative also has the support of the International Tunnelling Association and the International Association of Engineering Insurers. The international application of this new code is demonstrated with a few examples, which also includes Australian projects. It is the aim that this code forms in future part of the insurance coverage for tunnelling works in order to put this risk segment on a more sustainable basis. For that reason the insurance industry is seeking the support of all stakeholders of tunnelling construction projects.

The fines and legal costs even for failing to file the regulatory forms could deliver a serious blow to the financial well-being of a small business. Forms and reports under SARA Title III are not hard to complete., but they are time consuming. In this paper we will discuss the information and data compilation procedures, simplification of some legal terminology into simple technical language, and tips that help in filling out the forms?. Some actual case 'histories of EPA fines will also be reviewed.

En este sentido, surge el proyecto de integración de la unidad Antapaccay, donde se busca prolongar la vida útil de la mina y por consiguiente, el de la actividad económica minera en la provincia de Espinar. Por ello, el presente trabajo, describe los efectos de esta expansión de la explotación minera a través de la Unidad Minera Antapaccay que utiliza las instalaciones de Tintaya, manteniendo los planes de inversión social y el desarrollo económico local y regional.

Goldex Mine is an underground mine located near the town of Val-d?Or. Multiple innovations were required to work the mine. Due to its low grade, it was important to develop a bulk mining method to economically extract the ore. The method used is a combination between shrinkage stope and long hole stope methods. This modified method is more than adequate; however, all of the ore body must be liberated in a very short period of time. Therefore, to bring down 23 Mt of ore, large blasts up to 1.8 Mt are required. The situation is also complicated by the fact that the mine is located in a residential area. No less that 40 homes are built within a 1 km radius of the mine site. In addition to managing mass blasts, means must also be found to mitigate the vibrations produced by the blast to diminish the disturbance to the neighbourhood. Therefore, the mine used numerous tools to properly plan the blasts and reduce the impacts. A program to control vibrations was one of the first tools put in place. The program?s aim was to draw up an action plan to ensure a follow-up of the homes by inspecting them. The second tool was to develop a methodology to plot a reliable attenuation curve and to adequately determine the maximum charge weight for each blast. Finally, effective communication tools were developed to inform the neighbourhood. There was a huge amount of work to be done in relation to the bench. Reliable and fast loading techniques had to be found to load large blasts in a very short period of time. Adequate logistics had to be developed to send underground more than 20,000 kg of emulsion per day while performing the daily underground operations. Technology had to be developed to adapt the i-kon electronic blasting system to large blasts. Since the control of the charges was the major factor to reduce surface vibrations, it was important to employ loading techniques that would permit decking of the long holes to reduce the charge weight per delays without adversely affecting the loading. All of this work requires an enormous amount of energy but also close collaboration between the different stakeholders (various departments, suppliers and consultants).

This paper presents a dynamic analyses of heap leach pad stability under high phreatic levels and various levels of seismic loading. The dynamic analysis were conducted as fully non-linear two dimensional analyses with fully coupled liquefaction triggering using the Fast Lagrangian Analysis of Continua (FLAC) computer code. Also, the geotechnical parameters, appropriate earthquake record selection and modification, model calibration, numerical analyses and results are discussed.

ADECO, which stands for ?Analysis of Controlled Deformations in tunnels,? now allows us to fulfll Rabcewicz?s dream in any stress-strain condition. In order to achieve that dream and its consequent control over cost and schedule, however, NATM must be abandoned for the ADECO. The paper presents the basic concepts in the ADECO approach to design, construction and monitoring of tunnels together with some case histories, including: full face excavation for Cassia tunnel (width of 22 m, height of 14 m) in sands and silts under 5 m cover below an archeological area in Rome, Italy; Tartaguille tunnel (face area > 100 m2) advanced full face in highly swelling and squeezing ground under 100 m cover where NATM led to catastrophic failure, France; and 80 km of tunnels (face area > 100 m2) advanced full face in highly squeezing/swelling ground under 500 m cover for the high-speed railway line between Bologne and Florence, Italy (turnkey contract).

