This document contains most of the materials needed to use the exercise. The main part of the document is the instructor's copy. It tells how to use the exercise, presents the objectives, the master answer sheet, the scoring key, and discussion notes to be used following the exercise. The next section summarizes results from field tests of the exercise, and reports the miners' evaluation of the activity. The last part of this document is three appendices. Appendix A is the exercise problem booklet. This booklet can be duplicated locally. The booklets are reusable. One is needed for every person in the classroom. Appendix B is the answer sheet and the trainee's questionnaire. Copies of this answer sheet must have the latent image (invisible) ink answers that appear in Appendix C printed on them.1 Answer sheets are consumable. Overheating of electric cables on shuttle cars has long been recognized as a cause of premature insulation failure leading to shock and electrocution. Use of cable reels on shuttle cars can cause excessive heat build up which, in turn, causes the cable insulation to soften and become easily damaged This heat-softened insulation reduces the life of the cable. Repeated cycling of a cable in this manner can cause premature aging of the insulation; it becomes bridle, cracks and allows electrical leakage paths to farm. These leakage paths provide the opportunity for shock and electrocution to occur if miners come in contact with the damaged section of cable. It is imperative that cable operating temperatures be maintained at safe thermal limits. This paper discusses a research project conducted by the National Institute for Occupational Safety and Health, Pittsburgh Research Center to determine dynamically the conductor temperature of reeled shuttle car cables using fiber optics technology. The research portrays typical shuttle car loading cycles (both current and time cycles). Root-mean-square current loading is calculated based on the results. The thrust of this effort is to provide the scientific basis for ampacity derating factors specifically for mine machinery using reeled cables This information will reduce the probability of electrical hazards from reeled cables

Highly stressed rock in stopes continues to be a primary safety risk for miners in underground mines because it can result in failures of ground that lead to both injuries and death. Spokane Research Laboratory personnel investigated electromagnetic (EM) emissions in a deep underground mine in an effort to determine if these emissions could be used as indicators of impending catastrophic ground failure. Results suggest that (1) there is no increase in the number of EM emissions prior to recorded seismic activity, (2) some EM signals are generated during blasting, (3) interference from mine electrical sources mask seismic-generated EM signals, (4) EM emissions do not give enough warning (compared to seismic monitoring) to permit miners to leave a stope, (5) the distance an EM signal can travel in the rock is between 18 and 40 m, and (6) current data acquisition systems do not differentiate between EM signals generated from seismic activity and random mine electrical noise. These results preclude monitoring EM emissions as precursors of impending catastrophic ground failure.

This Bureau of Mines manual is oriented to the technical needs of an investigation of fire or explosion accidents and includes various practical and theoretical information that is needed in analyzing accidents or evaluating conditions that might lead to an accident. It is applicable to accidents in the chemical, mining, transportation, and other fuel-related industries. Specific procedures for conducting an accident investigation are outlined, and various guidelines are given for developing the fire or explosion evidence. Ignitability, flammability, and physicochemical data are presented for over 500 combustible gases, liquids, and solids; empirical and theoretical expressions are also given for extrapolating the behavior of combustible-oxidant-diluent systems to various environmental conditions. In addition, various data pertinent to damage analysis are summarized to facilitate the task of the investigator in developing plausible accident scenarios.

