The gold that is not recovered by a particular process is refractory to that process. A study to determine how the refractory gold is associated in the ore constituents usually indicates the needed improvement in the process or additional methods for bettering recovery. In preliminary work, segregation of the various minerals, gangue or impurities, and data showing the reactions of each in the recovery method readily confines the refractory gold to a certain part on which extensive research can be directed. In this dissecting plan, the microscope is an important aid in locating the refractory gold but should be supplemented by chemical tests.

This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. This U.S. Bureau of Mines study examined spontaneous heating episodes in coal mine pillars in an active underground coal mine. The information obtained from these incidents was then analyzed to learn which sampling methods provided the earliest indication of pillar heating. The objective of this study was to discover if the location of future events of pillar spontaneous heating could be inferred from the available information. The spontaneous heating-prone area in this evaluation involved pillars located just inby the mine portals. Several detection methods were used to determine gas levels outside as well as inside the affected pillars. It was hoped that, by incorporating external and internal sampling methods into an organized pro-gram, locations undergoing spontaneous heating could be determined more readily. This study found that by drilling small-diameter boreholes into the pillars, then obtaining gas samples from the affected pillars, the ability to locate early spontaneous heating episodes was improved. However, the ability to accurately predict future spontaneous heating events remains in question.

Underground mining, like many high-risk industries, is facing a crisis in the near future. This industry has traditionally relied on a master-apprentice relationship to train new workers, but experienced miners are retiring in large numbers, leaving few "masters" to properly train their replacements. With a recent resurgence in mining, many new miners are entering the workforce each day. The rise in the number of inexperienced workers has implications for worker safety in an industry in which one mistake can blow up the entire workplace. The explosion in the Darby Mine in May 2006 that killed five miners illustrates what can happen in hazardous environments such as underground mines. The Health Communications Team at the National Institute for Occupational Safety and Health Spokane Research Laboratory was asked to address this problem by developing new training and safety awareness materials to promote safe flame cutting and welding practices. This article describes how occupational ethnographic techniques provided insights needed for "new product development." The result of this work was a national initiative to raise awareness and to protect underground coal miners from the dangers of flame cutting and welding in an environment filled with solid fuel and methane (natural gas). Although this project was initially designed for the mining industry, the approach and lessons learned are applicable to any high-risk industry and should be of interest to other social marketing or occupational safety and health professionals.

A power supply to operate fluorescent or mercury vapor lamps from 300 volt dc coal mine trolley wires has been developed for the Bureau of Mines. A high frequency regulated inverter unit incorporates ballasts for a mix of lamp loads up to 650 watts. For 600 volt trolley systems an additional power converter unit was developed to step down the dc voltage to 300 volts for the inverter unit. Both units have operated in a working coal mine for over 12 months without need for servicing. The lamp load in these tests included two 100 watt mercury vapor head lamps and six 75 watt fluorescent lamps.

A review of technical literature of the last quarter century discloses that much experimental work has been carried on in an effort to develop commercial processes for making sponge iron. In addition to the work that has been recorded, other work has been done the results of which have not been published. The degree of success or failure of these investigators does not detract from the fact that the problem has its challenging aspects and practical possibilities that encourage further investigation. The purpose of this report is to record information derived from recent experimental work that .had its 'objective the development and demonstration of one method of producing sponge iron. Work of other investigators was studied and applied wherever possible.

The critical importance of wire rope integrity to the mining industry has led to the creation of standards for the retirement of wire rope as well as to the development of electromagnetic devices and procedures that would allow wire ropes to be tested against these standards without destroying them, The U.S. Bureau of Mines (USBM) conducted research to evaluate these devices by measuring loss of metallic cross-sectional area and the number of wire breaks, or local faults. The intent was to define the extent to which the technique can identify degradation within the rope, e.g., broken wires, corrosion, and wear. Experimental results showed that instrument sensitivity depended on a number of factors, including rope diameter, placement of sensors, location and characteristics of flaws, rope speed, and tension on the rope. The experiments also demonstrated the importance of having an elongated recorder trace and a properly trained and experienced operator to interpret overall results. A design of a rope and test procedures for use in laboratory evaluation of instruments have been recommended by the American Society for Testing and Materials Committee E07.07.10, Wire Rope Applications. The USBM is working through this committee to develop standards for an electro-magnetic method of wire rope inspection.

The behavioral accident simulator (BAS) is a computer program that can be used to simulate accident situations. It demonstrates how the potential to have an accident grows or diminishes as a result of changes in various environmental, organizational, and behavioral factors. It calculates the number of fatalities that may result from various conditions and graphically displays these data. The BAS is useful for studying systems of accident causes, for demonstrating the various ways to diminish fatalities, and for training employees. This Bureau of Mines report provides the detailed instructions needed to use the BAS.