Mine operators and managers for a number of years have known that the maintenance of health among the workers has an appreciable financial value, In order to maintain the health, it has been found ne cessery to imow; 1. The health hazards existing in the industry as a whole and the specific hazard in the individual mine. 2. The physical condition of applicants for employment as well as that of the old employees. This can be determined only by systematic physical examination of all applicants and of regular employees at intervals of at least once a year thereafter. 3. All cases of sickness that occur among the workers. No operator or manager, or personnel, medical, or safety organization can control or prevent sickness without knowing where, when, and under what conditions the sickness actually occurs. In order to obtain information as to the hazards of industry, investigations have been and are being made of the effects of the condi- tions both in the field under usual working practices and in the laboratory. In the field several factors are generally present which affect the health. The effect of these factors may be determined by studying each of them separately in the laboratory. However, both the field and the laboratory studies are limited in value in that such studies usually have been made over a comparatively short period. While we have some information as to their immediate effect, we do not know to what extent extreme heat, humidity, the presence of dust or other health hazards are the cause of disability. Mortality statistics are probably the most easily available indices as to the hazards of industry. As an example, a study of such statistics among coal miners of Indiana shows a marked increase in the death rate in 1918.

PJV Dome Mine has used some form of gravity concentration since start of milling operation in 1909 to recover free milling gold. Amalgamation and conventional jigs were first used and were replaced with the Knelson Concentrators in 1993. The Knelson concentrates produced were upgraded on a conventional shaking table prior to smelting and refining into gold bullion. In May 2002, a comprehensive review of the latest gravity recovery technology led to a piloting campaign aimed at further improving gravity recovery. The Knelson concentrates, subjected to intensive cyanide leaching in an Acacia pilot plant, were found to be amenable to intensive cyanide leaching with over 98% of the gold being dissolved within 24 hours. An Acacia Intensive Leach Reactor was successfully installed and commissioned in December 2002, to replace the shaking table. his paper describes pilot scale testing, installation, commissioning and optimization of the Acacia Intensive Leach Reactor. Also, the economic impacts on down stream processes are highlighted.

By March 2001, shrinking global markets, reduced consumer spending and declines in domestic manufacturing and industrial output ended the longest economic expansion in US history and pushed the nation’s economy into its first recession in a decade. Aggravated by the terrorist attacks of Sept. 11, the recession led to large reductions in the domestic production of processed mineral materials. Some of the most significant production declines in the US metals industry were registered by aluminum, copper and steel producers. They faced strong foreign competition, higher energy costs and lower prices for their products. However, homebuilding and other domestic construction sectors — major users of nonmetallic mineral products such as cement, brick, glass and stone — remained strong enough to help raise the output of industrial mineral materials slightly above previous year levels. A strong US dollar continued to weaken the competitive stance of US metal and nonmetallic mineral materials companies alike against foreign producers in markets at home and abroad.

It seems necessary, at times, that we should reconsider some of the more obscure causes that lead to explosions of gas and coal-dust in mines, in the hope that a recapitulation will keep before us that old and true saying "eternal vigilance is the price of safety". This explosion, while not extensive (resulting in only two much to be regretted deaths) enlarged our vision as to the possibility and probability of other explosions with more disastrous results. As this is only a partial report I shall have to omit many things that are generally included in describing a mine explosion. The mine was worked exclusively with electric cap-lamps -only firebosses and other officials carried safety-lamps. All shots were examined and fired by firebosses with a small hand-battery. Monobel was used exclusively for blasting, and clay for tamping. The system of haulage iri use was main-rope and winches, horses being employed for gathering. Compressed-air was the motive power for all winches and pumps, excepting the pumps at the shaft-bottom which were steam-driven. The mine was wet with practically no dust anywhere, except what was being made at the coal-face in the getting of the coal. Gas was found occasionally, generally in the brushing, especially where the brushing was carried up to a small seam, 2 to 6 feet above the main seam. The place where the explosion took place was known as Leigh's heading, which was being driven to the rise, immediately inbye of the siding (off No. 1 left, off the new slope) and was about 90 feet up from the level.