The The National Institute for Occupational Safety and Health (NIOSH) Pittsburgh Research Laboratory (PRL) is continuing to investigate the behavior of fully grouted roof bolts in the weak roof rock of the Safety Research Coal Mine (SRCM). This paper describes the results of three studies: • A series of 24 pull tests of bolts installed fully grouted and overcored to leave 12 inches of grouted bolt. The tests compared the pull-out performance of offset-head roof bolts with that of standard 5/8-inch bolts; • A second series of 24 tests that compared the pull-out loads obtained by bolts installed with 1 ft of resin (a Short Encapsulation Pull Test or SEPT) to that of fully grouted bolts overcored to leave only 12 inches of grouted bolt, and; • An investigation into the pressures generated during the installation of fully grouted resin bolts. All of the bolts installed and pulled were overcored and removed from the roof after the pull tests to allow inspection of the resin. The first study found that there was no significant difference in pullout load between the offset-head bolts and the standard bolts. The second study, comparing the SEPT bolts and the partially-overcored fully grouted bolts, indicated that SEPT can significantly underestimate bolt anchorage grip factor, suggesting that the SEPT is a conservative measure of actual bolt performance. Significant loss of resin to cracks in the roof was observed in the bolts overcored in the SRCM. The third study explored one possible explanation for the resin loss, high pressures generated during bolt installation. These tests employed bolts installed in strain gauged steel tubes. The results confirmed that significant pressures (greater than 4,000 psi) can be generated during bolt installation. The results of the pressure tests, combined with the observations of resin loss in bolts installed in the SRCM, suggest that under some circumstances resin loss and under-encapsulation could take place and lead to degraded bolt performance. Glove fingering was observed in all recovered bolts to various degrees. The limited comparative data suggested that the effect of glove fingering ranged from slight to moderate. Complete mixing of the resin was observed on all of the 40 bolts recovered (28 fully grouted and 12 SEPT). Australian and U.S. resins were compared and the Australian resins were observed to have much smaller catalyst compartment areas and lower resin viscosities. These differences are suggested as potential explanations for occasional resin mixing problems noted in Australian resin bolt installations.

Two types of airflow splitting methods for improving longwall dust control were investigated by NIOSH's Pittsburgh Research Laboratory. These methods included a translucent mesh barrier and a staged spray barrier system to confine the shearer-generated dust to the coal extraction side of the longwall face while maintaining a cleaner split of airflow on the opposing worker walkway side of the longwall face. The translucent mesh barrier was shown to provide notable dust control effectiveness up to 60 m downstream of the shearer in both the laboratory experiments and a longwall field evaluation, provided the mesh barrier remains relatively parallel to the airflow. The staged spray barrier system showed marginal dust control effectiveness in the laboratory and subsequently was not field tested.

The roof rock in underground limestone mines in Northern Appalachia can be subject to high horizontal stresses in spite of the shallow depth of the workings. The high stresses can cause roof stability problems. The National Institute for Occupational Safety and Health staff have observed a distinctive asymmetrical mode of failure at the pillar-roof contact in two underground stone mines, which is different from the typical failure mode observed in response to excessive levels of horizontal stress. A dip of greater than 5° was identified as a possible cause of this mode of failure. Numerical model experiments showed that an increase in the dip of the workings can cause an increase in the stress at the up-dip corner of the roof beam. A case study is presented in which failure at the pillar-roof contact was observed where the dip of the workings was 7° in a high horizontal stress field. Numerical modeling showed that the relatively small dip of the workings could have induced stresses changes in the immediate roof that would explain the failures. The model results also showed that this mode of failure is less likely to occur if the limestone pillars contain weak bedding planes that provide stress relief. In addition, the results showed that for the conditions at the case study site, the stress in the roof beam was not sensitive to the thickness of the roof beam or to the excavation span. The high horizontal stresses at this site are an important contributing factor to the observed failures.

Steel liner plates, which are bolted together to form a shell, are commonly used to provide temporary protective linings in the tunneling industry to ensure the stability of the tunnel and safety of the workers until more permanent ground reinforcement can be installed. As the ground deforms to try and close the opening, the liner plate is radially stressed, and by deforming redistributes these loads back to the surrounding insitu earth. Grouting behind the plates is done to fill any voids and ensure uniform contact to the strata around the back of the plates. Another function of the liner plate in some applications is to act as a platform or structure for the mining machine to react against in order to develop the necessary forward thrust to be able to excavate the face. In this case, the liner plates are loaded axially as a horizontal column of bolted liner plate rings, creating a jacking force against the liner plate assembly. In general, the buckling strength of the plate sections determines the jacking force capacity. Historically, the jacking force capacity of tunnel liner plates has been theoretically determined by some simplistic equations developed from tests of single liner plates. Recently, full-scale tests on a complete circular section of bolted liner plate ring segments have been conducted in the NIOSH Safety Structures Testing Laboratory using the unique Mine Roof Simulator to apply controlled loading in a manner that simulates the real world environment. Due to the unique size and capacity of the Mine Roof Simulator, these are believed to be the first full-scale tests conducted on the jacking force capability of tunnel liner plate rings. These tests indicate that the previously used theoretical equations tend to over predict the jacking force capacity of these tunnel liner plates. Unanticipated deformations in the field have been observed at loads below these theoretical design loads, therefore, a more accurate determination of the jacking force capability was needed. This report summarizes the full-scale test results, examines design parameters that impact the jacking force capability, and develops new mathematical models to improve the prediction of design loads.