"PURPOSE OF REPORTThe purpose of this report, which is the third of a. series on the coal mines of the Western States, is to give the findings of a study of the causes and methods of prevention of accidents from falls of root and coal in the mines of Lincoln County, Wyo. The three mines covered in this report, referred to as A. B. and C, account- for 9 per cent of the coal production in the county and nearly 9 per cent of the State's total production. For comparative purposes some statistical data are presented bearing upon the relation of injuries to their causes, to tonnage produced, to men employed, to days worked, and to days lost as the result of injuries.FATALITIES IN WYOMINGTo show the necessity for this study, Table 1 given the fatalities that occurred during 1928 in the bituminous—coal mines of the United States, of the State of Wyoming of Lincoln County, and in Mines A, B, and C, followed by a brief description of the Kemmerer coal dis-trict, its coal beds and immediate roof, thickness of cover, dip of coal beds, methods of mining timbering for roof support, roof testing, and supervision. This report presents the results of a study of mill tailings disposal practices and potential tailings dam stability problems. Recognizing the long period of time that would be required to develop the technology and practices necessary to significantly improve refuse disposal safety, the Bureau initiated this research effort as a first step in that development. Volume 1 includes a summary of conclusions and recommendations resulting from the overall research effort; discussions of disposal systems and methods, geology, seismicity, hydrology, and materials characteristics; evaluations of investigational techniques; discussions of usable interim generalized investigational, analytical and inspection procedures; discussion of general principles and presentation of a tailings technology manual outline; and discussion of needed future research am recommended programs.

To evaluate the potential use of battery-powered vehicles, the Bureau of Mines studied the performance of a battery-powered truck propelled by a 9.6 kw dc motor in considerable detail under various driving conditions. Some of the factors investigated were: Gear ratios, payload (1,468 lb maximum), road grade, stops made per mile, and charging time. With the exception of some difficulty with brush wear, operation and maintenance of the truck was generally trouble-free. On range tests carrying an 868-lb payload a distance of about 50 miles at an average speed of about 19 mph in normal driving, the Bureau's battery-powered truck averaged 1.6 to 1.9 miles per ac kwhr input. Stop-and-go tests indicated that prospects for the use of battery-powered trucks in house delivery service within city limits are good; performance of the Bureau truck was sufficiently high for that type of service. The estimated power cost of 2.65 cents per mile while making 10 stops per mile was attractively low from the standpoint of fuel costs.

"Ilmenite is the most important titanium-bearing constituent of titaniferous iron ores. Such ores, of which the United States has abundant reserves, are of interest because they constitute an important potential source of both iron and titanium that may be tapped in times of stress, and which eventually may be utilized in the normal course of events.As the iron and titanium constituents of titaniferous iron ores are not completely separable from each other by ore-dressing methods, any process for metal recovery will, of necessity, involve reduction of the iron in ilmenite. Consequently, knowledge of the thermodynamic properties of ilmenite should supplement information already available concerning those ores. With this in view, the Pacific Experiment Station of the Bureau of Mines undertook to evaluate thermodynamic properties; the data obtained are the subject of this report.Three types of experimental work were involved. First, low-temperature specific heats of ilmenite were measured in the temperature range 500 to 29S° K. These data serve to evaluate the entropy, a quantity necessary for a reliable evaluation and correlation of other thermodynamic properties. Second, high-temperature heat-content measurements were made in the temperature range 298° to 1,743° K. The data serve to bridge the gap between room temperature and elevated reduction-process temperatures. Finally, hydrogen-reduction equilibria were measured in the temperature range 1,220° to1,375° K. These data-give a direct measure of the free energy of reduction of ilmonite at these temperatures and, in conjunction with results of the other two types of measurements, serve to obtain the heat of formation of ilmenite, a quantity not readily determinable by direct calorimetric means.The data are correlated to form a composite, self-consistent portrayal of the thermodynamic properties of ilmenite, which may be used in evaluating the efficacy of any reducing agent of known thermodynamic properties. Selective-reduction of iron in ilmenite by hydrogen, carbon monoxide, and carbon are considered in this report."

On December 16, the General Services Administration (GSA) suspended weekly sales of surplus silver from the national defense stockpile because Congress approved the Defense Appropriation Act, which contained an amendment to halt silver sales. The amendment, introduced by Senator James A. McClure (R., Idaho), prohibits sales for 6 months while the impact of the sales and alternative methods of disposal are evaluated. The GSA sold 2 million troy ounces since sales started October 14. 46.5 million troy ounces were authorized to be sold during this fiscal year and an additional 58.6 million troy ounces were authorized to be sold over the following two fiscal years. The stockpile held 139.5 million troy ounces of silver when sales began in October. Brush Wellman, Inc., a vertically integrated producer of beryllium products announced plans to modify its Delta, Utah beryllium concentrate processing facility to utilize beryllium concentrates containing as little as 7% beryllium oxide. The company plans to invest $2.5 million for this modification with completion expected in mid-1983. Implementation of this plan to consume lower grade beryllium minerals will significantly increase world reserves of beryllium concentrates.