Permissible electric nine lamps are approved with the. understanding that if actively marketed they will be listed in Bureau of Mines publications so that state mine departments, compensation bureaus, mine operators, miners, and others interested in safety equipment for mines may know the permissible lamps available and the important safety features of each. The last list of permissible electric mine lamps was issued as Report of Investigations 33045 in June 1936, a supplement to Bulletin 332.6 All lamps described in Bulletin 332 are either obsolete or have been materially modified, as covered by extensions of the original approvals. These modified lamps and the new designs approved since 1930-totaling 12 cap lamps and 23 lamps of miscellaneous types-comprise the present list of actively marketed permissible electric mine lamps. This bulletin presents the Bureau's test requirements and procedure relative to permissible electric mine lamps and describes briefly the lamps, giving tabulated details of each.

Comprehensive world mineral industry investment data do not exist, but limited material published on aggregates of in vestment in some elements of the world mineral industry suggest a continued up-turn in the investment level, at least in market economy countries in 1989. Steel industry investments in Organization for Economic Cooperation and Development (OECD) countries are not yet available for 1989 but, as shown in table 6, increased by 22.1% in 1988, after showing a decrease of 5.2% (revised) in 1987. The pattern of investment ups and downs varied during 1988 within country blocs and economic alliances showing no particular trends. Investment in the steel industry by the United States increased by 58.3% between 1987 and 1988 and was 1 1.9% higher than the previous highest amount invested during the 1984-88 period but was still 41.5% less than the amount invested in 1983. Investment in the steel industries of the European Coal and Steel Community (ECSC) dropped again in 1988 from the revised amount for 1987, which became the first drop after several years of steady growth in what became the expanded ECSC in 1986. This drop in 1988 was the result of lower investments by Denmark, France, the Federal Republic of Germany, Ireland, Italy, Luxembourg, the Nether- lands, and Spain, investments grew or remained level in Belgium, Portugal, and the United Kingdom in 1987 and 1988. A decrease in investment in the Canadian (-26.6%) steel industry was offset by in- creases registered by the members of the European Free Trade Association (EFTA) of 31.7%, by Japan of 19.8% by Turkey of 22.4%, and a slight increase by the nations of Latin America. If data for the world steel industry as a whole, including those of the centrally planned economy countries, were available, the same pattern presumably would be shown as that reported; that is, it would remain fairly stable, with increases in some countries being offset by decreases in others. However, the lack of comprehensive information on the centrally planned economy countries makes it impossible to determine whether the overall trend was up or down despite recorded increases or decreases in production of the materials involved. Annual global steel industry investment is partly mirrored in the data for selected market economy countries that appear in table 13 for 1988-90. No reasonably comprehensive 1991 data were available for inclusion in this study, and the partial reporting available was so incomplete that it was impossible to demonstrate whether the level advanced or declined from the 1990 level of nearly $15.5 billion. Moreover, with the radical changes that are now being effected in the steel industries of the states that were the U.S.S.R. and its CMEA partners, it is most regrettable that comprehensive and comparable data, both current and historic, on investment in this area are not available. Updated information on market economy country petroleum industry capital and exploration expenditures by geographic area has been included in table 14, and it is evident that there was a substantial upturn in investment in 1991. The level of almost $74.5 billion was 14.5 % above the 1990 level and included gains in every topical category and geographic area. The source of information for this table has been changed to the Oil and Gas Journal Database from the previously used Chase-Manhattan Bank study of Capital Investments of the World Petroleum Industry, which has been discontinued. Because of this source change, it has been necessary to modify the structure of the table somewhat, but its total scope of coverage is regarded as basically unchanged. It should be noted that the grand total, as well as some of the detail appearing for 1987, corresponds precisely with those in the previous source.

World investment 10 all mineral and mining industry sectors is not available. However, partial information available provides some information useful for analysis. Information available in this section covers limited geographic information on investment in the steel industry, limited information for petroleum industry capital spending, and total U.S. foreign investment in comparison to the mineral-related sectors (tables 13, 14, and 15). Annual steel industry investment is available for the European Community, almost all countries within the European Free Trade Association (EFTA), Latin America, and other countries with significant mineral industries, such as Canada, Japan, and the United States. Although information was not available for 1992, data for 1991 indicate that for those countries aforementioned, total investment increased by 8 %. However, investment patterns were very different within the different groups of countries. While the European Community had a net increase of 12 %, dominated by an almost °40 % increase in Germany, EFTA member countries had a 26 % decrease in investment for the same period. Investment in EC countries was almost seven times that of EFT A countries. Japan's investment in the iron and steel industry, by far the largest of those available, almost twice that of the EC and almost four times that of the United States, increased by more than 20 %. In North America, investment in Canada and the United States decreased. Almost all of Latin America decreased investment in the sector. The notable exception was Mexico, with a 60 % increase. Changes in Mexico's investment policies of the recent past, its economic growth, and privatization of its iron and steel industry are some of the reasons for such an increase. The other country covered individually in Latin America, where investment in the iron and steel industry increased, although slightly, was Argentina. This limited information does not allow a comprehensive analysis of global industry trend, which in turn impacts many other mineral commodities, and with the transition in many countries within Europe and Central Eurasia that are now effecting the steel industries of the countries that were members of the U.S.S.R. and its CMEA partners, it is most regrettable that comprehensive and comparable data, both current and historic, on investment in this area are not available.

WHILE methods of utilizing blast-furnace slag have been developing, a great deal of literature on the subject has accumulated, but no comprehensive summary of information that would be helpful to engineers, contractors, architects, chemists, and others in under-standing and more successfully using this byproduct of the iron and steel industry has been published in the English language. The National Slag Association recognized the need for such a publication, and the Problems Committee of the Association under-took in 1943 to accumulate the necessary material. Over a period of several years this committee compiled pertinent information in manuscript form, and in 1948 the Bureau of Mines and the National Slag Association made an agreement whereby the Bureau was to prepare the data for publication as a cooperative bulletin of the Bureau of Mines and the National Slag Association.

The concept of isokinetic measurement of strength was originally related by Hislop and Perrine.(1)Characteristics of an isokinetic exertion are constant velocity throughout a predetermined range of motion. Strictly speaking, a means of speed control, and not a load in the usual sense, is applied in isokinetic exertions.(1)However, load and resistance are definitely present in this technique. In this case, the load is a result of the energy absorption process performed by the device to keep the exertion speed constant. The device prevents energy from being dissipated through acceleration in isokinetic exercise. The energy is instead converted into a resistive force, which varies in relation to the efficiency of the skeletal muscle. Since the speed of motion is held constant in isokinetic exercise, the resistance experienced during a contraction is equivalent to the force applied throughout the range of motion. For this reason, the technique of isokinetic exercise has sometimes been referred to as accommodating resistance exercise. This type of exercise allows the muscle to contract at its maximum capability a tall points throughout the range of motion. At the extremes of the range of motion of a joint, the muscle has the least mechanical advantage, and the resistance offered by the machine is correspondingly lower. Similarly, as the muscle reaches its optimal mechanical advantage, the resistance of the machine increases proportionally. It must be understood, however, that while isokinetic devices control the speed of the exertion, this does not assure a constant speed of muscle contraction. The speed of isokinetic contractions is constant during individual exertions; however, it is also possible to compare muscular performance over a wide range of isokinetic velocities. Increasing the isokinetic speed of contraction will place increasing demands on Type II muscle fibers (fast twitch and fast oxidative glycolytic). Workplace Assessment It is clear that isometric strength testing cannot substitute for dynamic strength assessment when examining highly dynamic occupational job demands. As most industrial work tasks contain a significant dynamic component, analysis of isokinetic strength capabilities appears to offer some advantage to isometric testing in this regard. However, it must be recognized that isokinetic devices are not entirely realistic compared with free dynamic lifting in which subjects may use rapid acceleration to gain a weight-lifting advantage.