Recent theoretical, experimental, and field studies conducted by the Bureau of Mines suggest that rock failures may be predicted and, in some instances, controlled. Theoretical studies that were later corroborated by laboratory and field studies predict that a seismic anomaly will precede rock bursts and/or roof falls in mines. The seismic anomaly is predicted to consist of an increase in the number of seismic events occurring in the immediate vicinity where the failure 1s nucleated. This increase will then be followed by a dramatic decrease of seismicity in a broad region that surrounds the location of the impending failure. The dimensions of this region allow a preliminary estimate of the time to failure (measured from the time the seismicity increase began) as well as an estimate of the energy that will be released at the instant of failure. Field observations indicating that this predicted behavior does precede rock bursts and a roof fall are presented in this report. Implications of results to the problem of failure control in mines that experience massive rock failures are discussed.

The social economy, or public interest, and national defense require wise development and efficient use of the oil and gas resources of the United States. In this development and use, and in the prevention of economic and physical waste of oil and gas, the objectives of the petroleum and natural-gas industries, the Government (Federal, State, and local), and the public are the same. Differences of viewpoint generally have originated in the failure of one of the triad to have a clear or complete understanding of the motives behind the activities of the other component groups or full knowledge of what each is trying to accomplish. The Unites States Bureau of Mines through its petroleum and natural-gas division makes technologic and scientific investigations pertaining to recovery,3 preparation, refining, and use of oil and natural gas with the view of reducing waste, increasing safety and efficiency and aiding economic development.

"INTR0DUCTIONThis publication is a third of a series devoted to falls of roof and coal in coal mines of the Western States. It gives the results of examinations of the mines of the Rock Springs coal District of Sweetwater County, Wyo., which represent the principal coal beds therein and approximately 75 percent of total production of the State.A study was made of safety conditions at all the producing mines of the district and suggestions were offered the operators regarding methods and practices which would, in the opinion of the writer, provide additional safeguards to prevent accidents from falls of roof and coal.In presenting the subject, consideration must be given to such features as character of the coal beds, depth of cover. methods of mining, timbering practice, roof testing and supervision, and for comparative purposes to certain statistical matter bearing upon relation of accidents to their causes, tonnage produced, men employed, days worked, and lost days due to accidents during the 3—year period covered by this study.ACKNOWLEDGMENTSThe writer acknowledges the cooperation of the officials of the various coal companies, without whose assistance this work could not have been completed.F0UR REPRESENTATIVE COAL OPERATIONSThis report relates to four representative coal operations in the Rook Springs district of Sweetwater County, Wyo., which are referred to as mines A, B, C, and D. This report gins the results of a study conducted hy the Ilnreau of Mines for the purpose of informing the petroleum industry on the recent progress in the de,·elopment nncl application of the absorption process for recO\·ering gasoline from natural gas. The work has been undertaken entirely with regard to plant practice and for the purpose of describing the features and the operation of units of the plants now in use, the improvements that have been made, and the reasons for any ehanges eontemplated by the engineers in eharge· of the plants studied. Throughout the United States the impro,·ements in the absorption process that are under consideration by engineers not only widen practice so as to include gases at all pressures and percentages of gasoline content, but seem to be dewloping definite standards as to dimensions of towers, l"elocities of flow, saturation of oi~ areas of eooling surfaces, and quantities of menstruum to be circulated. Although perfect practice has by no means been attained, the writer belie,·es thnt a description of the current practice at ,·arious plants, including those ob,'iously more efficient features that each plant hns <le\"eloped and uses at one or another stage of treatment, will intere;; t and possibly help all persons operating or designing absorption plants.

The Bureau of Mines has been active in promoting the purchase of coal for Government use under specifications that define the requirements in terms of heating value of the coal, expressed in British thermal units, and the composition as shown by proximate analyses. To these, when required, are added the ash-softening temperature, the free-swelling index, and the Hardgrove grindability index. Under most of these specification contracts the bidders guarantee the quality of the coal, and that guaranteed by the successful bidder becomes the standard of his contract. The deliveries are sampled in accordance with instructions4/ issued by the Bureau of Mines, the samples are analyzed in the Bureau laboratory to determine whether the coal is of the quality guaranteed by the contractor and, if it is not, a price adjustment is made. Analyses of the delivered coal and tipple samples (samples collected at mine tipples as coal is being loaded into railroad cars or trucks) provide valuable data for use in evaluating future bids. In addition, the continuous sampling of coal as delivered is a check on the practical results obtained in burning the coal.