"I. IntroductionThe alveolar surface of the lung is covered by two populations of pneumocytes, type I and type II epithelium. The type I cell, which accounts for most of the alveolar surface area and provides a highly specialized barrier for gas exchange (Rannels et al., 1991), is poorly understood. Although approaches to type I cell purification have been discussed, they have not been applied widely because of the lack of phenotypic markers to monitor isolation procedures (Dobbs et al., 1988; Danto et al., 1992). In contrast, the type II cell has been studied extensively, largely because of the development of reliable procedures for its isolation and primary culture. Investigations based on the use of purified populations of cultured type II cells have provided convincing evidence to support a role for the pneumocytes both in the maintenance of normal lung function and in the response of the lung to injury (for review, see Rannels and Rannels, 1989).While type II cells can be isolated by a variety of approaches, the protocols used most commonly are modifications of the gradient method of Dobbs and Mason (1979) or of the panning method of Dobbs et al. (1986). Purification of the cells based on density gradient centrifugation is reliable and consistent, with yields of 30 to 40 X 106 type II cells per rat of 230 g body weight. Isolation procedures applied to a minimum of three to four rats provide the best results, as lower cell yields are common with the use of fewer animals. Isometric strength is defined as the capacity to produce force or torque with a voluntary isometric (muscle[s] maintain[s] a constant length) contraction. The key thing to understand about this type of contraction and strength measurement is that no body movement occurs during the measurement period. The tested person's body angles and posture remain the same throughout the test. Isometric strength has historically been the type most studied and measured. It is probably the easiest to measure and to understand. Some strength researchers feel that isometric strength data may be difficult to apply to some "real life" situations because in most real circumstances people are moving - they are not static. Other researchers counter that it is equally difficult to determine the speed of movement of a person or group of persons doing a job (each moves in his or her unique manner and speed across the links and joints of the body). Thus, dynamic strength test data collected on persons moving at a different speed and/or in a different posture from the "real world" condition will be just as hard to apply. In truth, neither is better - they are different measurements. Both researchers and users should collect and use data that they understand and that fits their application. Workplace Assessment When a worker is called on to perform a physically demanding lifting task, the external load produces moments - tendencies to produce motion, also called torques - about various joints of the body.(1) Often these moments are augmented by the force of gravity acting on the mass of various body segments. For example, in a biceps curl exercise, the moment produced by the forearm flexors must counteract the moment of the weight held in the hands as well as the moment caused by gravity acting on the forearm's center of mass. To perform the task successfully, the muscles responsible for moving the joint must develop a greater moment than the combined moments of the external load and body segment. It should be clear that, at each joint of the body, there is a limit to the strength that the muscle can produce to move ever-increasing external loads. This concept forms the basis of isometric muscle strength prediction modeling.(1) The following procedures are generally used with this biomechanical analysis technique. First, workers are observed (and usually photographed or video-taped) as they perform physically demanding tasks. For each task the posture of the torso and the extremities are documented at the time of peak exertion. The postures are then re-created using a computerized software package, which calculates the load moments produced at various joints of the body as the task is performed. The values obtained during this analysis are then compared to population norms for isometric strength obtained from a population of industrial workers. This paper reviews research from the U.S. mining community to define issues relevant to an evolving national and international workforce and to relate them to the emergency response population. The authors further explore and relate the key problem of an aging workforce to the resultant changes and emerging physical and psychological needs of emergency workers. Finally, the authors cite examples from the mine emergency response community and mine rescue experiences to suggest practical recommendations that include both organizational and individual formats, with an emphasis on new approaches to training this changing workforce.

Vehicle vibration exposure has been linked to chronic back pain and low back symptoms among agricultural tractor drivers. The objectives of this study by the National Institute for Occupational Safety and Health (NIOSH) were to assess driver whole body vibration (WBV) exposures and recommend interventions to reduce the risk of back related injuries, particularly relative to vehicle jarring/jolting (the transient mechanical shock components of WBV). The methodology included collecting, from two independent samples, ?eld data and health and work history data of farm equipment operators. Data were collected during mowing, raking, baling, chiseling, tilling, and road travel for different model tractors. Spraying using a sprayer and shrub removal with a skid steer loader were also included. Based on ISO 2631 (1985), the American Conference of Governmental Industrial Hygienists, threshold limit values, presents 0.5 m/s2 as the action level recommended by the Commission of European Communities for overall weighted total RMS acceleration (vector sum for axes x, y, and z) [ACGIH, 2006.
